The amateur writes back : new theoretical directions for progressive left politics and social policy.

This work develops an opportunity for transgressive resistance to discursively formed structures of material and theoretical power and closure, based on a methodology of amateurism. The concept of amateurism draws heavily on the writing of Edward Said. This work synthesises Said with a broader corpus of postcolonial theory, following a theoretically postcolonial trajectory which applies the lessons from that referent to an engagement with traditional theoretical and cultural closure. The central thesis of the engagement follows a critique of strong ontology and vertical epistemology, or of expertise. Through an examination of health policy around birth, and sociological approaches to health, that critique is deployed to invigorate a new critical direction for the Left with a focus on subjectivity, social policy, social democracy and substantive citizenship. / Thesis (Ph.D.) - University of Adelaide, School of History and Politics, 2008

A intenção deste trabalho é compreender a formação de uma rede de debates intelectuais entre dois grupos da esquerda anglófona articulados em torno do Left Book Club, de Londres e em torno da revista Partisan Review, de Nova York durante os anos da Segunda Guerra Mundial. Entendo que, especialmente na primeira metade da década de 1940, os contatos entre estes dois núcleos da esquerda anglo-saxã se estreitaram em dois caminhos: por um lado, por uma crítica comum ao stalinismo; e, por outro, por uma crítica à esquerda da cultura nativa (e tradições burguesas) em ambos os países. Defendemos ainda que essas propostas articuladas por dentro dos movimentos intelectuais de esquerda nos dois países, ora baseadas ideologicamente numa releitura nãoinstitucionalizada do marxismo, ora no trotskismo, ora no pacifismo, ora na reconstrução de um discurso liberal, se aglutinavam em torno da contestação dos rumos do movimento comunista internacional, liderado pelo stalinismo. A articulação dos debates entre esses dois grupos se reforçará no momento em que o célebre romancista anglo-indiano George Orwell, famoso por duas das distopias mais famosas do século XX, o Animal Farm (1945) e o 1984 (1949), contribui simultaneamente com ambos os grupos. Na Partisan Review, Orwell ficara encarregado da publicação da coluna London Letters to Partisan Review, entre os anos de 1941 e 1946. A possibilidade de um diálogo franco com ambos os círculos, especialmente pela rede costurada com membros de ambos os grupos, possibilitou a Orwell criar uma ponte de contatos entre os dois ambientes. Entendo ainda que essa articulação construída por Orwell foi decisiva nas futuras participações de outros intelectuais, que já haviam publicado no Left Book Club e que participavam ativamente dos debates britânicos, como contribuintes da Partisan Review. / The intention of this work is to understand the constitution of a network of intellectual debates articulated among two groups of the Anglophone left around the London\'s Left Book Club, and around the New York\'s Partisan Review, during the years of World War II. I understand that, especially in the first half of the 1940s, contacts between the two cores of the Anglo-Saxon left narrowed in two ways: firstly, by a common critique of Stalinism; and on the other, a left-wing critic of the native culture (and bourgeois traditions) in both countries. We argue that those proposals articulated inside the left-wing intellectual movements in the two countries, based ideologically on a non-institutionalized reinterpretation of Marxism, or in Trotskyism, or in pacifism, or in rebuilding a liberal discourse, coalesced around the challenge of the international communist movement course, led by Stalinism. The joint discussions among those two groups will be strengthened at the time that the famous Anglo-Indian novelist George Orwell, famous for two of the most famous dystopias of the twentieth century, Animal Farm (1945) and 1984 (1949), contributes simultaneously with both groups. In Partisan Review, Orwell had been in charge of publishing the column \"London Letters to Partisan Review\", between the years 1941 and 1946. The possibility of an open dialogue with both circles, especially the stitched network with members of both groups, enabled Orwell to create a bridge between the two environments. I also believe that this articulation builted by Orwell was decisive in the future participation of other intellectuals, who had already published through the Left Book Club and participating actively in British debates, as contributors of Partisan Review.

[pas de résumé] / Jean Charbonnel was part of that generation of "young wolves", sponsored by Georges Pompidou, which - from the beginning of the 1960s - embarked on the conquest of those parts of the country that were hostile to Gaullism. That was how Jean Charbonnel succeeded in being elected a "député" in a part of the Corrèze that was "radical-socialiste", in which the initial contacts with local politics were more than difficult. With that victory on which to build, Jean Charbonnel made for himself a real stronghold in the south of the department (the "Pays de Brive") where he became a "conseiller général" (1964-1968), mayor of Brive (1966-1995), the initiator of the first forms of "intercommunalité", and a deputy for a total of fifteen years. With this base of practical experience on the ground, his Gaullism became clearly orientated towards a search for, and the defence of, a better social justice through the realisation of novel mechanisms whose aim was the integration of handicapped schoolchildren, and of young people into the world of work, or through the responsibility that was taken for older members of the community with the construction of "foyers-logements" (sheltered housing). Such local social action was echoed on the national level, particularly when Jean Charbonnel became "Ministre du Développement industriel et scientifique" (1972-1974), within the context of the "affaire Lipp" when he tried to obtain true recognition for the eminently "gaullien" principle of Participation, taking into account the interests of the workers, of the salaried employees and of the management. Jean Charbonnel's political action claimed to be resolutely progressive but above all reformist as it sought to associate the citizen - according to the place that he actually occupied within the Community - with the great reforms of the country. This thesis seeks therefore to comprehend how a Gaullist managed to establish himself in a left-wing area - the "Pays de Brive" -and the way in which his action made of him a left-wing Gaullist or rather a "gaulliste social" because he was intellectually nearer the Right than the Left, and above all because he never sought to break away from mainstream gaullism, by which he was quite different from the left-wing Gaullists

Non-Syndromic atrioventricular septal defects: a refined definition, associated risk factors, and prognostic factors for left atrioventricular valve replacement following primary repair

Congenital heart defects (CHDs) constitute a major proportion of clinically significant birth defects and are an important component of pediatric cardiovascular disease. Atrioventricular septal defects (AVSDs) include a range of anomalies characterized by atrial, ventricular, and atrioventricular (AV) valve defects. AVSDs commonly occur in the presence of a syndrome, most frequently Down syndrome; they also occur in isolation and are referred to as non-syndromic AVSDs (NSAVSDs). These studies were performed to evaluate for presence of an intermediate phenotype in parents and siblings of a child with a NSAVSD, risk factors associated with NSAVSDs, and prognostic risk factors for left AV valve replacement following primary repair of an AVSD. It was shown that the mean body surface area-standardized AV septal length (AVSL) was significantly shorter in the NSAVSD parents and siblings than in parents and siblings of syndromic AVSD case and control children. Using age- and gender-adjusted body surface area-standardized AVSL, it was determined that there was evidence for two component distributions in parents and siblings of NSAVSD children, suggesting the presence of an intermediate. Broadening the definition of AVSD to include those with a shortened AVSL may increase the power of genetic association and mapping studies to identify susceptibility genes. Risk factors associated with NSAVSD were examined using the 1997-2005 National Birth Defects Prevention Study database. Mothers who actively smoked or were exposed to passive smoke anytime from one month prior to pregnancy through the end of the first trimester were more likely to have an infant with a NSAVSD. There was a suggestive association between AVSDs and use of antibacterial, antifungal, and antiviral medications. Additional investigations are warranted to investigate associations with specific medications as well as to uncover possible gene-environment interaction effects that may modify these risks in order to develop improved primary prevention strategies. Using the Pediatric Cardiac Care Consortium database, factors associated with time to first reoperation and time to replacement following primary AVSD repair were evaluated. Type of AVSD repair, closure of the mitral valve cleft, moderate to severe postoperative left AV valve regurgitation, and presence of postoperative complete heart block were associated with earlier time to reoperation after adjusting for age and weight at AVSD repair. Down syndrome and presence of postoperative mitral stenosis were associated with earlier time to replacement. Prognostic risk factors following left AV valve replacement in children who had previously undergone AVSD repair were also identified. A prosthetic valve size to body weight ratio of greater than 3 and the presence of Down syndrome were identified as predictors of in-hospital death following left AV valve replacement. By adding to our knowledge of the AVSD familial and environmental risk factors from these studies, we will be able to (1) improve genetic counseling, (2) identify other family members for genetic testing, (3) begin to devise primary prevention strategies, and (4) improve treatment modalities. By recognizing prognostic factors which influence survival, optimal patient care can be devised which will not only improve treatment modalities, but also long-term survival.

The Effects of Spatial and Temporal Properties on a Viscoelastic Model of the Dyssynchronous Canine Heart

In this study, lumped parameter cardiovascular modeling has been used to understand the influence of muscle properties on mechanical dyssynchrony (MD) as well as general muscle dynamics. Incorporating viscous influence into the model allowed for an expanded view when analyzing muscle parameter response to MD. A unique method of ventricle segmentation was introduced that allowed fast analysis of regional and global ventricular properties. This segmentation process produced a ventricle with four identical sections each consisting of separately tunable muscle properties in the form of minimum and maximum elastance, elastance waveform delay, and myocardial viscous friction, yet these regional sections remained globally dependent. Elastance waveform delay proved to be the most influential property on MD as measured by internal flow fraction (IFF), followed by regional elastance magnitude, and finally regional viscosity influence. Due to the unique segmentation of this model, two metrics for IFF were derived: (1) the "true" IFF (IFF-4seg) and (2) the IFF as would be measured by an ideal conductance catheter (IFF-CC). The results of IFF-CC versus IFF-4seg show that conductance catheters are not capable of measuring IFF during a side-to-side volume transfer within the stacked cylinder under measurement. Finally, unique energetic situations were observed with this model that point to likely myocardium remodeling situations.

The androgyny of the Gethenians in The Left Hand of Darkness is a vehicle for Ursula Le Guin's rhetoric concerning gender roles. Le Guin attempts to make the reader identify with an ideal form of androgyny, through which she argues that many of the problems of human existence, from rape and war to dualistic thought and sexism, are products of gender roles and would be absent in an androgynous world. The novel also links barriers of separation and Othering with masculine thought, and deconstructs these separative boundaries of opposition, while promoting connective borders which acknowledge difference without creating opposition. The novel thus criticizes gendered thought processes and social roles, because they lead to opposition and separation.

Transmission Probability of Embolic Debris Through the Aortic Arch and Daughter Vessels During a Transcatheter Aortic Valve Replacement Procedure

Cerebral ischemia leading to an ischemic stroke is a possible complication of a transcatheter aortic valve replacement (TAVR) procedure. This is because embolic debris can become dislodged and travel through the aortic arch, where they either continue to the descending aorta and join the systemic circulation or travel into the cerebral vasculature through the three daughter vessels that branch off the top of the aortic arch. These three vessels are the brachiocephalic artery, the left subclavian artery, and the left common carotid artery. These three vessels lead either directly or indirectly to the cerebral vasculature, where the diameter of vessels become very small. If a large enough embolus travels into the cerebral vasculature, it can become stuck in the small cerebral vessels, blocking blood flow and cutting off the supply of oxygen to brain cells. The purpose of this study is to expand upon previous work in order to 1) create a more accurate physics simulation of blood and debris flow through the aortic arch 2) report on embolic debris distribution through the aortic arch and 3) analysis on which physical parameters affect embolic debris distribution. The physical parameters analyzed were particle diameter and particle density. This study was performed by creating a finite element model in COMSOL Multiphysics™ using a SolidWorks model of an aortic arch, with dimensions taken from a patient’s CT scan. Computational fluid dynamics was performed using a pulsatile pressure waveform throughout the aortic arch with a non-constant viscosity model. Once the velocity profile through the aortic arch matched with value ranges from literature, the particle tracing study was implemented. Both a pulsatile pressure waveform and a constant pressure model were analyzed, as well as a constant viscosity model and a non-constant viscosity model. The pulsatile pressure waveform influenced particle distribution and is recommended for future studies since this model leads to pulsatile flow, which is representative of flow through the aorta. It was seen that the non-constant viscosity model did not have a large effect on the velocity profile, but more than doubled the surface average value of viscosity. It also had an effect on the particle distribution through the aortic arch. Small diameter emboli were more likely to flow into the descending aorta, the brachiocephalic artery, and the left subclavian artery; larger emboli were more likely to flow into the left common carotid. Lower density emboli were more likely to flow into the descending aorta and the brachiocephalic artery. Averaging all densities and sizes, it was determined 44.8% of emboli flow into the three daughter vessels, but ultimately only 30.61% of emboli flow into the cerebral vasculature and have the potential to cause an ischemic stroke.

Uticaj dnevno-noćnog ritma arterijskog krvnog pritiska na funkciju i geometriju leve pretkomore i komore srca / Influence of circadian rhythm of arterial blood pressure on function and geometry of left atrium and ventricle of the heart

<p>Cilj: Cilj ovog istraživanja bio je da se utvrde promene koje arterijska hipertenzija dvojakim mehanizmima (dnevno-noćnim ritmom i svojim trajanjem) ostavlja na funkciju i geometriju leve pretkomore i komore srca. Ispitanici i metode: u ovo istraživanje bilo je uključeno 180 ispitanika koji su bili podeljeni u ispitivane grupe u odnosu na dnevno-noćni ritam arterijskog krvnog pritiska tokom 24-časovnog monitoringa arterijskog krvnog pritiska i to na sledeći način: grupa ispitanika sa očuvanim dnevno-noćnim ritmom arterijskog krvnog pritiska sastojala se od ukupno 90 ispitanika, a grupu onih sa naru&scaron;enim dnevno-noćnim ritmom arterijskog krvnog pritiska takođe je činilo 90 ispitanika. U svakoj od ovih grupa izvr&scaron;ena je dodatna podela ispitanika prema dužini trajanja arterijske hipertenzije na one kod kojih je arterijska hipertenzija trajala do 5 godina, one kod kojih je trajala od 5 do 10 godina i na one sa trajanjem arterijske hipertenzije vi&scaron;e od 10 godina. Svim ispitanicima rađen je incijalno 24-časovni ambulantni monitoring arterijskog krvnog pritiska, a nakon toga i ehokardiografski pregled radi određivanja parametara morfologije leve pretkomore i komore, kao i dijastolne funkcije leve komore. Rezultati i diskusija: nakon statističke obrade podataka dobijeni su rezultati koji ukazuju da se dijastolna disfunkcija stepena većeg od I statistički značajno če&scaron;će (p=0,011) uočava kod onih ispitanika koji nemaju očuvan dnevno-noćni ritam arterijskog krvnog pritiska. Naru&scaron;en dnevno-noćni ritam arterijskog krvnog pritiska takođe dovodi do povećanja indeksirane mase miokarda leve komore prema povr&scaron;ini tela ispitanika (LVM/BSA) &ndash; p=0,001; do zadebljanja zidova leve komore (IVS/PLW) &ndash; p=0,025, kao i do smanjenja sistolne brzine miokarda na nivou septalnog dela mitralnog anulusa (s&rsquo;) - p&lt;0,0005. Pored ovoga, u grupi onih sa naru&scaron;enim dnevno-noćnim ritmom arterijskog krvnog pritiska primećeno je če&scaron;će prisustvo ekscentrične hipertrofije miokarda leve komore (p=0,027). U odnosu na geometriju i funkciju leve pretkomore, naru&scaron;en dnevno-noćni ritam arterijskog krvnog pritiska dovodi do promena u smislu povećanja antero-posteriornog dijametra leve pretkomore (LA)-p=0,003; maksimalnog volumena leve pretkomore u komorskoj sistoli (LAVs) &ndash;p=0,007; indeksiranog LAVs prema povr&scaron;ini tela ispitanika (LAVs/BSA)-p&lt;,0005; E/e&rsquo; odnosa &ndash;p=0,040; rezervoarnog strain-a leve pretkomore (PLAS)- p=0,004; krutosti leve pretkomore (stiffness)-p=0,047, kao i brzine propagacije talasa kroz mitralni otvor (Vp)-p=0,029. Sa povećanjem dužine trajanja arterijske hipertenzije takođe su pokazane promene u morfologiji i funkciji leve pretkomore i leve komore srca. Dijastolna disfunkcija stepena većeg od I retko je bila uočena u grupi onih koji su arterijsku hipertenziju lečili kraće od 5 godina, a njeno prisustvo u ispitivanim grupama trajanja od 5 do 10 godina i duže od 10 godina bilo je statistički značajno če&scaron;će (p&lt;0,0005). Takođe, sa trajanjem arterijske hipertenzije statistički značajno se menjaju i parametri leve komore za koje smo pokazali da su pogor&scaron;ani kada dnevno-noćni ritam arterijskog krvnog pritiska nije očuvan, s tim &scaron;to smo ovde uočili i značajne promene u smislu postojanja lo&scaron;ijih vrednosti strain-a leve komore (LVGS)-p&lt;0,0005. Na sličan način kao i u odnosu na dnevno-noćni ritam arterijskog krvnog pritiska, menjao se predominantni oblik hipertrofije miokarda leve komore, te je uočeno značajno če&scaron;će (p=0,017) prisustvo ekscentrične hipertrofije kod ispitanika koji su duže lečili arterijsku hipertenziju. Uz ove promene, sa trajanjem arterijske hipertenzije, uočene su i promene sledećih ehokardiografskih parametara leve pretkomore (p&lt;0,0005): LA, LAVs, LAVs/BSA, E/e&rsquo;, PLAS, stiffness, Vp. Na kraju, formiran je i model za dijagnostiku dijastolne disfunkcije stepena većeg od I koji je testiran na 30 novih ispitanika i koji se pokazao kao kvalitetan uz visoku senzitivnost i specifičnost. Zaključak: arterijska hipertenzija dnevno-noćnim oscilacijama (ritmom), ali i svojim trajanjem dovodi do promena u funkciji i geometriji leve pretkomore i komore srca. Noviji ehokardiografski parametri (PLAS, LVGS, stiffness) su dobri pokazatelji ranih promena na nivou leve pretkomore i komore srca kod pacijenata sa arterijskom hipertenzijom.</p> / <p><!--[if gte
