A triagem de emergência tem como objetivo dar prioridade aos pacientes mais graves e melhorar a prestação do serviço de emergência, garantindo atendimento no momento adequado, melhor fluxo e segurança. Os objetivos deste trabalho foram traduzir e validar a parte referente às emergências odontológicas do instrumento para triagem de emergência Manchester Triage System (MTS) na língua portuguesa, avaliar a confiabilidade interobservadores e o impacto da aplicação do MTS no Setor de Urgência Odontológica da Faculdade de Odontologia da Universidade de São Paulo. O MTS foi traduzido através do método padrão de tradução/ retrotradução. A versão foi submetida à validação semântica e idiomática e também conceitual e cultural, feita por um comitê de juízes. Em ambas, a concordância final observada após os ajustes realizados foi superior a 80%. Na concordância interobservadores, 200 pacientes participaram das entrevistas realizadas por 2 voluntários independentes com conhecimentos em odontologia e a pesquisadora. Os coeficientes kappa foram de 0.58 e 0.60. Inicialmente foram entrevistados 120 pacientes sem a aplicação do MTS e em seguida 139 pacientes categorizados de acordo com a prioridade clínica. Houve diminuição no tempo médio de espera para atendimento, de 79.96 min para 36.86 min (Mann-Whitney (MW), p=0.00) e aumento do tempo médio de duração do atendimento, de 29.11 min para 34.78 min (MW, p=0.05). Dentre os pacientes categorizados segundo o MTS, os não-urgentes eram mais velhos (MW, p=0.01), tiveram menor duração de atendimento (MW, p=0.00) e menor intensidade de dor (MW, p=0.00). A versão traduzida e validada do MTS mostrou-se adequada e sua aplicação e útil no atendimento de pacientes do Setor. / The emergency triage aims to assign clinical priority to the patients and to improve emergency services, ensuring attendance at the right time, better patient flow and safety. The objectives of this study were to translate and validate part of the Manchester Triage System (MTS) related to dental emergency in Portuguese, to assess the inter-rater reliability and to evaluate the impact of implementation of triage at Setor de Urgência Odontológica of Faculdade da Odontologia of Universidade de São Paulo. The MTS was translated using the standard translation/ back-translation method. The instrument was then submitted to semantic and idiomatic validation, as well as conceptual and cultural validation by a committee of judges. In both, the final concordance after the adjustments was higher than 80%. In the inter-rater agreement, 200 patients were interviewed by two independent volunteers with some knowledge of dentistry and the researcher. The kappa coefficients were 0.58 and 0.60. Then 120 patients were interviewed without the application of the MTS and 139 patients were categorized according to their clinical priority. There was a reduction in the mean waiting time, from 79.96 min to 36.86 min (Mann-Whitney test (MW), p=0.00) and an increase in the mean of treatment duration, from 29.11 min to 34.78 min (MW, p=0.05). Among triaged patients, the non-urgent were older (MW, p=0.01), had shorter duration of treatment (MW, p=0.00) and lower pain intensity (MW, p=0.00). The translated and validated version of the MTS showed to be appropriate and useful in categorization of patients with dental emergencies in this setting.

Esta pesquisa toma a ficção como um método que nos permite complexificar as tramas sensíveis-conceituais com o mundo. Partindo de narrativas da personagem Helena, cartografam-se as linhas que configuram a questão da urgência entre uma lógica disciplinar e uma lógica do controle. De uma urgência concebida como acontecimento extraordinário, a ser isolado em um espaço e disciplinado por protocolos biomédicos, para uma urgência intensiva, constituinte do nosso cotidiano e de seus processos de subjetivação. Pelas narrativas de Helena, apreendemos as diversas operações de ambas as modulações da urgência, bem como entrevemos brechas para desviar das mesmas em fugas: a literatura menor e a possibilidade de afirmar paradas vibráteis são apostas deste trabalho como operação de resistência às capturas líquidas das urgências contemporâneas. / This research takes fiction as a method that allows us to complexize the conceptual-sensitive frames with the world. Starting from the narratives of the Helena character, the lines that configure the question of the urgency between a disciplinary logic and a control logic are mapped. From an urgency conceived as an extraordinary event, to be isolated in a space and disciplined by biomedical protocols, to an intensive urgency, constituent of our daily life and its subjectivation processes. Through the narratives of Helena, we learn the different operations of both modulations of urgency, as well as we look at gaps to divert them in fugues: the smaller literature and the possibility of affirming vibrating stops are bets of this work as an operation of resistance to net catches of emergencies contemporary.

An Exploration of Tactile Warning Design Based on Perceived Urgency

When there is information overload on the visual modality, another system of warnings must be adopted to prevent potential risks—tactile warning systems present a viable alternative. Building on work on design approaches for auditory warning systems that match appropriate warnings to the severity of risk, this thesis presents an approach to design tactile warnings based on perceived urgency. To do this, I use a subjective rating technique. I performed three experiments to demonstrate this approach. Our research approach uses subjective rating technique to evaluate perceived urgency. Three experiments were conducted to design tactile warnings with a tactile interface developed by attaching a grid of tactors on a vest. In Experiment 1 and 2, I evaluated perceived urgency of several warning designs with three important parameters of tactile warnings with subjective rating. In Experiment 3 I examined one warning design in the context of flight simulation. The results of Experiment 1 and 2 showed that participants can discriminate between all levels of perceived urgency from most warning parameters. In Experiment 3, the results showed that selected warning design was correctly mapped with the severity of most events. The findings suggest that tactile warnings based on perceived urgency can be a possible approach, but further studies will be required to evaluate different parameters of tactile warnings.

Negative Urgency, Pubertal Onset and the Longitudinal Prediction of Alcohol Consumption During the Transition from Preadolescence to Adolescence

Alcohol use in early adolescence is associated with numerous concurrent and future problems, including diagnosable alcohol use disorders. The trait of negative urgency, the tendency to act rashly when distressed, is an important predictor of alcohol-related dysfunction in youth and adults. The aim of this study was to test a model proposed by Cyders and Smith (2008) specifying a puberty-based developmental increase in negative urgency, which in turn predicts subsequent increases in early adolescent drinking. In a sample of 1,910 youth assessed semi- annually from spring of 5th grade through spring of 8th grade, we found support for this model. Pubertal onset was associated with both a mean increase and subsequent rises in negative urgency over time. Drinking frequency at any wave was predicted by prior wave assessments of drinking frequency, negative urgency, and pubertal onset. The slope of increase in drinking also increased as a function of pubertal onset. This model applied to negative urgency but not to other impulsivity-related traits. These findings highlight the importance of personality change in early adolescence as part of the risk matrix for early onset alcohol consumption.

Passados 20 anos desde a implantação do SUS, este salienta a necessidade de se reorganizar os serviços com outro olhar, evidenciando que o binômio saúde–doença está na dependência de fatores sociais. Considerando que os serviços de urgência são includentes, não discriminatórios, com uma ampla superfície de contato com a população, promovendo uma captação passiva da provável população de maior atividade de doença, com agravos não só de origem biológica, mas também advindos de iniqüidades sociais, estes devem se preparar para um atendimento humanizado, minimizar iniqüidades, monitorar e avaliar técnicamente, participar da rede de referência e contra referência, para cumprir os princípios do SUS. Esta pesquisa analisou a prática de um serviço de urgência e emergência do SUS, com atendimento em Saúde Bucal, no município de São Paulo, em 2006, visando sistematizar saberes que possam contribuir para uma reorganização da assistência e construção de parâmetros que subsidiem o desenvolvimento de modelos de atenção a urgência adequados e humanizados. Foi analisada uma amostra dos Boletins Emergenciais dos atendimentos de um mês por trimestre do ano, digitalizados no sistema HOSPUB. Os resultados demonstraram que a faixa etária que mais procura o serviço é a de 20 a 29 anos com 45% de freqüência, não havendo distinção entre sexo, sendo 85% de pessoas da própria região. Foram realizadas 106.632 consultas no ano, 5,1% de Saúde Bucal, com uma média de 462,6 atendimentos/mês e 15,4 atendimentos/ plantão de 24 horas. A cobertura regional para consultas de urgência foi de 1 consulta/ ano a cada 67,8 habitantes. Foram realizados 2,5 procedimentos/atendimento, com uma espera de 60 minutos em média... / Urgency in health care, assumes a new prominent role in The National Oral Health Policy as a citizenship right. In 20 years of the SUS’s implantation, this system proves the need of restructuring the services with a new approach, putting in evidence the binomial health illness is on the dependence of social factors. Considering that services in urgency are comprehensive, not discriminatory, a large contact surface with the population, a passive capture of the likely population with the most activity of the illness, with disorders not only of biological origin, but also from social iniquities, these services must be prepared for a humanized care of the population, minimizing inequities, with monitoring and technical assessment, participating in the network of reference and counter reference, fulfilling the principle of comprehensiveness in the SUS. This research examined the practice of an urgency and emergency service of the SUS, with Oral Health Care, in São Paulo, in 2006 with the goal of systematizing all knowledge that can contribute to a reorganization of such assistance towards the construction of parameters that subsidize the development of appropriate and humanized models of attention to the urgency. A sample of one month per quarter was analyzed, and inputted into the HOSPUB system. The results showed that the age group that most demand the service is from 20 to 29 years old with 45% of frequency and there is no meaningful distinction between sex and 85% of the demand is from individuals of the region itself. Regarding the conditions of service of the unit studied, from 106,632 consultations held in the year, 5.1% were of Oral Health, with an average of 462.6/month and 15.4 consultations/each 24 hours on-call.
