The Influence of Self-Monitoring on Return Rate Following Intake at a Child Guidance Clinic

Research has yet to identify any characteristics of clients, therapists, or treatment dyads which consistently identify those clients most likely to drop out of treatment. A frame of reference which may prove useful in identifying such clients is the social psychological construct of selfmonitoring. This theory proposes that individuals involved in any social encounter differ from each other in their approach to constructing a relevant self-presentation. High self-monitors emphasize matching their behavior to situational cues while low self-monitors match their behavior to perceived internal values and traits. The present study demonstrates the effects that selfmonitoring styles of therapists and clients have on the effectiveness of a therapeutic intake interview and the client's decision whether or not to return for treatment. Additionally examined are the effects of therapist selfmonitoring style on theoretical orientations toward psychotherapy. The hypothesis that pairings of high self-monitors would be most effective is tested by Chi-square and found to be nonsignificant. Using the Chi-square test, low self-monitoring therapists are found to endorse a single approach to therapy and to strongly endorse the psychoanalytical orientation. Low self-monitors are found to be eclectic in approach. Satisfaction with the interview is examined using ANOVA. Results are nonsignificant with the exception that low self-monitoring therapists are more satisfied with the intake interview than are high selfmonitoring therapists. Finally, within-cell Pearson correlations are examined to measure agreement about satisfaction between therapist and client. Pairs of high self-monitors show the highest rate of agreement. Implications for further research in this area are discussed.

Here SD is defined as the act of the therapist to disclose verbally something about him or her self during the session. The literature presents controversial positions concerning the use of SD as a therapeutic intervention. At the one hand there are practices that promote it enthusiastically. On the other, we observe strong opposition. The aim of the present study is to seek support for the use or not of SD and to investigate the therapist s goals, as well as his or her vision concerning the impact of SD on him or herself, on the client and on the process. A qualitative, exploratory method is used including 10 therapists with behavioral training, and who work from a contextualist point of view. The data collection occurred through semi-structured interviews, recorded and transcribed verbatim for analysis. A Grounded theory approach was used, which consists of an inductive process of analysis, organization, categorization and interpretation with the aim to elaborate a theory that explains contexts. The results suggest that SD is frequently used, mainly by therapists whose training includes contact with the ideas of the third wave . The use of SD to intensify the therapist-client relationship, to demonstrate comprehension, and to offer models was observed to be quite general. A sub-group considered that SD does not have to be genuine to be valid. It is suggest that experience and training may be variables that influence the use of AR. / No relacionamento terapêutico, aqui considerado como um encontro de subjetividades no contexto interpessoal, não é raro a Auto-Revelação (AR) pelo terapeuta frente ao seu cliente, doravante definida como o ato do terapeuta de revelar verbalmente algo de si durante a sessão. A literatura traz posições controversas sobre a utilização de AR como intervenção terapêutica. De um lado existem práticas que a promovem de forma entusiasta, e do outro, observa forte oposição. O objetivo do presente estudo é buscar amparo para utilização ou não da auto-revelação e investigar a intenção, assim como a visão do terapeuta do impacto da AR sobre si, sobre o cliente e sobre o processo. Trata-se de um estudo qualitativo e de caráter exploratório com 10 terapeutas de formação behaviorista e que atuam dentro de uma visão contextualista. Para a coleta de dados foram realizadas entrevistas simi-estruturadas, que foram gravadas e posteriormente transcritas para efeito de análise. A abordagem utilizada para análise dos dados, foi a Grounded Theory que consiste num processo indutivo de análise, organização, categorização e interpretação no intuito de elaborar uma teoria que explique o contexto. Os resultados indicam, que AR é freqüentemente usada, principalmente por terapeutas cuja formação inclui exposição às idéias da terceira onda da terapia comportamental. O uso de AR para intensificar o relacionamento terapêutico, para evidenciar compreensão, normalizar experiência do cliente e para oferecer modelos foi constatado ser bem geral. Um sub-grupo considera que AR não precisa ser genuína para ser válida. Sugere que a experiência e a formação podem ser variáveis que influenciam o uso de AR.

Intimidade no relacionamento terapeuta-cliente: alcances e limites

The occurrence of questions or comments by the clients that make the therapist talk about particularities of his or her life: The effects these have on the therapist and the therapist-client relationship, what variables control the therapist s behavior of responding to these questions, or, summing up: possibilities for coping and clinical intervention. The study uses a qualitative, explanatory approach, using semi-structured interviews for data-collection. The interviews were transcribed for analysis according to the Grounded Theory model, an inductive strategy that consists in organizing, categorizing and interpreting data that lead to the construction of a theory that explains the studied phenomenon. The results indicate that therapists make intentional use of emerging clinical situations, like questions by the therapist about his or her private life. The reaction of the therapist to questions about his or her private life can, depending on the therapist, be converted in interventions and make the relationship more intimate; and this may be directly linked to the professional repertoire the therapist has in coherence with what is proposed by treatment models like Functional Analytical Psychotherapy (FAP) and Therapy through the Contingencies of Reinforcement (TCR). FAP facilitates the comprehension and classifications of behaviors emitted by the client as clinically relevant behaviors and promotes the acceptance by the therapist of the byproducts of the interaction, not evaluating them as negative, but as opportunities in the session for the development of the relationship. TCR prioritizes the consciousness both therapist and client have, concerning the contingencies that are operating in the different contexts of the client s life, and emphasizes the importance of the therapist to be able to describe the adopted strategies. / Na literatura concernente ao relacionamento terapêutico há considerações importantes sobre o papel central da intimidade, apesar de não serem investigados os determinantes dos comportamentos íntimos dos terapeutas. Buscou-se investigar nessa pesquisa a ocorrência, na sessão, de perguntas ou colocações dos clientes que levam o terapeuta a falar de particularidades de sua vida, o efeito que elas têm sobre o terapeuta e a relação terapêutica, o que controla o comportamento do terapeuta de responder a essas questões (se é considerada a relevância clínica), possíveis reações (espontâneas ou planejadas) que podem reforçar ou enfraquecer esses comportamentos, enfim, possibilidades de manejo e atuação clínica diante do fenômeno intimidade. Trata-se de uma pesquisa qualitativa e de caráter exploratório cuja coleta de dados se deu através de entrevistas semi-estruturadas, feitas com oito terapeutas comportamentais. Para efeito de análise, as entrevistas foram gravadas e depois transcritas. Os dados foram analisados segundo os princípios da Grounded Theory. Os resultados encontrados demonstraram que os terapeutas comportamentais têm seus estilos próprios de prevenir, manejar e aproveitar situações que envolvem intimidade no relacionamento com o cliente e, no geral, os terapeutas que falam sobre aspectos da sua vida (emitem comportamentos íntimos) o fazem com intenção terapêutica. Verificou-se que a intimidade pode tornar a relação mais intensa, mas é a consciência do terapeuta das variáveis envolvidas no processo que converte o que ocorre entre ele e o cliente numa intervenção. Observa-se coerência entre o que é proposto pelos modelos de terapia apresentados - Psicoterapia Analítica Funcional (FAP) e Terapia por Contingências de Reforçamento (TCR) - e o que os terapeutas entrevistados na sua prática clínica fazem. Porém, não se pode negar a influência direta de outras variáveis, como características pessoais do terapeuta. A atuação do profissional está estreitamente ligada com seu estilo. Estes resultados apontam para a importância de se analisar repertórios pessoais e profissionais do clínico no estudo do relacionamento terapêutico.
