A vivência da puérpera-adolescente com o recém-nascido, no domicílio / The adolescent mother´s experience with the newborn, at home

Este estudo qualitativo foi motivado pela carência de pesquisas com enfoque na maternidade na adolescência e no período puerperal. Teve como objetivo compreender a vivência da puérpera-adolescente sobre o cuidado do recém-nascido, em domicílio. Adotou-se o conceito de Maternidade como referencial de análise e o método do Discurso do Sujeito Coletivo (DSC) no tratamento dos dados. O projeto foi aprovado pela Comissão de Ensino e Pesquisa, e pelo Comitê de Ética em Pesquisa do Hospital Universitário da USP. Participaram do estudo 15 puérperas-adolescentes, primíparas, que ficaram internadas na unidade de Alojamento Conjunto do HU-USP, juntamente com o recém-nascido. Os dados foram coletados em 2006, por meio de entrevista realizada com as puérperas após o mínimo de 30 e o máximo de 40 dias, no domicílio. As respostas da questão aberta, “conte-me como está sendo em casa com seu bebê ?" possibilitaram a elaboração de 17 DSC apresentados em dois blocos, segundo os temas centrais que emergiram: “cuidados do recém-nascido" e “contexto sociocultural das puérperas-adolescentes". Em relação ao primeiro bloco, os discursos mostraram uma construção diária do ser mãe-adolescente e o desejo da puérpera pela maternidade e pela maternagem, pois assumiam integralmente as tarefas de mãe-cuidadora. A princípio considerada de difícil adaptação, a maternidade gerou na jovem a necessidade de aprender a conviver com as abdicações e ambivalências inerentes ao novo status. O suporte familiar, as orientações recebidas na unidade de Alojamento Conjunto e a experiência anterior no cuidado de recém-nascidos favoreceram a adaptação à maternidade e a superação de suas limitações iniciais. Quanto ao segundo bloco (contexto sociocultural), os dados mostraram a expressão de vivências e de mudanças nas relações sociais, com abandono de projetos de vida imediatos e de atividades de lazer. Verificou-se, ainda, que a puérpera-adolescente vivencia o cuidado do recém-nascido com erros e acertos, e, a cada dia, constrói o próprio modelo de ser mãe, vencendo medos e dificuldades, e despertando para sua capacidade de atender às necessidades de higiene, de alimentação e de afeto do recém-nascido. Diante dos dados, creio que o profissional deve repensar como abordar essas jovens mães nas unidades de saúde, priorizando o atendimento de situações geradoras de conflitos no cuidado do bebê e na relação com familiares. Além disso, deve estar disponível para compartilhar e possibilitar o esclarecimento de suas dúvidas de modo a facilitar a superação de dificuldades. Portanto, oferecer-lhe a chance de uma vivência da maternidade-adolescente com base em cuidados construídos diariamente, com superação / This is a qualitative study that was motivated by the lack of researches related to the maternity during the adolescence and the postpartum period. The aim of this study was to understand the adolescent mother’s experience while taking care of the newborn at home. The maternity concept was adopted as the referential for the analysis. For the data treatment the Collective Subject’s Speech was adopted. The project was approved by the Research Committee and also by the Ethic Committee of the School Hospital of University of São Paulo. Fifteen adolescent mothers participated in the research. They were all mothers for the first time and they stayed together with their babies in the rooming-in of the School Hospital of University of São Paulo. The data were collected in 2006 by interviews done with the mothers after the minimum of 30 days and the maximum of 40 days they were in their houses. The answers given to the open question: “Tell me what it is like to be home with your baby", made possible to elaborate 17 Collective Subject’s Speeches that were presented in two parts, according to the following central themes that emerged: “taking care of the newborn" and “social and cultural contexts of the adolescent mothers". In the first part, the speeches showed that everyday, the women were building their role as adolescent mothers as well as their desire for the maternity and care. They totally assumed the tasks related to the caregiver mother. In the beginning, the adolescent mothers considered they had difficult in adapting to the maternity, which forced them to learn how to live with the abdication and ambivalences related to their new status. The support from the family, the orientation given in the hospital and the previous experience taking care of newborns helped the adolescent mother to be adapted to the maternity and also to overcome the first limitations. In the second part of the speeches (social and cultural contexts), the data showed experiences and changes related to their social lives as putting behind some immediate life projects as well as their leisure activities. It was also verified that the adolescent mother experiences the care of the newborn by making the right and wrong things and each day she builds her own model of a mother, overcoming her fears and difficulties. This process makes her understand her capacity to fulfill the newborn’s needs concerning hygiene, feeding and affection. According to the findings, I believe the professional should rethink the way the care is given to these young mothers in the health units and give priority to the situation of conflict in the care of the baby and in the relationship with the members of the family. The professional should also be available to share and clear their doubts in order to make easier for them to overcome their difficulties and give them the opportunity to experience an adolescent maternity based on everyday care built with triumph

<p>The purpose of this study was to examine how some young mothers, who previously have been in foster care or residential care, describe the way motherhood has affected their lives and identities. Narrative interviews were conducted with three teenage mothers and one mother aged 23. As tools for analyzing the narratives, theories about narrative psychology and how people present themselves through story-telling and language were used. The results of the study show that becoming a mother was life-changing in a positive way, according to the young mothers. They described how they, since becoming mothers, had become more mature and now lived less risk-full lives. Motherhood had brought a new focus to their lives and had given them motivation to study and to become self-supporting. The mothers expressed a wish to give their children a better childhood, than they themselves had. The results of the study were similar with the results reported in previous, qualitative studies of the experiences of young mothers.</p>

Psychometric properties of the Spanish version of the 10-item Connor Davidson Resilience Scale (CD-RISC) among adolescent mothers in Peru

El texto completo de este trabajo no está disponible en el Repositorio Académico UPC por restricciones de la casa editorial donde ha sido publicado. / The objective of this research is to assess the psychometric properties of the Connor-Davidson Resilience Scale (CD-RISC) in order to contribute to the literature identifying validated resilience measures in low-resource settings where individuals face significant adversity. This cross-sectional study included 789 adolescent mothers who delivered at a maternity hospital in Lima, Peru. The Spanish version of the 10-item CD-RISC was used to measure resilience. Internal consistency and construct validity were assessed by evaluating individual item characteristics as well as the association of CD-RISC score with symptoms of depression, anxiety and sleep disturbance. Exploratory factor analysis (EFA) was performed to test the factorial structure of the CD-RISC. The CD-RISC was found to have good internal consistency (Cronbach’s alpha = 0.85). CD-RISC scores were positively associated with school attendance, financial hardship, and history of childhood abuse; scores were negatively associated with household dysfunction, depression, anxiety and poor sleep quality. The results of the EFA showed that the CD-RISC contained a two-factor solution, which accounted for 46% of the variance. Overall, these findings suggest that the Spanish-language version of the CD-RISC-10 is an adequate measure of resilience in this population. Adolescent mothers in an intervention study a qualitative analysis of variables relating to their teaching interactions with their infants

The intent of this thesis was to study, in depth, the experiences of four adolescent mothers who underwent a home intervention program. I studied two mothers who did well with teaching their 12-month old children during play, and two mothers who did not do as well. All four mothers received weekly intervention from the time of their child's birth through 12-months of age. I studied the following variables: 1) how much time the home visitors spent on selected child development and parenting topics; 2) the mother's perceived social support; 3) how many community resources the mother used; and 4) if the infant was healthy and within normal developmental range. All of the mothers struggled in their lives, yet varied in the quality and time of most of the variables. It was striking how different each one was from the other. The implications of the study are important for child development specialists who can use the qualitative data within this document to better understand first time adolescent mothers in order to improve the outcomes of the home visitation services that they provide to mothers and infants. After spending time studying these four adolescent mothers, I would recommend that adolescent women not get pregnant. Adolescence is a time that is meant for experiences and self-discovery and should be spent free from a dependent child who critically needs them. Future research and funding should be spent on preventing adolescent pregnancy and ensuring that flexible curriculum be utilized by the home visitors in order to meet the varying needs of adolescent mothers.

No que tange à assistência ao recém-nascido, significativos avanços tecnológicos e de humanização do cuidado são descritos na história recente. O presente estudo tem como objetivo principal analisar, sob a ótica de enfermeiras e mães jovens, quais os sentidos atribuídos à atividade de leitura dirigida a bebês em Unidade de Terapia Intensiva Neonatal, e quais as possibilidades da leitura atuar como facilitadora da relação mãe jovem-bebê, considerando a metodologia do Projeto Biblioteca Viva. Trata-se de estudo com abordagem qualitativa. Paraa coleta de dados, foi utilizada a técnica de entrevista semi-estruturada. Foram entrevistadas 10 (dez) enfermeiras que trabalham em UTINe 7 (sete) mães jovens de bebês internados na unidade referida. A pesquisa demonstrou que a leitura de histórias infantis para bebês em UTI neonatal, que tem por objetivo aproximarmãe e bebê, possibilita para esse par um momento em que o foco principal não seja relativo ao adoecimento e vem a somar às propostas de atenção a essa clientela e às estratégias de aproximação da dupla mãe-bebê. Este estudo revelou ainda que mães e profissionais de enfermagem compreendem a atividade de leitura comouma qualificação da assistência em UTIN, percebendo a promoção de leitura como meio de tornar a oferta de carinho uma dinâmica institucional e como alternativa de oferecer ao RN estímulos positivos ao seu desenvolvimento. / The high number of needed-hospitaliz ation babies born in NICU and its complications lead us to reflect and pro pose alternatives ways in the field of medical care. The development/kepping of mother-baby relationship as a manner to overcome the difficulties by them selves, as well as their families, is a thing to be considered. These difficu lties are even harde r when it is an adolescent or young mother. As regard the newborns care, signifi cant technological advances and in the humanization care field were recently described. This study has the aim to analyze, under the nurses and young mothers perspective, which are the meanings attributed to the reading activity to babies in Neonatal Intensive Care Unit, as well as its possibilities to become easier the young mother-baby relationship, considering the methodology of the Biblioteca Viva Project. This is a qualitative study. For t he data collection, a half-structured interview approach were used. We interv iewed 10 (Ten) nurses that work in the NICU and seven (7) young mothers of hospi talized babies in the same unit. The results showed that the readi ng of children's stories to babies in NICU, allows them a time when the major focus is no t the illness but the approximation of the mother-baby. This study also showed that mothers and nurses understand the reading activity as a qualification for NICU assistance, realizing that the promotion of reading is a way to bec ome the care offering in a dynamic institution and as an alter native way to offer the newborn positive stimulation for their development.

A strategic alignment framework for the prevention and combat of early marriage and maternity in Zambezia Province, Mozambique

Despite all the legislative efforts regarding child protection and campaigns to prevent and combat early marriage and maternity, Mozambique was ranked 9th globally in terms of the prevalence of early marriage, with 48% of girls aged 20-24 marrying before the age of 18 years. The aim of this study was to develop a strategic alignment framework for the prevention and combat of early marriage and maternity in Zambézia Province, Mozambique. This study used a two-stage equal-status concurrent sequential mixed-method design. Data were collected through a cross-sectional survey, administered to 383 early married, maternity and pregnant girls; life story interviews with early married, maternity and pregnant girls (25) aged 10-19 years; semi-structured interviews with professionals from the education and health sectors, local authorities, families of the early married, maternity and pregnant girls (37), and group discussions with members of a child committee (16). The results indicated that the majority of early married, maternity and pregnant girls only completed primary education (55.9%), followed by secondary education (39.9%), and higher education (2.9%). About 65% of adolescent girls became pregnant at the age of 15-17. Among adolescent girls, 18.8% had their first baby before the age of 15 years and 99.2% had their first baby before they were 18 years old. Among the early maternity girls (362), 24.3% responded “yes” to the questions about health complications during their first baby’s birth and 75.7% of the respondents said “no”. Socio-cultural meanings, such as socialisation into roles, legitimising having children, the value and benefits of the bridewealth, the role of initiation, the social meaning of the first menstruation, geographical and transport issues were the main drivers for school dropout, forcing adolescent girls to marry. Engaging in sexual practices was found to provide the girls a sense of meaning and purpose, or as a result of poverty. Physical aspects, interpersonal relations, education, work, and emotional distress were some of the negative consequences of early marriage and maternity. There were some relevant interventions and efforts to prevent and combat early marriage and maternity in Maganja da Costa and Morrumbala districts in Zambézia Province, Mozambique. However, the alignment of the activities implemented by different NGOs and CBOs to MNSPCM (2016-2019) was still a challenge. Only World Vision was implementing programmes aligned to the National Strategy. Based on the results, I developed a strategic alignment framework for the prevention and combat of early marriage and maternity in Zambézia Province, Mozambique. / Health Studies / D. Litt. et Phil.
