Os indivíduos do estudo foram submetidos a um protocolo de suplementação com cogumelo ou placebo por 24 semanas, e foram distribuídos aleatoriamente nos seguintes grupos: Grupo tratado com placebo (5g/dia n=14) e Grupo tratado com cogumelo (5g/dian= 9).Todas as análises foram obtidas antes e após os tratamentos (placebo ou cogumelo). O estado nutricional (dados antropométricos, de composição corporal, da bioquímica nutricional e do consumo alimentar), foi semelhante entre os grupos. Several species of edible fungi have been explored as to its potential medical and many patients now have to seek a solution to their condition in complementary therapies. Objective: To evaluate the effects of supplementation of Agaricus brasiliensis on the nutritional status, the frequency of adverse effects of antiviral therapy and liver damage in patients with hepatitis C treated with pegylated interferon and ribavirin. We performed a prospective trial randomized controlled double-blind in the service of Viral Hepatitis Hospital of the Medical School of Botucatu, UNESP. Those in the study were subjected to a memorandum of supplementation with mushroom or placebo for 24 weeks, and were randomly distributed in the following groups: placebo-treated group (5g/dia n = 14) and mushroom-treated group (n 5g/dia- = 9). All tests were obtained before and after the treatments (placebo or mushroom). The nutritional status (anthropometric data, body composition, nutrition and biochemistry of food intake), was similar between the groups. There was improvement of liver damage in both groups, reducing transaminase (AST / ALT and AST / ALT, p <0.05), demonstrating the effectiveness of treatment antiviral use of the mushroom was shown to be beneficial in reducing the frequency and intensity of the adverse effects of drug therapy (myalgia, dysgeusia, headache, reduction in sexual desire, hair loss, hipoanorexia, malaise, dry mouth and irritability, p <0.05). Modelling the transmission dynamics of multi-strains influenza with vaccination and antiviral treatment

Thesis (MSc)--Stellenbosch University, 2012. / ENGLISH ABSTRACT: Recently, new strains of influenza such as bird flu and swine flu have emerged. These strains have the capacity to infect people on a quite large scale and are characterized by their resistance to existing influenza treatment and their high mortality rates. In this thesis, we consider two models for influenza transmission dynamics that include both sensitive and resistant strains and accounts for disease induced mortality. The first model allows for immigration/migration and does not include any control measure. The second one explores the effects of vaccination and treatment of the sensitive strain but ignores immigration/migration. We studied the two models mathematically and numerically. We started with the model without any control measures; we calculated the basic reproductive numbers, determined the equilibrium points and investigated their stability. Our analysis showed that when the basic reproduction numbers of both strains are less than one then the two strains will die out. When at least one of the basic reproduction numbers is greater than one, then the strain with the higher basic reproduction number is the one that will persist. Numerical simulations were carried out to confirm the stability results and a bifurcation diagram was given. We also studied numerically the impact of the mortality rate of influenza on the dynamics of the disease. Especially, we investigated the effect of the mortality rate on the time needed for the pandemic to reach its peak, the value at the peak for each strain and, when eradication is possible, the time it takes for the disease to be eradicated. For the model with control, we also calculated the control reproductive number and the equilibrium points. The stability analysis was carried out numerically and bifurcation diagrams with vaccination and treatment parameters were given to determine the regions where eradication of the disease is possible. Our results suggest that in the presence of a resistant strain, treating more infected individuals will not eradicate the disease as the resistant strain will always persist. Depressão no Tratamento da Hepatite C

To prospectively evaluate, depressive symptoms and risk factors for major depression in patients with hepatitis C virus (HCV) treated with antiviral combined therapy. This study is a convenience cohort that evaluated 50 patients with HCV by the structured diagnostic interview - Mini International Neuropsychiatric Interview (MINI) to screen for depressive symptoms before antiviral combined therapy, and in the follow-up visits (4 and 12th week). Laboratorial analysis were performed during the follow-up. The study was approved by the University s Ethics Committee (151.642). We evaluated 50 patients, in which prevalence of genotype 1 was 42%. Pegylated interferon alpha (IFN-&#945;) and ribavirin was the most prevalent treatment used for HCV (86%). During the follow-up of patients, treatment for HCV increases the risk of depression in the 4th week (43.5.9%), but not at 12th week (30.7%) treatment compared with the baseline (25.6%) (p=0.04). We found differences between the prevalences of depression and genotypes of the virus in regard to time of the follow-up with higher odds ratio in the 4th week (OR=2.2) compared to baseline and 12th week (OR=1.8) using pairwise comparisons with Bonferroni adjustment (p=0.03). Also, patients with genotype 2 and 3 had significantly lower odds of presenting depression compared genotype 1 (p&#8804;0.05). However, the average score on the BDI-II did not differ in the follow-up.This study provide evidence of an association between HCV genotype and major depression. During the follow-up, depressive symptoms increase in 4th week, corresponding to conditions of immune activation. Metodologia: Este estudo é uma coorte de conveniência , que avaliou 50 pacientes com HCV por entrevista diagnóstica estruturada - Mini International Neuropsychiatric Interview ( MINI ) - para triagem de sintomas depressivos antes da terapia antiviral combinada , e nas visitas de acompanhamento (4 e 12 semanas ) . A análise laboratorial foi realizada durante o follow-up. O estudo foi aprovado pelo Comitê de Ética da Universidade (151,642). Resultados: Foram avaliados 50 pacientes, nos quais a prevalência do genótipo 1 foi de 42% . Interferon peguilado alfa (IFN - &#945;) e ribavirina era o tratamento mais prevalente utilizado para HCV ( 86 % ) . Durante o seguimento de pacientes, o tratamento para HCV aumenta o risco de depressão na 4 ª semana ( 43.5 %) , mas não a 12 ª semana (30,7%) em comparação com o tratamento inicial ( 25,6% ) (p = 0,04) . Foram encontradas diferenças entre as prevalências de depressão e genótipos do vírus em relação ao tempo de seguimento com maior razão de odds na 4 ª semana (OR = 2,2 ) em relação à linha de base e 12 ª semana (OR = 1,8 ), utilizando comparações pareadas com ajuste de Bonferroni (p = 0,03). Além disso, os pacientes com genótipo 2 e 3 tiveram chances significativamente menores de apresentar depressão, em comparação genótipo 1 (p &#8804; 0,05). No entanto, a pontuação média no BDI- II não foi diferente no estudo follow- up. Conclusão: Este estudo fornece evidências de uma associação entre o genótipo HCV e depressão maior. Durante o seguimento, os sintomas depressivos aumentam na 4 ª semana, o que corresponde a condições de ativação imune . A depressão maior em pacientes HCV influencia a qualidade de vida e sua adesão ao tratamento antiviral, sendo importantes programas de rastreio, para a identificação precoce e tratamento da depressão induzida por interferon.

Tyrimo tikslas - įvertinti sergančiųjų lėtiniu virusiniu C hepatitu gyvenimo kokybę ir nustatyti ją įtakojančius veiksnius. Tyrimo uždaviniai. Įvertinti demografinių veiksnių įtaką sergančiųjų lėtiniu virusiniu C hepatitu gyvenimo kokybei. Ištirti sergančiųjų lėtiniu virusiniu hepatitu C žinių apie šią ligą lygį. Įvertinti sergančiųjų lėtiniu virusiniu C hepatitu turimų žinių įtaką gyvenimo kokybei. Įvertinti specifinio priešvirusinio gydymo šalutinių reiškinių įtaką gyvenimo kokybei. Įvertinti slaugytojų vaidmenį gerinant šių pacientų gyvenimo kokybę. Tyrimo populiacija. VUL Santariškių klinikų Hepatologijos, gastroenterologijos ir dietologijos centre gydomi virusiniu hepatitu C sergantys 52 pacientai. Tyrimo metodai. Tyrimas atliktas pritaikant anketavimo ir medicininių dokumentų analizės metodus. Taikyta pacientų apklausos anketa žinių apie ligą ir specifinį gydymą įvertinimui ir SF- 36 gyvenimo kokybės vertinimo klausimynas. Statistinė analizė atlikta naudojant „Microsoft Office Excel 2003“ ir SPSS 15,0 for Windows versijos statistinę programą. Darbo rezultatai ir išvados. Tyrimo metu nustatyta, kad statistiškai patikimai žemesniais balais vyrai vertino šias gyvenimo kokybės sritis – „aktyvumo apribojimą dėl fizinių problemų“ ir „socialinę veiklą“, lyginant su tais pačiais moterų rodikliais. Amžius bei išsilavinimas neturėjo įtakos pacientų gyvenimo kokybei. Ištyrus sergančiųjų lėtiniu VHC žinių apie ligą lygį, nustatyta, kad jų žinios apie ligą nepakankamos. Geriausios...
