The Differences Between the Energy Metabolism of the Annulus Fibrosus and the Nucleus Pulposus Cells of the Intervertebral Disc

Back pain is one of the most common physical conditions in the United States, for which approximately 15% of the population will visit a doctor every year. The most common type of back pain is low back pain (LBP) and millions of dollars are spent every year healthcare are a due to LBP. Although poorly understood, low back pain has been associated to interveterbral disc (IVD) degeneration. The IVD is an important structure that helps maintaining normal skeletal support. It is composed of three different tissues called the annulus fibrosus (AF), and the nucleus pulposus (NP), attached to a cartilage endplate (CEP) at its top and bottom surfaces. The AF tissue is composed of chondrocyte-like cells, while the NP tissue is composed of notochordal cells at a young age, which are replaced by mature NP cells later in life. Common signs of degeneration are the inability to maintain extracellular matrix integrity and calcification of the cartilage endplate. Extracellular matrix synthesis and cartilage endplate calcification are closely related to production of adenosine triphosphate (ATP) or energy metabolism of the cells. AF and NP tissues are known to be structurally and compositionally different; therefore it is believed that their metabolic pathways are also distinct. The objective of this study was to determine the differences between AF and NP cells, specifically in their energy metabolism with and without dynamic loading.

An Investigation of the Effects of Exogenous Crosslinking of Bovine Annulus Fibrosus Tissue

This study investigates the changes due to crosslinking treatment in stiffness, permeability, and glycosaminoglycan (GAG) content of bovine intervertebral discs. The objective of this study was to determine the mechanical and biochemical effects of crosslinking treatment on lumbar bovine tissue. Previous studies have found that crosslinking can increase stiffness and permeability in the intervertebral disc. These changes have not yet been investigated by confined compression, stress-relaxation tests of young bovine tissue. Eleven lumbar motion segments were harvested from calf spines and soaked in a saline solution or one of four crosslinking treatments (genipin, methylglyoxal, proanthrocyanidin, and EDC). Five mm diameter samples were removed from the midannulus region at anterior / anterior-lateral locations, confined in a saline bath, swelled to equilibrium, and tested in confined compression stress-relaxation to 15% strain in 5% increments. Radial samples were also harvested, treated with saline solution and EDC, and tested in the same manner. The aggregate modulus and hydraulic permeability were calculated using the nonlinear biphasic theory. Swelling pressure was calculated as the load at swelling equilibrium. GAG content was measured using the dimethylmethylene blue assay. Differences with P value < 0.05 were considered significant. In the axial orientation, all crosslinking treatments except methyglyoxal at least doubled the aggregate modulus relative to soaked controls (P less than 0.05). Genipin treatment resulted in 78% lower axial permeability, proanthrocyanidin (PA) 50% lower, and EDC treatment 84% lower relative to soaked controls (P &lt; 0.05). GAG content measured in the methyglyoxal treatment group was 25% lower than in soaked control group. Genipin (G), proanthrocyanidin (PA), and EDC treatment increased the swelling pressure by at least 65% (P less than 0.05). In the radial orientation, EDC treatment increased the stiffness by 75%, and did not significantly affect the permeability or swelling pressure. Some crosslinking treatments proved effective in increasing the stiffness and swelling pressure of the disc. The increased swelling pressure in G, PA, and EDC treatment groups relative to soaked controls suggests reduced GAG leaching during soaking treatment, further confirmed by the reduction in permeability in these groups.

Local heat transfer coefficients in an annular passage with flow turbulation

In this experimental and numerical investigation, the use of flow turbulation was considered as a method to increase local heat transfer coefficients in annular heat transfer passages. Experimental data was obtained for cases with and without inserted ring turbulators within a horizontal annular test section using water for average Reynolds numbers ranging from 2000 to 7500 and average Prandtl numbers ranging from 6.73 to 6.79. The test section was heated uniformly on the inner annular wall and had a hydraulic diameter of 14.8mm, a diameter ratio (inner wall diameter to outer wall diameter) of 0.648, and a length to hydraulic diameter ratio of approximately 74. A set of circular cross sectioned ring-type turbulators were used which had a thickness of 1mm, a ring diameter of 15.1mm and a pitch of 50mm. It was found that the presence of the flow turbulators increased the average Nusselt number by between 33.9% and 45.8%. The experimental tests were followed by numerical simulations to identify the response in the heat transfer coefficient by changing the geometry of the turbulators. For this, the turbulator diameters were ranged from 0.5 mm to 2 mm, and the gap size (between the inner wall and a turbulator ring) ranged from 0.125 mm to 4 mm at a pitch of 50 mm. The results showed that the use of turbulators increased the Nusselt numbers by a maximum of 34.8% and that the maximum can be achieved for a turbulator diameter of 2 mm and a gap size of 0.25 mm, for all the Reynolds numbers tested. Bedömningen av höger kammares systoliska funktion kan med fördel göras med ekokardiografi, bl.a. med tekniker som tvådimensionell ekokardiografi, motion-mode (M-mode) och Tissue Doppler Imaging (TDI). Mätvärden som kan fås ut av dessa tekniker är Tricuspid Annular Plane Systolic Excursion (TAPSE) och s`. TAPSE beskriver annulus tricuspidalis longitudinella rörlighet. s´ beskriver den maximala systoliska hastigheten i annulus tricuspidalis. Syftet med examensarbetet var att undersöka sambandet mellan TAPSE och s´ vid högerkammarfunktionsbestämning. Studien är en retrospektiv tvärsnittsstudie och består av 99 deltagare i åldrarna 18–90 år. Både friska och hjärt-och kärlsjuka deltagare inkluderades, men inte patienter med förmaksflimmer eller individer som var yngre än 18 år. Deltagarna undersöktes med transthorakal ekokardiografi (TTE) vid Länssjukhuset Ryhov i Jönköping, vid undersökningen mättes TAPSE och s´. Det var samma biomedicinska analytiker (BMA) som utförde undersökningarna. McNemars test utfördes och ett Kappa-värde räknades ut. Resultatet visade signifikant skillnad och Kappa-värdet var lågt vilket talar för dåligt samband mellan de båda variablerna. Utfallet i studien visar att fler deltagare klassas med normal högerkammarfunktion mätt med TAPSE, medan samma individer klassas med högerkammardysfunktion mätt med s´. Orsaken till det dåliga sambandet mellan parametrarna är okänt och bör studeras vidare i kommande studier. / The assessment of right ventricular systolic function is done by echocardiography, with techniques as twodimensional echocardiography, motion-mode (M-mode) and Tissue Doppler Imaging (TDI). Measurements ​​obtained from these techniques are Tricuspid Annular Plane Systolic Excursion (TAPSE) and s`. TAPSE describes the longitudinal movement of annulus tricuspidalis. s´ describes annulus tricuspidalis maximal systolic velocity. The purpose of this essay was to explore the relationship between TAPSE and s´. The study consists of 99 participants between the ages of 18 and 90. The population was consisted of individuals with or without cardiovascular disease, but not individuals with atrial fibrillation. Participants were investigated with transthorakal echocardiography (TTE) at Länssjukhuset Ryhov in Jönköping. At the examination both TAPSE and s´ was measured. McNemar's test was performed and a Kappa value was calculated. The result showed a significant difference and the Kappa value was low which indicates a low relationship between the variables. The outcome of the study shows that several participants are classified with normal right ventricular function measured with TAPSE, but the same individuals are classified with right ventricular dysfunction measured by s'. The reason for the poor relationship between the parameters is unknown and should be studied further in future studies.

Ce travail de thèse a pour ambition d'apporter une contribution à l'étude de la croissance du disque intervertébral. Il s'inscrit dans un projet de recherche qui vise à développer de nouveaux outils prédictifs afin d'améliorer le traitement de pathologies liées au phénomène de croissance des cartilages et des fibrocartilages. Un second objectif est d'apporter des éléments de compréhension qui pourraient être utiles pour le développement de techniques novatrices d'ingénierie tissulaire.La première partie expérimentale de ce travail consiste en l'identification de propriétés mécanique quasi-statique de l'annulus fibrosus du disque intervertébral, au travers d'une loi de comportement hyperélastique anisotrope, ainsi que d'un champ de déformations résiduelles présent au sein de l'annulus fibrosus, par le biais d'un outil de corrélation d'images numériques. Cette double identification permet à la fois d'estimer le champ de contrainte in vivo de l'annulus fibrosus, mais aussi d'obtenir une trace historique du processus de croissance qui nous servira de référence dans la suite de l'étude.La prise en compte des déformations résiduelles dans un modèle numérique d'annulus fibrosus a permis de constater l’homogénéisation spatiale des déformations lors de chargements physiologiques. Ces résultats soulignent l’importance de la considération des déformations résiduelles dans l’estimation des déformations et des contraintes subies in vivo par l’annulus fibrosus.De plus, un scénario de croissance de l'annulus fibrosus associé à deux critères mécaniques générateurs de croissance ont aussi été implémentés en utilisant la méthode des éléments finis. Les résultats de cette étude numérique n'ont pas permis de reproduire le champ de déformations résiduelles estimé expérimentalement. Seul le modèle de croissance utilisant un critère mécanique anisotrope prenant en compte la direction des fibres présentes dans l’annulus fibrosus et dans le cas d’un chargement omettant volontairement un chargement vertical sur l’annulus fibrosus a permis de reproduire qualitativement les déformations résiduelles tangentielles mesurées expérimentalement.Afin de compléter la compréhension du phénomène de croissance du cartilage, une étude biomécanique d'un modèle in vitro de cartilage de synthèse a mis en évidence l'impact du facteur de croissance TGF-beta3 sur la rigidité de la matrice extra-cellulaire. Une corrélation forte entre les expressions géniques des cellules de cartilage et les propriétés mécaniques de la matrice extra-cellulaire a été trouvée. Cette corrélation forte entre l’activité cellulaire et la rigidité de la matrice extra-cellulaire, couplée à la difficulté des modèles de croissance actuels uniquement basés sur des critères mécaniques, ouvre d’intéressantes perspectives d’études sur la compréhension du phénomène de croissance sous contrainte mécanique. L'étude approfondie de cet échantillon permettra à terme d'enrichir les modèles de croissance afin de prendre en compte les différents phénomènes physiques présent au sein des tissus biologiques. / This thesis aims to contribute to the study of the intervertebral disc growth. It is a part of a global reasearch program which focuses on the development of new predictive tools to improve treatments related to cartilage and fibrocartilage growth phenomenon pathologies. A second objective is to provide elements of understanding that could be useful for the development of innovative tissue engineering technics.The first experimental part of this work consists in the identification of quasi-static mechanical properties of the annulus fibrosus of the intervertebral disk, through an anisotropic hyperelastic constitutive law, as well as a residual strains field present within the annulus fibrosus, through a digital image correlation tool. This double identification makes it possible both to estimate the stress field in vivo of the annulus fibrosus, but also to obtain a historical trace of the growth process which will serve us as a reference in the rest of the study.We note the spatial homogenization of the deformations during physiological loadings when residual strains field is take into account in a numerical model of annulus fibrosus. These results underline the importance of the consideration of residual strains in the estimation of the deformations and stresses undergone by the annulus fibrosus.In addition, a growth scenario of the annulus fibrosus associated with two mechanical growth criterias has also been implemented using the finite element method. The results of this numerical study did not make it possible to reproduce the residual strains field estimated experimentally. Only the growth model using an anisotropic mechanical criterion taking into account the fibers direction present in the annulus fibrosus and in the case of a deliberately omitted load in the vertical direction on the annulus fibrosus has made it possible to qualitatively reproduce the tangential residual strains measured experimentally.In order to complete the understanding of cartilage growth, a biomechanical study of a synthetic in vitro cartilage model revealed the impact of the TGF-beta3 growth factor on the extracellular matrix cartilage stiffness. A strong correlation between the cartilage cell gene expressions and the mechanical properties of the extracellular matrix was found. This strong correlation between cellular activity and the rigidity of the extracellular matrix, coupled with the difficulty of the current growth models based solely on mechanical criteria, opens up interesting prospects for studies on the understanding of the phenomenon of growth under mechanical stimulus. In-depth study of this sample will eventually enrich the growth models to take into account the different physical phenomena present in biological tissues.

Le transporteur anionique TAT1 (SLC26A8) : rôle physiologique et implication dans les asthénozoospermies humaines / Anion transporter TAT1 (SLC26A8) : physiological role and involvement in human asthenozoospermia

La protéine TAT1 (Testis Anion Transporter 1 ; SLC26A8) appartient à la famille des SLC26, une famille de transporteurs d’anions qui contribuent dans différents épithelia à l’homéostasie cellulaire. La protéine TAT1 s’exprime exclusivement dans les cellules germinales mâles, chez l’homme et chez la souris. Sur le spermatozoïde mature, la protéine TAT1 est localisée à la jonction des pièces intermédiaire (PI) et principale (PP) du flagelle, au niveau de l’annulus, une structure en forme d’anneau composée de différents polymères de Septines (1, 4, 6, 7 et 12).Le modèle murin d’invalidation du gène Tat1 présente une infertilité mâle par asthénozoospermie totale (absence de mobilité des spermatozoïdes) et des défauts de capacitation associés à des anomalies structurales du flagelle (plicature du flagelle, disjonction entre la PI et la PP, atrophie de l’annulus). Ce modèle indique que la protéine TAT1 pourrait avoir un rôle structural dans le maintien de l’annulus et dans la mise en place du flagelle. Par ailleurs, la protéine TAT1 possédant une activité de transport d’anions, il est vraisemblable qu’elle puisse influer directement sur la régulation de la mobilité et de la capacitation puisqu’il est bien établi que les échanges ioniques sont essentiels au contrôle de ces deux processus.En effet, les ions chlorure, bicarbonate et calcium participent à l’activation de la voie de signalisation AMPc/PKA, au cours des processus de mobilité et de capacitation (i.e. processus de maturation ayant lieu dans le tractus génital féminin et conférant au spermatozoïde un mouvement hyperactivé et la capacité à interagir avec l’ovocyte).Plusieurs travaux ont montré une interaction physique et fonctionnelle des membres de la famille SLC26 avec le canal chlorure/bicarbonate CFTR (Cystic Fibrosis Transmembrane conductance Regulator) dont les mutations sont responsables de la mucoviscidose. De manière intéressante des données récentes ont montré l’expression de CFTR dans le spermatozoïde et son rôle dans la régulation des flux de chlorure au cours de la capacitation. Au cours de ma thèse, nous avons testé la coopération entre les protéines TAT1 et CFTR ; nous avons pu montrer que la protéine TAT1 est capable d’interagir physiquement avec CFTR et de stimuler son activité de transport d’anions, suggérant qu’in vivo les deux protéines forment un complexe moléculaire impliqué dans la régulation des flux de chlorure et de bicarbonate dans le spermatozoïde.Tout comme TAT1, plusieurs membres de la famille SLC26 ont une expression tissulaire spécifique. Par ailleurs, les mutations génétiques de certains SLC26 sont associées à des pathologies humaines (surdité, diarrhée chlorurée congénitale et chondrodysplasie). De par le phénotype du modèle murin Tat1 et l’importance des SLC26 en pathologie humaine, TAT1 constitue un bon candidat dans la recherche des causes génétiques des asthénozoospermies humaines.Le laboratoire a mis en place au cours de ma thèse, un projet de recherche de mutations du gène TAT1 dans les asthénozoospermies humaines. Le séquençage des régions codantes du gène TAT1 dans une cohorte de 147 hommes infertiles par asthénozoospermie a ainsi permis d’identifier des variations de séquence inédites du gène chez 7 sujets. L’étude in vitro de certains variants indique pour trois d’entre eux une instabilité des formes mutantes associée à un défaut de stimulation du canal CFTR, in vitro. Par ailleurs, les spermatozoïdes de ces patients présentent d’importantes anomalies flagellaires dans la mise en place de la pièce intermédiaire, compatible avec un rôle de la protéine TAT1 et de ses partenaires (les septines) dans la genèse du flagelle / TAT1 (Testis Anion Transporter 1 ; SLC26A8) belongs to the SLC26 family of anion transporters, which is implicated in cellular homeostasis of different epithelia. TAT1 is exclusively expressed in male germ cells, in human and mouse. On mature spermatozoa, TAT1 is located at the annulus, a ring-shaped structure composed of different septins polymers (1, 4, 6, 7 and 12), at the junction of the midpiece (MP) and principal piece (PP) of the flagellum.The knock-out mouse model of Tat1 gene shows a male infertility by complete asthenozoospermia (lack of sperm motility) and capacitation defects combined with flagellar structural abnormalities (flagella bending, MP and PP disjunction and atrophy of the annulus). This model suggests that the TAT1 protein could fulfill structural roles in the annulus and during flagellum biogenesis. Moreover TAT1 displayind an anion transport activity, it could also be implicated in the control of sperm motility and capacitation by regulating anions exchannges, which are well known to be essential for both processes.Indeed, chloride, bicarbonate and calcium ions are involved in the activation of the cAMP/PKA pathway, controlling sperm motility and capacitation processes (i.e. maturation events occuring in the female genital tract and providing the spermatozoa an hyperactivation movement and the ability to interact with oocyte).Several publications have reported a physical and functionnal interaction between SLC26 family members and the chloride/bicarbonate CFTR channel (Cystic Fibrosis Transmembrane conductance Regulator), which mutations are responsible of cystic fibrosis. Interestingly, recent data showed CFTR expression in spermatozoa and its role in the regulation of chloride fluxes during capacitation. During my thesis, we tested TAT1 and CFTR cooperation; we showed that TAT1 can interact physically with CFTR and stimulate its anion transport activity, suggesting that in vivo they form a molecular complex involved in the regulation of chloride and bicarbonate fluxes during sperm capacitation.Like TAT1, several SLC26 family members have a tissue specific expression. Furthermore genetic mutations in several SLC26 members result in human pathology such as deafness, congenital chloride diarrhea and chondrodysplasia. According to the phenotype of the KO Tat1 mouse model and the role of SLC26 members in human pathology, TAT1 constitutes a good candidate for the search of genetic causes of human asthenozoospermia.During my thesis, the laboratory has set up, a research project aiming at identifying mutations in the TAT1 gene that are responsible for human asthenozoospermia.Sequencing of the TAT1 gene coding regions in a cohort of 147 infertile men presenting with asthenozoospermia allowed us to identify several new sequence variations in in the TAT1 gene. In vitro study of these variants shows that 3 of them are associated with protein instability and abrogate CFTR stimulation. Besides, patients sperm show important flagellar abnormalities in the midpiece, consistent with a role of TAT1 and its partners (septins) in flagellum biogenesis.

Diplomová práca je zameraná na popísanie prúdenia tekutiny medzikruhovým potrubím. Tok tekutiny je skúmaný pri viacerých podmienkach, čo vedie k laminárnemu ale aj turbulentnému prúdeniu. Práca obsahuje rešeršnú časť, ktorá opisuje doteraz známe vzťahy popisujúce daný typ prúdenia a takisto skúma spôsoby merania rýchlostného profilu v medzikruhových medzerách. Nasledujúca časť obsahuje návrh výpočtového modelu, popis simulácií prúdenia pri konkrétnych podmienkach a ich vyhodnotenie. Výsledky zo simulačnej časti sú použité pri popise charakteristických vlastností turbulentného prúdenia v medzikruží. Tieto charakteristiky sú použité v ďalšej časti práce, ktorá je cielená na získanie analytického vzťahu, ktorý by popisoval časovo stredovaný rýchlostný profil turbulentného prúdenia v medzikruhovej medzere.

Echocardiographic measurements at Takotsubo cardiomyopathy : transient left ventricular dysfunction

Takotsubo cardiomyopathy (TTC) is a disease characterized by transient left ventricular (LV) dysfunction and typical wall motion abnormalities in apical parts, without obvious signs of coronary influence. Due to its elusive natural cause and the lack of clarified pathology, further studies are needed. Thirteen patients presented with an episode of TTC, and referred to Örebro University Hospital (USÖ), were prospectively included and investigated by comparisons made at onset (acute phase) against at follow-up three months later (recovery phase). Including echocardiographic measurements, focused on biventricular systolic long-axis function and conventional diastolic function (DF) variables. Systolic improvement was shown, while most DF data were unchanged, suggesting that TTC is mainly a systolic disease affecting both ventricles. Diagnosis should include multidisciplinary engagement, as TTC associates both with emotional stress and pathological markers of physiological stress. In this thesis, such approach was offered to the aforementioned patients; to see if a common denominator could be found, thus, contributing to better handling. Emotional state was assessed, along with an array of cardiac investigations in addition to echocardiography. Acutely, imbalance in the autonomic cardiac control was shown, as well as a trend toward posttraumatic stress, but specific findings allowing conclusions on differential diagnosis could not be demonstrated. By adding another 15 TTC patients (i.e. 28 in total), through collaboration with observers from USA, a retrospective echocardiographic analysis could be done to further study DF; concluding that TTC associates with impairment of conventional DF variables which tends to parallel the systolic recovery, in contrary to the initial result but in line with other causesof LV dysfunction. Magnetic resonance imaging (MRI) is another method of choice at TTC. The USÖ patients had cardiac MRI, thus, a retrospective analysis was done to investigate the effect on LV geometry, both echocardiographic and by MRI; suggesting that TTC is consistently associated with increased LV mass, due to a local impact that seems to follow the change in LVconcentric wall motion.
