Previous issue date: 2012-01-09 / This investigative work, intitled Naming and verbal fluency in patients with multiple sclerosis , is developed through two studies: the first crosssectional and correlational (Study A) and the second case study (study B). The objectives of the study are: to investigate language functioning through naming ability and verbal fluency in patients with multiple sclerosis; to check the connections between performances on naming and verbal fluency and performance on other neuropsychological tests; to verify the influence of gender and education variables on performance on language and cognitive tests; to evaluate the language skills of patients diagnosed with multiple sclerosis with focus on dysarthria and dysgraphia; to evaluate intensity of neuronal activation the pattern in patients in verbal fluency tasks - phonological and semantic and naming, through functional magnetic resonance technique. Neuropsychological assessments were performed in 42 patients with multiple sclerosis and in 30 participants of a control group. The evaluations were compared to the base of patients with multiple sclerosis of Hospital S?o Lucas of the PUCRS. In addition we evaluated a patient with Relapsing-Remitting Multiple Sclerosis through reading assignments, copying and spontaneous speech, and also through functional magnetic resonance imaging in naming task, phonological and semantical verbal fluency. The results allowed us to make the following statements: verbal fluency, both semantic and phonological, is impaired in patients with multiple sclerosis; naming is not affected in these patients. Although multiple sclerosis compromises verbal fluency, it does not necessarily compromise memory and intelligence; there is a close relationship between fluency deficits and reduced speed of information processing; gender and education variables do not affect cognitive performance in these patients, except in the Vocabulary subtest. The results of the case study allow us to say: the subject had a significant level of dysgraphia but did not show dysarthria, which reinforces the heterogeneous symptomatic manifestations of the disease; Neuroimaging analysis showed crucial injuries of the disease; the activations in the task of naming and fluency occur predominantly in the left hemisphere with less intense parallel activations in the right hemisphere, which may suggest recruitment of nerve cells due to injuries in the temporal lobe of the left hemisphere. / O presente trabalho investigativo, que consta como t?tulo nomea??o e flu?ncia verbal em portadores de esclerose m?ltipla, desenvolve-se por meio de dois estudos: o primeiro, transversal e correlacional (estudo A), e o segundo, estudo de caso (estudo B). Os objetivos do estudo s?o: investigar o funcionamento da linguagem atrav?s da capacidade de nomea??o e de flu?ncia verbal de pacientes com Esclerose M?ltipla; verificar as correla??es entre os desempenhos em nomea??o e flu?ncia verbal e os desempenhos nos demais testes neuropsicol?gicos; verificar a influ?ncia das vari?veis sexo e escolaridade no desempenho dos testes lingu?sticos e cognitivos; avaliar as habilidades lingu?sticas de paciente com diagn?stico de Esclerose M?ltipla com aten??o ? disartria e ? disgrafia, al?m de avaliar caracter?sticas e intensidade de ativa??o neuronal desse paciente em atividade de flu?ncia verbal fonol?gica e sem?ntica e nomea??o, atrav?s de t?cnica de Resson?ncia Magn?tica Funcional. Foram realizadas avalia??es neuropsicol?gicas em 42 portadores com Esclerose M?ltipla e 30 participantes para grupo de controle. As avalia??es foram comparadas com os resultados do banco de dados de portadores de Esclerose M?ltipla do Hospital S?o Lucas da PUCRS. Al?m disso, para estudo de caso, foi avaliada paciente com Esclerose M?ltipla Remitente-Recorrente atrav?s de tarefas de leitura, c?pia e fala espont?nea e, ainda, atrav?s de exame de resson?ncia magn?tica funcional em tarefa de nomea??o, flu?ncia verbal fonol?gica e flu?ncia verbal sem?ntica. Os resultados permitem fazer as seguintes afirma??es: a flu?ncia verbal, tanto fonol?gica quanto sem?ntica, ? comprometida em portadores de Esclerose M?ltipla; a nomea??o n?o ? afetada nesses pacientes; embora a Esclerose M?ltipla comprometa significativamente a flu?ncia verbal, n?o compromete necessariamente mem?ria e intelig?ncia; h? estreita rela??o entre d?ficits em flu?ncia e redu??o da velocidade de processamento de informa??es; a vari?vel sexo n?o interfere no desempenho cognitivo desses pacientes; a vari?vel escolaridade n?o interfere no desempenho cognitivo desses pacientes, exceto no subteste Vocabul?rio, cuja diferen?a foi significativa entre escores de participantes com mais e com menos escolaridade. Os resultados do estudo de caso permitem afirmar: a participante apresentou n?vel importante de disgrafia e n?o apresentou disartria, o que refor?a a caracter?stica heterog?nea nas manifesta??es sintom?ticas dessa doen?a; o Teste de Domin?ncia Manual mostrou que a participante ? destra; a an?lise das neuroimagens mostrou que h? les?es importantes caracter?sticas da doen?a; as ativa??es em tarefa de nomea??o e flu?ncia ocorrem predominantemente no hemisf?rio esquerdo com ativa??es paralelas menos intensas no hemisf?rio direito, o que pode sugerir recrutamento de c?lulas nervosas devido a les?es no lobo temporal do hemisf?rio esquerdo.
