Previous issue date: 2005-08-16 / Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / Objective: The goal of this research was to describe the peripherical and central audiological findings of a child with auditory neuropathy/auditory dys-synchrony. Methods: It was performed an audiological evaluation, eletrophysiological and behavioral, with tests that appraise the peripherical and central auditory system. Results: The result found in audiometry revealed a variation or floating of pure tone thresholds and the beginning of a sensorioneural hearing loss in high frequencies; the results of speech audiometry were compatible with pure tone thresholds. It could be observed a presence of the contralateral acoustic reflex in the frequency of 500 Hz in both ears and 1 kHz on the left ear, with absence of reflexes in the other frequencies, with normal timpanometry. The otoacoustic emissions were not found, when compared to previous evaluations, it can be observed that a hearing loss is occuring comparing the results from previous exams. The ABR register showed absence of response with presence of cochlear microphonics, in the minimum level of 50 dBnHL on the right ear and 60 dBnHL on the left ear. The auditory evoked potentials for middle and long latency were presents. In the evaluation of the auditory processing we can find normal results for the following tests: low-pass filtered speech, auditory fusion test, dichotic digits and pitch pattern sequence test; the results for the following tests showed altered: speech in noise test, synthetic sentence identification, staggered spondaic words, masking level difference and random gap detection. Conclusions: The use of all of the audiological exams, eletrophysiological and behavioral, in order to evaluate the peripherical and central auditory system is fundamental in the cases with auditory neuropathy/auditory dys-synchrony, once they can make possible a better comprehensions of these cases; it s important to follow up them carefully even when normal thresholds can be observed because light modifications can occur which may need a therapeutic intervation / Objetivo: Este estudo teve como objetivo descrever os achados da avaliação audiológica periférica e central de um sujeito com neuropatia auditiva/dessincronia auditiva. Metodologia: Foi realizada avaliação audiológica, eletrofisiológica e comportamental, com testes que avaliam o sistema auditivo periférico e central. Resultados: O resultado obtido na audiometria tonal limiar revelou uma variação ou flutuação dos limiares auditivos e o início de uma perda auditiva sensorioneural nas freqüências altas; os resultados logoaudiométricos foram compatíveis com os limiares obtidos na audiometria tonal. Pode-se observar presença de reflexo acústico contralateral nas freqüências de 500 Hz em ambas as orelhas e 1kHz na OE, com ausência de reflexos nas demais freqüências, com curva tipo A bilateral. As emissões otoacústicas encontraram-se ausentes (EOAT e EOAPD), quando comparada às avaliações anteriores, sendo possível observar que está ocorrendo a perda das emissões otoacústicas. A pesquisa do PEATE registrou ausência de respostas, com presença do MC, sendo estes encontrados no nível mínimo de 50 dBnHL na OD e 60 dBnHL na OE. Os potenciais auditivos evocados de média e longa latência - PEAML e P300 - estiveram presentes. Na avaliação do processamento auditivo, encontraram-se dentro da normalidade os resultados obtidos nos testes: fala filtrada (FF), fusão binaural (FB), dicótico de dígitos (DD) e teste seqüencial de padrão de freqüência (PPST); alterados estiveram os resultados dos testes: fala com ruído (FR), identificação de sentenças sintéticas (SSI), dissílabos sobrepostos e alternados (SSW), limiar diferencial de mascaramento (MLD) e detecção de intervalos aleatórios de silêncio (RGDT). Considerações Finais: A utilização da bateria completa de exames audiológicos, eletrofisiológicos e comportamentais, que avaliem o sistema auditivo periférico e central é fundamental nos casos de neuropatia auditiva/dessincronia auditiva, uma vez que possibilitam uma melhor compreensão dos casos; é importante a realização do acompanhamento e do monitoramento mesmo nos casos de neuropatia auditiva/dessincronia auditiva que apresentam limiares tonais próximos da normalidade, pois podem ocorrer mudanças no quadro audiológico que gerem necessidade de intervenção terapêutica
