The effects of progressive resisted exerxcises on performance-oriented mobility in persons with HIV related poly-neuropathy

Background: Distal symmetrical poly-neuropathy (DSP) has emerged as one of the major neurological complication associated with HIV/AIDS and antiretroviral therapy. People with DSP commonly have problems with pain, mobility, altered gait and balance all which affect their quality of life. While therapeutic strengthening exercise has been reported to attenuate these impairments in other co-morbid conditions like diabetes mellitus and in HIV/AIDS, there is no evidence available on the effects of exercise on DSP in people living with HIV/AIDS (PLWHA).The purpose of this study was to determine the effects of progressive resisted exercises (PRE) on performance oriented mobility and health related quality of life in (PLWHA) related DSP. Objectives of this study were to determine the effects of PRE on gait, balance and pain levels and establish if there is a relationship between performance-oriented mobility and health-related quality of life in PLWHA related DSP. Methods: In order to fulfil the objectives, an assessor-blinded randomized controlled trial was conducted over two studies, with a combined sample of 160 participants sourced from two family care clinics at two central hospitals and ten anti-retroviral therapy dispensing municipal clinics in Harare, Zimbabwe. While the experimental group with 80 participants had an intervention program of PRE sessions of one hour for the lower limbs, done twice per week over 12 weeks, the control group of 80 participants was given advice to walk unsupervised at home. Loss to follow up in this study was at 60% (n=97) and the data was analysed using an intention to treat analysis approach. Results: Participants of an average age of 42.2 years (SD=8.5) constituted of 70.6% (n=113) female participants. Combination antiretroviral therapy containing stavudine, was used by 59% (n=94) of the participants and 59% (n=94) of the participants had moderate to severe neuropathy. Proximal muscles exhibited weakness (hamstring muscles strength = 3.43 kg force (SD=1.5)) when compared to leg muscles (gastrocnemius muscles strength = 12.8 kg force (SD=2.0)). Gait and balance scores did not show differences in effect between the intervention and the control group (95%CI 0.00-0.02, p = 0.8). Similarly there were no differences of effect for muscle strength (95%CI 0.00-0.08, p=0.13-0.8) and pain (95%CI 0.0-0.06, p>0.13). However the effect on quality of life changes were significantly different between the two groups (95%CI 0.00-0.12 p= 0.04). Quality of life was positively associated with gait, odds ratio 1.01 (95%CI 1.00 – 1.04), moderately associated with balance odds ratio 0.68, (95%CI 0.52 – 0.93) negatively associated with pain odds ratio 0.98 (95%CI 0.97 – 0.99). Conclusion: This research study established that progressive resisted exercises have positive effects on the health related quality of life in PLWHA related DSP. However this study did not show a difference of the effects of progressive resisted exercises on performance oriented mobility in PLWHA related DSP when compared to advice to exercise at home. The study findings may not be generalized to all individuals living with HIV/AIDS who have DSP as the participants were from a particular demographic setting. This project may be continued at the participating family care clinics as a roll on of the perceived benefits of exercise for people with HIV related DSP.

La maîtrise de la température des composants à forte dissipation, notamment dans les systèmes électroniques nomades, constitue un verrou à leurs développements. Que ce soit pour l'électronique de puissance ou les calculateurs, les densités de puissance requièrent l'utilisation de systèmes de refroidissement de plus en plus performants, en particulier dans le cas des microprocesseurs qui associent miniaturisation et augmentation des fréquences d'horloge. Les conséquences sont multiples, limitation des performances, augmentation de la consommation et du taux de défaillance. C'est dans ce contexte que le projet CANOPEE, réunissant un consortium de partenaire industriel et académique, propose de développer et démontrer les avantages d'une solution technologique active récemment brevetée et appelée OnduloTrans. Elle consiste en un dispositif échangeur-pompe, permettant à la fois d'obtenir un excellent transfert thermique et d'assurer le pompage du fluide caloporteur. OnduloTrans est une solution active de refroidissement basée sur la déformation dynamique d'une paroi d'un canal. Le dispositif est fixé directement à l'aplomb du composant à refroidir. Le canal est déformé suivant une onde progressive pour créer un pompage péristaltique. L'intensification des transferts est obtenue lorsque les variations dynamiques des dimensions du canal viennent perturber la couche limite à l'interface conduction/convection. Le travail de la thèse consiste à concevoir et développer une solution d'actionnement intégrée permettant de mouvoir la paroi souple. Il s'accompagnera d'essais concrets témoignant des performances thermiques de cette solution. L'actionnement mis en oeuvre doit respecter les contraintes hydrauliques et thermiques de l'application embarquée visée, tout particulièrement le faible encombrement et une consommation électrique minimale. La difficulté réside dans les critères antagonistes que sont la production d'une onde progressive de grande amplitude dans un milieu aux dimensions centimétriques. Le manuscrit s'articule en trois parties. Dans un premier temps les solutions d'actionnement envisageables seront explorées. Pour ce faire, la solution OnduloTrans est d'abord décrite. Un état de l'art des solutions de conversion électromécanique, puis des micropompes péristaltiques est entrepris. Les phénomènes hydrauliques propres au micropompage péristaltique sont abordés pour cerner la problématique du développement de l'actionnement. Ensuite les deux chapitres suivants détaillent l'étude de deux solutions piézoélectriques distinctes. Une première solution piézoélectrique à onde discrète est développée dans le second chapitre. Elle consiste en une répartition d'actionneurs flextensionnels le long de la paroi souple. Une méthodologie de prédimensionnement basée sur des modèles mécaniques simples est présentée. L'onde progressive créée étant très particulière, une méthode analytique de calcul du débit, validée par simulations numériques, permet d'étudier l'influence de la commande des actionneurs. Un prototype est finalement réalisé avec l'aide des partenaires. De nombreux essais sont ensuite réalisés afin de valider les différentes hypothèses et déterminer les premières performances hydrauliques et thermiques du dispositif. Le troisième chapitre aborde une solution à onde progressive continue et actionnement intégré à la membrane. Le but est ici de prouver le concept de pompage péristaltique par flexion contrôlée d'une plaque intégrant une couche piézoélectrique. Un modèle de dimensionnement 1D constitué de tronçons piézoélectriques répartis à la surface est tout d'abord présenté. La répartition des segments piézoélectriques fait ensuite l'objet d'une étude paramétrique afin de définir judicieusement leur disposition, ceci en vue de maximiser le débit théorique. Cette étude paramétrique est finalement couplée à une optimisation des commandes électriques, évaluée par les résultats d'un prototype.

The intellectual development of Charles A. Beard, 1874-1923

This dissertation traces the development of Charles A. Beard’s social, political, legal and historical thought. It covers his early education in Indiana, his cosmopolitan postgraduate studies at the University of Oxford, Cornell University and Columbia University, and professional work as political scientist and as expert on municipal government. By following thematically and chronologically the relationship between Beard’s historical writings and his early life as teacher of politics and government, this dissertation offers both a reinterpretation of the meaning of Beard’s interpretation of the origins of the U.S. Constitution and a glimpse of the shifting intellectual trends in political thinking that emerged during the Progressive era. Contrary to the idea that Beard was a moral absolutist interested in denouncing the interference of economic interests in government, this thesis argues that Beard developed a pluralistic, functionalistic, and anti-majoritarian conception of politics that was at odds with many Progressive thinkers. Most previous research on Beard has lacked archival research and has ignored Beard’s teachings on politics at Columbia University, thus projecting into Beard’s thought concepts and values he did not adopt. In this study Beard appears as an early advocate of a new pluralistic ethics and utilitarian morality that allowed him to picture the framers of the Constitution as modern pragmatic politicians interested in creating a strong government by the art of integrating the major economic interests of the society in the process of law-making. This dissertation also reveals a broader intellectual world informing Beard’s scholarly work and highlights his readings in modern sociology at DePauw and German sociological jurisprudence as two key factors in understanding Beard’s conception of law and politics. As such, it offers a much more complicated image of Beard’s thought and his intellectual world.

L’enseignement secondaire général est souvent décrit comme une citadelle repliée sur elle-même, une institution figée, dont les enseignants seraient réfractaires au changement. L’histoire de l’éducation nouvelle en son sein en attesterait. Hormis la courte parenthèse enchantée de la Libération, celle-ci aurait échoué à le transformer. Ce bilan d’insuccès est-il justifié ? S’il l’est, dans quelle mesure ? Et s’il ne l’est pas totalement, comment expliquer sa prégnance dans des discours aux modalités variées, militantes comme universitaires ? Afin de répondre à ces questions, l’étude menée ici propose de prendre du recul et de porter l’analyse sur la longue durée pour comparer trois périodes : 1930-1944, 1944-1970 et 1970-2016. Pour chacune d’elle, il s’agit de croiser des sources multiples. Une lecture minutieuse des textes officiels rend possible une évaluation de la prégnance de l’éducation nouvelle dans les injonctions ministérielles. Un examen des réseaux (Inspection générale, hauts fonctionnaires, associations enseignantes, syndicats...) permet de retracer le parcours de la diffusion de ses idées et de tenter d’en mesurer l’impact dans les collèges et les lycées. À l’issue de ce cheminement, il apparaît que l’éducation nouvelle a su participer à l’élaboration d’un nouveau modèle pédagogique et a modifié, au moins en partie, les pratiques enseignantes. Au final, les raisons du ressenti d’un échec de l’éducation nouvelle dans l’enseignement secondaire ne tiennent pas tant aux réalisations effectives qu’à l’identité de celle-ci, aux stratégies de diffusion à l’œuvre (de l’explicite à l’implicite) et à des combats de représentations entre acteurs et observateurs du champ éducatif. / Secondary education is often described as a withdrawn citadel, a rigid institution, whose teachers would be resistant to change. The history of progressive education within its system would tend to evidence this statement. Except for the enchanted Liberation episode, it would have failed to transform it. Is this assessment of failure justified ? If so, to what extent is it justified ? And if it isn’t completely, how can its prominence in various discourses, whether militant or academic, be explained ? To answer these questions, the study conducted here offers to take a step back and to carry the analysis on the long term, in order to allow comparisons between three periods : 1930-1944, 1944-1970 and 1970-2016. For each of them, it is a question of crossing sources of varied nature. An assesment of the prominence of progressive education in ministerial injunctions is made possible thanks to a meticulous reading of the official texts. An examination of networks (general Inspectorate, senior officals, teacher associations, unions…) enables to trace the path of the dissemination of its ideas and to try to assess its impact on middle and high schools. At the conclusion of thiscourse, it seems that progressive education has managed to participate in the elaboration of a progressive educational model and to change, at least partly, teaching practices. In the end, the reasons for the feeling of a failure of progressive education in secondary education do not lie in the actual achievements as much as in its identity, in the dissemination strategies at work (from explicit to implicit) and in representation fights between actors and observers of the educational field.

A study of music and/or muscle relaxation and specific participant variables and their relationship with state anxiety reduction

A study was conducted to determine the relationship between three different methods of relaxation (listening to music, progressive muscle relaxation (PMR), and a combination of music listening and PMR) and state anxiety reduction (as indicated by a feeling of reduced tension for the participant, measured on a self-rating scale). The study also examined the relationship between specific variables (age, sex, music training, music preferences, familiarity with music, degree of liking for music, knowledge of, and experience in, PMR, and learning style) and response to a relaxation method. The study involved forty male and female volunteers, ranging in age from 18 - 60 years. Participants were randomly assigned to one of three experimental groups or a control group. Both quantitative and qualitative methods were used in the analysis of results. Results indicated that for the participants in this study, all methods were associated with a decrease in state anxiety, the combined methods being no more effective in achieving reduced tension levels than the independent methods. However, a significant difference was achieved between the PMR group and the control group, with the mean decrease in tension level for the PMR group higher on average than that of the control group. There was also no significant difference observed between the relaxation sessions and the controlled opportunity for social interaction, attended by all participants. Further, case studies indicated that response to relaxation methods is a highly individual one.

Die hier vorliegende Magisterarbeit beschäftigt sich mit der bisher unbekannten pädagogischen Biographie des jüdischen Lehrers, Schulleiters, Museumsgründers und Zionisten Hans Herbert Hammerstein/ Yisrael Shiloni (1901-1996). Eingebunden in die Darstellung des bildungshistorischen Kontexts beleuchtet der Beitrag alle relevanten Lebensabschnitte des Protagonisten. Wichtigste Station seines pädagogischen Werdegangs ist die „Private Jüdische Volksschule Bonn“. Zwischen 1934 bis 1937 gründete und leitete Hammerstein/ Shiloni diese Einrichtung, die vielleicht die einzige jüdische Volksschule ihrer Zeit war, die ausschließlich nach reformpädagogischen Prinzipien arbeitete. Nicht unerwähnt soll auch das „Museum Deutsches Judentum“ bleiben, dass den Abschluss seines pädagogischen Lebensweges bildet. Hammerstein/ Shiloni gründete diese Institution 1971 in Israel, um die Geschichte des deutschen Judentums zu dokumentieren. Das Museum besteht bis heute als „Museum der deutschsprachigen Juden – Kulturzentrum der Jeckes“ in Tefen und nimmt einen festen Platz in der israelischen Museumslandschaft ein. Jahrhundert. / This MA-Thesis describes the so fare unknown pedagogic biography of the Jewish teacher, headmaster, founder of a museum and Zionist Hans Herbert Hammerstein/ Yisrael Shiloni (1901-1996). While displaying the historic educational background the thesis specifies all relevant stages of the protagonist’s life. The most important achievement in his pedagogic career was the “Private Jüdische Volksschule Bonn” (Private Jewish Elementary School in Bonn). Hammerstein/ Shiloni founded this institution in 1934 and led it until 1937. The school was probably the only Jewish school in that time, which worked solely with progressive educational principles. His last undertaking in his pedagogic life was the foundation of the “Museum Deutsches Judentum” (Museum of German Jewry) in 1971 in Israel. Hammerstein/ Shiloni were also the curator of this institution, which documented the history of the German Jewry. The museum still exists as the “Museum der deutschsprachigen Juden – Kulturzentrum der Jeckes” (Museum of the German- speaking Jewry – Cultural Centre of the Jekkes”) in Tefen and is part of the Israeli museum scene. Hammerstein/ Shiloni’ s life is, despite it’s uniqueness, exemplarily for the generation of German Zionists, that saw the Zionism as an educational project, after World War I and gained personal, cultural and political identity from this ideology. Therefore this biographic paper contributes to the history of the Jewish progressive education, which was almost forgotten and to the history of the education of the German Jewry in the 20. century.

Progressive Exercise To Address Impaired Balance And Mobility In Older Adults Referred for Home Care Physiotherapy: Is It Beneficial To Target Vestibular Control And Lower Limb Muscle Strength

Purpose: The primary purpose of this study was to evaluate the effect of a progressive exercise program on vestibular control of standing balance, in older adults referred for home care physiotherapy because of balance impairment. Methods: Ability to use vestibular inputs for postural control in standing was assessed using the Clinical Test of Sensory Interaction and Balance (CTSIB). Participants who had CTSIBTest 5 scores of < 15 seconds were accepted into the study. Participants were randomly assigned to an 8 week intervention of progressive balance exercise targeting ability to use vestibular control and high intensity progressive resistance exercise (PRE) or high intensity PRE only. Results: The difference in CTSIBTest 5 scores of the RBE Group (median 23.3 s) was greater than the difference in CTSIBTest 5 scores for the RE Group (median 0.60 s) (W = 18.0, p <0.05). Conclusions: The results of this study provide preliminary evidence that the ability to use vestibular control in older adults, referred for home care physiotherapy for balance impairment, can be modified by progressive balance training and resistance exercise but not by resistance exercise alone.

Meanwhile , the Cross-Straits relations become the enough factor influencing government-business relations inside Taiwan, therefore Chinese Government make state apparatus pass the operation of trading ties of the Cross-Straits gradually to influence government-business relations of Taiwan and decision-making process of the g state apparatus of Taiwan inside Taiwan, even weaken the state apparatus independence of Taiwan, thus state apparatus of Taiwan face the double crisis coming from native folk capital and Chinese state apparatus .
