Anwendung der Bibliometrie und Altmetrik in der Kinderchirurgie

1. Bibliometric study in pediatric minimally invasive surgery (MIS) Pediatric MIS is a standard technique worldwide for many pediatric surgical diseases and we aimed to analyze the research activity in this field. Articles on pediatric MIS (1991–2020) were analyzed from the Web of ScienceTM for the total number of publications, citations, journals, and IFs. Of these, the 50 most cited publications were evaluated in detail and classified according to the level of evidence (i.e., study design) and topic (i.e., surgical procedure). In total, 4464 publications and 53,111 citations from 684 journals on pediatric MIS were identified. The 50 most cited papers were published from 32 institutions in the USA/Canada (n = 28), Europe (n = 19), and Asia (n = 3) in 12 journals. Four authors (USA/Europe) contributed to 26% of the 50 most cited papers as first/senior author. Hot topics were laparoscopic pyeloplasty (n = 9), inguinal hernia repair (n = 7), appendectomy, and pyloromyotomy (n = 4 each). The majority of publications were retrospective studies (n = 33) and case reports (n = 6) (IF 5.2 ± 3.2; impact index 16.5 ± 6.4; citations 125 ± 39.4). They were cited as often as articles with high evidence levels (meta-analyses, n = 2; randomized controlled trials, n = 7; prospective studies, n = 2) (IF 12.9 ± 22.5; impact index 14.0 ± 6.5; citations 125 ± 34.7; p > 0.05). 2. Altmetric study in pediatric surgery Altmetric analysis assessing online mentions of publications is a new method to evaluate awareness to research output. We aimed to identify and characterize the top 100 articles with the highest Altmetric Attention Score (AAS) in pediatric surgery. Publications from core pediatric surgical journals (J Pediatr Surg, J Pediatr Surg Case Rep, Eur J Pediatr Surg, European J Pediatr Surg Rep, Pediatr Surg Int, Semin Pediatr Surg) were retrieved from www.altmetric.com in January 2023 and the top 100 publications were identified. Characteristics of each publication were analyzed. Publications ranked 93 to 101 had an identical AAS of 21. Thus, 101 articles were included for further analysis. The top 101 AAS articles were published between 1974 and 2022, preferentially from the United States (64%) and mainly in J Pediatr Surg (73%), followed by J Pediatr Surg Case Rep, Pediatr Surg Int, Semin Pediatr Surg, and Eur J Pediatr Surg. Their AAS ranged between 21 and 389 (median 33) with X (formerly Twitter) being mostly responsible for online mentions (n=2189, 75%). The number of citations ranged from 0 to 358 (median 16) and did not correlate with the AAS. Retrospective study design (33%) with low evidence level IV (49%) was the dominant study type. 3. Conclusions According to our bibliometric study in pediatric MIS, research activity increased over the last 30 years, with a golden decade in the early 21st century. Laparoscopic pyeloplasty and inguinal hernia repair accounted for most of the top 50 citations. Retrospective studies and case reports were the most common type of publication. Studies with high level of evidence such as randomized controlled trials are missing, especially on advanced techniques in pediatric MIS. For our altmetric study of publications in core pediatric surgery journals, we analyzed the top 101 most mentioned pediatric surgery articles in six pediatric journals. Among the pediatric surgical journals, the Journal of Pediatric Surgery is the main source for high-profile publications in pediatric surgery. The altmetric score of articles is predominantly achieved by their propagation via X (formerly Twitter), irrespective of study design, level of evidence or recognition by the scientific community. Therefore, active “twitterism” may play the key role in reaching high AAS.

Dissémination et communication des résultats de la recherche clinique dans les médias / Dissemination and communication of clinical research in mass media

Les médias et les réseaux sociaux constituent une source importante de diffusion et de communication des résultats de la recherche clinique. Le terme « spin » est utilisé lorsque la présentation et l’interprétation des résultats d’une étude sont déformées par les auteurs que ce soit intentionnellement ou involontairement. Les spins exagèrent les effets bénéfiques des interventions et sous-estiment les effets indésirables. Les principaux objectifs de ce travail étaient :1) d’évaluer la prévalence des « spins » dans les articles de presse, d’identifier les différentes stratégies de spin et de développer une classification de spin ; 2) d’identifier les facteurs associés à une diffusion des résultats via les réseaux sociaux et 3) d’étudier comment les articles scientifiques rapportés avec des spins diffusent via les réseaux sociaux, à partir de l’exemple de l’essai DAPT 2014. Dans un premier temps, nous avons réalisé une revue systématique des articles de presse décrivant les résultats d’études évaluant une intervention et indexés dans la rubrique santé de Google. Nous avons développé une classification des stratégies de spin pour les articles de presse et montré que la prévalence des spins est élevée. Dans un deuxième temps, nous avons réalisé une étude de cohorte d’articles évaluant les traitements contre le cancer pour identifier les facteurs associés à une attention médiatique élevée. Le critère de jugement principal était l’attention portée par les médias et les réseaux sociaux mesuré par le « score Altmetric ». Nos résultats ont montré l’importance de l’accès libre et des communiqués de presse. Enfin, nous avons effectué une analyse systématique de l’attention portée par les médias et les réseaux sociaux autour de l'étude DAPT qui était rapporté avec des spins sous estimant les effets indésirable de l’intervention. Nous avons montré que l’interprétation des résultats par les auteurs sont rarement contredits. Ces travaux ont permis de mettre en évidence l’importance des spins dans les médias et de développer des outils (classification de spin) pour améliorer la diffusion des résultats de la recherche. / Mass media and social networks are important sources of disseminating and communicating clinical research. The term “spin” is used when the presentation and interpretation of the results of a study is distorted by the authors intentionally or unintentionally. Spins exaggerate the beneficial effects of interventions and underestimate adverse effects. The main objectives of this PhD were (I) to assess the prevalence of spin in health news, as well as identify and classify different strategies of spin; (II) to identify factors associated with dissemination of research results through online media, and (III) to explore how results of a trial reported with spin were disseminated to the scientific community and online media, using the 2014 DAPT trial as a case study. For the first aim, we performed a cross-sectional study of health news and described the distortion of research results of studies evaluating an intervention in Google health news. We developed a classification of spin for health news and showed a high prevalence of spin. For the second aim, we performed a cohort study of articles evaluating cancer treatments and identify factors associated with high online media attention. The primary outcome was the attention received by media and social networks measured by Altmetric score. Our results highlighted the importance of open access and press releases. Finally, we performed a systematic review of attention received by media and social networks surrounding the DAPT study which were reported with spin and undermine the adverse effects of the treatment. We showed that the interpretation of results by authors was rarely criticized. These results highlighted the importance of spin in mass media and provided a tool (classification of spin) to improve the dissemination of research results.

However, IRs have deficiencies with regard to visual externalization of their vast scientific content in the form of bibliometric and altmetric indicators. In this context, the aim of this paper was to investigate the capability of national initiatives in Open-Access IRs in the use of bibliometric and altmetric indicators based on information visualization concepts. The research method used was the exploratory case study and the analysis unit comprises 81 IRs, active in Brazilian Science and Technology (S&T) Institutions, identified in the Directory of Open Access Repositories (OpenDOAR). Quantitatively, the existence of bibliometric and altmetric indicators in the Brazilian articles was verified in the literature, as well as the existence of plug-ins for IRs that use the DSpace system and, in a qualitative way, the analysis involved national articles with the objective of verifying which bibliometric and altmetric indicators are being used by national researchers, thus guiding the indicators that were used for the analysis with Brazilian IRs. The results obtained included the identification and analysis of the presence in Brazilian RIs of bibliometric and altmetric indicators and their graphic visualization, existing plug-ins for RIs using DSpace technology and the development of a new solution. It was observed that the bibliometric indicators showed a higher rate of occurrence in the analyzed sample than altmetric indicators and paucity of IRs in providing the indicators using a visual representation. It is concluded that the discussion of the results can contribute to the understanding of the issues surrounding the use of IRs, as sources of information for the preparation and visualization of bibliometric and altmetric indicators on the dynamics of scientific activities performed by Brazilian S&T Institutions. / Os Repositórios Institucionais (RIs) apresentam significativo potencial como fontes de informação para a elaboração dos estudos métricos, visando à compreensão das dinâmicas da atividade científica institucional. Contudo, os RIs sinalizam deficiências no tocante à externalização visual de seu vasto conteúdo científico na forma de indicadores bibliométricos e altmétricos. Neste contexto, o objetivo desta pesquisa foi investigar a capabilidade das iniciativas nacionais em RIs de Acesso Aberto na utilização de indicadores bibliométricos e altmétricos, com base nos conceitos de visualização da informação. O método de pesquisa utilizado foi o estudo de caso exploratório e a unidade de análise compreendeu 81 RIs, ativos nas Instituições de Ciência e Tecnologia (ICTs) brasileiras, identificados no Diretório de Repositórios de Acesso Aberto (OPENDOAR). De forma quantitativa, verificou-se na literatura a existência de indicadores bibliométricos e altmétricos nas publicações de artigos brasileiros, bem como a existência de plug-ins para RIs que utilizam o sistema DSpace e, de forma qualitativa, a análise envolveu artigos nacionais com o objetivo de verificar quais indicadores bibliométricos e altmétricos estão sendo utilizados por pesquisadores nacionais, norteando dessa forma, os indicadores que foram utilizados para a análise junto aos RIs brasileiros. Os resultados alcançados compreenderam a identificação e a análise da presença nos RIs brasileiros dos indicadores bibliométricos e altmétricos e sua visualização gráfica, plug-ins existentes para RIs que utilizam a tecnologia DSpace e o desenvolvimento de uma nova solução. Observou-se que os indicadores bibliométricos apresentaram um índice maior de ocorrência na amostra analisada do que os indicadores altmétricos e a incipiência dos RIs em disponibilizar os indicadores utilizando uma representação visual. Conclui-se que a discussão dos resultados alcançados pode contribuir para a compreensão da problemática em torno da utilização dos RIs, como fontes de informação, para a elaboração e visualização de indicadores bibliométricos e altmétricos, e consequentemente para o entendimento da dinâmica das atividades científicas desempenhadas pelas ICTs brasileiras.
