Objetivou-se conhecer a possível dinâmica da violência sexual contra meninos. A tese é composta por três artigos: um artigo no qual um modelo integrativo conceitual da dinâmica da violência sexual contra crianças e adolescentes é proposto; um artigo empírico de apresentação e avaliação das parcerias estabelecidas para a coleta de dados; e um artigo empírico com os resultados do estudo sobre a dinâmica da violência sexual contra meninos. Participaram do principal estudo dessa tese quatro meninos vítimas de violência sexual e quatro psicólogos da rede de atendimento às vítimas de violência sexual. A análise das entrevistas evidenciou a menor visibilidade das situações de violência sexual contra meninos, a influência de padrões sociais de masculinidade e o despreparo da rede de atendimento para intervenção nesses casos. A maior compreensão da violência sexual contra meninos é necessária para a implantação de estratégias de intervenção adaptadas às necessidades de meninos vítimas. / The aim of this doctoral dissertation was to investigate the possible dynamics of sexual violence against boys. The dissertation is composed of three articles: an article proposing a conceptual integrative model of the dynamics of sexual violence against children; an empirical article regarding the evaluation of partnerships with assistance networks developed for data collection; and an empirical article with the results of the study on the dynamics of sexual violence against boys. Participants were four boys victims of sexual violence and four psychologists of assistance networks. The results showed a lower visibility of sexual violence against boys, the influence of social standards of masculinity, and the lack of professional specialization for delivering intervention for the victims. A greater comprehension of sexual violence against boys is necessary for the implementation of more effective intervention strategies adapted to the needs of boy victims.

Esta dissertação teve por objetivo investigar variáveis cognitivas e emocionais em meninas vítimas de abuso sexual infantil (ASI), observando as relações entre ASI, Transtorno de Estresse Pós-Traumático (TEPT) e desempenho em funções cognitivas. Para isso, foram organizados três artigos, sendo um teórico e dois empíricos. O primeiro estudo apresenta uma revisão teórica sobre as relações entre ASI, TEPT e disfunção cognitiva. O segundo estudo avaliou o funcionamento cognitivo (memória, atenção e flexibilidade cognitiva/funções executivas) em um grupo de meninas vítimas de ASI (n=12) e comparou-o a um grupo controle (n=16). O terceiro estudo investigou a presença de sintomas emocionais e comportamentais em 16 meninas vítimas de ASI, bem como indicadores de risco à ocorrência do ASI nas famílias destas participantes. Foi observada uma alta manifestação de TEPT e um maior número de erros na tarefa de atenção visual concentrada, no grupo de vítimas de ASI, quando comparado ao grupo controle. Estes resultados ressaltam a importância de intervenções psicoterapêuticas e neuropsicológicas junto a esta população. / The objective of this dissertation was to investigate the cognitive and emotional variables in girls, victims of child sexual abuse (CSA), observing relationships between CSA, Posttraumatic Stress Disorder (PTSD) and performance in cognitive functions. For that, three papers were organized, one theoretical paper and two empirical papers. The first study shows a theoretical review about relationships between CSA, PTSD and cognitive dysfunction. The second study evaluated the cognitive functioning (memory, attention and cognitive flexibility/executive function) in a group of girls (n=12), victims of CSA and their performance was compared with a control group (n=16). The third study investigated the presence of emotional and behavioral symptoms and CSA risk factors in the family in 16 girls, victims of CSA. A high manifestation of PTSD and high number of errors on concentrated visual attention test was observed in the group of victims of CSA, when was compared to the control group. These results highlight the importance of psychotherapeutic and neuropsychological interventions with this population.

A metodologia geral adotada foi a análise qualitativa por triangulação de métodos. A técnica de entrevista foi complementada por questionário descritivo. Os resultados foram dispostos em três artigos. O primeiro constou de uma análise documental conduzida pelo método de análise do conteúdo, onde foram discutidos os limites e potencialidades das ações do então chamado Programa Sentinela, com o objetivo de proceder a uma revisão das primeiras repercussões do Plano Nacional de Enfrentamento da Violência Sexual contra Crianças e Adolescentes e a ação da rede destacando um dos importantes parceiros do setor Saúde. O segundo apresentou um estudo de caso das ações constituídas pela Secretaria Municipal de Saúde e Defesa Civil do Rio de Janeiro a nível central, para garantir atendimento especializado às crianças e adolescentes vítimas de violência sexual, seus familiares e autores da agressão. O estudo envolveu gestores das Gerências de Programa da Mulher, do Adolescente e da Criança, membros do Conselho de Direitos da Criança e do Adolescente, Assessoria de Promoção da Saúde e Superintendência de Atenção Primária do nível central da Secretaria Municipal de Saúde e Defesa Civil do Rio de Janeiro. O terceiro artigo fez uma análise das condições reais de atendimento à população infantojuvenil vitimizada em três unidades de referência municipais, uma de emergência, uma de acompanhamento de crianças e outra de seguimento de adolescentes, sob a ótica dos gestores e profissionais locais. Resultados: Verificou-se uma frágil articulação do então denominado programa Sentinela com a Saúde, com deficiência de encaminhamentos e interlocução. As ações da Secretaria Municipal de Saúde e Defesa Civil do Rio de Janeiro mostraram-se mais voltadas para as vítimas, e de forma coadjuvante às famílias. A assistência ao agressor depende da parceria com organizações não governamentais. Apesar de engajada no combate à violência sexual, gestores da Secretaria nem sempre conhecem os documentos oficiais instituídos para a temática, enfraquecendo, com isso, as políticas de enfrentamento. Dentro do nível central ainda existe uma lacuna na interlocução dos gestores de programas que prejudica a efetivação de projetos e uniformidade de ações. Segundo os profissionais, as capacitações oferecidas, apesar de presentes, são assistemáticas, pontuais e não documentadas. As unidades apresentam problemas de espaço físico e déficit de recursos humanos. Parte dos profissionais que atuam no atendimento referem nunca ter recebido capacitações para a temática e outros, já capacitados sentem falta de uma atualização periódica e permanente. Conclui-se que, apesar de todos os investimentos realizados pela Secretaria Municipal de Saúde e Defesa Civil do Rio de Janeiro para fornecer atendimento às vitimas, ainda se fazem necessários ajustes em vários níveis, dotados de complexidade diversa para se prover uma assistência de qualidade a crianças e adolescentes em situação de violência sexual, seus familiares e também aos autores desta forma de violação. / This thesis took as its object of study the actions taken by the Municipal Health and Civil Defense of Rio de Janeiro to ensure specialized care to children and adolescent victims of sexual violence, their families and perpetrators of aggression in order to analyze them from the perspective of the central level managers of the City Department of Health and directors and health professionals from three local institutions of reference for the attendance of these patients.The general methodology adopted was the qualitative analysis by triangulation methods. The interview technique was complemented by descriptive questionnaire. The results were arranged in three articles. The first consisted of a documentary analysis conducted by the method of analysis of content, where the limitations and potentialities of the actions of the formerly called Programa Sentinela were discussed, with the objective to proceed to a revision of the first repercussion of the National Plan to Combat Sexual Violence against Children and Adolescents and the action of the assistance net highlighting one of the important partners of health sector. The second presented a case study of the actions made by the central level of the City Department of Health and Civil Defense of Rio de Janeiro to ensure specialized attendance to the children and adolescents victims of sexual violence, their families and perpetrators of aggression. The study involved managers of the Programs for Women, Adolescents and Children, members of the Rights Council of Children and Adolescents, Office of Health Promotion and Supervision of Primary Attention of the central-level of the City Department of Health and Civil Defense of Rio de Janeiro. The third article made an analysis of the actual conditions of attendance of the children and teenagers sexually victimized in three municipal units of reference, an emergency, one of accompaniment of children and one for follow-up of adolescents under the perspective of local managers and professionals. Results: There was a weak articulation between the formerly called Programa Sentinela and the Health System, disabled referrals and a lack of dialogue. Shares of the City Department of Health were more focused on the victims, placing the families in a secondary position. The assistance to the aggressor depends on partnership with nongovernmental organizations. Although engaged in the combat against sexual violence, the Department managers don’t have much knowledge about the official documents on the subject, weakening thereby the politics of confrontation. Within the central level there is still a gap in the interaction of program managers that affect the execution of projects and the uniformity of actions. According to the professionals, the training offered, although present, are unsystematic and not documented. The units have problems of space and lack of human resources. Part of professionals working in care refers having never received training related to the theme and others already trained miss a periodic and permanent update. One concludes that, despite all the investments made by the City Department of Health and Civil Defense of Rio de Janeiro to provide attendance to the victims, adjustments are still needed at various levels, endowed with diverse complexity, to provide assistance with quality to children and adolescents in situations of sexual violence, their families,and the authors of this form of abuse.
