A Probe through Empirical Study into the Quality of Medical Treatment Services of Public Hospitals in Penghu County¡XAn Utilization of the PZB Model

The Department of Health executed the contract with Tri-Service General Hospital on January 2, 2003, consigning the latter to render Penghu medical treatment services with the newly built ¡§Penghu Medical Care Building of Department of Health¡¨. Under the consignment, the Tri-Service General Hospital will launch an overall integration of the medical care resources of the current Penghu Hospital of Armed Forces and Penghu Hospital of Department of Health. Starting from July 1, 2006, Tri-Service General Hospital, Penghu Branch has officially come into being, bringing in human resources of the Tri-Service General Hospital, creating the medical treatment facitities in Penghu area easily accessible to upgrade the professional medical treatment services in Pengh area, enabling the local community people of Penghu area to enjoy diverse local welfare in medical services. In Penghu County, with limited medical care resources, other than the efforts to help patients release from suffering and reduce the rate of patients being arranged for medical treatment beyond Penghu area, the efforts to upgrade the quality of medical treatment services have gradually become the major goals in the consigned management. The present study aims at the concept mode of ¡§Service Quality Model, PZB¡¨ offered by Parasuraman, Zeithaml&Berry(1988) as the very ground, taking the two local service oriented public hospitals, i.e., ¡§Penghu Branch of Tri-Service General Hospital¡¨ and ¡§Penghu Hospital of Department of Health¡¨ as the target of survey, to probe into the levels of satisfaction on hospital quality of services felt by the hospital department heads, medical care service crew, patients and patients¡¦ families. The study further analizes to make sure whether the hospital department heads, medical care service crew, patients and patients¡¦ families show difference in the level of satisfaction. As indicated by the findings yielded by the present study, the two public hospitals show shortfalls in quality of services which call for immediate improvement. Penghu Hospital of Department of Health takes advantage of the geographical benefits, easy access to transportation, the close connection with the local patients and public praise and should team closely up with Penghu Branch of Tri-Service General Hospital. Penghu Branch of Tri-Service General Hospital attempts to localize the three messions in medical teratment, education and research. The doctors of the two hospitals should participate in various seminars to co-share the resources, strengthen the professional know-how to upgrade the quality of medical treatment services. The management administrator should make the department heads and medical service crew feel of how important the quality of services is. The present study may help better satisfy the needs of patients and their familities in the perspective of a consumer. The upgrade of quality of medical treatment services is a pivotal part of the public affairs on offshore island areas. The administrator should offer prominent systematic strategies in systematic services to take firm command of the quality of services and technological advantages. Only through such efforts can those hospitals create sound image and public praise in medical treatment in Penghu area to assure sustained survival and existence. On the grounds of the results yielded by the present study, the thesis, finally offers proposals for betterment to Penghu Branch of Tri-Service General Hospital and Penghu Hospital of Department of Health in terms of quality of services. Such proposals will function as handy reference to the government authorities and the subsequent researchers regarding medical treatment in Penghu area.

Exploring experiences of men accessing residential addictions services: towards an anti-oppressive policy development and implementation perspective

Many men seek to address their substance use issues in a variety of resources including hospitals, detoxification centres, treatment programs, supportive recovery housing and more. How policy is constructed and implemented has a direct impact on how effectively clients achieve their recovery goals. There is little literature on the topic of how policy is developed and implemented in residential addictions services. What research could be found did not include the voices of the recipients of these services. The goal of this research is to gain insight into the experiences of people accessing residential addictions services to inform policy development and implementation. Thorne’s (2008) interpretive description was the methodology used to guide this study. Eleven qualitative interviews with men accessing residential addictions services were conducted. Several themes were identified and an interpretive description was made. Eight theories are proposed. Several recommendations were formulated. Calling time : a discursive analysis of telephone calls to an alcohol helpline

This thesis takes Discursive Psychology as its main theoretical influence. Drawing on the resources of Discursive Psychology and utilising analytic tools provided by Conversation Analysis, these principles are applied to the study of addiction, and specifically alcohol problems. The data explored are telephone calls to an alcohol helpline. Four analytic chapters are presented. The first focuses on the concept of loss of control over drinking, identifying features of how this concept is constructed in talk and suggests possible functions of control talk for both callers and Advice Workers. The second analytic chapter examines how Advice Workers respond to callers' professed impaired control over their drinking and I demonstrate that embedded in discursive sequences of problem formulation and advice giving are issues of agency, accountability and responsibility. The thesis moves on to explore the role of knowledge in calls to an alcohol helpline and the analysis reveals that both the expert status of the Advice Worker and the speciality of the topic are co-constructed between the speakers on the helpline. The final analytic chapter features just one telephone call and demonstrates the application of such an analysis for alcohol service providers. The thesis ends with a discussion of the main overall findings and the implications of the research for clinical practice. I close by arguing that initial agency contact is a very important site of study and recommend that this should be explored utilising naturally-occurring talk.

Nepilnamečių priklausomybės ligų gydymo organizavimas paslaugų teikėjų ir gavėjų požiūriu / Organization of addictive disorders’ treatment to adolescents in the attitude of receivers and providers of the services

Darbo tikslas. Įvertinti nepilnamečių priklausomybės ligų gydymo organizavimą paslaugų gavėjų ir teikėjų požiūriu. Darbo uždaviniai: 1) Išanalizuoti nepilnamečiams teikiamas paslaugas priklausomybės ligų srityje; 2) Ištirti psichikos sveikatos priežiūros paslaugų teikėjų požiūrį apie priklausomybės ligų gydymo paslaugų teikimo organizavimą nepilnamečiams; 3) Atskleisti nepilnamečių, besigydančių Kauno apskrities priklausomybės ligų centre, bei jų tėvų požiūrį į priklausomybės ligų gydymo paslaugų organizavimą nepilnamečiams. Tyrimo metodika. Tyrimui atlikti naudoti mišrūs tyrimų metodai: kiekybinis ir kokybinis tyrimai. Paslaugų teikėjų, PSC ir PLC dirbančios specialistų komandos, požiūriui ištirti buvo atlikta anketinė anoniminė apklausa, kuri apėmė 31 įstaigą (26 PSC ir 5 PLC), perduota 170 anketų. Dalyvavo 26 įstaigos (22 PSC ir 4 PLC), grįžo 118 anketų. Įstaigos atsako dažnis siekia beveik 84 procentus, anketos - 69 procentus. Interviu buvo paimta iš 5 priklausomybe sergančių nepilnamečių, kurie gydėsi KAPLC, bei 5 tėvų/globėjų. Statistinei duomenų analizei naudotas SPSS 17.0 duomenų analizės paketas, bei „Microsoft Office Exell“ programa. Rezultatai. Ambulatorinės abstinencijos detoksikacijos paslaugą dažniau teikia PLC (60 proc.), nei PSC (9,6 proc., p<0,05), taip pat kaip ir palaikomojo gydymo ir atkryčių profilaktikos paslaugą (atitinkamai 62,9 proc. ir 36,1 proc., p<0,05). To evaluate the organization of addictive disorders’ treatment to adolescents in the attitude of receivers and providers of the services. Objectives. 1) To analyze services that are provided to adolescents in the field of addictive disorders; 2) To study the attitude of providers of mental public health care services towards the organization of providing addictive disorders treatment services to adolescents; 3) To reveal the attitude of adolescents, who are under the treatment in Kaunas district center for addictive disorders, and their parents’ attitude towards organization of addictive disorders treatment services to adolescents. Methods. Mixed analysis methods were used for the performance of research: quantitative and qualitative. In order to study the attitude of providers of services, team of professionals working in Mental health centers and Centers of addictive diseases, anonymous questionnaire survey was performed. Survey was implemented in 31 establishments (26 Mental health centers and 5 Centers of addictive diseases), 170 questionnaires were distributed. 26 establishments (22 Mental health centers and 4 Centers of addictive diseases) participated in the research and 118 questionnaires came back. Youth Receiving Treatment Service in the Juvenile Justice System: An Examination of Funding Sources and Recidivism

abstract: The dissertation examines treatment services received by youth on probation in the Maricopa County, Arizona. The project focuses on three primary issues: 1) the factors associated with receiving treatment services while on probation, 2) the factors associated with receiving treatment services through different funding streams, and 3) whether treatment services and specific characteristics of treatment services, particularly the funding source, influence recidivism outcomes of youth. To answer these questions the research used data obtained from the Maricopa County Juvenile Probation Department from July 2012 thru August 2014. Multivariate regression, along with statistical techniques to control for selection bias, were used to identify the factors associated with receiving treatment services, the factors associated with the funding source of treatment services, and the effect of treatment services on recidivism. The findings from the current dissertation suggest that the receipt of treatment services is not equal across groups, and particularly that minorities are less likely to receive treatment services compared to their White counterparts. Additionally, the findings reveal that certain characteristics of youth and the type of treatment service received influence the funding source, but the source of funding does not influence the effectiveness of the treatment services. Finally, using propensity score matching, the current dissertation found that treatment services were effective in reducing recidivism while under probation supervision and 6 months after probation supervision has ended. BACKGROUND. Substance use is a widespread phenomenon in prisons, with a numberous associated problems, for instance high incidence of infectious diseases, recidivism, poor physical and mental health and high costs of repressive measures, which still do not eradicate drug use in prison facilities. In 2016, the first addictologist was admitted to the prison system in Czechia. Two years later, 14 more table places were established. The thesis brings an overview at this period (2018 - 2021). AIMS. The aim of the thesis is to provide an information about starting and forming of specialized addiction services in prison seeting from the perspective of addictologists. The theoretical part of the thesis also presents the results of foreign research on the effectiveness of selected interventions and strategies and recommendations for good practice. RESEARCH QUESTIONS. The research questions address the experience of addictologists and are focused on four main areas: workplace formation, clientele, quality assurance and the impact of Covid-19 on the workplace. Stigma, Help-Seeking Behaviors, and Use of Services Among College Students with Self-Reported Posttraumatic Stress Disorder

People experiencing mental health illnesses such as posttraumatic stress disorder (PTSD) who do not receive mental health treatment services (MHTS) are at a higher risk of committing crimes. The research problem of this study was to fill the gap in the literature concerning gender, sex at birth, and gender identity differences as predictors of attitudes toward perceived stigma in help-seeking behavior (HSB) and use of MHTS. The sample size included 5,000 participants in the de-identified secondary data set of students from 26 universities and colleges across the United States. The Healthy Minds Study collected these data in 2016-2017 using the Patient Health Questionnaire. To address the research questions guiding the study, one-way ANOVA was used to test for differences in groups based on sexual orientation and gender identity for measures of perception of stigma, use of MHTS, and HSB regarding receiving mental health services. Between groups, MANOVA was used to assess differences in groups based on gender identity and sexual orientation on a linear combination of the dependent measures of perception of stigma in use of MHTS and HSB. There were directional differences between groups based on independent variables gender and sexual orientation on measures of the dependent variables perception of stigma in use of MHTS and of HSB. However, a closer examination of the results indicated that the effect size associated with the directional differences was weak. The results from this study may help clinicians to identify treatment challenges related to biological sex and gender identity and help to influence future interventions to better accommodate the contemporary population of men and women experiencing symptoms of PTSD.

Presently the Department of Correctional Services conducts pre-release preparation programmes in larger institutions. The inmates are, however, detained in institutions where the influence from other inmates is not conducive to the effective preparation of inmates for adjustment in free society after release or placement on parole. The use of halfway houses can combat the latter problems and play an important role in providing educational and training programmes. Specialised services such as social work; religious work, counselling, psychological treatment and psychiatry receive attention. Parents Served by Assertive Community Treatment: A Needs Based Assessment

Indiana University-Purdue University Indianapolis (IUPUI) / Assertive Community Treatment (ACT) represents an effective treatment for individuals with severe mental illness. Though studies estimate that as many as half of all people with severe mental illness are parents, little is known about consumers receiving ACT services who are parents. Thus, the purpose of the present study was to 1) estimate the prevalence of parent ACT consumers, 2) identify current ACT team policies and practices for treating parent consumers, and 3) examine the perspective of parent consumers served by ACT teams. Quantitative and qualitative data were collected and analyzed via two separate studies. In study 1, eighty-two ACT providers from 76 teams across the United States and Canada were surveyed to determine the prevalence of parent ACT consumers, ACT team policies for identifying the parental status of consumers, treatment services available for parent consumers, and provider attitudes about parent consumers. Providers estimated roughly 21.6% of ACT consumers were parents. Less than half of providers (46.3%) reported formally asking about parental status during intake and only 20.7% providers belonged to ACT teams that provide special programs/services designed for parent consumers. The majority of providers (75.6%) reported negative or mixed attitudes about parents with severe mental illness. In study 2, seventeen parents with severe mental illness being served by ACT teams were interviewed about parenting, the relationship between parenting and severe mental illness, parenting needs, and suggestions for improved treatment services for parents. All parents were able to identify at least one positive aspect of parenting and most parents (76.5%) also identified negative aspects of parenting. Loss of custody emerged as a significant parenting problem, with the majority of parents (88.2%) experiencing loss of custody at least once. Given the difficulties of being a parent and having to manage a severe mental illness, parents expressed interest in several parent-focused treatment services, including family therapy, parenting skills, communication skills training, resources/finances for children, and social support groups with peers. When asked about overall satisfaction with ACT services, most participants with adult children (87.5%) reported having no unmet parent-related needs and high satisfaction (4.63 out of 5) with ACT services, whereas parents with young children (77.8%) reported having numerous unmet parenting needs and low satisfaction (3.78 out of 5) with ACT services. Thus, the age of participants’ children was a significant factor, indicating that the ACT treatment model may not be adequately serving parents of young, dependent children.
