Objective: To assess the therapeutic response to inhaled corticosteroids of patients with Sickle Cell Anemia (SCA). Methods: A longitudinal intervention study with children, adolescents and young adults, both genders, aged between seven and twenty-seven years old, clinically stable, without acute adverse events, with the diagnosis of SCA confirmed by hemoglobin electrophoresis. They were initially submitted to pulmonary function testing, transcutaneous oxygen saturation and peak flow. Then, they were submitted to the use of innalatory budesonide for 03 months and afterwords they were reassessed with pulmonary function testing, transcutaneous oxygen saturation and peak flow. Results: The study included 44 patients with mean age of 14.9±4,6 years, with spirometric altered profile, and 63.6% had a restrictive ventilatory disease, 22.7% obstructive and 13.6% mixed obstructive and restrictive. The measures of transcutaneous oxygen saturation, after the treatment, showed an improvement, with 43.1% of the patients with higher values than 95%, 31.8% with values between 90 to 94% and 25% values below 89%. As to the improvement of the symptoms, there was a decrease of 4.6% in the number of dyspneic, 22.7% in the number of those with cough and 2.2% in the number of those who complained of chest pain. The number of asymptomatic patients increased from 50% to 79.5%. Conclusion: The patients with obstructive and mixed obstructive and restrictive profile answered better than restrictive patients, having or not having asthma. / Objetivo: Avaliar o efeito da utilização do corticoesteróide inalatório sobre a função pulmonar de pacientes com anemia falciforme (AF). Métodos: Estudo de intervenção longitudinal com crianças, adolescentes e adultos jovens, no qual foram avaliados pacientes de ambos os gêneros, com idades entre sete e vinte e sete anos, clinicamente estáveis, sem intercorrências agudas, com o diagnóstico de AF confirmado por eletroforese de hemoglobina e alteração na prova ventilatória completa. Os pacientes foram submetidos à prova ventilatória completa, à saturação transcutânea de oxigênio e ao Peak flow. Foram em seguida submetidos a três meses de uso de budesonida inalatória na dose de 400 a 800mcg/dia. Após este tratamento foram reavaliados quanto aos mesmos parâmetros. Resultados: Participaram do estudo 44 pacientes, todos com perfil espirométrico alterado, com média de idade de 14,9 ± 4,6 anos, sendo que 63,6% apresentavam distúrbio ventilatório restritivo, 22,7% obstrutivo e 13,6% misto. Os pacientes com padrão obstrutivo e misto responderam de forma semelhante ao tratamento com corticoesteróide: 50% dos pacientes com padrão obstrutivo apresentaram melhora do VEF1 e 33,3% dos pacientes com padrão misto responderam com melhora do VEF1 e do CVF, enquanto que 14,3% dos pacientes com padrão restritivo responderam favoravelmente, com melhora do CVF. As medidas da saturação transcutânea de oxigênio após o tratamento apresentaram melhora, com 43,1% dos pacientes apresentando valores maiores que 95%, 31,8% com valores entre 90 e 94%, e 25% com valores inferiores a 89%. Houve diminuição de 22,7% no número dos pacientes que apresentavam queixa de tosse persistente, e a proporção de pacientes assintomáticos passou de 50% para 79%. Conclusão: Os pacientes com anemia falciforme e prova ventilatória com padrão obstrutivo e misto responderam favoravelmente ao uso do corticoesteróide inalatório.
