[pt] No Brasil, as Infecções Hospitalares se constituem na principal causa de
mortalidade de pacientes internados, gerando grandes prejuízos financeiros. Em
2008, a ANVISA publicou alerta de emergência epidemiológica devido à
ocorrência de infecções por micobactérias de crescimento rápido em todas as
regiões do país, infecções fortemente relacionadas a falhas nos processos de
limpeza, desinfecção e esterilização de produtos médicos. A validação é uma
ferramenta da metrologia que possibilita estabelecer se o processo de
esterilização é executado de modo eficaz e reprodutível. Na norma ISO/TS
17665-2:2009 sobre validação da esterilização de produtos para saúde, se
destacam recomendações de revalidações periódicas, distribuição espacial de
sensores térmicos, critérios de aceitação quanto à temperatura calculada a partir
da pressão medida na câmara e estimativa de incerteza de medição. Com base
nos requisitos, recomendações, orientações e exigências descritas em
documentos nacionais e internacionais, no presente trabalho se desenvolvem
sistema e procedimento para avaliação da confiabilidade metrológica de
esterilizadores por vapor saturado. Com o sistema foram avaliadas quatro
autoclaves gravitacionais em uso hospitalar e industrial. Os resultados reforçam
a importância de aspectos como: revalidações, condições ambientais e impacto
da estabilidade temporal e uniformidade espacial na incerteza global das
câmaras. Em todas as autoclaves avaliadas o processo se realizou por vapor
superaquecido, que é menos eficiente e não-conforme à ISO 17665. As
limitações identificadas corroboram com os novos requisitos da ANVISA (RDC
15:2012), tanto com relação à exigência de avaliações periódicas de
esterilizadores, quanto à proibição do uso de autoclaves gravitacionais com
capacidade superior a 100 L. / [en] In Brazil, the Hospital Infections are the main cause of mortality during
hospitalization, generating huge financial losses. In 2008, ANVISA issued a
warning of epidemiological emergency due to the occurrence of infections by
rapidly growing mycobacteria in all regions of the country, strongly related to
failures in the processes of cleaning, disinfection and sterilization of medical
products. Validation is a metrological tool that allows establishing whether the
sterilization process developed is running efficiently and with reproducibility.
ISO/TS 17665-2:2009, on validation of sterilization products for health,
highlights the need of periodic revalidations, measurements of temperature with
sensors distributed throughout the chamber volume, specifies acceptance criteria
based on measurements of pressure in the internal volume of the chamber
(theoretical temperature) and introduces the measurement uncertainty
requirement. In this work, based on the requirements and recommendations
outlined in national and international documents, a system and procedure are
developed for the metrological reliability evaluation of saturated steam
sterilizers. With the developed system, four gravitational autoclaves were
evaluated. The results reinforce the importance of revalidation, environmental
conditions, and the contribution of temporal stability and spatial uniformity in
the overall uncertainty of the cameras. All the autoclave process was carried out
by means of superheated steam, which is less efficient and non-conform to ISO
17665. The limitations identified corroborate with the recently published
requirements of RDC ANVISA 15:2012, concerning both the need of periodic
evaluations of sterilizers and the prohibition of the use of gravitational
autoclaves with capacity exceeding 100 L.

[pt] O presente trabalho visa desenvolver um sistema de avaliação metrológica de autoclaves, que atenda às normas técnicas vigentes, de baixo custo e acessível a todos Estabelecimentos Assistenciais de Saúde (EAS), visando à validação do processo de esterilização por vapor saturado e a melhoria no controle de infecção hospitalar associada a problemas decorrentes do processo de esterilização. Por meio de uma abordagem matemática, o sistema deverá ser capaz de validar o processo de esterilização, garantindo assim o nível de esterilidade necessário e com isso reduzir as barreiras tecnológicas e financeiras que hoje impedem a garantia da qualidade da esterilização de materiais, sobretudo, em EAS, tanto público como privado. Apesar de instituições como a Organização Internacional de Metrologia Legal (OIML) externarem sua preocupação no que diz respeito à necessidade do acompanhamento da qualidade das medições realizadas por Equipamentos Eletromédicos (EEM) na fase pós-comercialização, a maioria dos países, entre eles o Brasil, apesar de já terem incorporado algumas recomendações publicadas pela OIML, têm concentrado suas exigências da fase de fabricação ao registro do produto. As iniciativas de calibração e ensaios de EEM dependem da cultura metrológica dos interessados ou na busca de algum tipo de acreditação, para a qual o controle metrológico dos equipamentos vem a ser exigência obrigatória. Aliado à falta de cultura metrológica depara-se com a existência de poucas instituições capacitadas para a realização desta tarefa e com a falta de recursos disponíveis. A exemplo dos EEM, no caso dos esterilizadores por vapor saturado ou autoclaves, existe apenas um único laboratório acreditado junto à Rede Brasileira de Laboratórios de Ensaio (RBLE), acreditado em janeiro de 2008. Utilizando-se dos estudos da esterilização, em particular das autoclaves e da Norma NBR ISO 11134/2001. Foi desenvolvido um sistema destinado à validação metrológica de autoclaves. Este sistema é composto de um Hardware (sensores, sistema de aquisição e computador portátil) e de um software desenvolvido em LabView. Neste sistema os sensores são posicionados no interior da câmara de esterilização, coletando a temperatura ao longo de todo o ciclo de esterilização. Com as temperaturas coletadas, tanto nos estudos de distribuição de temperatura como nos estudos de penetração de calor, foi possível calcular as taxas de letalidade do processo e apresentar os dados estatísticos, que confirmarão a efetividade do processo ou não. Após a realização de testes visando à comprovação do funcionamento do sistema, foi realizado o teste em um equipamento utilizado em um hospital público do Rio de Janeiro. Os testes compreenderam a realização de quatro estudos sendo, dois de distribuição de temperatura e dois de penetração de calor. Foram encontradas algumas limitações decorrentes da necessidade de desenvolvimento de alguns acessórios como, por exemplo, um flange para a utilização do sistema em autoclaves com acesso à câmara através de orifícios laterais, e a necessidade da construção de uma armação articulada para a perfeita disposição dos sensores no interior da câmara. O sistema desenvolvido no presente trabalho contribui para a disseminação da realização da validação do processo de esterilização por vapor saturado, avaliação fundamental para o
controle da infecção hospitalar. / [en] The present work aims at developing a system for metrological evaluation of autoclaves in accordance with the standards, having a low cost and being accessible to all the healthcare system, aiming at the validation of the process of sterilization and the improvement of the control of hospital infection. By means of a mathematical approach, the system has to be capable to validate the sterilization process, thus guaranteeing the level of sterility necessary and reducing the technological and financial barriers that today hinder the guarantee of the quality of the sterilization of materials, in public and private health system. Although organizations as the International Organization of Legal Metrology (OIML) recommend the follow-up of the quality of the measurements carried through Medical Electrical Equipment (MEE) in the after-sales phase, the majority of the countries, including Brazil, even though already having incorporated some recommendations published by OIML, have concentrated its requirements on the phase of manufacture and register of the product. The initiatives of calibration and essays of MEE depend on the metrological culture of the interested parties or in the search of some type of accreditation process, for which the metrological control of the equipment is a requirement. Ally to the lack of metrological culture comes across itself with the existence of few institutions enabled to the accomplishment of this task and with the lack of available resources. In the case of the autoclaves, in Brazil, there was only one laboratory accredited in last January. A system destined to the metrological Evaluation of autoclaves was developed. This system is composed by hardware (sensors, system of acquisition and computer) and software, developed in LabView. In this system the sensors are arranged inside of the sterilization chamber, collecting the temperature throughout the sterilization cycle. With the collected temperatures, as much in the studies of distribution of temperature as in the studies of penetration of heat, it is possible to calculate the taxes of lethality of the process and to present the statistical data, that will confirm or not the effectiveness of the process. After the tests aiming at evidencing the correct operation of the system, it has been carried out the test in an equipment used in a public hospital of Rio de Janeiro. The tests consisted of 4 (four) studies, 2 (two) of distribution of temperature and 2 (two) of penetration of heat. Some limitations due to the necessity of
development of some accessories have been found, for example, a flange for the use of the system in sterilizers with access to the chamber through lateral orifices, and the necessity of the construction of a articulated scaffolding for the perfect placement of the sensors in the interior of the chamber. The system developed in the present work contributes for the dissemination of the accomplishment of the validation of the process of sterilization for saturated vapor, basic evaluation for the control of the hospital infection.

[pt] Esta dissertação analisará a (re)produção das formas de controles de corpos
e sexualidades em diferentes espaços e tempos. Tal análise é possível através do
entendimento, desenvolvido em trabalhos anteriores, de que o pensamento moldado
pela Inquisição e a caça às bruxas na Idade Média foi reproduzido durante a
construção do Brasil enquanto sociedade. No trabalho presente, será somada a tal
entendimento a hipótese de que o pensamento medieval continua permeando o
imaginário social da sociedade brasileira na atualidade. Para tal, serão analisadas
práticas de esterilização involuntárias de mulheres negras no Brasil a partir da
década de 1980; assim como, discursos e práticas violentas contra a comunidade
LGBTQIA+ (Lésbicas, Gays, Bissexuais, Travestis/Transexuais/Transgêneros,
Queers, Intersexuais, Agêneros/Assexuais/Arromânticos e outras possibilidades de
orientação sexual e identificação de gênero) perpetrados por grupos contra a
ideologia de gênero, representados aqui em grande parte por membros do
governo Bolsonaro. Com base em correntes teóricas feministas, principalmente dos
feminismos negro e decolonial, serão ressaltados os efeitos das posicionalidades de
gênero, sexualidade e raça dos sujeitos alvos de violência. / [en] This dissertation will analyze the (re)production of forms of control of
bodies and sexualities in different spaces and times. Such an analysis is possible
through the understanding, developed in previous works, that the thought shaped
by the Inquisition and the witch hunt in the Middle Ages was reproduced during the
construction of Brazil as a society. In the present work, it will be added to this
understanding the hypothesis that the medieval thought continues to permeate the
social imaginary of Brazilian society today. To this end, involuntary sterilization
practices of black women in Brazil from the 1980s will be analyzed; as well as,
violent speeches and practices against the LGBTQIA+ (Lesbians, Gays, Bisexuals,
Trans/Transsexual/Transgender, Queer, Intersex, Asexuals/Aromantics and other
possibilities of sexual orientation and gender identification) community perpetrated
by groups against the gender ideology, represented here largely by members of
the Bolsonaro government. Based on feminist theoretical currents, mainly black and
decolonial feminisms, the effects of the gender, sexuality and race positions of the
subjects targeted by violence will be highlighted.
