Depression is caused by afunctional deficit of monoamines in their signaling pathways (serotoninergic, dopaminergic, noradrenergic) in certain parts of the brain. Although there are various forms of clinical treatment of mood disorders such as psychotherapy, antidepressants, ECT and TMS, the prescription of antidepressant drugs is the main form of management of calls endogenous depressions. The functional modulation of neurochemical transmission paths can interfere with the processing performance of cognitive functions like memory. Objective: Our study sought to investigate the possible changes in different types and stages of memory caused by chronic use of antidepressants from different managements; monotherapy and combination therapy. Methods: We use this research to chronic administration of three antidepressant drugs and their associations (paroxetine, venlafaxine and bupropion) widely used in clinical practice. To evaluate the performance mnemonic to use behavioral models predictive of mice with eigen value memory (radial maze, passive avoidance) and exploratory activity (open field). They measured levels of BDNF and AChE in the hippocampus of rats in groups of antidepressant users. Results: Radial Maze - acquisition phase: the Venlaf group (11 ±4.85), associated with Venlaf Parox (10,88±5,30) associated with Buprop and Venlaf (11 ± 3.85) showed a number of training trials significantly higher compared to that of the control group (5.11± 1.36), while groups with Parox (4, 14 ± 2.19), Buprop (7.1 ± 3.66) associated with Parox alone and Buprop (4.75 ± 1.75) showed no change in the number of significant attempts. Radial Maze - MC: Venlaf groups (2.16 ±1.47) associated with Venlaf Parox (2.85 ± 1.35) associated with Buprop and Venlaf (2.62 ± 1.76) showed no significant changes compared to the user group of saline (2.25 ± 1.58) while users of antidepressants Parox (3.11 ± 1.69), Buprop (4.9± 2.99) associated with Buprop and Parox (4.12 ± 2.58) had significantly (p <0.001) greater errors in the control group (2.25 ± 1.58). Radial Maze – ML: Venlaf users groups (4.5 ± 1.64), Buprop (4 ± 2.3) associated with Buprop and Venlaf (2.87 ± 1.72) showed no significant than the control group (3.11 ± 1.36),where as the group user Parox (4.4± 1.50) howed a significantly greater errors in the control group (3.11 ± 1 ,36) Venlaf users groups associated with Parox (2.66 ± 0.85) associated with Buprop and Parox (3.12 ± 2.16) had a number of errors significantly lower when compared to the number of errors in Paroxetine (4.4 ± 1.50). Avoidance passive –MC and ML : Parox users groups(218.4 ± 113.1) Buprop (39.57 ± 23.19) and associated with Parox Buprop (300 ± 0.0) showed a significant lag time higher in the control group (270 ± 37.30). ML: Venlaf users groups associated with Parox (218.1 ± 103.6) associated with Buprop and Parox (296.7 ± 8.69) revert the increase in the latency time of the Parox (218,1±103,6) and caused a significant decrease the time spent in the compartment of the mouse course over the same group. Open Field: Parox (20.80 ± 3.85) increased significantly in the control (14.90 ± 3.85) while the user group Buprop associated with Parox (12 ± 2.50) reversed the increase in locomotor activity of the Parox group (20.80 ± 3.85). Conclusions: the test of acquisition show loss of performance in groups o antidepressant users with little selectivity for serotonin reuptake. The MC and ML in visual-spatial skills, we found deficits in groups of antidepressants that potentiate both serotonin action with dopaminergic action. The potentiation of β-adrenergic is associated with increased level of alertness and improves the performance of establishment and consolidation of MC and ML. / Depressão é uma condição psicopatológica crônica e recorrente, com evidentes prejuízos sócio-ocupacionais e de qualidade de vida, tanto na fase aguda quanto no longo prazo. A depressão é causada por um déficit funcional das monoaminas nas suas vias transmissoras (serotoninérgicas, noradrenérgica e dopaminérgicas) em certos locais do cérebro. Apesar de existirem diversas formas de tratamento clínico desse transtorno do humor como: psicoterapia, antidepressivos, ECT e TMS, a prescrição das drogas antidepressivas representa a principal forma de manejo das chamadas depressões endógenas. A modulação funcional das vias de transmissão neuroquímica pode interferir no desempenho do processamento de funções cognitivas como a memória. Objetivo: Nosso estudo procurou investigar as possíveis alterações em diferentes tipos e fases da memória provocadas pelo uso crônico de diferentes manejos de antidepressivos; monoterapia e terapia combinada. Métodos: Utilizamos nessa pesquisa a administração crônica de três drogas antidepressivas e suas associações (Paroxetina, Venlafaxina e Bupropiona) de largo uso na prática clínica. Para avaliação do desempenho mnemônico utilizamos modelos comportamentais para ratos com alto valor preditivo para memória (labirinto radial, esquiva passiva) e para atividade exploratória (campo aberto). Foram medidos os níveis de BDNF e a atividade enzimática da AChE do hipocampo dos grupos de ratos usuários de antidepressivos. Resultados: No Labirinto Radial – fase de aquisição: os grupos Venlaf (11±4,85), Venlaf+Parox (10,88±5,30) e Venlaf + Buprop (11±3,85) apresentaram número de tentativas de treinos significativamente maior comparado ao desempenho dos animais do grupo controle (5,11±1,36), enquanto os grupos com Parox (4,14±2,19), Buprop (7,1±3,66) sozinha e Parox + Bup (4,75±1,75) não apresentaram alterações do número de tentativas significantes. Labirinto Radial – MC: os grupos Venlaf (2,16±1,47), Venlaf+Parox (2,85±1,35) e Venla + Bup (2,62±1,76) não apresentaram alterações significantes em comparação ao grupo usuário da solução salina (2,25±1,58), enquanto que os usuários dos antidepressivos Parox (3,11±1,69), Bup (4,9±2,99) e Parox + Bup (4,12±2,58) apresentaram um número significativamente (p<0,001) maior de erros em relação ao grupo controle (2,25±1,58). Labirinto Radial – ML: grupos usuários de Venla (4,5±1,64), Buprop (4±2,3) e Venlaf+ Bup (2,87±1,72) não apresentaram alterações significativamente diferente do grupo controle (3.11±1,36), enquanto que o grupo usuário de Parox (4,4±1,50) apresentou um número significativamente maior de erros em relação ao grupo controle (3.11±1,36). Os grupos usuários de Venlaf + Parox (2,66±0,85) e Parox+Buprop (3,12±2,16) apresentaram um número de erros significativamente menor quando comparados ao número de erros na Parox (4,4±1,50). Esquiva Passiva – MC e ML:os grupos usuários de Parox (218,4±113,1), Bup (39,57±23,19) e Parox+Buprop (300±0,0) apresentaram um tempo de latência significativamente (p<0,001) maior em relação ao grupo controle (270±37,30). ML: os grupos usuários de Venlaf+Parox (218,1±103,6) e Parox+ Bup (296,7±8,69) reverteram o aumento do tempo de latência do grupo de Parox (218,1±103,6) e causou uma diminuição significativa (p<0,001) do tempo de permanência do rato no compartimento claro em relação ao mesmo grupo. Campo Aberto – N0 cruzamentos: Parox (20,80±3,85) apresentou um aumento significativo do número de cruzamentos em relação ao grupo controle (14,90±3,85), enquanto que o grupo usuário de Parox+Buprop (12±2,50) reverteu o aumento da atividade locomotora do grupo da Parox (20,80±3,85). Conclusões: no teste de aquisição houve prejuízo de desempenho nos grupos usuários de antidepressivos com pouca seletividade para recaptação de serotonina. O desempenho da MC e ML, em habilidades visuo-espaciais, obtivemos déficits nos grupos de antidepressivos que potencializam tanto a ação serotoninérgica, com a ação dopaminérgica. A potencialização β-adrenérgica induziu uma melhora do desempenho da formação e consolidação da MC e ML.

Segurança da terapia antirretroviral para a infecção pelo vírus HIV-1 em ensaios clínicos randomizados envolvendo pacientes multiexperimentados utilizando terapia otimizada de base : uma revisão sistemática

Introdução: O desenvolvimento de antirretrovirais (AVRs) para o vírus HIV em pacientes multiexperimentados é prioridade em saúde pública e, além de supressão virológica máxima, recuperação e estabilização imunológica, os ensaios clínicos randomizados (ECRs) que envolvem esquemas de antirretrovirais utilizados em terapias de resgate devem ser capazes de demonstrar segurança na sua utilização, a curto e longo prazos. Objetivo: Realizar revisão sistemática e caracterizar os achados de segurança da terapia antirretroviral (TARV) para a infecção pelo vírus HIV-1 nos ECRs que utilizaram esquemas otimizados de base (EOB) em pacientes multiexperimentados através, principalmente, da avaliação da qualidade da informação sobre riscos baseada na extensão do CONSORT sobre riscos e da avaliação da apresentação de achados laboratoriais que são proxy de desfechos conhecidamente relevantes. Metodologia: Foram revisadas as bases de dados MEDLINE, EMBASE, LILACS, Colaboração Cochrane, SCOPUS e ISI Web of Science visando identificar publicações entre janeiro/2003 e agosto/2014. Também foram revisadas as bases de dados dos seguintes congressos científicos internacionais: International AIDS Conference; Conference on Retroviruses and Opportunistic Infections (CROI); Interscience Conference on Antimicrobial Agents and Chemotherapy (ICAAC); e International Congress on Drug Therapy in HIV Infection. Não foram feitas restrições em relação à língua da publicação dos estudos. Os critérios para inclusão dos estudos foram: ECRs com pelo menos 16 semanas de duração e com pelo menos 100 indivíduos que avaliaram segurança da terapia antirretroviral (TARV) em pacientes com infecção pelo HIV- 1 caracterizados como multiexperimentados. Resultados: Vinte e quatro ECRs foram incluídos, sendo 16 estudos originais e 8 extensões. Vinte e três (95,8%) estudos foram exclusivamente financiados pela indústria farmacêutica. Apenas 15 (62,5%) estudos apresentaram eventos adversos (EAs) clínicos graves. Ocorrência de limiar para a apresentação de resultados foi observada em 13 (86,7%) estudos que apresentaram EAs graves, 22 (95,6 %) estudos que apresentaram EAs considerados comuns e em 8 (42,1%) estudos que apresentaram informações sobre desfechos laboratoriais. Seis (25%) ECRs não apresentaram informações sobre os desfechos laboratoriais investigados. Oito (33,3%) estudos não apresentaram informações sobre triglicerídeos, colesterol total, LDL (low density lipoprotein) e hiperglicemia, resultando em ausência de informação sobre os referidos desfechos para os medicamentos maraviroque (MVC) e vincriviroc (VIC). Nove estudos não apresentaram informações sobre marcadores hepáticos, resultando em ausência de informação destes desfechos para o medicamento enfuvirtida (ENF). Dezoito (75%) estudos não apresentaram informações sobre creatinina, resultando em ausência de informação sobre este desfecho para os medicamentos ENF, tipranavir (TPV) e darunavir (DRV). Conclusões: A informação sobre riscos em ECRs envolvendo TARV para pacientes multiexperimentados é altamente seletiva e insuficiente. Além disso, desfechos conhecidamente relevantes não tem sido incluídos a priori nestes estudos. Os ECRs que avaliam novos ARVs ainda apresentam as características dos estudos do início da era TARV, quando, diante de um cenário com grande mortalidade associada ao HIV, o principal objetivo era preservar a vida dos pacientes, em detrimento das questões de segurança da terapia. / Introduction: The development of antiretroviral drugs (ARVs) for treating the HIV virus in treatment-experienced patients is a priority in public health. In addition to virological suppression, recovery and stabilization of the immune system, randomized clinical trials (RCTs) involving antiretroviral regimens used in recovery therapies must be able to demonstrate short and long term safety in their use. Objective: To perform a systematic review and to characterize the findings regarding the safety of cART for HIV-1 virus infections in RCTs that used optimized background regimen in treatmentexperienced patients primarily through assessing the quality of information based on the extent of the CONSORT on risks and evaluating the overall presentation of laboratory findings that are known to proxy relevant outcomes. Methodology: The following databases were researched: MEDLINE, EMBASE, LILACS, The Cochrane Collaboration, SCOPUS and ISI Web of Science. We searched for works published between January/2003 and August/2014. The databases of the following international scientific conferences were also researched: International AIDS Conference; Conference on Retroviruses and Opportunistic Infections (CROI); Interscience Conference on Antimicrobial Agents and Chemotherapy (ICAAC); and International Congress on Drug Therapy in HIV Infection. There were no restrictions regarding the language of the studies. The criteria for inclusion were the following: RCTs that lasted at least 16 weeks and with at least 100 participants who evaluated the safety of antiretroviral therapies (cART) in treatment-experienced patients with HIV-1. Results: Twenty-four RCTs were included, sixteen original studies and eight extensions. Twenty-three (95.8%) of the studies were funded exclusively by the pharmaceutical industry. Only fifteen (62.5%) of the studies showed serious clinical adverse events (AEs). The occurrence threshold for the presentation of results was observed in thirteen (86.7%) of the studies that showed serious AEs, twenty-two (95.6%) of the studies that showed AEs considered to be common, and eight (42.1%) of the studies which presented information on laboratory outcomes. Six (25%) RCTs have not provided information on laboratory outcomes investigated. Eight (33.3%) studies did not provide information on triglycerides, total cholesterol, LDL (low density lipoprotein) and hyperglycemia, resulting in lack of information on these outcomes for the drugs maraviroc (MVC) and vicriviroc (VIC). Nine studies did not provide information on hepatic markers, resulting in lack of information on these outcomes for the drug enfuvirtide (ENF). Eighteen (75%) studies did not provide information on creatinine, resulting in lack of information on this outcome for the drugs ENF, tipranavir (TPV) and darunavir (DRV). Conclusions: Information on the risks of RCTs involving cART for treatment-experienced patients is highly selective and insufficient. Moreover, known relevant outcomes have not been included a priori in these studies. RCTs evaluating new ARVs have the same characteristics of studies of early ARVs, when, faced with a scenario of high mortality associated with HIV, the main goal was to preserve the lives of patients, at the expense of the safety in therapy.

O objetivo desta investigação foi analisar a percepção do doente de tuberculose sob tratamento supervisionado em uma Unidade Distrital de Saúde do município de Ribeirão Preto-SP. Optou-se pela pesquisa de natureza qualitativa. A população do estudo constituiu-se 6 doentes de tuberculose inscritos no Programa de Controle de Tuberculose sob o regime de tratamento supervisionado (TS). Para a coleta de dados utilizou-se o prontuário do doente e a Ficha Epidemiológica de Notificação da doença e a entrevista semi-estruturada como tendo como questões orientadoras "o significado da tuberculose" e "o tratamento supervisionado na vida do doente". Para a análise dos dados utilizou-se a técnica de análise de Conteúdo, modalidade Temática. A unidade temática central é o tratamento supervisionado na tuberculose: a percepção do doente foi conformada a partir dos seguintes núcleos de sentido: Enfoque da ação terapêutica no Tratamento Supervisionado (A ingestão medicamentosa e Fortalezas e Debilidades do Tratamento Supervisionado ); e Singularidades do doente sob tratamento supervisionado, (Percepção sobre a doença, Convivência com a doença, As repercussões do tratamento supervisionado na vida do doente). As fortalezas percebidas no processo da doença e no tratamento: medicação gratuita; a cesta básica, o vale transporte; a supervisão através da visita domiciliária permite o vínculo entre os trabalhadores de saúde e os doentes e suas famílias. As debilidades do TS percebidas foram: a fiscalização na tomada da medicação; a dependência do horário da visita para ingerir a medicação. A família e a equipe de saúde (na figura da visitadora) são reconhecidos como fator de adesão ao tratamento. / This study aimed to analyze the perception of tuberculosis patients under supervised treatment (ST) in a District Health Unit in the City of Ribeirão Preto - SP. A qualitative analysis was selected as the methodological approach. The study population consisted of 7 tuberculosis patients participating in the Tuberculosis Control Program under the a regimen of supervised treatment. The patients' medical records, the Epidemiological Form of Disease Notification and semi-structured interviews were used as instruments for data collection. The following were used as guiding questions: "the significance of tuberculosis" and "the supervised treatment in the patient's life". The Content Analysis technique - Thematic Modality - was used for data analysis. The main thematic unit "Supervised treatment in tuberculosis: the patient’s perception" was found from the following meaning units: the focus of ST as a therapeutic action (medication ingestion and the weaknesses and strengths of ST); and the Singularities of patients under supervised treatment (The perception of the disease, Living with the disease, The consequences of ST on the patient's life). The following were perceived by patients as strengths in the disease and treatment process: the provision of free medication; the availability of other forms of incentive such as the basic food basket and transportation vouchers; supervision through home visits allowed the link between health workers, the patients and their families. The weaknesses of ST were perceived as: the surveillance of medication ingestion and the dependence on the visiting hours for medication ingestion. The social actors involved in the treatment, such as the family and the health care team (in the visiting figure), were recognized as a factor of adherence to the treatment.

Perfil da saúde bucal de crianças e jovens tratados pelas terapias alopática e homeopática

Este estudo teve como objetivo comparar a saúde bucal de pacientes, portadores de problemas respiratórios que se tratavam com medicações alopáticas e homeopáticas, no mínimo por dois anos. Foram examinados 90 pacientes com idades de 6 a 14 anos, emparelhados por sexo e idade, representando um estudo analítico observacional, semelhante aos de caso-controle. Os índices utilizados para avaliar a saúde bucal foram: CPO-S e ceo-s (Índices de superfícies cariadas, perdidas e obturadas), IP (Índice de placa), IG (Índice gengival) e, ainda, VFS (Velocidade do fluxo salivar) e CTS (Capacidade tampão salivar). Foi realizado um questionário sobre os hábitos de higiene, alimentação e condição geral de saúde, assim como sobre as condições socioeconômicas das famílias. Os Graus de instrução dos responsáveis (P= 0,007), a Escola do tipo particular (P< 0,000) em que os participantes estudavam mostraram-se associados, significativamente, ao grupo que utilizava Homeopatia, mas a Renda familiar não apresentou diferença entre os dois grupos (P=0,456). Este grupo mostrou-se, também, estatisticamente mais saudável quanto aos índices de saúde bucal (CPO-S, ceo-s, IP e IG, com P<0,05) em análises bivariadas pelos testes “t” de Student e U de Mann Whitney. A Velocidade do fluxo salivar foi significativamente maior apenas pelo teste U de Mann Whitney (P= 0,015). Ao serem comparados por testes multivariados, em que a variável Grau de instrução dos pais foi controlada, as diferenças significativas entre os tratamentos se mantiveram para todas as variáveis com exceção do CPO-S. A análise de regressão logística que foi usada para comparar os tratamentos quanto ao Perfil de saúde bucal, variável criada a partir das variáveis CPO-S, ceo-s, IP e IG, evidenciou que o tratamento homeopático é fator de proteção à saúde bucal (Odds Ratio=28,02). Quando na comparação foram associadas as variáveis, Grau de instrução dos pais apresentou Odds Ratio=23,26 e ao incluir o tipo de escola em que os pacientes estudavam o Odds Ratio foi de 19,23, todos valores sendo significativos. Através das análises realizadas, não se encontraram semelhanças quanto à saúde bucal dos dois grupos examinados, tendo o grupo homeopatizado melhor condição bucal. Novos estudos são recomendados, procurando comparar estes tratamentos em outros grupos.

Comunidade terapêutica: relato e análise de uma experiência

As a social practice that institutions, the psychiatry and the relationships, resulting from them are is realized for the social and human a privileged field of application OD social and human sciences discoveries and its study will hardly be done by only one point of view. Such stand allowed us to countersign this work under an institutional approaching. Whereas the hospital that was examined as making part of a more extensive context of the society. This study was realized by comparing the inforrnations about the patients and about the attendment team of a psychiatric hospital that ilustrated this work. We considered the first period as the classic therapy one and the second that of the implantation and the expansion of the therapeutical community experience. We also considered this second period like being a more important moment of the therapeutical practices history of that hospital, as it fixes the beginning of a much more different period than the earlier one. This difference is characterized not only by the ideas or even the approaching of psychiatrical problems in general but specifically by the attention paid to the patients, restraining more and more an unilateral treatment of mental disease, not only in theoretical terms but opened up new perspectives in term of psychiatric praxis and besides all its, goals, in face of strengthning of the patient - family -community entailment. The realized comparison allowed us to conclude that within the exposed reports there was no significative differences between the two periods still considering that the psychiatrical practice in public hospitais is organized in accordand with the actual policy of health. / Neste trabalho, procuramos analisar fatores que contribuem para o sucesso ou insucesso de determinadas práticas terapêuticas em instituições psiquiátricas. Como prática social, que se realiza através de instituições, a psiquiatria e as relações sociais e humanas dela decorrentes, são, um campo privilegiado de aplicação de descobertas das ciências sociais e do comportamento e seu estudo dificilmente se fará a partir de uma só perspectiva, que nos possibilita referendar este trabalho sob uma abordagem institucional, considerando o hospital estudado como fazendo parte de um contexto mais amplo da sociedade. A análise foi realizada através da comparação de dados referentes aos pacientes e à equipe de atendimento do hospital psiquiátrico que ilustra este trabalho. Estes se referem a dois períodos diferentes de atendimento psiquiátrico. Consideramos o primeiro período como de terapêutica clássica e o segundo como o da implantação e expansão da experiência de comunidade terapêutica. Colocamos este segundo momento como o mais relevante na história das práticas terapêuticas daquele hospital, posto que marca o início de um período bastante diferente do anterior , tanto no que concerne às ideias e à abordagem dos problemas psiquiátricos em geral, quanto à atenção dispensada aos pacientes especificamente, deixando cada vez menor a margem para um tratamento unilateral da doença mental, não somente em termos teóricos, mas também, em termos práticos em virtude da tentativa de fortalecimento da vinculação paciente/família/comunidade. Entretanto a comparação realizada nos permite concluir que de acordo com os dados apresentados pelo hospital os dois períodos não diferem significativamente no que se referem aos pacientes e a equipe de atendimento. Considerando ainda que a prática terapêutica em hospitais públicos está organizada em função de objetivos da política de saúde vigente.

Eficácia do treinamento assertivo na depressão

However, 'little has been done to evaluate its effectiveness on depression. In this work an attempt was made to show the importance of the technique in alleviating neurotic depression. A non-analogue study of the effects of assertion training on depression, using single-case experimental design strategie, was tried. To a certain extent results supported the effectiveness of treatment. Some considerations were given to recent behavioral approaches concerning depression and a small, historical approach relate to the development of assertive training was presented as well. It was concluded that if the loss of response-contingent reinforcement is the basis for depressed behavior, itis possible to alleviate depression by restoring and increasing the incidence of positive reinforcement using assertive training, even if we subscribe to the idea that lack of control over environmental events is ultimately at the root of depressed behavior. / O treinamento assertivo tem se tornando uma técnica de 'tratamento cada vez mais' popular e tem sido usado com uma ampla variedade de problemas interpessoais. Entretanto, pouco tem sido feito para avaliar-se sua eficácia na depressão. Neste trabalho foi feito uma tentativa para mostrar a importância da técnica em aliviar depressões neur6ticas. Tentou-se um estudo singular dos efeitos do treinamento assertivo na depressão', utilizando a estratégia do delineamento experimental para caso único. Em certa medida os resultados apoiaram a eficácia do tratamento. Algumas considerações foram tecidas com base em abordagens comportamentais recentes, concernentes à depressão, e uma pequena abordagem histórica relacionada ao desenvolvimento do treinamento assertivo, também, foi apresentada. Concluiu-se que se a perda de reforçamento de contingências de resposta é a base do comportamento depressivo, é possível aliviar a depressão restaurando e aumentando a incidência de reforçamentos positivos, utilizando o treinamento assertivo, mesmo se subscrevermos a ideia de que a falta de controle sobre os eventos ambientais esteja: na raiz de todo o comportamento depressivo.

Therefore, this study aimed to evaluate the cognitive performance of AD patients benefit from medication for Assistance Program Holders of Alzheimer's disease (PAPDA) in Picos-PI, as well as verify adherence of users to therapy and characterize the profile of these patients. Cognitive assessment was performed using the Mini Mental State Examination (MMSE), Trail Making Test and Clock Drawing Test (CDT). For statistical analysis we used the chi-square test (Pearson coefficient), considering the significance level at p <0.05. The study included 20 people who were assessed on cognitive tests, adherence to treatment (MAT), through home visits. The sample with respect to socioeconomic status were 60% (n = 12) were female and 40% (n = 8) were male, and 70% (n = 14) of these were married. Also, there were the following average schooling found: approximately 7 ± 4.48 years of study and age 76 ± 7.89 years. All survey participants reported that to renew PAPDA made use of private consultation to specialized medical care. Regarding cognitive assessment measured by the MMSE, 40% (n = 8) had scores between 17 and 24 points, 30% (n = 6) had scores from 9 to 16 points, 20% (n = 4) had scores between 1 8 points and 10% (n = 2) had scores between 25 to 30 points verifying statistical difference between MMSE and age (p = 0.04), and significant clue to MMSE and education (p = 0.053). The tracks test was not completed by any of the participants. TDR showed that 30% (n = 6) did not try or failed to understand the request and 35% (n = 7) understood the request and made circles with numbers not connected. The MMSE and TDR correlated (p = 0.038), with a decrease in MMSE scores found in accordance with the increase of TDR score. Evaluation of adherence measure to treatment (MAT) was answered by caregivers and found that all patients adhere to therapy. From the analyzes, it is worth highlighting the use of benzodiazepines for some seniors. There was no significant difference between MMSE and the start of treatment, even with used cholinesterase inhibitors. It is concluded that patients with AD have cognitive decline with severe attention deficit disorder, motor coordination and reduced visual field and good adherence to treatment, but the rational use of anticolisterasicos seems to have no direct association with disease progression and or start time treatment. / Doença de Alzheimer (DA), patologia crescente entre os idosos, e que limita a autonomia e independência destes, é contemplada com dispensação gratuita de medicamentos pelo Estado. para DA. Diante disso, o presente estudo teve como objetivo avaliar o desempenho cognitivo de pacientes com DA beneficiados com medicamentos pelo Programa de Assistência aos Portadores da Doença de Alzheimer (PAPDA) em Picos-PI, assim como verificar a adesão dos usuários à terapêutica e caracterizar o perfil destes pacientes. A avaliação cognitiva foi realizada com a utilização do Mini Exame do Estado Mental (MEEM), Teste de Trilhas e Teste do Desenho do Relógio (TDR). Para análise estatística dos dados foi utilizado o teste Qui-quadrado (coeficiente de Pearson), considerando o nível de significância a partir de p<0,05. Participaram do estudo 20 pessoas que foram avaliadas em testes cognitivos, adesão ao tratamento (MAT), por meio de visitas domiciliares. A amostra estudada com relação ao perfil socioeconômico foi composta por 60% (n=12) do sexo feminino e 40% (n=8) do sexo masculino, sendo que 70 % (n= 14) destes eram casados. Também, foram encontradas as seguintes médias de escolaridade: aproximadamente 7 ± 4,48 anos de estudo e de idade de 76± 7,89 anos. Todos os participantes da pesquisa informaram que ao renovar o PAPDA fizeram uso de consulta particular para atendimento médico especializado. Com relação a avaliação cognitiva medida pelo MEEM, 40% (n=8) obtiveram escores entre 17 a 24 pontos, 30% (n=6) obtiveram escores entre 9 a 16 pontos, 20% (n=4) obtiveram escores entre 1 a 8 pontos e 10% (n=2) obtiveram escores entre 25 a 30 pontos verificando-se diferença estatística entre MEEM e a idade (p=0,04), e indício de significância para MEEM e escolaridade (p=0,053). O teste de trilhas não foi concluído por nenhum dos participantes. O TDR apresentou que 30 % (n=6) não tentaram ou não conseguiram entender a solicitação, e 35% (n=7) compreenderam o solicitado e fizeram círculos, com números não conectados. O MEEM e o TDR apresentaram correlação (p=0,038), havendo uma diminuição dos escores encontrados MEEM de acordo com o aumento da pontuação TDR. A avaliação da medida de adesão ao tratamento (MAT) foi respondida pelos cuidadores e concluiu-se que todos os pacientes aderiram à terapêutica. Das análises, vale destacar o uso de benzodiazepínicos por alguns idosos. Não houve diferença significativa entre MEEM e o inicio do tratamento, nem mesmo com os anticolinesterasicos usados. Conclui-se que os pacientes com DA apresentam declínio cognitivo com déficit severo de atenção, coordenação motora e campo visual reduzidos e boa adesão ao tratamento, contudo o uso racional dos anticolisterasicos parece não ter associação direta com a progressão da doença e ou tempo de inicio do tratamento.

Paracoccidioidomycosis, an example of systemic fungal disease, has presented between 1 and 3 cases per 100,000 inhabitants in endemic areas. Its main therapeutic regimen is the use of itraconazole associated with antibiotics. However, due to a shortage of effective antifungal drugs on the market and the high cost of available medicines, access to medicines is not viable for the poor. With a view to promoting health, RDC 134/2003 was approved, which requires that bioequivalence tests be carried out between similar medicines and their respective reference in order to ensure the efficacy and safety of the like. The objectives of this study were: to evaluate comparatively the bioavailability of two pharmaceutical formulations of itraconazole in order to determine its bioequivalence; draw a profile between the anthropometric parameters and investigate comparatively factors that may influence the bioavailability of itraconazole in the formulations evaluated. The study was carried out with 59 healthy male volunteers and consisted of an open, randomized, cross-over study with 2 treatments, 2 periods (two sequences) with a minimum interval of seven days between hospitalizations. Blood samples were collected at regular intervals to construct the plasma concentration versus time curves. Itraconazole concentrations were analyzed by High Efficiency Liquid Chromatography coupled to Mass Spectrometry (LC-MS / MS). The geometric mean Itraconazole test / Itraconazole reference and the 90% confidence interval (CI) for Cmax and ASC0-last were 82.79% (76.27-89.88%) and 59.73% (54.40-65 , 60%). Considering that the formulations studied were not within the ratio between C max and ASC0-last between 80% and 125%, and considering a 90% confidence interval, as proposed by the Food and Drug Administration and National Surveillance Agency It is concluded that the two formulations of itraconazole did not present similar bioavailabilities both in rate and extent of absorption and therefore are not bioequivalent. / Infecções fúngicas, especialmente sistêmicas, estão aumentando progressivamente no Brasil. A paracoccidioidomicose, um exemplo de doença fúngica sistêmica, tem apresentado entre 1 a 3 casos por 100 mil habitantes em áreas endêmicas.Seu principal esquema terapêutico é o uso de itraconazol associado a antibióticos. No entanto, devido a escassez de medicamentos antifúngicos similares eficazes no mercado e ao alto custo dos medicamentos disponíveis, o acesso medicamentoso torna-se inviável por parte da população carente. Visando a promoção da saúde, foi aprovada a RDC 134/2003 que exige a realização de testes de bioequivalência entre medicamentos similares e seu respectivo referência a fim de assegurar a eficácia e segurança dos similares. Os objetivos deste trabalho foram: avaliar comparativamente a biodisponibilidade de duas apresentações farmacêuticas de itraconazol a fim de determinar sua bioequivalência; traçar um perfil entre os parâmetros antropométricos e investigar comparativamente fatores que possam influenciar na biodisponibilidade do itraconazol nas formulações avaliadas. A pesquisa foi realizada com 59 voluntários sadios do sexo masculino e consistiu de um estudo aberto, randomizado, cruzado, com 2 tratamentos, 2 períodos (duas sequências) com intervalo mínimo de sete dias entre os internamentos. Amostras de sangue foram coletadas em intervalos regulares para construção das curvas de concentração plasmática versus tempo. As concentrações de itraconazol foram analisadas por Cromatografia Líquida de Alta Eficiência acoplada à Espectrometria de Massa (LC-MS/MS). A média geométrica do Itraconazol teste/ Itraconazol referência e o intervalo de confiança (IC) de 90% para Cmax e ASC0-túltimo foram 82,79% (76.27-89,88%) e 59,73% (54,40-65,60%). Considerando que as formulações estudadas não apresentaram-se dentro da razão entre as médias de Cmax e ASC0-túltimo entre 80% e 125%, e considerando um intervalo de confiança de 90%, como proposto pelo Food and Drug Administration e Agência Nacional de Vigilância Sanitária, conclui-se que as duas formulações de itraconazol não apresentaram biodisponibilidades semelhantes tanto na taxa quanto na extensão de absorção e, portanto, não são bioequivalentes.

Segurança da terapia antirretroviral para a infecção pelo vírus HIV-1 em ensaios clínicos randomizados envolvendo pacientes multiexperimentados utilizando terapia otimizada de base : uma revisão sistemática

Introdução: O desenvolvimento de antirretrovirais (AVRs) para o vírus HIV em pacientes multiexperimentados é prioridade em saúde pública e, além de supressão virológica máxima, recuperação e estabilização imunológica, os ensaios clínicos randomizados (ECRs) que envolvem esquemas de antirretrovirais utilizados em terapias de resgate devem ser capazes de demonstrar segurança na sua utilização, a curto e longo prazos. Objetivo: Realizar revisão sistemática e caracterizar os achados de segurança da terapia antirretroviral (TARV) para a infecção pelo vírus HIV-1 nos ECRs que utilizaram esquemas otimizados de base (EOB) em pacientes multiexperimentados através, principalmente, da avaliação da qualidade da informação sobre riscos baseada na extensão do CONSORT sobre riscos e da avaliação da apresentação de achados laboratoriais que são proxy de desfechos conhecidamente relevantes. Metodologia: Foram revisadas as bases de dados MEDLINE, EMBASE, LILACS, Colaboração Cochrane, SCOPUS e ISI Web of Science visando identificar publicações entre janeiro/2003 e agosto/2014. Também foram revisadas as bases de dados dos seguintes congressos científicos internacionais: International AIDS Conference; Conference on Retroviruses and Opportunistic Infections (CROI); Interscience Conference on Antimicrobial Agents and Chemotherapy (ICAAC); e International Congress on Drug Therapy in HIV Infection. Não foram feitas restrições em relação à língua da publicação dos estudos. Os critérios para inclusão dos estudos foram: ECRs com pelo menos 16 semanas de duração e com pelo menos 100 indivíduos que avaliaram segurança da terapia antirretroviral (TARV) em pacientes com infecção pelo HIV- 1 caracterizados como multiexperimentados. Resultados: Vinte e quatro ECRs foram incluídos, sendo 16 estudos originais e 8 extensões. Vinte e três (95,8%) estudos foram exclusivamente financiados pela indústria farmacêutica. Apenas 15 (62,5%) estudos apresentaram eventos adversos (EAs) clínicos graves. Ocorrência de limiar para a apresentação de resultados foi observada em 13 (86,7%) estudos que apresentaram EAs graves, 22 (95,6 %) estudos que apresentaram EAs considerados comuns e em 8 (42,1%) estudos que apresentaram informações sobre desfechos laboratoriais. Seis (25%) ECRs não apresentaram informações sobre os desfechos laboratoriais investigados. Oito (33,3%) estudos não apresentaram informações sobre triglicerídeos, colesterol total, LDL (low density lipoprotein) e hiperglicemia, resultando em ausência de informação sobre os referidos desfechos para os medicamentos maraviroque (MVC) e vincriviroc (VIC). Nove estudos não apresentaram informações sobre marcadores hepáticos, resultando em ausência de informação destes desfechos para o medicamento enfuvirtida (ENF). Dezoito (75%) estudos não apresentaram informações sobre creatinina, resultando em ausência de informação sobre este desfecho para os medicamentos ENF, tipranavir (TPV) e darunavir (DRV). Conclusões: A informação sobre riscos em ECRs envolvendo TARV para pacientes multiexperimentados é altamente seletiva e insuficiente. Além disso, desfechos conhecidamente relevantes não tem sido incluídos a priori nestes estudos. Os ECRs que avaliam novos ARVs ainda apresentam as características dos estudos do início da era TARV, quando, diante de um cenário com grande mortalidade associada ao HIV, o principal objetivo era preservar a vida dos pacientes, em detrimento das questões de segurança da terapia. / Introduction: The development of antiretroviral drugs (ARVs) for treating the HIV virus in treatment-experienced patients is a priority in public health. In addition to virological suppression, recovery and stabilization of the immune system, randomized clinical trials (RCTs) involving antiretroviral regimens used in recovery therapies must be able to demonstrate short and long term safety in their use. Objective: To perform a systematic review and to characterize the findings regarding the safety of cART for HIV-1 virus infections in RCTs that used optimized background regimen in treatmentexperienced patients primarily through assessing the quality of information based on the extent of the CONSORT on risks and evaluating the overall presentation of laboratory findings that are known to proxy relevant outcomes. Methodology: The following databases were researched: MEDLINE, EMBASE, LILACS, The Cochrane Collaboration, SCOPUS and ISI Web of Science. We searched for works published between January/2003 and August/2014. The databases of the following international scientific conferences were also researched: International AIDS Conference; Conference on Retroviruses and Opportunistic Infections (CROI); Interscience Conference on Antimicrobial Agents and Chemotherapy (ICAAC); and International Congress on Drug Therapy in HIV Infection. There were no restrictions regarding the language of the studies. The criteria for inclusion were the following: RCTs that lasted at least 16 weeks and with at least 100 participants who evaluated the safety of antiretroviral therapies (cART) in treatment-experienced patients with HIV-1. Results: Twenty-four RCTs were included, sixteen original studies and eight extensions. Twenty-three (95.8%) of the studies were funded exclusively by the pharmaceutical industry. Only fifteen (62.5%) of the studies showed serious clinical adverse events (AEs). The occurrence threshold for the presentation of results was observed in thirteen (86.7%) of the studies that showed serious AEs, twenty-two (95.6%) of the studies that showed AEs considered to be common, and eight (42.1%) of the studies which presented information on laboratory outcomes. Six (25%) RCTs have not provided information on laboratory outcomes investigated. Eight (33.3%) studies did not provide information on triglycerides, total cholesterol, LDL (low density lipoprotein) and hyperglycemia, resulting in lack of information on these outcomes for the drugs maraviroc (MVC) and vicriviroc (VIC). Nine studies did not provide information on hepatic markers, resulting in lack of information on these outcomes for the drug enfuvirtide (ENF). Eighteen (75%) studies did not provide information on creatinine, resulting in lack of information on this outcome for the drugs ENF, tipranavir (TPV) and darunavir (DRV). Conclusions: Information on the risks of RCTs involving cART for treatment-experienced patients is highly selective and insufficient. Moreover, known relevant outcomes have not been included a priori in these studies. RCTs evaluating new ARVs have the same characteristics of studies of early ARVs, when, faced with a scenario of high mortality associated with HIV, the main goal was to preserve the lives of patients, at the expense of the safety in therapy.

Thus, it is necessary to develop research efforts that result in actual contributions to the clinical practice of nurse assistants. So, we chose to perform this integrative review, one of the methods of Evidence-Based Practice that aims to incorporate the evidence found in the literature to assistance and/or management practice. Therefore, this research aimed to analyze the evidence available in the literature on the nursing interventions applied to the PU treatment in bedridden patients. For articles selection we used five databases (CINAHAL, Scopus, Cochrane, LILACS and PubMed) and Estima Journal. The sample of this review was composed of 27 articles related to interventions of general purpose used in PU treatment. After analyzing the articles included, the results showed that electromagnetic therapy has evidence of its benefit for healing of PU; however there are no guidelines for its applicability. This has also been described for Therapeutic ultrasound and VAC therapy. Regarding the wound cleaning process, they described the use of many substances, such as aloe vera, tap water and saline solution. They also approached the use of hydro massage. We did not find any evidence that among these substances one stood out more than others. Regarding the food issue, we verified that patients who receive supplemental protein, zinc and vitamin C in their diets tend to heal the PU in a shorter period. By dealing with the etiology of PU, we focused on repositioning and support surfaces (SS). It became evident that the patients should be repositioned every two hours when lying down and every 15 minutes when sitting, as a way of minimizing the pressure and shear forces. However, the evidence for this care has limitations that don not allow us to conclude on the real pressure relief it causes. Regarding the method of PU measurement, a single study composed the sample of this dissertation, so it only described the application of the Pressure Ulcer Scale for Healing (PUSH) as a way of documenting PU. Another crucial point was the approach to knowledge of nurses and care techniques employed by them when treating PU. We noticed the prevalence of lack of information among them that starts at graduation. Given this situation, we reflect on how PU is being treated, and thus we analyzed the evidence used against pain, odor and exudate. As for the pain, we verified the recommendation of benzydamine gel and EMLA cream, especially before performing procedures such as debridement. As for the odor, we did not find any study describing the best way to minimize it. As for the amount of exudate, it is closely related to the healing period of PU. Regarding the coverage itself, the studies selected approached the hydrocolloid, aloe vera, normothermic wound therapy and collagen. The coverage were approached in PU with different healing stages, and both studies presented appropriate cure rate when describing the use of each one of them. However, there were no evidences to prove the superiority of any of these therapies. Thus, we reinforce the need for the continuous pursuit of knowledge made by nurses, as well as the importance of the overall assessment of patients who are submitted to their care, given that the actual contributions to the PU treatment practice are still limited. Therefore, there is urgent need to develop clinical trials to elucidate many of the answers that are not yet explicit. / Com o aumento da expectativa de vida populacional, tem-se a úlcera por pressão (UP) como um dos agravos com elevada incidência. Assim, esforços para o desenvolvimento de pesquisas que resultem em contribuições reais para a prática clínica dos enfermeiros assistenciais são necessários. Por isso, optou-se em realizar esta revisão integrativa, um dos métodos da Prática Baseada em Evidências que tem por objetivo incorporar as evidências encontradas na literatura à prática assistencial e/ou gerencial. Para isso, a presente investigação objetivou analisar as evidências disponíveis na literatura sobre as intervenções de enfermagem aplicadas para o tratamento das UP em pacientes acamados. Para seleção dos artigos, foram utilizadas cinco bases de dados (CINAHAL, SCOPUS, COCHRANE, LILACS e PUBMED) além da revista ESTIMA. A amostra dessa revisão foi composta por 27 artigos, referentes a intervenções, de âmbito geral, utilizadas para tratar UP. Após análise dos artigos incluídos, os resultados apontaram que a eletromagnético-terapia possui evidências do seu beneficio para cicatrização das UP, porém não existem diretrizes sobre sua aplicabilidade. Isto também foi descrito para o Ultrassom terapêutico e a V. A. C. terapia. Quanto ao processo de limpeza de ferida, foram descritas utilização de várias substâncias, dentre essas, não foi encontrada evidência de que uma se sobressaia em relação à outra. Na questão alimentar, ficou evidente que os pacientes que recebem suplementação de proteínas, zinco e vitamina C em suas dietas, tendem a cicatrizar a UP em um menor período. Ao abordar a etiliologia das UP, enfocou-se o reposiciosamento e as superfícies de suporte (SS). Ficou patente que os pacientes devem ser reposicionados a cada duas horas, quando deitados, e a cada 15 minutos, quando sentados, como forma de minimizar as forças de pressão e cisalhamento. No entanto, a evidência referente a esse cuidado possui limitações que impedem concluir quanto ao real alívio de pressão que causa. Com relação à forma de mensurar a UP, um único estudo fez parte da amostra desta dissertação, de modo que este apenas descreveu a aplicação da Escala de PUSH como forma de documentar a UP. Outro ponto crucial foi a abordagem quanto ao conhecimento dos enfermeiros e as técnicas de cuidados empregadas por eles ao tratar UP. Percebeu-se que prevalece a desinformação destes em decorrênca do desconhecimento que inicia na graduação. Diante desse quadro, pensa-se em como as UP estão sendo tratadas e para isso analisaram-se as evidências empregadas diante da dor, do odor e do exsudato. Quanto à dor, ficou clara a recomendação do gel de benzidamina e a pomada EMLA, principalmente antes da realização procedimentos, como desbridamento. Quanto ao odor, não foram encontrados estudos que descrevessem a melhor forma de minimizá-lo. Para a quantidade de exsudato, este está intimamente relacionado ao período de cicatrização que a UP encontra-se. Em se tratando das coberturas propriamente ditas, os estudos selecionados abordaram o hidrocoloide, a aloe vera, terapia normotermica e o colágeno. Estas coberturas foram abordadas em UP com diferentes estágios de cicatrização e ambos os estudos apresentaram taxa de cura adequada com ao descrever o uso de cada uma delas. Entretanto, não foram identificadas evidências que comprovem a superioridade de uma destas terapias. Logo, reforça-se a necessidade da busca de conhecimento contínuo por parte dos enfermeiros, bem como a importância da avaliação global dos pacientes que são submetidos a seus cuidados, haja vista que as contribuições reais para prática de tratamento de UP ainda são limitadas. Com isso, urgem que sejam desenvolvidos ensaios clínicos para elucidar muitas das respostas que ainda não estão explícitas.
