Prävalenz und klinische Bedeutung des koronaren Linksversorgungstyps bei Patienten mit valvulärer Aortenklappenstenose

In der Literatur wird bei kleineren Patientenkohorten über eine erhöhte Prävalenz des linkskoronaren Versorgungstyps bei Patienten mit valvulärer Aortenklappenstenose berichet. Es wird auch postuliert, dass dies nur Patienten mit Aortenklappenstenose und bikuspider Aortenklappe betrifft. Die funktionelle Konsequenz der Kombination aus einem linksdominanten koronaren Versorgungstyp und valvulärer Aortenklappenstenose ist bisher nicht klar. Ziel der vorliegenden Arbeit war es, die Prävalenz des Linksversorgungstyps in einer großen Kohorte von Patienten mit diagnostizierter relevanter Aortenklappenstenose im Vergleich zu einer Kontrollkohorte ohne Aortenklappenstenose zu analysieren. Es galt ferner zu prüfen, ob sich eine etwaig höhere Prävalenz des koronaren Linksversorgungstyps auf Patienten mit bikuspider Aortenklappe beschränkt, ob der koronare Linksversorgungstyp ein unabhängiger Risikofaktor für die Progression einer Aortenklappenstenose ist und ob die koronare Linksdominanz Relevanz für das postoperative beziehungsweise postinterventionelle Outcome nach Aortenklappenersatz hat. Über eine Datenbankabfrage wurden alle Patienten identifiziert, die vom 01.01.2007 bis 31.12.2008 in der Klinik für Innere Medizin und Kardiologie im Herzzentrum der Universität Leipzig aufgenommen wurden. Zwei Subkohorten wurden extrahiert. Kohorte 1 umfasste alle Patienten mit einer bekannten oder neu diagnostizierten, symptomatischen und isolierten Stenose der nativen Aortenklappe. Kohorte 2 umfasste alle Patienten, welche einer invasiven Koronarangiographie unterzogen wurden und keine beginnende oder fortgeschrittene Aortenklappenstenose zeigten sowie ≥70 Jahre waren. Kohorte 1 umfasste 721, Kohorte 2 6990 Patienten. Der linkskoronare Versorgungstyp zeigte sich bei Patienten mit Aortenklappenstenose signifikant häufiger als bei Kontrollpatienten ohne Aortenklappenstenose (13.2% versus 10.2%, p=0.003). Eine signifikant niedrigere Prävalenz des rechtskoronaren Versorgungstypen fand sich bei Patienten mit einer Aortenklappenstenose (66.9% versus 71.6%, p=0.008). Beide Prävalenzen waren unabhängig vom Vorliegen einer bi- oder trikuspiden Klappenarchitektur. Es bestand lediglich ein geringer Trend zu einer höheren Prävalenz der linkskoronaren Dominanz bei Patienten mit bikuspider Klappe im Vergleich zu Patienten mit trikuspider Klappe (15 [20.8%] versus 60 [13.9%], p=0.12). Keine Unterschiede zeigten sich indessen für die intrahospitale Mortalität und Auftreten eines akuten Myokardinfarktes zwischen den koronaren Versorgungstypen. Die linkskoronare Dominanz ist kein Risikofaktor für eine Progression der Aortenklappenstenose.

Left-behind Children in China : A qualitative study about the experience of left-behind children concerning their childhood

The overall aim of the thesis is to explore the negative life experience of left-behind children in a dynamic and integrated perspective taking left-behind children, their guardians, parents, school and other social communities into consideration. It explores the current situation of some left-behind children in China, and discusses the causes and negative effects on children‟s comprehensive development. A qualitative research method has been used because of the in-depth outcome accessible through face-to-face interview. Three left-behind children in different family compositions and one teacher were interviewed. The outcome of this study is discussed in multiple dimensions to explore consequences of left-behind children in China. The results suggest that parents’ migration can lead to more serious problems for the children than expected. The management of left-behind children requires concerted efforts from various social systems, not only parents, schools, children welfare center, and the whole nation, otherwise it would worsen deteriorated social issues.

The Impact of Georgia's Accountability System on School Performance and Subgroup Populations

This dissertation examines the impact of Georgia’s accountability system on both school and student performance. We focus on two components within Georgia’s accountability system – the sanctioning of failing schools and binding subgroup requirements. Schools within Georgia become subject to sanctions upon two consecutive years of failing to show Adequate Yearly Progress (AYP). The subgroup binding requirements, introduced by the No Child Left Behind (NCLB) Act, hold schools independently responsible for the performance of given subgroups contingent upon enrollment. The first question of this dissertation examines the factors that influence a school’s ability to meet assessment standards. We examine the relative importance of school characteristics, as they relate to accountability components, in determining AYP in practice. A binary response model is used as AYP is determined on a pass/fail basis. More specifically, we apply a correlated random effects probit model with a Chamberlain-Mundlak adjustment. The second question of this dissertation examines the impact of binding requirements on subgroup performance, where subgroup performance is defined as the percentage of students scoring at or above proficiency. We employ a regression discontinuity design that compares the performance of bounded and unbounded subgroups to determine the treatment effect. Each question of this dissertation is addressed through evaluating both mean and distributional effects. We find that imposing sanctions on failing schools has a positive impact on future performance. However, increasing the number of binding requirements has a negative impact on a school’s probability of passage. This result suggests that heterogeneous schools, or schools with several large subgroup populations, are negatively impacted by the requirement. While we find that accountability components have a statistically significant impact on probability of AYP passage, factors related to school resources and quality appear to have a greater influence. The mechanism for the negative impact of binding requirements remains unidentified as we also find that binding requirements have a slight positive impact on individual subgroup performance. The magnitude of this impact is dependent upon the subgroup examined, school type, and position of the subgroup within the Meets/Exceeds distribution. Overall, our results suggest the need for re-examination of the binding requirements as a method of targeting disadvantaged populations.

The left hand has always been perceived as the weaker of the two hands and since about 1840, a substantial amount of piano repertoire for the left hand has seen the light. Composers continue to show an interest in composing works in this genre and the reasons are mainly because of injuries to the right hand, to aid in the pianist's technical development, because of the challenge it poses to the composer and finally to delight audiences with an unique technical display. The technical challenges that are present in these works, differ greatly from the difficulties found in piano music for both hands. In order to give a commanding performance, the pianist is required to make certain adjustments. In music for the left hand alone, this hand is suddenly solely responsible for both the melodic lines and accompanying textures simultaneously. By addressing the technical difficulties of a composition, the interpretation of the work is invariably influenced. One of the most well-known works in this genre, A. Scriabin's Prelude for the left hand, Op. 9, no. 1, has been used to identify the technical aspects in piano music for the left hand. The question that arises, is: which specific technical aspects can be identified in A. Scriabin's Prelude for the left hand, Op. 9, no. 1? The following questions are subsequently raised: firstly, how will the pianist approach these technical aspects, and secondly, how will certain technical adjustments benefit the performance and interpretation of the work? The hypothesis holds that the pianist will have to identify and address the unique technical aspects of piano works for the left hand alone, in order to perform these works successfully. The popular politics of the poll tax : an active citizenship of the left?

The Community Charge (poll tax) was seen by both its supporters and opponents alike as an attempt to promote the British New Right's concept of responsible active citizenship in local politics. The reaction of different groups of citizens to the tax is explored through a detailed case study of events in the London Borough of Ealing, an archetypal London suburb. Here, as in most urban areas, organised anti-poll tax protestors clashed with MPs, councillors and the local magistracy, who played a large role in enforcing the measure. It shows how the protestors attempted to mobilise a 'moral community' built around the idea of 'fair' taxation and promote a campaign of civil disobedience to force abolition. This in turn compelled local actors to make principled choices about the enforcement of a law of which many of them strongly disapproved. The protestors' tactics seemed to strike a popular chord and at least a fifth of all Ealing charge-payers (and eight million people nationally) failed to pay the tax in 1990/91. However, the detailed evidence also suggests that non-payment can best be seen as a mass expression of bloody mindedness, rather than a concerted and organised campaign of civil disobedience. Nevertheless the protests had important implications for the practise of left-wing citizenship in contemporary Britain and served to highlight growing divisions between the mainstream and radical Left. Previously published academic accounts have addressed the 'high' politics of the poll tax. The thesis explores instead the 'popular' politics of the poll tax crisis in a suitably local setting and so redresses an imbalance in the literature. It therefore makes an original contribution to knowledge and understanding of the relationship between conventional means of political participation, radical popular protest movements and competing concepts of citizenship.

From old socialists to new democrats : the realignment of the Japanese left

In 1996, a new left of centre party emerged in Japan called the Democratic Party of Japan (DPJ) and effectively replaced the Japan Socialist Party (JSP) as the main opposition party. This thesis asks what conditions caused this realignment and how the DPJ differs from the JSP. An increasing distrust and disinterest of politics and politicians has meant that the non-aligned voter in Japan forms the largest group of the electorate. Every party has lost support, but the left faced the worst drop of support. With the end of the Cold War, and the intensifying call for Japan to reassess its role on the World stage, the traditional ideology of the Japanese left, which has become synonymous with peace and preservation of the Peace Constitution, has lost its stabilising effect on the party and on its supporters. The labour unions, which were once the key mobilisational force for the left-wing parties at election time, began to question their relationship with the JSP and found new links to government. Simultaneously, they were also losing members so mobilisation of voters for the left also declined. Finally, a new electoral system did not reward the opposition as much as the LDP. Overall, the mobilisation of the electorate has become increasingly difficult for the Japanese left as a result of these factors. The DPJ has had to find ways of dealing with them and also has had to create its own identity. The way in which the party has dealt with this is by 'widening out' its types of candidate and using new methods to attract support. Furthermore, the DPJ has become more aware of its party coherence and has ensured that party unity is maintained even when ideological disputes occur.

The politics of the Chilean right from the Popular Front to 1964

The right in Chile has been crucial to the democratic politics of the country ever since the development of the party system. In the period covered by this thesis (1939- 1964) the Chilean right faced the combined challenge of reformist governments and the emergence of mass political parties on the left. This thesis describes the Chilean right as being composed by political parties, entrepreneurial associations and a leading newspaper, El Mercurio. which represented and expressed the interests, thoughts and common perspectives of the right as a whole. This thesis argues that the right in Chile was able not only to survive the reformist governments of the 1940s, but that it remained very influential politically and as an economic force. The strategies it successfully carried out were those of negotiation and co-optation, which rested in the right's congressional power. With the advent of populism at the beginning of the 1950s, the argument goes, the right faced new threats, especially the discredit of political parties and of Congress conceived as the arena par excellence for negotiation and compromise. The danger came both from within the right itself, threatening to divide it, and from populist governments, which put at risk the right's sources of influence. Thus, the rightist sectors had to develop new political strategies, which in essence pointed towards the formulation of a project of capitalist modernisation centered in private enterprise and free markets. With the election of Jorge Alessandri --an entrepreneurial figure-- as President of the Republic, the right had the opportunity to put into practice its novel economic ideas. In the long run, though, the results were negative. The thesis analises the political dimension of this failure. Once its project collapsed, the right became ideologically marginal, just at a time when Chilean politics had become strongly ideological as well as reformist if not revolutionary. This explains why the leaders of the long established parties of the right, the Conservatives and Liberals, dissolved themselves in 1965. Finally this dissertation speculates on the right at the present time looking for elements of continuity and change between today's right and the one studied for the 1939-1964 period.

Assessment of ventricular morphology using echocardiography in Ornate tinamous (Nothoprocta ornata) and domestic chickens (Gallus domesticus)

The Ornate Tinamou (Nothoprocta ornata), an ancient bird, has adapted to life at high altitude (>2.400 m.a.s.l) for a longer period than the domestic chicken (Gallus domesticus), which came to South America with the Spanish conquerors. Ornate tinamous have a smaller heart in relation to body size than domestic chickens. This study was made to evaluate heart morphometric measurements comparing Ornate Tinamou and domestic chicken using echocardiography measurements to determine wall thickness and chamber size and to evaluate whether it can retrieve measurements consistent with previous results on dissected hearts. I was also interested in evaluating potential adaptations of the Ornate Tinamou to life in hypoxic environments by exposing the heart to positive inotropic stimulation. The results were compared with those previously obtained on dissected hearts. The results showed that the chamber size of the domestic chicken was significantly larger than in Ornate Tinamou, both in conscious and anesthetized birds. Injection of 1µg/kg isoproterenol caused domestic chickens’ systolic chamber size to decrease significantly and fractional shortening to increase significantly. The same changes were seen in the Ornate Tinamou but they were not significant. In conclusion, this study confirms that echocardiography is a valid method for retrieving cardiac measurements without euthanizing animals, opening for the possibility of taking several measurements at different ages.
