[pt] Através da memória episódica o indivíduo tem a capacidade de recordar experiencias anteriormente armazenadas, além de utiliza-la em processos como a aprendizagem e ao elaborar metas futuras. Entretanto é uma função que sofre declínio no envelhecimento, repercutindo negativamente na vida social e psicológica do idoso. As diversas estratégias de intervenção cognitiva surgem como propostas que visam reduzir as perdas da memória. A presente dissertação tem como intuito abordar aspectos teóricos e empíricos da intervenção cognitiva e seu efeito na memória episódica. O primeiro capitulo consiste em uma revisão sistemática, na qual buscou-se identificar os principais modelos de treino cognitivo da memória episódica em idosos saudáveis e clínicos. Foi realizado uma busca em quatro bases de dados: PubMed; PsycNET, Web of Science e Scielo através do método PRISMA, restando 23 artigos finais. Os dados coletados dos trabalhos analisados sugerem eficácia dentre as diferentes abordagens de treino cognitivo, porém, apesar de ser um tema bastante estudado, carece de mais estudos empíricos acerca de outras formas de treino cognitivo. O segundo capitulo ilustra um estudo empírico que mensurou o efeito da intervenção cognitiva em idosos saudáveis participantes do programa de estimulação cognitiva Neuro Academia da memória. Quinze idosos saudáveis participaram da pesquisa e como instrumentos para avaliação da memória episódica foram utilizados o RAVLT e o Ruche. Como resultado, encontrou-se diferença dentre grupos experimental e controle em momentos pré e pós intervenção cognitiva baseando-se na curva de aprendizagem. Entretanto, a mensuração da memória episódica através do treinamento cognitivo necessita de novas produções. / [en] Through episodic memory, the individual has the ability to recall previously stored experiences, in addition to using it in processes such as learning and in developing future goals. However, it is a function that suffers a decline in aging, with a negative impact on the social and psychological life of the elderly. The different cognitive intervention strategies appear as proposals that aim to reduce memory losses. This dissertation aims to address theoretical and empirical aspects of cognitive intervention and its effect on episodic memory. The first chapter consists of a systematic review, in which we sought to identify the main models of cognitive training of episodic memory in healthy and clinical elderly. A search was performed in four databases: PubMed; PsycNET, Web of Science and Scielo through the PRISMA method, leaving 23 final articles. The data collected from the analyzed studies suggest efficacy among the different approaches to cognitive training, however, despite being a widely studied topic, it lacks further empirical studies about other forms of cognitive training. The second chapter illustrates an empirical study that measured the effect of cognitive intervention in healthy elderly people participating in the Neuro Academy of Memory cognitive stimulation program. Fifteen healthy elderly people participated in the research and RAVLT and Ruche were used as instruments to assess episodic memory. As a result, a difference was found between experimental and control groups in moments before and after cognitive intervention based on the learning curve. However, measuring episodic memory through cognitive training requires new productions.

Working memory in Alzheimer’s disease and mild cognitive impairment (MCI) : assessment and intervention

L’objectif principal de cette thèse est d’examiner et d’intervenir auprès des déficits de la mémoire de travail (MdeT) à l’intérieur de deux populations cliniques : la maladie d’Alzheimer (MA) et le trouble cognitif léger (TCL). La thèse se compose de trois articles empiriques. Le but de la première expérimentation était d’examiner les déficits de MdeT dans le vieillissement normal, le TCL et la MA à l’aide de deux versions de l’empan complexe : l’empan de phrases et l’empan arithmétique. De plus, l’effet de «l’oubli» (forgetting) a été mesuré en manipulant la longueur de l’intervalle de rétention. Les résultats aux tâches d’empan complexe indiquent que la MdeT est déficitaire chez les individus atteints de TCL et encore plus chez les gens ayant la MA. Les données recueillies supportent également le rôle de l’oubli à l’intérieur de la MdeT. L’augmentation de l’intervalle de rétention exacerbait le déficit dans la MA et permettait de prédire un pronostic négatif dans le TCL. L’objectif de la deuxième étude était d’examiner la faisabilité d’un programme d’entraînement cognitif à l’ordinateur pour la composante de contrôle attentionnel à l’intérieur de la MdeT. Cette étude a été réalisée auprès de personnes âgées saines et de personnes âgées avec TCL. Les données de cette expérimentation ont révélé des effets positifs de l’entraînement pour les deux groupes de personnes. Toutefois, l’absence d’un groupe contrôle a limité l’interprétation des résultats. Sur la base de ces données, la troisième expérimentation visait à implémenter une étude randomisée à double-insu avec groupe contrôle d’un entraînement du contrôle attentionnel chez des personnes TCL avec atteinte exécutive. Ce protocole impliquait un paradigme de double-tâche composé d’une tâche de détection visuelle et d’une tâche de jugement alpha-arithmétique. Alors que le groupe contrôle pratiquait simplement la double-tâche sur six périodes d’une heure chacune, le groupe expérimental recevait un entraînement de type priorité variable dans lequel les participants devaient gérer leur contrôle attentionnel en variant la proportion de ressources attentionnelles allouée à chaque tâche. Les résultats montrent un effet significatif de l’intervention sur une des deux tâches impliquées (précision à la tâche de détection visuelle) ainsi qu’une tendance au transfert à une autre tâche d’attention divisée, mais peu d’effets de généralisation à d’autres tâches d’attention. En résumé, les données originales rapportées dans la présente thèse démontrent un déficit de la MdeT dans les maladies neurodégénératives liées à l’âge, avec un gradient entre le TCL et la MA. Elles suggèrent également une préservation de la plasticité des capacités attentionnelles chez les personnes à risque de développer une démence. / The principal aim of this dissertation is to investigate and intervene upon working memory (WM) impairments in two clinical populations: Alzheimer’s disease (AD) and mild cognitive impairment (MCI). The dissertation is comprised of three empirical articles. The goal of the first study was to examine WM impairments in normal aging, MCI and AD using two versions of the complex span task: sentence span and operation span. In addition, the effect of forgetting was assessed by manipulating the length of the retention interval. Results indicate impaired WM on complex spans in MCI and, to a greater extent, in AD. Data also support a role for forgetting within WM. Increasing the retention interval augmented deficit in persons with AD, and showed potential in predicting a negative prognosis in those with MCI. The objective of the second article was to investigate the feasibility of a computerized cognitive training paradigm for the attentional control component of WM in healthy older individuals and those with MCI. Data from this experiment revealed positive effects of the intervention for both groups. However, the absence of a control group limited interpretation of results. Based on those data, the third article aimed to implement a double-blind randomized controlled study of training of attentional control in MCI with executive deficits. This involved a dual-task paradigm comprised of a visual detection task and a visual alpha-arithmetic judgment task. While the control group performed simple dual-task practice over six one-hour sessions, the experimental group received variable-priority training, in which participants managed their attentional control by varying the proportion of attentional resources allocated to each task. Results show a significant effect of intervention on one of the two tasks involved (accuracy on the visual detection task) and a trend for transfer to another task of divided attention, but little generalization to other tasks of attention. In summary, the original data reported in this thesis demonstrates WM impairment in age-related disorders, with a gradient between MCI and AD, and suggest preserved plasticity of attentional capacities in persons at risk of developing dementia.

Working memory training and transcranial electrical brain stimulation

Working memory training improves performance on trained and untrained working memory tasks, but there is little consistent evidence that these gains benefit everyday tasks that rely on working memory. Evidence has shown that transcranial electrical stimulation (tES) may be an effective tool for enhancing cognitive training and promoting transfer. In the first study, participants completed Cogmed working memory training with either active or sham transcranial random noise stimulation (tRNS). Training was associated with substantial gains on the training activities and on transfer measures of working memory with common processing and storage demands to the training tasks. tRNS did not enhance gains on trained or untrained activities. The second study systematically investigated the boundary conditions to training transfer by testing whether gains following backward digit recall (BDR) training transferred within- and across-paradigm to untrained backward recall and n-back tasks with varying degrees of overlap with the training activity. A further aim was to test whether transcranial direct current stimulation (tDCS) enhanced training and transfer. Participants were allocated to one of three conditions: (i) BDR training with active tDCS, (ii) BDR training with sham tDCS, or (iii) visual search control training with sham tDCS. The results indicated that training transfer is constrained by paradigm, but not by stimuli domain or stimuli materials. There was no evidence that tDCS enhanced performance on the training or transfer tasks. The results of Study 1 and Study 2 provide no evidence that tES enhances the benefits of working memory training. The absence of transfer between backward recall training and n-back in Study 2 suggested the tasks might tap into distinct aspects of working memory. Consequently, the final study used a latent variable approach to explore the degree of overlap between different forms of backward recall and n-back tasks containing digits, letters, or spatial locations as stimuli. The best-fitting factor model included two distinct but related (r = .68) constructs corresponding to backward recall and n-back. Both categories of task were linked to a separate fluid reasoning construct, providing evidence that both are valid measures of higher-order complex cognition. Overall, the experiments in this thesis suggest that working memory tasks tap into separate processes and that training may be targeting and improving these distinct processes, explaining the absence of cross-paradigm transfer.

Executive Function Deficits in Seriously Ill Children—Emerging Challenges and Possibilities for Clinical Care

The past years have seen an incredible increase in the quality and success rates of treatments in pediatric medicine. One of the resulting major challenges refers to the management of primary or secondary residual executive function deficits in affected children. These deficits lead to problems in the ability to acquire, understand, and apply abstract and complex knowledge and to plan, direct, and control actions. Executive functions deficits are important to consider because they are highly predictive of functioning in social and academic aspects of daily life. We argue that current clinical practice does not sufficiently account for the complex cognitive processes in this population. This is because widely applied pharmacological interventions only rarely account for the complexity of the underlying neuronal mechanisms and do not fit well into possibly powerful “individualized medicine” approaches. Novel treatment approaches targeting deficits in executive functions in seriously ill children could focus on neuronal oscillations, as these have some specific relations to different aspects of executive function. Importantly, such treatment approaches can be individually tailored to the individuals’ deficits and can be transferred into home-treatment or e-health solutions. These approaches are easy-to-use, can be easily integrated into daily life, and are becoming increasingly cost-effective.

Puppy Valley: An App-Mediated Board Game to Enhance Scam And Fraud Awareness Among US Seniors

<p>Scams and fraud have become a significant problem for the American population within the past few years. While victims range from young to older adults, it is the seniors who are the major targets. In 2018 the Federal Trade Commission reported that senior citizens aged 60 years and above formed 35% of all the scam victims it recorded. According to the U.S Senate Committee on aging, scams like the grandparent scam, IRS impersonation scam, romance scam; have become popular. Even at this level of notoriety, most victims are usually unaware of it entirely or how its perpetrators commit these crimes. Being aware of the situation is a critical step towards curbing this problem. This thesis project uses User-Centered Design (UCD) to understand, conceptualize, and prototype a solution to promote social interaction while improving the cognitive abilities of seniors and creating awareness about fraud. The design requirements and direction were formed through multivariate research methods like literature review, user interviews, and focus group sessions. The proposed solution is Puppy valley, an app-mediated trivia board game that has a physical wheel and mobile application and uses narratives inspired by real fraud cases to create activities for users to play with. After the research and design, the game was evaluated by five experts, and refinements within the scope of the study were made based on the insights from the evaluation.</p>

Imagerie cérébrale du développement du contrôle inhibiteur et de son entraînement intensif à l'adolescence / Brain imaging of inhibitory control development and its intensive training at adolescence

Les fonctions exécutives (FE), et en particulier le Contrôle Inhibiteur (CI), jouent un rôle très important dans la réussite académique et professionnelle ainsi que dans la physiopathologie de nombreux troubles psychiatriques. L'adolescence est une période critique du développement du CI, ce dernier étant sous-tendu en particulier par la maturation tardive du cortex préfrontal jusqu’au début de l'âge adulte. Le premier objectif de cette thèse a été de cartographier les bases neurales du CI durant le développement et d'en évaluer leurs spécificités en les comparant avec celles de la mémoire de travail (MdT), une autre composante clef des FE. À partir d'une méta-analyse des études en IRMf du CI et de la MdT incluant 845 enfants, 1377 adolescents et 10235 adultes, nous avons identifié des modifications de l'activité fonctionnelle, à savoir le passage d'un réseau diffus à un réseau focal plus spécialisé avec l'âge, en accord avec un modèle dynamique du développement cérébral. Un large recouvrement de régions fronto-pariétales pour le CI et la MdT a également été détecté, ce qui soulève la question de la spécificité des processus et des tâches de ces deux FE. Par la suite, nous avons analysé l'effet à long terme du neuro-développement précoce sur le CI à partir de l'étude de la morphologie sulcale, un paramètre anatomique du cerveau déterminé lors de la vie fœtale. Dans un premier temps, nous avons montré, d'après une analyse longitudinale de 243 IRM, la stabilité du motif des sillons durant le développement. Nous avons par la suite établi que les polymorphismes sulcaux du cortex cingulaire antérieur et du sillon frontal inférieur contribuaient, de manière complémentaire, à l'efficience du CI chez l'enfant et également chez l'adulte. Enfin, nous nous sommes intéressés à l’entraînement cognitif au CI à l'adolescence, une période de très grande plasticité cérébrale et de sensibilité à l'environnement. Nous avons étudié chez 49 adolescents de 16-17 ans l'effet d'un entraînement intensif sur tablette tactile (25 sessions de 15 minutes par jour) au CI versus Contrôle Actif aux niveaux cognitif et cérébral (IRMf : tâches de stop-signal, de matrice de points, du réseau attentionnel et de gratification retardée). Nous avons en particulier évalué l'effet des facteurs neurodéveloppementaux précoces sur la réceptivité à l’entraînement au CI. Ces travaux s'inscrivent dans un nouveau champ de recherche interdisciplinaire à l'interface entre les neurosciences et la psychologie. Dans une perspective translationnelle éducative et thérapeutique, il vise à évaluer le plus finement possible, grâce à l'imagerie cérébrale anatomique et fonctionnelle, quelles interventions pédagogiques et thérapeutiques sont susceptibles d'aider au mieux le cerveau à surmonter des difficultés d'ordre cognitif. / Executive functions (EF), including Inhibitory Control (IC), play a major role in academic and professional achievement, as well as in the pathophysiology of many psychiatric disorders. Adolescence is a critical period in IC development as it is underlain by the protracted maturation of prefrontal cortex until early adulthood. The first objective of this thesis was to examine the neural bases of IC during development and to evaluate their specificities by comparing them with the working memory (WM), another key component of EF. Based on a meta-analysis of IC and WM fMRI studies including 845 children, 1377 adolescents and 10235 adults, we identified changes in functional activity with a shift from a diffuse to a more focal and specialized network with age. These results support the model of dynamic neurofunctional development. Moreover, a large overlap of fronto-parietal regions was found for IC and WM, which raises issues regarding the specificities of IC and WM processes and tasks. Second, we analyzed the long-term effect of early neurodevelopment on IC based on the sulcus morphology, an anatomical brain feature determined during fetal life. We showed, using a longitudinal analysis of 243 MRIs, that folding patterns are fixed from childhood to adulthood. Subsequently, we established that the sulcal polymorphisms of the anterior cingulate cortex and the inferior frontal sulcus complementary contributed to IC efficiency in both children and adults. Finally, we studied IC training in adolescence, a period of high brain plasticity and environmental sensitivity. We examined the effects of an intensive IC training (25 sessions of 15 minutes per day) versus active control training group on touchscreens in 49 adolescents (16-17 years-old) on cognitive and brain levels (fMRI: stop-signal, dot matrix, attentional network and delayed gratification tasks). We also assessed the effect of early neurodevelopmental factors on IC training receptivity. This thesis is part of a new field of interdisciplinary research, at the interface between neurosciences and psychology. It includes translational educational and therapeutic perspectives, with aims at evaluating as finely as possible, using anatomical and functional brain imaging, what pedagogical and therapeutic interventions are likely to help the brain to overcome cognitive difficulties.

Using non-medical risk factors related to dementia and cognitive decline for developing an evidencebased e-health tool

The number of dementia cases is increasing worldwide. Most research and development in this area is related to the prevention of dementia, and to the development of various prediction tools for dementia. The tools made available take most of the medical data into account while calculating risk scores, with only a small amount of non-medical data. There is a lot of data related to medical and non-medical risk factors available from various sources which can be retrieved and analysed in real time, but this is today not used in any risk score tool for risk score calculation. As part of the project Multimodal strategies to promote a healthy brain in ageing: Innovative evidence-based tools (MULTI-MODE), a new risk score is being developed to be used in a new ICT-based tool for dementia prediction. Identification of non-medical data and a good model to fill the gap between data available at the server and using this data in risk score calculation may help in increasing the predictability of tools. In this thesis, some of the existing risk factors for the prediction of dementia are described, and the importance of non-medical factors in calculating risk scores is discussed. Additional non-medical factors are identified that could be included in future versions of the risk score. A database design for storing risk score information efficiently is presented, as is an app structure that can be used at the server side to validate the user input and to increase the effectiveness of a prediction tool. / Antal demensfall ökar över hela världen. Forskning och utveckling inom detta område är relaterat till att förebygga demens och att utveckla olika prognosverktyg för demens. Flera tillgängliga verktyg tar hänsyn till medicinska data i beräkning av riskpoäng, med endast en liten mängd av icke-medicinska data. Det finns en hel del data om medicinska och icke-medicinska faktorer online, men de används idag inte för riskpoängberäkning. Som en del av projektet Multimodala strategier för att främja en frisk hjärna i åldrande: Innovativa evidensbaserade verktyg (MULTI-MODE), så har en ny metod utvecklats för att användas i ett nytt IT-baserat verktyg för demensförutsägelse.
