The role of systems-level variables in family adaptation to bereavement: a concept-validation study of cohesion and expressiveness

Understanding and prediction of children's adaptation to loss requires attention to family characteristics and interpersonal patterns in addition to individual variables. Empirical inquiries into family variables have indicated that the concepts of cohesion and expressiveness in particular may be useful in explaining members' adjustment. Using both deductive and inductive methods this study developed a reliable behavioral coding system for observing family members as they described the story of a child's death. The study also examined the relationship of these observable behaviors to both self-reports of cohesion and expressiveness and measures of individual adjustment. In terms of convergent validity the Expressiveness subscale of the Family Environment Scale (Moos & Moos, 1986) was more readily associated with observable behaviors than was the Cohesion subscale. Discriminant validity was not established, however, and possible explanations for this were discussed. Examination of criterion-related and predictive validity demonstrated the uti1ity of both se1f-report and behavioral measures of cohesion and expressiveness in accounting for parental depression, child behavior problems, and other specific indicators of distress. Implications for clinical intervention with bereaved families were discussed. / Ph. D.

A Canonical Correlational Analysis Exploring Characteristics of Children Presenting to Counseling for Grief and Loss

To date, researchers who have explored the complexity of childhood bereavement have utilized unstandardized assessment instruments and/or have independently evaluated specific constructs rather than factoring in the dimensionality of loss. The purpose of this study was to use parents' completion of established instruments--the Child Behavior Checklist and the Parenting Stress Index--to examine the multivariate shared relationship between characteristics of bereaved children referred for counseling--their ages, genders, ethnicities, types of loss, and life stressors--and their behavioral manifestations as well as the relationship between these characteristics and levels of parent-child relational stress. Utilizing archival clinical files, I examined these characteristics from bereaved children (N = 98) whose parents sought counseling services from two university-based counseling clinics. The sample consisted of 67 boys and 31 girls between the ages 3 and 11 years old (M = 6.28). The majority of participants (67%, n = 66) identified as Caucasian, 10% (n = 10) as African American, 10% (n = 10) as Hispanic/Latino, 6% as Bi-racial (n = 6), 4% as Native American (n = 4), and 2% as Asian (n = 2). A canonical correlational analyses (CCA) was conducted to examine relationship between characteristics of children and their subsequent behavioral manifestations. The full model was found to be statistically significant using the Wilks’s λ = .611 criterion, F(25, 328.41) = 1.862, p = .008. The R2 type effect size was .389, which indicates the full model explains about 39% of the variance shared between the two variable sets. A second CCA was conducted to explore the relationship between characteristics of bereaved children and levels of parent-child relational stress. The full model was found statistically to be significant using the Wilks’s λ = .790 criterion, F(10, 154) = 1.926, p = .045. The R2 type effect size was .210, which indicates the full model explains about 21% of the variance shared between the two variable sets. Overall, correlational findings from this study provided insight into bereaved children’s manifestations of loss and levels of parent-child relational stress as contingent upon specific characteristics. Specifically, results indicated a strong relationship between age and bereaved children’s behavioral manifestations. This finding reinforced the importance for clinicians to understand developmental implications when working with bereaved children. Furthermore, caregivers who reported minimal overall external stressors also reported less parent-child relational interference. This finding further emphasizes the importance for caregivers to maintain utmost stability for bereaved children.

Thesis (PhD)--Stellenbosch University, 2014. / ENGLISH ABSTRACT: This thesis deals with pastoral grief care for children orphaned through HIV and AIDS in Nairobi City County in Kenya (formerly Nairobi Province in Kenya). This concern developed as a result of realising that children’s grief is rarely attended to before, during and after burial in most Kenyan communities. The research question that guided the research was: What theoretical, theological and contextual pastoral intervention strategy can the church use to provide grief care to children orphaned through HIV and AIDS? To respond to this question, the research had four objectives, which helped the researcher to remain focused on the key problem. An empirical research design was adopted to establish the impact the death of parents has on children and to examine how their grief has been responded to, particularly by the church. It was clear that children do experience grief that manifests in various forms including emotional, behavioural, cognitive, spiritual and physical responses. Grief is also heightened by the events that transpire after the death of parents plus the status of the children as double orphans, paternal orphans or maternal orphans. Despite the fact that children are overwhelmed by grief and grief-related issues when their parents die, the family and community as well as the church, rarely provide children with grief care. Through a literature study the research engaged with other disciplines in order to broaden the understanding of children’s grief. The study revealed that children’s development in general is affected by their grief and their stage of development, in turn, has an effect on their grief. However, it should be noted that African cultures have great impact on the development of African children and their grief. It was also noted that African people have their own way of grieving and mourning when a family loses a person through death. Various rituals that are calculated to help the mourning family work to through the grief process are performed. Interestingly, children rarely participate in the performance of such rituals. It was very clear that, in order to provide pastoral grief care to children orphaned through HIV and AIDS, it is of utmost importance to understand the nature of human beings in relation to God. Such an understanding could determine how the church responds to the grieving children’s situation. It is suggested that the church should work closely with the family, community and institutions in order to provide meaningful pastoral grief care within an African setting. It was argued that various rituals performed during funeral ceremonies are of help in grieving and the mourning process; hence it is suggested that children should also be given the opportunity to participate in performing the rituals. It is noted that the Christian and cultural rituals that are relevant to grief care could be employed. Given the above understanding, a pastoral intervention strategy for grief care to children orphaned through HIV and AIDS is proposed. Different interventions that respond to children’s grief and grief-related issues before, during, and after burial are discussed. / AFRIKAANSE OPSOMMING: Hierdie tesis ondersoek die pastorale vertroosting van kinders wat in die Nairobi City County in Kenya (die voormalige Nairobi Provinsie) as gevolg van MIV en vigs wees gelaat is. Dit bied 'n pastorale ingrypingstrategie in reaksie tot die verdriet van kinders wat as gevolg van MIV en vigs as wese atergelaat is. Kommer hieroor het ontstaan by die besef dat die bedroefdheid van kinders in die meeste Keniaanse gemeenskappe selde vóór, tydens of ná ‘n begrafnis aandag kry. Die navorsingsvraag wat die navorsing gerig het, was: Watter teoretiese, teologiese en kontekstuele pastorale intervensiestrategie kan deur die kerk gebruik word om vertroosting vir treurende kinders wat vanweë MIV en vigs wese geword het, te bied? Vier doelstellings is vir die beantwoording van hierdie vraag opgestel om te verseker dat die navorser se fokus op die kernprobleem ingestel bly. ‘n Empiriese navorsingontwerp is gekies om die impak van die dood van ouers op kinders te bepaal en uit te vind hoe hul bedroefdheid, veral deur die kerk, hanteer is. Dit was duidelik dat kinders wel treur, en dat hul droefheid in die vorm van emosionele, gedrags-, kognitiewe, geestelike en fisiese reaksies geopenbaar word. Die intensiteit van kinders se droefheid word ook verhoog deur gebeure wat op die dood van die ouers volg, sowel as die kinders se status as dubbele weeskinders of as vaderlose of moederlose weeskinders. Ten spyte van die feit dat kinders deur hartseer en verdriet-verwante aangeleenthede oorweldig word wanneer hul ouers sterf, bied die familie en die gemeenskap, sowel as die kerk, selde vertroostende sorg aan sulke kinders. Die navorser het ander dissiplines deur middel van ‘n literatuurstudie betrek om begrip van kinderdroefheid te verbreed. Die studie het onthul dat kinders se ontwikkeling in die algemeen deur hul bedroefdheid geaffekteer word en dat hul fase van ontwikkeling weer hul droefheid beïnvloed. Daar moet egter in ag geneem word dat Afrika-kulture ’n groot impak op die ontwikkeling van kinders van Afrika en hul verdriet het. Dat mense van Afrika droefheid en rou op ‘n eie manier bedryf wanneer ‘n familielid te sterwe kom, moet ook in ag geneem word. Daar is ‘n verskeidenheid rituele wat daarop gemik is om die roubeklaers in staat te stel om die rouproses te deurwerk. Kinders neem selde hieraan deel. Dit was duidelik dat dit van uiterste belang is om die natuur van die mens in verhouding tot God te verstaan om vertroostende pastorale sorg aan kinders wat vanweë MIV en vigs wees geword het, te bied. Die verstaan hiervan kan bepaal hoe die kerk op die situasie van kinders wat treur, reageer. Die voorstel is dat die kerk noue samewerking met die familie, die gemeenskap en instellings moet handhaaf om betekenisvolle vertroostende pastorale sorg binne die Afrika-opset te bied. Daar word aangevoer dat die verskillende rituele wat gedurende begrafnis seremonies uitgevoer word, van hulp is in rou en die rouproses, dus word voorgetel dat kinders ‘n geleentheid moet kry om aan die rituele deel te neem. Christelike en kulturele rituele wat van toepassing is kan gebruik word. 'n Pastorale intervensiestrategie vir vertroostende sorg aan kinders wat as gevolg van MIV en vigs wees gelaat is, word aan die hand van die bogenoemde insig voorgestel. Verskillende ingrypings wat op die kind se hartseer en rou-verwante kwessies vóór, gedurende en ná die begrafnis betrekking het, word bespreek.

An exploration of the experience of a memory box programme by children affected by HIV and AIDS.

The purpose of this research was to highlight the benefits of a Memory Box Programme on the social and emotional developmental functioning of orphaned children. The study focused on the memory box approach used by Sinomlando Centre with orphaned children. The Sinomlando Memory Box Programme has been in existence since 2000. The overall objective of this Memory Box Programme is to enhance resilience in vulnerable children and orphans affected by HIV and AIDS through the methodology of memory boxes In contributing to the discourse on memory work, an interpretive method of data collection and analysis in the form of a focus group discussion and thematic analysis was utilized in this study. Three focus groups were conducted with 26 orphaned children between the ages of 12 years to 15 years. Sampling of the children was based on a purposive sample, targeting children orphaned by AIDS who attended a 5-day camp conducted through a local NGO. The results of the study indicate that children who attended the Sinomlando Memory Box Programme had benefited from their involvement in the programme in diverse ways. They felt that being involved in a Memory Box Programme equipped them with better coping mechanisms and skills. This thesis investigates the literature on trauma and grief in order to explore the existence of the phenomenon of traumatic grief in children. Appropriate therapies are explored for treating traumatic grief in children. The focus in this research is on non-directive art therapy as the literature has found it to be useful in traumatic circumstances. The therapeutic relationship and the focus on containment are valuable in addressing the primary need of the child in therapy especially when traumatic grief is to be addressed. A qualitative case study approach was chosen. Two case studies were selected to investigate the phenomenon of traumatic grief in children. Purposeful sampling was used to select the cases to observe the phenomenon of traumatic grief. The case studies included pre-therapy and post-therapy assessments, which included cognitive, emotional and behavioural assessments. Aan die hand van ‘n literatuurstudie is die aard, gevolge en terapie-benaderings met betrekking tot die verlies- en treurproses bestudeer. Die Opvoedkundig-sielkundige Relasieteorie is verken, en is as raamwerk in ’n empiriese studie gebruik. Die hantering van die verlies- en treurproses is kwalitatief ondersoek. Deur terapeutiese intervensie in drie deelnemers se verlies- en treurproses, is insig verkry met die oog op die samestelling van ’n terapieprogram, sowel as vir riglyne vir die terapeut en die kliënt. Ek het wegbeweeg van navorsing wat primêr beskrywend van aard is en op diagnostiese oorwegings fokus, na raamwerke waar intervensie-strategieë en spesifieke tegnieke gebruik kan word wanneer die terapeut en die kliënt betrokke is in deurlopende psigoterapie. Die navorsingsresultate toon dat dit belangrik is om nie slegs op die verlies te fokus nie, maar ook op onopgeloste kwessies wat vantevore in die kliënt se lewe plaasgevind het. Insig en kennis van hulleself, hul verhoudings, verdedigingsmeganismes, emosies, denke en gedrag, en die invloed wat dit op hulle verlies- en treurproses het, was nodig sodat die kliënte hul ongewenste aangeleerde gedrag kon wysig, om sodoende hulleself beter te verstaan in die hantering van hulle verlies en die treurproses. / The purpose of this study was to investigate the nature and negative consequenses of, and to set guidelines for the therapeutic handling of the loss and grief process. A literature study was undertaken to determine the nature, consequences, and therapeutic techniques, with regard to the loss and grief process, based on Psychological Relations theory. By means of therapeutic intervention in the three participants’ loss and grief processes, I gained insight to compose a therapeutic programme, as well as therapeutic guidelines and intervention strategies and specific techniques that can be used when the therapist and the client are involved in continuous psychotherapy. The results of the study indicated that it is important to focus on unresolved issues that happened earlier in the client’s life.
