Experiencias en el alivio del dolor del neonato en enfermeras de un hospital II-2 de Chiclayo, 2022

En la actualidad el dolor en el neonato es una necesidad que debe ser atendida por los profesionales de enfermería, debido a que, durante la hospitalización, se le somete a diferentes procedimientos dolorosos y un cuidado deficiente puede llevar a alteraciones en su salud a futuro, por lo que debe darse la importancia respectiva en un momento oportuno. El objetivo del presente estudio fue describir y analizar las experiencias en el alivio del dolor del neonato en enfermeras de un hospital II-2 de Chiclayo, 2022. El abordaje metodológico fue de tipo cualitativo con diseño descriptivo, se tuvo como técnica e instrumento de recolección de datos la entrevista semiestructurada, la cual fue validada por juicio de expertos; el estudio fue aprobado por el comité de ética institucional y acreditado por estudio piloto. La muestra fue obtenida por saturación y redundancia, en la que participaron 14 enfermeras del servicio de neonatología, los datos fueron procesados con la técnica análisis de contenido; durante el proceso de la investigación, se asumieron los principios éticos y de rigor científico, se halló como resultado 3 categorías: Identificación del dolor en el neonato, Tratamiento utilizado para aliviar el dolor y Uso de instrumentos para valorar el dolor. Finalmente se llegó a la conclusión que, la valoración del dolor no se encuentra estandarizada en el servicio de neonatología y las enfermeras emplean según su criterio estrategias farmacológicas y no farmacológicas para aliviar el dolor en el neonato. / At present, pain in the newborn is a need that must be addressed by nursing professionals, because, during hospitalization, he is subjected to different painful procedures and poor care can lead to changes in his health in the future. Therefore, the respective importance should be given at an opportune moment. The objective of the present study was to describe and analyze the experiences in the relief of pain in the newborn in nurses of a II-2 hospital in Chiclayo, 2022. The methodological approach was qualitative with a descriptive design, it was used as a collection technique and instrument. of data the semi-structured interview, which was validated by expert judgment; the study was approved by the institutional ethics committee and accredited as a pilot study. The sample was obtained by saturation and redundancy, in which 14 nurses from the neonatology service participated. The data was processed with the content analysis technique; During the research process, ethical principles and scientific rigor were assumed, 3 categories were found as a result: Identification of pain in the newborn, Treatment used to relieve pain and Use of instruments to assess pain. Finally, it was concluded that pain assessment is not standardized in the neonatology service and nurses use pharmacological and non-pharmacological strategies to relieve pain in the newborn according to their criteria.

The impact of language on the expression and assessment of pain in children aged 4-7 years : a mixed methods study

The assessment of pain in children has been an enduring theme in the research literature over many decades, with particular focus on how pain can be adequately measured and the extent of under-measurement of pain (American Academy of Pediatrics 2001; Coyne, 2006; McCaffery & Beebe 1989; Subhashini et al., 2009). Definitions of pain, and hence development of pain measurement tools, are often criticised for not addressing the influence of culture and ethnicity on pain (Bates et al., 1993; McCaffery & Beebe 1989; Zinke, 2007), in children, the perception and expression of pain is also affected by cognitive development (Hallström and Elander, 2004). Whilst there has been an increase in the number of children living in the United Kingdom (UK) who do not speak English as their first language, it has been acknowledged that the measurement and management of pain by health service professionals relies predominantly on their experience with English speaking children (RCN, 2009). This study aimed primarily to examine how primary school age children in key stage 1 who speak English as a primary or additional language experience, express, and explain pain. This aim was addressed through three research questions: (1) how do primary school age children in key stage 1 talk about pain? (2) What are the similarities and differences in the language used to talk about pain by children with English as a primary and additional language? (3) Are there differences in the perceptions of pain by children of different age, gender, language background, and country of birth? A second aim was to examine whether language would affect actions taken by final year child health students and nurses working in Minor Injuries Units to manage pain in primary school age children. Study objectives were addressed using a two phase mixed methods design. The first aim was addressed using six focus group interviews with groups of primary school children (aged 4-7) (Phase 1). Two methods were used in the interviews: use of drawings from the Pediatric Pain Inventory (Lollar et al., 1982) to capture the language used by children to describe pain and observation of the children’s placing of pain drawings on red/amber/green paper to denote perceived severity of pain. Following data collection, the vocabulary of each child was assessed using a standardised lexical test (British Picture Vocabulary Score version II - BPVS II) (Dunn et al., 1997). To address the second aim, a factorial survey was conducted (Phase 2) with nurses working in Minor Injuries Units and child health nursing students to determine whether language has an impact on decisions made about the management of children in pain following a minor injury. Phase 1 findings demonstrated that children from English as an Additional Language (EAL) backgrounds used less elaborate language when talking about pain but tended to talk about the pictures prior to deciding where they should be placed. The children’s placement of pain drawings varied according to language background, gender, and age. The calculated language age of English lexical comprehension (BPVS II score) of monolingual children (M=69.85, SD=19.27) was significantly higher than EAL children (M=47.93, SD=14.32; t (32) = 3.60, p =0.001, two-tailed). However, when these differences were explored in terms of year group, the differences remain significant with foundation and year 2 but not with year 1. For the EAL children, there were also significant relationships between BPVS II score and length of stay in the UK (spearman’s rho 0.749, p = 0.33). The Phase 1 findings were used to construct vignettes, describing hypothetical care situations, for Phase 2. Multinomial logistic regression was used to analyse the impact of a child’s age, gender, language, parent’s language, injury mechanism, and reaction to pain on the way in which the child’s pain would be assessed and whether parents or an interpreter would be invited to assist in pain assessment. Findings demonstrated that observing the child’s behaviour is the most significant assessment process that is used to assess EAL children, rather than the Visual Analogue Scale (VAS), which was used with non-EAL children. This is significant as VAS is the mostly widely used tool to assess pain in health care settings. However, VAS is only effective if it can be understood by the child. Further, MIU nurses and child health students were more likely to involve parents who speak English well than those who speak English poorly but would ask for an interpreter if their involvement was necessary. In order for the respondents to explain their decisions, they were asked an open ended question for each vignette. They reported that language and age of children are the most common difficulties they faced during assessment of pain. Therefore, they suggested some solutions, like using an age appropriate tools for assessing younger children. Respondents also identified that using an interpreter is a time consuming process, which might delay the management of pain. In light of the growing numbers of EAL children in the UK; this research has application in a number of contexts. The variation in language would apply if children were reporting their own pain. However, the findings emphasise the need for sufficient time to be allocated to pain assessment to allow an individualised approach. Study findings suggest several factors that may be important in assessing pain in EAL children; these should be explored further in the context of clinicians’ assessment of pain. The implications of the study impact on policy, practice, education, and future research.

Estudo da estimulação elétrica nervosa transcutânea (TENS) nível sensório para efeito de analgesia em pacientes com osteoartrose de joelho / Study of transcutaneous electrical nerve stimulation (TENS) for the sensory effect of analgesia in patients with knee osteoarthritis

INTRODUÇÃO: A osteoartrose de joelho (OAJ) é uma das patologias que mais afetam o aparelho locomotor e interferem de forma considerável nas atividades da vida diária, acometendo os indivíduos em faixa etária variável e em diversos graus e níveis de dor articular. As propostas terapêuticas são bastante diversificadas para a osteoartrose (OA), a terapia física, a TENS está para colaborar no tratamento dos pacientes com osteoartrose de joelho. OBJETIVOS: Devido à importância da articulação do joelho nas atividades locomotoras e sua relação com as atividades da vida diária. Os pacientes com osteoartrose de joelho sofrem não somente pelo problema localizado, existe uma alteração funcional global que pode levar a processos dolorosos em outras articulações. Para uma eficaz orientação e avaliação do paciente primeiro é necessário que este faça a descrição clara de sua dor para que o profissional perceba como que esse estímulo doloroso prejudica sua vida. O objetivo do presente estudo foi avaliar o efeito da estimulação elétrica nervosa transcutânea (sigla em inglês TENS) nível sensório para tratamento de dor em pacientes com diagnóstico de osteoartrose de joelho, utilizando escalas de avaliação de dor, escala de avaliação funcional. MÉTODOS: Foram estudados 10 pacientes com diagnóstico de osteoartrose de joelho, utilizando uma modalidade fisioterapêutica, a eletroterapia, especificamente a (TENS), com parâmetros de 80 Hz e 140s, com um total de 10 sessões, 30 minutos para cada sessão, o período total de tratamento compreendeu 4 semanas, nesse total de 10 sessões, cada paciente foi entrevistado com um questionário na 1ª sessão, na 5ª e na 10ª sessão. Os formulários utilizados foram Questionário da Dor McGill Melzack, escala Analógica visual da dor, escala numérica com expressão facial de sofrimento, escala de atividades da vida diária da knee Outcome Survey - RESULTADOS: O resultado com a TENS nível sensório para efeito de analgesia mostra que as diferenças foram estatisticamente significativas (p<0,05) para escala Analógica visual da dor e o Questionário da Dor McGill, em alguns itens da escala de atividades da vida diária da knee Outcome Survey houve significância estatística. CONCLUSÃO: Nossos resultados sugerem que a TENS nível sensório reduz a dor e melhora a funcionalidade do joelho em alguns pontos da escala de atividades da vida diária em pacientes com OAJ / INTRODUCTION: Knee osteoarthritis (KOA) is one of the diseases that most affect the locomotor system and interfere significantly in activities of daily life, affecting individuals at varying ages and in different degrees and levels of joint pain. The proposals are quite diverse therapies for osteoarthritis (OA), physical therapy, TENS is to collaborate in the treatment of patients with osteoarthritis of the knee. OBJECTIVES: Because of the importance of the knee joint in locomotor activity and its relation to the activities of daily living. Patients with osteoarthritis of the knee not only suffer the problem located, there is a change in overall functional processes that can lead to pain in other joints. To provide efficient guidance and evaluation of the patient first must make this clear description of their pain for the professional to realize how painful stimulus that affect their lives. The aim of this study was to evaluate the effect of transcutaneous electrical nerve stimulation (TENS acronym in English) to the sensory processing of pain in patients with osteoarthritis of the knee, using rating scales of pain, functional scale. METHODS: We studied 10 patients with osteoarthritis of the knee, using a modality physiotherapy, electrotherapy, specifically (TENS), with parameters of 80 Hz and 140s, with a total of 10 sessions, 30 minutes for each session, the period Total treatment included 4 weeks in total of 10 sessions, each patient was interviewed with a questionnaire in the first session, on the 5th and 10th session. The forms used were the McGill Pain Questionnaire - Melzack, visual analogue pain scale, numerical scale with facial expression of pain, range of activities of daily living of the Knee Outcome Survey - RESULTS: The result with TENS for the effect of sensory analgesia shows that the differences were statistically significant (p <0.05) for visual analogue pain scale and McGill Pain Questionnaire, in some items of the scale of activities of daily living knee Outcome Survey of statistical significance. CONCLUSION: Our results suggest that sensory TENS reduces pain and improves knee function in some points of the scale of activities of daily living in patients with KOA

Avaliação e mensuração da dor no envelhecimento: instituições de longa permanência / Assessment and measurement of pain in aging: long-term institutions

O envelhecimento é um fenômeno novo e uma tendência mundial. Com ele, há um aumento de doenças e de queixas de dor. Este estudo permitiu que os profissionais de saúde tivessem uma melhor percepção na avaliação da dor no envelhecimento. O objetivo geral foi avaliar os diferentes tipos de dor crônica no envelhecimento. Os objetivos específicos foram mensurar a dor percebida, avaliar os descritores de dor crônica de maior atribuição, identificar diferentes temáticas de dor crônica percebida e avaliar sinais e sintomas de depressão. Foi realizada identificação sócio-demográfica, com perguntas relacionadas ao sexo, à idade, à escolaridade, à religião, ao estado civil, às atividades desenvolvidas na instituição, à situação econômica, ao tempo de institucionalização, ao recebimento de visitas e às doenças diagnosticadas e, posteriormente, foram feitos quatro Experimentos (amostras dependentes). Experimento 1 - Mensurar a dor percebida - a intensidade da dor crônica foi avaliada pelo método psicofísico de estimação de categorias e se percebiam dor no momento da entrevista, para identificar o local, o tempo e o horário. Experimento 2 - Avaliar os descritores de dor crônica de maior atribuição - a intensidade da dor foi avaliada por dois métodos psicofísicos independentes: estimação de categorias e estimação de postos. Experimento 3 - Identificar diferentes temáticas de dor crônica percebida - utilizou-se uma entrevista semiestruturada com sete perguntas relacionadas à dor crônica. Experimento 4 - Avaliar sinais e sintomas de depressão - utilizou-se a Escala de Depressão Geriátrica de 15 pontos. Participaram 46 idosos residentes em duas Instituições de Longa Permanência em Ribeirão Preto, São Paulo; dentre estes, 25 eram de instituição filantrópica e 21 de instituição particular. Os resultados foram a média de idade de 78,26±8,33 anos, 26 do sexo feminino, 23 viúvos, 28 católicos, 23 cursaram o ensino fundamental incompleto, 46 aposentados e 37 recebem até 1 salário mínimo, 16 tem hipertensão arterial, 37 residem nas instituições referidas no tempo de 0 a 5 anos, 34 declararam não realizar atividade nas instituições pesquisadas e 24 recebem visitas de familiares. No Experimento 1, 14 idosos atribuíram o valor 10 para a intensidade da dor percebida na última semana, sendo a média aritmética de 7,02±2,74 pontos; 28 relataram sentir dor no momento da entrevista e 32 não haver horário específico, sendo as regiões mais afetadas os membros inferiores e a região dorsal. No Experimento 2, no método de estimação de categorias, o descritor de dor de maior atribuição foi \"dolorosa\" e o de menor foi \"desastrosa\". No método de estimação de postos, o descritor de dor de maior atribuição foi \"desastrosa\" e o de menor atribuição foi \"dolorosa\". No Experimento 3, foram identificadas unidades temáticas como \"percepção quanto ao tempo\", \"dimensão da dor\", \"estratégias de enfrentamento\", \"causas relacionadas à dor\", \"percepção da situação atual\" e \"outras percepções\". No Experimento 4, observou-se que 33 idosos responderam à Escala de Depressão Geriátrica (EDG) - 15 pontos - e obtiveram um escore de 5 ou mais pontos podendo sugerir episódio de depressão, ao passo que 13 obtiveram um escore abaixo de 5 pontos, não sugerindo episódio de depressão. / Aging is a new phenomenon and a global trend. The specific objectives were to measure the perceived pain, to evaluate the descriptors of chronic pain with higher scores, identify different issues of chronic pain perceived and assess for signs and symptoms of depression. Identification was carried out socio-demographic, with questions related to sex, age, education, religion, marital status, the activities of the institution, the economic situation at the time of institutionalization, to receive visits and disease diagnosed and later, four experiments were conducted (dependent samples). Experiment 1 - Measuring the perceived pain - chronic pain intensity was assessed by the psychophysical method of category estimation and perceived pain during the interview, to identify the location, time and time. OBJETIVOS: Devido à importância da articulação do joelho nas atividades locomotoras e sua relação com as atividades da vida diária. Os pacientes com osteoartrose de joelho sofrem não somente pelo problema localizado, existe uma alteração funcional global que pode levar a processos dolorosos em outras articulações. Para uma eficaz orientação e avaliação do paciente primeiro é necessário que este faça a descrição clara de sua dor para que o profissional perceba como que esse estímulo doloroso prejudica sua vida. O objetivo do presente estudo foi avaliar o efeito da estimulação elétrica nervosa transcutânea (sigla em inglês TENS) nível sensório para tratamento de dor em pacientes com diagnóstico de osteoartrose de joelho, utilizando escalas de avaliação de dor, escala de avaliação funcional. MÉTODOS: Foram estudados 10 pacientes com diagnóstico de osteoartrose de joelho, utilizando uma modalidade fisioterapêutica, a eletroterapia, especificamente a (TENS), com parâmetros de 80 Hz e 140s, com um total de 10 sessões, 30 minutos para cada sessão, o período total de tratamento compreendeu 4 semanas, nesse total de 10 sessões, cada paciente foi entrevistado com um questionário na 1ª sessão, na 5ª e na 10ª sessão. Os formulários utilizados foram Questionário da Dor McGill Melzack, escala Analógica visual da dor, escala numérica com expressão facial de sofrimento, escala de atividades da vida diária da knee Outcome Survey - RESULTADOS: O resultado com a TENS nível sensório para efeito de analgesia mostra que as diferenças foram estatisticamente significativas (p<0,05) para escala Analógica visual da dor e o Questionário da Dor McGill, em alguns itens da escala de atividades da vida diária da knee Outcome Survey houve significância estatística. CONCLUSÃO: Nossos resultados sugerem que a TENS nível sensório reduz a dor e melhora a funcionalidade do joelho em alguns pontos da escala de atividades da vida diária em pacientes com OAJ / INTRODUCTION: Knee osteoarthritis (KOA) is one of the diseases that most affect the locomotor system and interfere significantly in activities of daily life, affecting individuals at varying ages and in different degrees and levels of joint pain.
