On the other hand, the use of computational techniques for prediction has obtained promissing results in several areas of knowledge, and can be seen as a fundamental tool in obtaining blood donations, however, are little explored in this sector. Given this gap, this research aimed to analyze and develop prediction techniques to optimize the search for donors with higher conversion rate to the donation, focusing on data mining techniques. For this, we first analyzed the performance of traditional literature classifiers applied to a real database, which produced unsatisfactory prediction results. Seeking for higher quality results we propose a top-k recommendation approach of blood donors, which uses heuristics to estimate a confidence degree in donation. Computational experiments show that the top-k recommendation approach achieves good results for all three developed heuristics. The support vector-based heuristic achieving 94.09% of precision among the top-10 recommended, and 99.90% of precision for top-1, for the same data set that the classifiers were not successful. It is expected that the results of this research will contribute to the academic community due to the variety of classifiers analyzed and especially due to the proposed top-k recommendations approach. In the future, this approach can be better analyzed with other databases and even improved by the development of new heuristics. In addition, it is believed that the developed top-k approach can be used in health prediction systems, with a focus on predicting blood donors, especially in emergency situations. / Unidades Hemoterápicas encontram dificuldades para otimizar a busca por doadores de sangue em situações de emergência, assim como para manter seus estoques sanguíneos em níveis adequados. Por outro lado, a utilização de técnicas computacionais de predição tem obtido ótimos resultados em várias áreas do conhecimento, podendo ser vista como uma ferramenta fundamental na obtenção de doações de sangue, contudo, são pouco exploradas neste setor. Dado esta lacuna, este trabalho objetivou em analisar e desenvolver técnicas de predição para otimizar a busca por doadores com maior taxa de conversão à doação, com foco em técnicas de mineração de dados. Para isto, primeiramente analisou-se o desempenho de classificadores tradicionais da literatura aplicados a uma base de dados real, o que produziu resultados de predição insatisfatórios. Na busca de resultados de maior qualidade foi proposta uma abordagem de recomendação dos top-k, que utiliza heurísticas para a estimar a confiança em doação. Experimentos computacionais demonstram que a abordagem de recomendação top-k alcança bons resultados para todas as três heurísticas desenvolvidas. A heurística baseada em vetores de suporte obteve taxas de precisão de 94,09% entre os top-10 recomendados, chegando a 99,90% de precisão para o top-1, para a mesma base em que não se obteve sucesso com o uso de classificadores. É esperado que os resultados deste trabalho contribuam para a comunidade acadêmica devido a variedade de classificadores analisados e principalmente pela abordagem de recomendações top-k proposta. Futuramente esta abordagem poderá ser melhor analisada com outras bases de dados e até mesmo aprimorada pelo desenvolvimento de novas heurísticas. Além disso, acredita-se que a abordagem top-k desenvolvida possa ser utilizada em sistemas predição na área da saúde, com foco na predição de doadores de sangue principalmente em situações de emergência.

Elaboração e validação de material audiovisual para conscientização de doadores de sangue / Development and implementation of audiovisual material for blood donors awareness

As transfusões sanguíneas constituem-se uma ferramenta indispensável na prática médica atual, sendo a captação e retenção de doadores uma preocupação de todo serviço hemoterápico, assim como a segurança dos doadores e receptores. Neste sentido, a educação prévia à doação de sangue exerce papel fundamental, sendo etapa obrigatória no processo de doação. Um doador bem orientado significa menor risco transfusional e maior probabilidade de retorno. Este trabalho descreve a elaboração e aplicação de um vídeo contendo informações necessárias à conscientização prévia à doação de sangue. Após o levantamento das necessidades de conteúdo, através da legislação, literatura e de grupo focal com doadores, produziu-se um vídeo que foi apresentado previamente à doação. Foram incluídos 500 doadores no grupo conscientizado com o vídeo e 500 no grupo controle, conscientizados com palestras, conforme rotina do serviço, sendo 25% de primeira vez e 75% de retorno. Após a doação, os doadores responderam a um questionário para avaliação dos conhecimentos adquiridos e os grupos foram comparados entre si. Responderam ao questionário 118 e 121 doadores de primeira vez e 375 e 345 de retorno, nos grupo do vídeo e controle, respectivamente. A distribuição dos doadores entre os grupos foi homogênea, não variando em relação a faixas etárias, gênero, escolaridade, número de comparecimentos e motivação para a doação. Quanto à nota obtida no questionário, foi significativamente superior para o grupo do vídeo em relação ao controle, marcadamente ente os doadores de primeira vez. Dentre as faixas etárias analisadas, foi inferior para a faixa etária maior de 50 anos. Os doadores de primeira vez tiveram notas inferiores em relação aos de retorno. A apresentação do vídeo não interferiu nas taxas de inaptidão clínica à doação, de situação de risco para doenças transmissíveis, de reações vasovagais, nem nos resultados sorológicos, sendo semelhantes ao grupo controle. A satisfação geral com o atendimento, medida através de instrumento padrão do serviço, foi superior no grupo do vídeo em relação ao grupo controle. Conclui-se que a ferramenta audiovisual é adequada à informação e educação dos doadores de sangue antes da efetivação da mesma, sendo de especial interesse para os doadores de primeira vez, gerando maior satisfação com o atendimento. / Blood Transfusions are an essential tool in the current medical practice, in which the capture and retention of donors is a concern of every hemotherapy service, as well as the safety of both, the donors and the receptors. Therefore, prior education to blood donation has a fundamental role for being a compulsory stage in the donation process. A well-targeted donor means less transfusion risk and more probability of return. This work describes the development and implementation of a video containing information necessary for prior awareness regarding to blood donation. After collecting data about the need of content, through the legislation, literature and focus group with donors, it was produced a video which was presented before the donation. There were 500 donors included in the group aware of the video and 500 in the control group, conscious with speeches, according to the service routine, being 25% of first time and 75% of regular donors. After the donation, the donors answered a questionnaire to evaluate the knowledge gained and the groups compared with each other. Donors from video and control group answered the questionnaire: 118 and 121 of the first time donors, and 375 and 345 regular donors, respectively. The distribution of donors among the groups was homogeneous, not varying according to age, gender, schooling, number of attendances and motivation for donation. Regarding the score of the questionnaire, it was significantly higher in the video group compared to the control group, especially for first time donors. It was lower for people over 50 years old related to the other ages, and for first time donors, related to the regular ones. The video presentation did not interfer in rates such as clinical deferral to donation, the situation of risk for communicable diseases, vasovagal reactions, not even in serological results, being similar to the control group. The general satisfaction with the attendance, which was measured through a service standard instrument, was higher in the video group compared to the control group. In conclusion, the audiovisual tool is suitable to the information and education of the blood donors before the completion of it, being of a special interest to all first time donors, generating greater satisfaction with the attendance.

Custo-efetividade das ferramentas de captação / Cost-effectiveness of the tools for donors recruitment

A sociedade tem se tornado cada vez mais instruída e exigente, na qual os clientes possuem grande influência nas estruturas da organização, fazendo com que as empresas busquem aumentar a satisfação dos clientes, atendendo-os de modo a superar as suas expectativas. Nesse contexto, estão inseridos os doadores de sangue. No Brasil, a lei que regulamenta o funcionamento dos serviços hemoterápicos preconiza que a doação de sangue deve ser um ato voluntário e altruísta, no qual o doador não deve receber nenhum tipo de vantagem para realizar a doação de sangue. Segundo dados da Coordenação Geral de Sangue e Hemoderivados, no ano de 2014, a região Sudeste, na qual está concentrada a maior parte da população brasileira, registrou uma das menores taxas de doação: a cada 1.000 habitantes, apenas 17,22 % doaram sangue. É fundamental para os hemocentros identificarem a maneira mais convincente e estimulante de convocar/captar doadores de sangue, de modo a manter os estoques suficientes, além de atender os doadores conforme suas expectativas. Portanto, o objetivo desse estudo foi avaliar qual a ferramenta de abordagem individual e coletiva que apresenta o melhor custoefetividade somado à preferência dos doadores, a qual será essencial para o aumento do número de doações. Para isso, os dados utilizados abrangem um período de 4 anos (2012 a 2015) e representam o número de candidatos à doação de sangue de duas instalações do Hemocentro RP, ambos localizados na cidade de Ribeirão Preto (Sede e Posto de Coleta). Valores-p para o teste de Dickey e Fuller de estacionariedade sugerem que as séries analisadas não são estacionárias, sendo então diferenciadas, passando a apresentar p<0,01. Foi observado que existe correlação (p = 0,31) entre o número de malas diretas e o retorno dos doadores seis meses após o envio das mesmas. No Posto de Coleta, não foi constatada essa correlação. Além disso, as ferramentas para captação que trabalham com os doadores de reposição apresentaram melhores resultados, tais como as ferramentas com atendimento mais personalizado (contato telefônico) juntamente com as campanhas realizadas pelo Facebook Inc. / Society has become increasingly educated and demanding, in which the customers have a strong influence on the organization\'s structures, causing companies to seek to increase the customer satisfaction, meeting them in a way that exceeds their expectations. In this context, blood donors are inserted. In Brazil, the law that regulates the operation of hemotherapy services recommends that blood donation should be a voluntary and altruistic act in which the donor should not receive any type of advantage in order to donate blood. According to data from the \"Coordenação Geral de Sangue e Hemoderivados\", in 2014, the Southeast region, where most of the Brazilian population is concentrated, had one of the lowest rates of donation: for every 1,000 inhabitants, only 17.22% donated blood. It is essential for blood centers to identify the most convincing and stimulating way to recruit and invite blood donors, to maintain sufficient stocks by serving donors according to their expectations. Therefore, the aim of this study was to evaluate which tool of individual and collective approach presents the best cost-effectiveness besides to donor preference, which will be essential for increasing the number of donations. To do so, the data cover a period of 4 years (2012 to 2015) and represent the number of candidates for blood donation from two Hemocentro RP facilities, both located in the city of Ribeirão Preto (\"Sede\" and \"Posto de Coleta\"). p-values for the test of Dickey and Fuller of stationarity suggest that the series analyzed are not stationary, being then differentiated, with p <0.01. It was observed that there is a correlation (p = 0.31) between the number of direct mailings and the donors return six months after sending them. No correlation was verified at the \"Posto de Coleta\". Moreover, recruitment tools for replacement donors present better results, such as the tools with more personalized service (telephone contact) along with the campaigns carried out by Facebook Inc.

Qualitätskontrolle von Blutkomponenten nach GMP ("Good Manufacturing Practice")

Um die Qualität von Blutprodukten nach GMP zu überprüfen, wurden 72 Blutspenden bzw. ihre Folgeprodukte Erythrozytenkonzentrat in PAGGS-M, gefiltertes Erythrozytenkonzentrat und Fresh Frozen Plasma untersucht. Zusätzlich wurden an jeweils 12 Blutderivaten, die z.T. nicht zum Routineprogramm des Blutspendedienstes der Abteilung für Klinische Hämostaseologie und Transfusionsmedizin in Homburg/Saar gehören, Messungen durchgeführt, um ihre Qualität am Herstellungstag und am Ende der Haltbarkeit zu bestimmen. Als letztes wurden schließlich auch rheologische Parameter an 21 Erythrozytenkonzentraten in PAGGS-M und Fresh Frozen Plasma über einen Zeitraum von 28 Tagen gemessen und dokumentiert. Die Messung der Parameter, sowohl hämatologische als auch hämostaseologische und hämorheologische, erfolgte nach Standardmethoden. Die Ergebnisse waren insgesamt nicht zufriedenstellend. Ausschlaggebende Grundlage der Untersuchung sind die GMP-Richtlinien, die sich an den europäischen Normen orientieren. Zwar zeigten die Produkte, die routinemäßig hergestellt werden, wie auch die, die nur für diese Untersuchung produziert wurden, in vielen Bereichen ein Übereinstimmen mit den Vorschriften. Speziell auch die Qualität der gefilterten Erythrozytenkonzentrate, die besonders für Immunsupprimierte wegen ihres niedrigen Gehalts an Leukozyten verwendet werden, war in vielen Fällen gut. Die Ergebnisse der rheologischen Messungen glichen denen anderer Studien: so wurde ein Abfall der Fließfähigkeit über den Meßzeitraum hinweg beobachtet, was auf ein leichtes Schwellen der Zellen und eine Zunahme der Rigidität zurückzuführen ist. Fast durchgängig wich der Meßwert für den Hämatokrit der EK von der Richtlinie ab, am stärksten in PAGGS-M. Als Grund hierfür wurde eine Meßungenauigkeit des elektronischen Zellzählers bzw. die Methode der Bestimmung angesehen, so daß eine echte Abweichung als unwahrscheinlich anzusehen ist. Jedoch wurde die vorgeschriebene maximale Hämolyserate mit Ablauf der Haltbarkeit häufig überschritten, ebenso wie die gemessenen Leukozytenzahlen. Eine Vielzahl der Plasmen war durch Erythrozyten und Leukozyten kontaminiert. Durch häufige Waschvorgänge beim Auftauen von tiefgefrorenen EK wurde ein erhöhter Hb-Überstand erzeugt, das geforderte Mindestvolumen wurde dabei unterschritten. Daher ist festzuhalten, daß für jede untersuchte Produktgruppe Abweichungen festgestellt wurden, die in diesem Maße nicht tolerabel sind. Damit ist die Qualität der Produkte insgesamt als unzureichend zu bezeichnen. Der hier geführte Nachweis dieser Abweichungen ermöglichte die Einführung geänderter Herstellungsverfahren, so daß die heute erzeugten Produkte den Qualitätsvorschriften entsprechen. / 72 blood donations and their follow-up products (red blood cell concentrates resuspended in PAGGS-M, leukocyte-reduced red cell concentrates and fresh frozen plasma) were investigated to check their quality. Additionally, tests were performed on blood components of which most were not part of the routine program in the Department of Hemostaseology and Transfusion Medicine in Homburg/Saar. Finally, rheological parameters of red blood cell concentrates in PAGGS-M and of fresh frozen plasma were measured and documented over a period of 28 days. The results were not satisfying. Both the routine products and the blood components which were only produced for this study met the GMP-guidelines in many cases. Especially the quality of leukocyte-reduced red blood cell concentrates which are used especially for the transfusion for immunocompromised patients was good. The results of the rheological measurements were similar to those of other studies: decreasing flow over a period of time which was put down to slight swelling of the cells and to a rise in erythrocyte rigidity. The average hematokrit of red cell concentrates was generally too low especially in PAGGS-M. The reason for this was thought to be the electronic cell counter or rather the method of measuring. Hemolysis after storage and mean leucocyte count often exceeded the official dates. Many samples of the FFP were contaminated with red and white blood cells. Repeated washing after thawing of frozen red blood cell concentrates caused an excess of extracellular haemoglobin and low concentrate volumes. Any deviation from the guidelines is not tolerable. After this study adjustments in the production could be made so blood products now meet all of the requirements.

Knowledge, attitudes and practices amongst black employees of the University of Limpopo, Turfloop Campus, Limpopo Province, South Africa

Thesis (MPH.) -- University of Limpopo, 2012 / The objectives of the study: To determine the knowledge, attitudes and practices of black employees of The University of Limpopo (Turfloop campus) about blood donation. To determine the degree of willingness by blacks to donate as well as suggesting some solutions to improve the performance by the South African National Blood Services. Design: A descriptive study utilizing a self-reported questionnaire was carried out. Setting: The University of Limpopo Turfloop Campus in the Limpopo Province of South Africa. Material and Methods: A total of 138 employees participated in the study. With the aid of Predictive Analytics Software (PASW), 136 male and 101 female participants were randomly selected from the 40 University departments. The mean ages by gender was 36.91, ±10, 06 years for males, and 41.93, ±9, 35 years for females. The mean ages by donor status was 40.24, ±10, 15 years for donors and 37.94, ±9, 9 years for non-donors. Outcome measures: Subjects demographic variables were determined by the use of a pre-tested self-reported questionnaire, which covered personal characteristics such as gender, age, residence, number of household members, marital status, educational background, faculty or department of vi employment, as well as the monthly income bracket. Knowledge, attitudes and practices regarding blood donation were also assessed using the pre-tested self-developed questionnaire. The following information was included: previous practices, current and lifetime practices of blood donation. Results: There was a general lack of practice of blood donation despite the presence of average knowledge relating to matters of blood donation amongst the black employees of the University of Limpopo, (Turfloop campus). There was no difference between black females and males as far as blood donation was concerned. There was a reflection of similar practices of blood donation by both genders. Blood donation generally started at a very late age due to early lack of exposure, information and awareness. Conclusion: Findings from this study showed that the prevalence of non-donors amongst the blacks is high. The attitudes and practices of non-donors towards donation were generally less favourable, but it can be argued that a high percentage of these findings may likely change with the right interventions.

Carbon monoxide in biological systems : An experimental and clinical study

Background: Carbon monoxide (CO) is a toxic gas, but it is also produced endogenously when haem is degraded. When produced in vivo, CO is believed to have positive biological effects. For example it activates the production of cyclic guanosine mono-phosphate and causes vasodilatation. CO is also believed to have anti-inflammatory properties by binding to Mitogen activated protein (MAP) kinase. Several studies in cells, mice and rats support this opinion regarding both the circulatory as well as the anti-inflammatory properties. However, studies in larger animals regarding circulatory effects have demonstrated contradictory results. The only study in humans regarding anti-inflammatory properties of CO could not demonstrate such effects. Methods: This thesis consists of four different models. In paper I a method for analysis of CO in blood was developed using gas chromatography. In paper II a porcine model was used to investigate the elimination time for CO. The pigs in paper II had a high concentration of CO administered via blood, and CO concentrations were followed over time and kinetically parameters calculated. Circulatory parameters were also measured to evaluate if there were any circulatory changes after CO administration. In paper III CO´s anti-inflammatory properties were investigated in an endotoxin-induced systemic inflammatory model in pigs. Paper III was a randomized study where one group inhaled CO and the other group served as controls. Plasma cytokine concentrations were measured and followed over time as an indication of the inflammatory state. In paper IV, CO concentrations in blood from blood donors at the Blood Centre in Umeå were investigated. The blood donors also completed a questionnaire about age, smoking history and other possible sources for exogenous contamination of CO in the blood. Results and conclusions: In paper I we developed a method suitable for analysis of low concentrations of CO in blood. The half-life of CO at levels of 250 µM in pigs was found to be 60 minutes. CO did not show anti-inflammatory effects after an endotoxin-induced systemic inflammation in pigs. In banked blood CO was present at concentrations up to six times higher than normal concentrations. This could be a risk when transfusing such blood to susceptible patients.

Thus, the aim of this dissertation consists to understand the factors that influence the individuals to donate blood. Upon the presentation of the theoretical reference, a structural model was elaborated based on the relations between the models of blood donation, which is based on fifteen main hypotheses. The strategy used in this research was the data collection with the data analysis under a quantitative approach, using the application of structural equation modeling technique. The sample was of 346 people 100 students, 94 people in João Pessoa s city downtown, and 150 people from the internet. In general, two hypotheses were accepted, that deals with the influence of fear and reference group in predisposition of the individuals to donate blood. This suggests the necessity that the actions of social marketing to blood donation have to be focused on understanding the process of interaction between these constructs, turning the advertisement campaigns more elaborated to catch and retain blood donators. / As discussões entre a temática da doação de sangue e o marketing social mostram-se relevantes, à medida que contribuem para a o planejamento das ações de órgãos públicos e instituições de saúde, tendo em vista a ineficiência dos processos de recrutamento e retenção de doadores retratarem a ausência de compreensão dos aspectos comportamentais que influenciam os indivíduos a adotar tal conduta. Desse modo, o objetivo geral deste trabalho consiste em compreender os fatores que influenciam os indivíduos a doarem sangue. Mediante a apresentação do referencial teórico, um modelo estrutural foi elaborado com base nas relações entre os modelos de doação de sangue, o qual se constitui de quinze hipóteses centrais. A estratégia de pesquisa utilizada foi o levantamento, com a análise dos resultados adotando uma abordagem quantitativa, com a aplicação da técnica de modelagem de equações estruturais. A amostra da pesquisa foi de 346 respondentes, sendo 100 estudantes, 94 respondentes das ruas centrais de João Pessoa, e, 150 participantes via internet. De maneira geral, foram aceitas duas hipóteses, que tratam da influência do medo e do grupo de referência na predisposição dos sujeitos em doar sangue. Isto sugere a necessidade de que as ações de marketing social para a doação de sangue devem estar focadas em compreender o processo de interação entre estes construtos, tornando mais efetiva as campanhas elaboradas para captação e retenção dos doadores de sangue.
