Previous issue date: 2014 / Coordenação de Aperfeiçoamento de Pessoal de Nível Superior (CAPES) / Introdução: A sensibilidade do complexo areolo-papilar (CAP) pode ser relevante para a sexualidade feminina. Objetivo: Avaliar a sensibilidade do complexo areolo-papilar e a funcao sexual apos a mamoplastia redutora com pediculo superomedial. Avaliar a associacao entre alteracao da sensibilidade do complexo areolo-papilar e presenca de disfuncao sexual. Metodos: Foram selecionadas 80 pacientes, distribuidas em tres grupos: Controle (GC, n=20), mamas eutroficas; Hipertrofia (GH, n=20) e Mamoplastia (GM, n=40). O GC e GH foram avaliados uma unica vez e o GM foi avaliado no pre e pos-operatorio de seis meses da mamoplastia. A sensibilidade do CAP foi avaliada nos quatro quadrantes areolares e na papila, para quatro modalidades: toque superficial, temperatura (quente/fria), vibracao e pressao. Para avaliacao da funcao sexual utilizou-se a versao brasileira do Female Sexual Function Index (FSFI) que apresenta seis dominios (desejo, excitacao, lubrificacao, orgasmo, satisfacao e dor) e um escore total, que determina a presenca ou nao de disfuncao sexual. Resultados: Comparado ao GC, o GM no sexto mes pos-operatorio apresentou pior sensibilidade para temperatura e pressao (ρ<0,01). O GH apresentou a pior sensibilidade para toque superficial (ρ<0,01). Comparando pre e pos-operatorio do GM, houve diminuicao significante para temperatura e pressao (ρ<0,05). Em comparacao com GC e GH, o GM, no pos-operatorio, apresentou melhor resultado para e citacao (ρ=0,0001), lubrificacao (ρ=0,0004), orgasmo (ρ<0,0001) e satisfacao (ρ<0,0001). Houve associacao significante entre disfuncao sexual e baixa sensibilidade em pontos do CAP a temperatura e vibracao (ρ≤0,041) no pre e pos-operatorio do GM e ao toque superficial, temperatura e pressao comparando-se os tres grupos. Conclusao: A mamoplastia redutora com pediculo superomedial promoveu diminuicao na sensibilidade do CAP e nao interferiu na funcao sexual / Introduction: The sensibility of nipple-areola complex (NAC) may be
relevant to female sexuality. Objective: To evaluate the sensitivity of the
NAC and sexual function after breast reduction with superomedial
pedicle. To evaluate the association between changes in sensitivity of the
NAC and the presence of sexual dysfunction. Methods: Eighty patients
were selected divided into three groups: Control (CG, n=20), with
eutrophic breasts; Hypertrophy (HG, n=20) and Mammoplasty (MG,
n=40). The MG was assessed preoperatively and six months
postoperatively, and HG and CG were evaluated once. The sensibility of
the NAC was assessed in the four quadrants of areola and the nipple for
four different modalities: touch, temperature, vibration and pressure.
Sexual function was assessed by the Brazilian version of the Female
Sexual Function Index (FSFI), which has six domains (desire, arousal,
lubrication, orgasm, satisfaction and pain) and a total score that
determines the presence or absence of sexual dysfunction. Results:
Compared with the CG, the MG in the sixth postoperative month showed
worse sensibility to temperature and pressure (ρ<0.01). The worst
sensibility to touch was shown by HG (ρ<0.01). Comparing pre and
postoperative, MG had a significant decrease in temperature and pressure
(ρ<0.05). Compared with CG and HG, MG at postoperatively showed
better results for excitation (ρ=0.0001), lubrification (ρ=0.0004), orgasm
(ρ<0.0001) and satisfaction (ρ<0.0001). There was a significant
association between dysfunction and NAC low sensibility to temperature
and vibration (ρ≤0.041) in MG comparing post and preoperative and to
touch, temperature and pressure comparing the three groups. Conclusion:
The breast reduction with superomedial pedicle caused a decrease in
sensitivity of the NAC and not interfere with sexual function. / BV UNIFESP: Teses e dissertações
