Acompanhamento terapêutico como dispositivo psicanalítico de tratamento das psicoses na saúde mental / Therapeutic acompaniment as a mental health device in the treatment of psychosis

Essa pesquisa interroga a respeito do exercício da prática clínica quando seus fundamentos, conceitos e teorias próprios não estão referidos aos espaços tradicionais de tratamento, como é o caso da prática do Acompanhamento Terapêutico. Trata-se de um levantamento bibliográfico das articulações teórico-práticas realizadas pelos pesquisadores brasileiros que escreveram sobre o tema do AT e posteriormente um recorte específico dos trabalhos que se situam no campo da psicanálise, tendo a finalidade de reunir a produção teórica desta prática e analisá-la. A pesquisa se propõe a oferecer um panorama do campo mostrando como e em que momento histórico o AT surgiu, quais as elaborações teóricas embasaram seu fazer e como a psicanálise contribui para este trabalho. Como conseqüência a disponibilização do conjunto de material produzido neste campo pode contribuir na elaboração de uma posição ou um lugar teórico sobre o AT. O AT surgiu no campo da saúde mental no momento histórico conhecido como Reforma Psiquiátrica, visto que se caracteriza pela aproximação à loucura e por seus novos modos de tratamento. Por ter surgido neste espaço as teorizações no campo se enquadram, em sua maioria, nos enfoques da reabilitação psicossocial e da psicanálise. De acordo com o material bibliográfico pesquisado é possível afirmar que O AT seria definido por uma prática e não por uma abordagem teórica, já que a rigor, não há uma teorização acerca do AT, pois seus autores importaram conceitos de diferentes correntes com bases epistemológicas distintas. O AT pode ser sustentado teoricamente a partir dos preceitos que orientam a clínica psicanalítica com as psicoses, compreendido como uma oferta clínica que sustenta saídas e promove a circulação. É, assim, portanto que o AT propicia em ato uma aproximação e experimentação de laços sociais, ou seja, possibilidades de encontro com efeitos de real. Conclui-se que o AT não é uma prática psicanalítica, no entanto, ao ser desempenhado por um psicanalista, alguns lugares lhe são possíveis, podendo se prestar a ser suporte das identificações imaginárias, ser secretário do alienado até contribuir para a construção do sinthome, que se referem às posições que o AT pode se colocar na transferência / This research delves into the clinical practice when its foundations, concepts and theories do not refer to the traditional spaces of treatment, as is the case with the practice of Therapeutic Acompaniment . It deals with a bibliographical research of the theoreticalpractical articulation by Brazilian researchers who write about Therapeutic Acompaniment and subsequently with a specific section of the psychoanalysis work, with the goal of assembling the theoretical product of this practice and analyzing it. The research intends to offer a view of the field showing how and at what point in history Therapeutic Acompaniment came to be, what theoretical elaborations founded its application and how psychoanalysis contributes to this work. As a consequence, the availability of the set of materials produced in this filed may contribute to the elaboration of a position or a theoretical space regarding Therapeutic Acompaniment. Therapeutic Acompaniment emerged in the mental health field at a historic moment known as the Psychiatric Reform, given that it is characterized by its affinity to insanity and by its new treatment methods. Having arised in this space, most theorizations in the field conform to the focus on psychosocial rehabilitation and psychoanalysis. According to the bibliographical material researched, it is possible to state that Therapeutic Acompaniment would be defined by a practice and not by a theoretical approach, since, strictly, there is no theorization regarding Therapeutic Acompaniment, since its authors imported concepts from different trends, with different epistemological bases. Therapeutic Acompaniment can be theoretically support from the precepts that guide the psychoanalytic clinic of psychosis, understood as a clinical practice that sustains egress and promotes circulation. Therefore Therapeutic Acompaniment provides an approximation and experimentation of social links, that is, possibilities of encounters with effects of the real. In conclusion, although Therapeutic Acompaniment is not a psychoanalytical practice, when it is used by a psychoanalyst it may find its space; it may provide support for imaginary identifications, function as secretary of the insane, and contribute to the construction of the sinthome, spaces which refer to the positions Therapeutic Acompaniment may assume during transference

Perceptions of clinical supervisors about their preparedness for clinical teaching at a university in the Western Cape

Magister Curationis - MCur / Background: Clinical supervision or clinical accompaniment is considered an integral part of nursing education and is crucial for the development of nursing students’ clinical competence. In order to achieve this, clinical supervision requires skilled human resources which vary from one educational institution to another and may include lecturers, clinical supervisors/facilitators and professional nurses. Clinical supervisors are required to be good educators as well as excellent clinicians. In addition, they often draw on their individual, personal and professional experiences to guide their teaching to meet the demands of both the clinical and academic contexts in which they work. However, the clinical teaching model or framework used by educational institutions is often not aligned to clinical practice activities and vice versa. This poses challenges for students due to the different expectations of educators and professional nurses in practice. Inadequately prepared clinical supervisors can have a detrimental effect on the delivery of the nursing programme that may include poor clinical teaching and inadequate integration of theory and clinical competencies, which ultimately leads to poorly trained nursing students. Aim of the study: The aim of this study was to explore the perceptions of clinical supervisors regarding their preparedness for clinical teaching. Methods: The study adopted a qualitative research approach, utilising an exploratory descriptive design. A non-probability purposive sampling method was used to select 12 clinical supervisors in the undergraduate programme. The researcher collected the data by means of semi-structured interviews with open-ended questions and analysed this data using content data analysis. Analysis of the data using ATLAS, ti 8 research software programme generated four themes and 18 categories. Findings: The findings indicated that clinical supervisors required time to adapt to their role and improve their knowledge and skills despite them having a positive experience during their orientation. Although appreciative of the support and guidance, clinical supervisors stated that not all colleagues were supportive, which in some instances had a negative impact on interpersonal relationships. The findings furthermore indicated that clinical supervisors apply all the steps of the five phases in the skills lab methodology employed by the School of Nursing at the university where the study was conducted. Clinical supervisors are required to be well informed and committed to continuing education in order to incorporate theory into practice with the latest developments and equipment in facilities. Furthermore, the findings indicated that inconsistency and failure to attend to students’ clinical learning needs can have a negative impact on student learning. Ethics: The researcher adhered to all principles of research ethics throughout the study.

In line with this, the performance of expatriates has become crucial. Foreign postings for executives are, however, costly undertak-ings for multinational corporations, especially when they fail. Hence the acculturation of an expatriate to the host country is a highly relevant topic. The research on expatriate accultura-tion to a host country has changed substantially over the past few years. Yet to date, the majority of research has been focus-ing on male expatriates. The purpose of this paper is to exam-ine whether there are any differences between male and female expatriates, and the impact of partner accompaniment on their adjustment to the host country. An exploratory multiple-case study has been made and data has been collected through a survey of the expatriates of three major multinational corpora-tions. Five propositions have been formulated and verified. The contribution of this paper is to illuminate the presumptive differences between male and female expatriation.</p>

A Fragmented Parallel Stream: The Bass Lines of Eddie Gomez in the Bill Evans Trio

Master of Music (Performance) / Eddie Gomez was the bassist in the Bill Evans Trio for eleven years. His contribution to the group’s sound was considerable, but while there has been some recognition of his virtuoso solos in the trio there has been little academic interest in his bass lines. This essay examines bass lines from the album Since We Met, recorded in 1974 by Evans, Gomez and drummer Marty Morell. Analysis of the bass accompaniments to the piano solos on “Since We Met” and “Time Remembered” reveals that they form a fragmented two-feel. A traditional two-feel employs two notes to emphasise the first and third beats in bar of 4/4 time. In Gomez’s bass lines these two notes are frequently replaced with short rhythmic motifs. These motifs occur in a variety of forms and at different metric displacements that alternately propel and retard the forward motion of the music. Additionally, Gomez uses a wide range of register and varied articulations to create a richly diverse bass line. The resulting effect has often been interpreted as interactive or conversational with the soloist. However there is very little interaction between the bass line and Evans’ solo. The bass line is a parallel stream to the solo that energises and colours the music.

Public vulnérable : mise en œuvre d’un dispositif d’accompagnement socioprofessionnel intensif / Vulnerable public : establishment of a system of intensive socio-professional accompaniment

Cette thèse, réalisée par Alexandre LANGARD, psychologue du travail, sous la direction du Professeur Martine BATT (Interpsy EA 4432), s’inscrit dans le cadre d’une recherche action, répondant à une politique gouvernementale mise en œuvre suite au plan de lutte contre la pauvreté signé en 2013. Ce travail a consisté à mettre en œuvre un dispositif d’accompagnement de personnes dites « vulnérables », âgées de 18 ans à 25 ans, en situation de précarité, sans emploi et sans formation, qui pourraient bénéficier d’une prise en charge plus adaptée. Cette action revient à permettre à l’ensemble de la population identifiée de s’inscrire dans une démarche d’insertion, à travers un accompagnement novateur et intensif, favorisant l’accès à une situation professionnelle et permettant la levée des freins à l’emploi. L’objectif de cette recherche action est donc double. D’une part, mettre en œuvre un dispositif d’insertion, répondant à une demande gouvernementale afin de proposer in fine des solutions répondant à des problématiques professionnelles et sociales rencontrées par le public cible. D’autre part, évaluer les leviers individuels favorisant ainsi la réussite de l’accompagnement proposé. La démarche a conjugué des méthodes de recensement, de recueil d’informations, d’évaluation et d’analyse des données quantitatives et qualitatives. L’évaluation de l’apport de l’accompagnement a été réalisée sur une période de trois années à partir des variables individuelles essentielles considérées comme leviers dans une démarche d’insertion : niveau d’estime de soi, sentiment d’efficacité personnelle, décision vocationnelle, motivation, obstacle externe et désinvestissement du travail. Des entretiens individuels ont permis de dresser un bilan qualitatif de l’accompagnement dispensé. Les données discursives recueillies au cours des entretiens ont été analysées selon des méthodes classiques d’analyse thématique de contenu du discours. Les résultats montrent l’impact du dispositif sur les sentiments personnels positifs, l’accès à l’emploi-formation et l’importance du rôle du conseiller. La recherche action portée est un projet SHS, à dimension psychologique et psychosociale, qui a permis d’évaluer et de promouvoir des actions au service de personnes vulnérables. / This thesis written by Alexandre LANGARD, occupational psychologist, under the direction of Professor Martine Batt (Interpsy EA 4432), is part of an action research project, responding to a government policy implemented following the poverty reduction plan signed in 2013.This study consisted in implementing a support system for unemployed without training « vulnerable » people, aged 18 to 25, in a precarious situation, who could benefit from more appropriate care. Through innovative and intensive support, this action amounts to enabling the entire identified population to participate in an integration process, thus promoting access to a professional situation and allowing the removal of barriers to employment.The aim of this action research is therefore twofold. On the one hand the objective is to implement an integration system responding to a government request, in order to finally propose solutions that correspond the professional and social issues faced by the target audience.On the other hand, the analysis and evaluation of the individual levers has to be taken into consideration to ensure the success of the proposed support. The approach combined methods of census, information gathering, evaluation and analysis of quantitative and qualitative data.The evaluation of this support system was carried out over a three-year period, on the basis of the essential individual variables considered as levers in an integration process: self-esteem level, sense of personal effectiveness, vocational decision, motivation, external obstacles and disinvestment from work. Individual interviews made it possible to draw up a qualitative review of the support provided. The discursive data collected during the interviews were analysed using traditional methods of thematic analysis of language content. The result shows the impact of the system on positive personal feelings, access to employment-training and the importance of the role of the counsellor. The action research is an SHS project, with psychological and psychosocial dimension, which has made possible to evaluate and to promote actions helping vulnerable people.
