Estudo dos efeitos do risedronato e da calcarea phosphorica 6CH na reparação óssea em ratos machos castrados /

Fraturas decorrentes da osteoporose são muito comuns e podem apresentar consolidação mais lenta. O objetivo deste trabalho foi avaliar o efeito do risedronato (medicamento alopático) e da Calcarea phosphorica 6CH (medicamento homeopático) no processo de reparo ósseo em ratos machos com osteoporose induzida por castração. Para tanto, foram utilizados oitenta e quatro ratos de três meses de idade separados em quatro grupos de vinte e um animais, sendo três grupos submetidos à castração e um grupo a falsa cirurgia (sham). Um mês após, foram realizadas lesões ósseas monocorticais na tíbia de todos os animais e a partir do dia seguinte, os respectivos tratamentos foram iniciados de acordo com os seguintes grupos: CR - castrado/ risedronato (1mg/kg/dia); CCp - castrado/Calcarea phosphorica 6CH (três gotas/dia); CP - castrato/placebo e SP - sham/placebo que receberam apenas três gotas ao dia de água destilada. Os animais foram sacrificados aos sete, catorze e vinte e oito dias após o início do tratamento e as tíbias removidas. Radiografias digitais foram realizadas e avaliadas pelo programa Image Tool para obter a densidade óptica na área do defeito. Então as tíbias foram descalcificadas e processadas para a realização das análises histológicas. A histomorfometria mediu a porcentagem de osso formado utilizando um retículo graduado colocado no centro da lesão através do programa Image J. Para a análise estatística, os dados foram submetidos aos testes de ANOVA, Tukey e Dunnett, ao nível de 5%. Segundo a análise da densidade óptica, o grupo CCp apresentou os melhores resultados aos sete e catorze dias, mas foi superado pelo grupo CR aos vinte e oito dias. It results in a much higher risk for fractures and might cause slower bone consolidation. The aim of this work was to study the effects of Risedronate (allopathic medicine) and Calcarea phosphorica 6CH (homeopathic medicine) to repair bone lesions in male rats with osteoporosis, induced by castration. For this, eighty-four three-months-old rats were used divided in four groups of twenty-one animals. Three groups where castrated and one group was submitted to Sham surgery. One month later, monocortical lesions were made in all animals' tibiae and after one day they began the different treatments according to the following groups: CR - castrated/Risedronate (1mg/kg/day); CCp - castrated/Calcarea phosphorica 6CH (3 drops/day); CP - castrated/placebo and SP - Sham/placebo that received 3 drops/day of distilled water. The animals were sacrificed at seven, fourteen and twenty-eight days after the beginning of treatment and had their tibiae removed. The tibiae's digital XR was analyzed in order to evaluate optical density, using the Image Tool program. Then, they were decalcified and processed for histologic analysis. Histomorphometric measures of bone percentual formation were evaluated using a graticule in the central area of the lesion, with the Image J program. Data was submitted to ANOVA, Tukey and Dunnett tests (5% level). According to the optical density, the CCp group showed the best results at seven and fourteen days, but it was surpassed by the CR group at the 28th day. Os princípios biológicos da regeneração óssea guiada (ROG) têm contribuído para o desenvolvimento de membranas que, em odontologia, são utilizadas em diversas situações como tratamentos com implantes dentários, aumento de rebordo alveolar e reparo de defeitos ósseos de origem traumática e patológica. Resultados de experimentos in vitro comparando a membrana obtida pela associação do polímero poli(fluoreto de vinilideno-trifluoretileno) e da cerâmica titanato de bário (P(VDFTrFE)/ BT) à membrana de politetrafluoretileno (PTFE) mostraram uma resposta favorável de osteoblastos, fibroblastos e queratinócitos à membrana de P(VDFTrFE)/ BT. O objetivo do presente estudo foi avaliar o efeito da membrana de P(VDFTrFE)/ BT sobre a formação óssea in vivo. Foram criados defeitos ósseos com 5 mm de diâmetro em calvárias de ratos machos Wistar (peso 200-250 g), distribuídos em três grupos com relação à utilização ou não de membranas nos defeitos ósseos: (1) membrana de P(VDF-TrFE)/BT; (2) membrana de PTFE; (3) nenhum tipo de membrana. Ao final de 4 e 8 semanas os animais foram eutanasiados e as amostras foram submetidas à: (1) análise por microtomografia computadorizada (micro-CT) para avaliar volume ósseo, superfície óssea, superfície óssea específica, número de trabéculas, separação trabecular e espessura trabecular, (2) análise histológica com base em cortes histológicos não descalcificados, (3) análise por reação em cadeia da polimerase em tempo real (PCR em tempo real) para avaliar a expressão gênica dos marcadores ósseos runt-related transcription factor 2 (RUNX2), fosfatase alcalina (ALP), sialoproteína óssea (BSP), osteocalcina (OC), osteoprotegerina (OPG) e receptor activator of nuclear factor-kappa B ligand (RANKL) e (4) análise da expressão de microRNAs (miRs) pela técnica de sequenciamento na plataforma Illumina. Os dados das análises morfométricas e da expressão gênica foram submetidos ao teste de Kruskal-Wallis, seguido pelo teste de Fischer quando apropriado, e para a análise da expressão de miRs foi utilizado o teste de Mann-Whitney para comparar a membrana de P(VDF-TrFE)/BT à de PTFE. Para todas as comparações o nível de significância adotado foi de 0,05. Os defeitos que não receberam a membrana tiveram uma formação óssea insignificante. Ambas as membranas favoreceram a formação óssea, sendo a superfície óssea maior sobre a membrana de P(VDF-TrFE)/BT comparada à de PTFE em 4 (p=0,01) e 8 (p=0,001) semanas e a separação trabecular maior sobre a membrana de PTFE comparada à de P(VDF-TrFE)/BT em 4 (p=0,05) e 8 (p=0,001) semanas. A expressão gênica de BSP (p=0,01), OC (p=0,001) e OPG (p=0,001) foi maior e de RANKL (p=0,001) foi menor sobre a membrana de P(VDFTrFE)/ BT comparada à de PTFE em 4 semanas. A expressão gênica de RUNX2 (p=0,001) e OC (p=0,05) foi maior e de ALP (p=0,05), RANKL (p=0,01) e OPG (p=0,001) foi menor sobre a membrana de P(VDF-TrFE)/BT comparada à de PTFE em 8 semanas. Quarenta e cinco e 13 miRs foram regulados positivamente (>2 vezes) em 4 semanas e 8 semanas, respectivamente, e 11 e 39 miRs foram regulados negativamente (>2 vezes) em 4 semanas e 8 semanas, respectivamente, pela membrana de P(VDF-TrFE)/BT comparada à de PTFE. Os resultados indicam que a membrana de P(VDF-TrFE)/BT favorece a formação óssea quando comparada à membrana de PTFE e, portanto, pode ser considerada um biomaterial promissor para ser utilizado em procedimentos de ROG. / Biological principles of guided bone regeneration (GBR) have contributed to development of membranes that, in dentistry, are used in several situations such as treatment with dental implants, alveolar bone augmentation and repair of traumatic and pathological bone defects. Results of in vitro experiments comparing membrane obtained by the combination of poly(vinylidene fluoride-trifluoroethylene) and barium titanate ceramics (P(VDF-TrFE)/BT) with polytetrafluoroethylene (PTFE) membrane showed a favorable response of osteoblasts, fibroblasts and keratinocytes to the P(VDF-TrFE)/BT membrane. The aim of this study was to evaluate the effect of P(VDFTrFE)/ BT membrane on in vivo bone formation. Bone defects with 5 mm in diameter were created in calvaria of male Wistar rats (weight 200-250 g), distributed into three groups regarding the use or not of membranes on defects: (1) P(VDF-TrFE)/BT membrane; (2) PTFE membrane; (3) no membrane. At the end of 4 and 8 weeks, the animals were euthanized and samples were subjected to: (1) computed microtomography analysis (micro-CT) to assess bone volume, bone surface, specific bone surface, trabecular number, trabecular thickness and trabecular separation; (2) histological analysis based on non-decalcified histological sections; (3) real time polymerase chain reaction (real time PCR) to evaluate gene expression of the bone markers runt-related transcription factor 2 (RUNX2), alkaline phosphatase (ALP), bone sialoprotein (BSP), osteocalcin (OC), osteoprotegerin (OPG) and receptor activator of nuclear factor-kappa B ligand (RANKL); (4) microRNAs (miRs) sequencing analysis at Illumina platform. Data from morphometric and gene expression analyses were submitted to the Kruskal-Wallis test followed by Fischer\'s test, when appropriate, and from miRs expression to the Mann-Whitney test to compare P(VDF-TrFE)/BT with PTFE membrane. For all comparisons, the significance level was 0.05. Defects without membrane exhibit a non significant bone formation. Both membranes favored osteogenesis with an increased bone formation on the P(VDF-TrFE)/BT membrane compared with PTFE at 4 (p=0.05) and 8 weeks (p=0.001). Trabecular separation was greater on PTFE membrane compared with the P(VDF-TrFE)/BT at 4 (p=0.05) and 8 weeks (p=0.001). At 4 weeks, the gene expression of BSP (p=0.01), OC (p=0.001) and OPG (p=0.001) were higher on P(VDF-TrFE)/BT membrane compared with PTFE, while RANKL (p=0.001) was lower. The gene expression of RUNX2 (p=0.001) and OC (p=0.05) were higher and ALP (p=0.05), RANKL (p=0.01) and OPG (p=0.001) were lower on the membrane of P(VDF- TrFE)/BT compared with PTFE at 8 weeks. Fortyfive and 13 miRs were up-regulated (> 2 fold) at 4 weeks and 8 weeks, respectively, and 11 and 39 miRs were negatively regulated (> 2 fold) at 4 weeks and 8 weeks, respectively, on the P(VDF-TrFE)/BT membrane compared with PTFE membrane. The results indicate that the P(VDF-TrFE)/BT membrane favors bone formation compared with PTFE membrane and, therefore, may be considered a promising biomaterial for using in GBR procedures.

The use of phosphorous containing polymers to mimic the action of bisphosphonate drugs in bone repair

Bone has the capacity to regenerate itself, however for challenging defects such as non-uniform factures, repair can be problematic. A similar challenge is presented in the repair of osteoporotic bone. Osteoporotic bone becomes increasingly porous and brittle and the risk of fracture is greatly increased. A drug mimic, poly(vinyl phosphonic acid – co – acrylic acid)(PVPA), has been incorporated into FDA approved poly(ε-caprolactone)(PCL), and aims to mimic the action of bisphosphonates to reduce the activity of osteoclasts. The PVPA polymer contains pendant phosphonic acid groups which are hypothesised to mimic the P-C-P backbone found in bisphosphonates. The PCL/PVPA scaffold has been found to have sufficient mechanical strength in order to be used as a bone void filler as well as providing a hydrophilic surface for superior cell attachment. The substrate has been found to significantly enhance the deposition of collagen, alkaline phosphatase activity and the expression of osteocalcin. Alizarin red staining revealed an increase in the rate of mineralisation in the presence of the drug mimic. The PCL/PVPA substrates have been suggested to induce osteoblast cells from a proliferative phase to a mineralisation stage. This is believed to be due to the presence of phosphorous within the scaffold which could lead to the critical concentration required for the initiation of mineralisation being reached more rapidly and effectively. The PVPA polymer has been found to mimic the action of bisphosphonates by inducing osteoclast apoptosis in vitro, and its actions of osteoclast apoptosis are comparable to that of Alendronate, a commonly administered bisphosphonate. A critical size defect model has demonstrated that the PVPA polymer has the ability to heal critical size defects; the healing potential was two fold greater than the control PCL substrate. Initial in vivo studies using a subcutaneous model demonstrated an improvement in mineralisation in the presence of PVPA. Untreated PCL/PVPA substrates displayed a high level of branched blood vessel formation, essential for healthy bone formation. However PVPA samples pre-treated with VEGF, hindered blood vessel formation and the infiltration of cells. This suggests that the PVPA alone is capable of inducing neovascularisation without the addition of VEGF. The findings suggest that the PCL/PVPA system could be used to treat challenging bone defects such as non-unions and osteoporotic fractures.

Cellulose Nanocrystal Aerogels: Processing Techniques and Bone Scaffolding Applications

This thesis investigates new processing methods and bone tissue engineering applications of cross-linked cellulose nanocrystal (CNC) aerogels. Aerogels are highly porous, low-density materials that have been praised for their high surface area and interconnected pores, but criticized for their brittleness. This prompted a search for new aerogel “building blocks” to produce more flexible materials; CNCs meet this need and chemically cross-linked CNC aerogels have good compressive strength and shape recovery properties in air and liquid environments. CNCs are high aspect ratio, non-toxic and renewably-sourced nanoparticles. Literature has demonstrated CNC aerogel production using cryo-templating with controlled drying. In this work, we produce aerogels using a new scalable process called pressurized gas expansion (PGX) and compare them to conventional cryo-templated aerogels. PGX aerogels were found to have more expanded fibrillar morphology, a range of mesopore sizes and smaller macropores, in contrast to cryo-templated aerogels that had a sheet-like morphology surrounding larger macropores. Additionally, PGX aerogels had higher specific surface area and porosity, but lower compressive strength due to a lower cross-link density. While neither CNC aerogel type dispersed in water, PGX aerogels partially shrank whereas cryo-templated aerogels did not; this is attributed to their morphological differences. This work shows that new aerogel processing methods can introduce new properties and thus broaden the potential applications of CNC aerogels. One specific biomedical application was evaluated for CNC aerogels – their use as bone tissue scaffolds. Cryo-templated aerogels comprised of CNCs with different surface chemistries, either sulfate or phosphate groups, were found to have attractive chemical, physical and mechanical properties for bone tissue engineering. This work shows that both types of CNC aerogels can facilitate cell proliferation, favorable differentiation, and can nucleate uniform hydroxyapatite growth. These positive in vitro results and the bimodal pore morphology of CNC aerogels make them promising bone scaffolds for in vivo studies. / Thesis / Master of Applied Science (MASc) / Aerogels are light, porous, sponge-like materials that are essentially 99% air by volume. In this work, the aerogels are made from non-toxic plant-based nanoparticles called cellulose nanocrystals (CNCs). This thesis investigates: 1) new ways to control CNC aerogel properties and pore size through different processing methods and 2) the use of CNC aerogels to aid in the repair of damaged bones. High-resolution microscopy and nano-characterization tools show that CNC aerogels have tunable properties, which may extend their possible applications. The internal structure, sponge-like mechanical properties and biocompatibility of CNC aerogels allowed them to be successfully utilized to support bone cells and grow bone-like mineral.

Osteoblast Response to Zirconia-Hybridized Pyrophosphate Stabilized Amorphous Calcium Phosphate

Biodegradable polyesters, such as poly(DL-lactic-co-glycolic acid) (PLGA), have been used to fabricate porous bone scaffolds to support bone tissue development. These scaffolds allow for cell seeding, attachment, growth and extracellular matrix production in vitro and are replaced by new bone tissue when implanted into bone sites in vivo. Hydroxyapatite (HAP) and μ-tricalcium phosphate (μ-TCP) ceramics have been incorporated into PLGA bone scaffolds and have been shown to increase their osteoconductivity (support cell attachment). Although HAP, μ-TCP, and biodegradable polyesters are osteoconductive, there is no evidence that these scaffold materials are osteoinductive (support cell differentiation). Calcium and phosphate ions, in contrast, have been postulated to be osteogenic factors that enhance osteoblast differentiation and mineralization. Recently, a zirconia-hybridized pyrophosphate stabilized amorphous calcium phosphate (Zr-ACP) has been synthesized which permits controlled release of calcium and phosphate ions and thus is hypothesized to be osteoinductive. Incorporation of Zr-ACP into a highly porous poly(DL lactic-co-glycolic acid) (PLGA) scaffold could potentially increase the osteoinductivity of the scaffold and therefore promote osteogenesis when implanted in vivo. To determine the osteoinductivity of Zr-ACP, a MC3T3-E1 mouse calvarial-derived osteoprogenitor cell line was used to measure cell response to Zr-ACP. To accomplish this objective, Zr-ACP was added to cell culture at different stages in cell maturation (days 0, 4 and 11). DNA synthesis, alkaline phosphatase (ALP) activity, osteopontin synthesis and collagen synthesis were determined. Results indicate that culture in the presence of Zr-ACP significantly increased cell proliferation, ALP activity and osteopontin synthesis but not collagen synthesis. To determine the feasibility of incorporating Zr-ACP into a PLGA scaffold, PLGA/Zr-ACP composite foams (5% or 10% (w/v) polymer:solvent with 25 wt% or 50 wt% Zr-ACP) were fabricated using a thermal phase inversion technique. Scanning electron microscopy revealed a highly porous structure with pores ranging in size from a few microns to about 100 μm. The amorphous structure of the Zr-ACP was maintained during composite fabrication as confirmed by X-ray diffraction measurements. Composite scaffolds also showed significantly greater compressive yield strengths and moduli as compared to pure polymer scaffolds. The results of this study indicate that Zr-ACP enhances the osteoblastic phenotype of MC3T3-E1 cells in vitro and can be incorporated into a porous PLGA scaffold. Les affections ostéo-articulaires concernent des millions de personnes. L’ingénierietissulaire osseuse, associant cellules souches mésenchymateuses humaines (CSM) etmatériaux synthétiques, pourrait répondre aux besoins cliniques. Pour cela, les différentescomposantes de cette approche et leur association doivent être mieux étudiées pour la rendreutile cliniquement. Durant cette thèse, une première étude animale proche du cas cliniquenous a permis de définir les points à améliorer pour le traitement des pertes osseuses. Nousavons ainsi pu développer un milieu spécifique induisant une différenciation rapide etterminale des CSM en ostéoblastes. Par la suite, l’utilisation de particules de céramiquescomme support cellulaire nous a permis d’obtenir des hybrides riches en matriceextracellulaire. Cet environnement 3D biomimétique permet l’engagement spontané des CSMvers un phénotype ostéoblastique et l’obtention d’une quantité osseuse importante in vivo.L’ensemble de ces résultats met en évidence l’importance de l’environnement et du stade dedifférenciation cellulaire pour la formation osseuse par ingénierie tissulaire osseuse. / Osteo-articular disorders affect millions of people over the world. Bone tissueengineering, an approach combining human mesenchymal stem cells (MSC) and syntheticmaterials, could potentially fulfill clinical needs. However, the different components of thisapproach and their association should be investigated further to make it clinically useful. Inthis thesis, an initial animal study close to clinical situation allowed us to identify areas thatneed improvement for regenerating bone defect. We were then able to develop a specificmedium which induces a rapid and terminal osteoblastic differentiation of MSC.Subsequently, the use of ceramic particles as cell support has allowed us to obtain hybridmainly composed of extracellular matrix. This biomimetic 3D environment allowsspontaneous osteoblastic commitment of MSC and induces a large bone quantity in vivo.Overall, these results highlight the importance of the environment and the cell differentiationstate for bone formation using bone tissue engineering.

TISSUE ENGINEERING COMPOSITE BIOMIMETIC GELATIN SPONGES FOR BONE REGENERATION

The field of tissue engineering aims to develop viable substitutes with the ability to repair and regenerate the functions of damaged tissue. Common practices to supplement bone regeneration in larger defects include bone graft biomaterials such as autografts, allografts, xenografts, and synthetic biomaterials. Autologous bone grafting is the current gold-standard procedure used to replace missing or damaged bone. However, these grafts have disadvantages such as donor site morbidity, limited availability, and the need for a secondary surgery. The focus of this study is to tissue engineer a lyophilized gelatin composite sponge composed of hydroxyapatite (HA), chitin whiskers (CW), and preparations rich in growth factors (PRGF) to provide sufficient structural support to the defect site while enhancing the body’s own reparative capacity, ultimately eliminating the need for autologous tissue harvesting or repeat operations. The present study investigates several in vitro evaluations on multiple compositions of modified gelatin sponge scaffolds for use in bone graft applications. Gelatin sponges were fabricated via freeze-drying, enhanced with PRGF, HA, and/or CW, and cross-linked with 50 mM 1-Ethyl-3-(3-dimethylaminopropyl)carbodiimide hydrochloride (EDC) either during or post gelation. Initial evaluation of all scaffold combinations indicated that scaffolds released contents up to 90 days, EDC cross-linking during gelation allowed for more protein release, and had the ability to swell. Since the incorporation of PRGF, HA, and CW increased cell infiltration, and production of cell-created mineral matrix over 90 days in culture, these scaffolds were further characterized. Freeze-dried gelatin sponges enhanced with PRGF, HA, and CW and cross-linked during gelation with EDC (PHCE) were mineralized (M) in 5x revised simulated body fluid (r-SBF) for 1 hour to create a bone-like mineral surface. Gelatin EDC scaffold controls (GE), GE-M, PHCE, and PHCE-M scaffolds were characterized for their ability to swell, mineralizing potential, surface morphology, growth factor incorporation and release, uniaxial compression properties, and cell attachment, proliferation, infiltration, and protein/cytokine secretion.. After mineralization, scanning electron microscopy showed sparse clusters of mineral deposition for GE-M scaffolds while PHCE-M scaffolds exhibited a more uniform mineral deposition. Both GE and PHCE scaffolds were porous structures that swelled up to 50% of their original volume upon hydration. Over 21 days incubation, PHCE-M scaffolds cumulatively released about 30% of their original protein content, significantly more than all other scaffolds. Multiplex Luminex assays confirmed the successful incorporation of PRGF growth factors within PRGF sponges. For acellular uniaxial compression testing, PHCE-M scaffolds reported lower Young’s modulus values (1.3 - 1.6 MPa) when compared to GE and GE-M scaffolds (1.6 – 3.2 MPa). These low modulus values were comparable to values of tissue found in early stages of bone healing. DAPI (4',6-diamidino-2-phenylindole) staining and imaging showed an increase in initial cell attachment and infiltration of PHCE and PHCE-M scaffolds on day 1. GE-M scaffolds also appeared to attach more cells than the GE control. MTS cell proliferation assay results indicated that on days 4 and 7, PHCE scaffolds increased cell proliferation (compared to GE controls). MTS also illustrated that the addition of a mineral coating increases and decreases cell proliferation on GE-M and PHCE-M scaffolds, respectively. Multiplexer analysis of MG-63 protein/cytokine secretion suggests that cells are responding in a bone regenerative fashion on all scaffolds, as evidence of osteocalcin secretion. Little to no secretion of osteopontin, IL-1β, and TNF-α demonstrates that scaffolds are not influencing cells to secrete factors associated with bone resorption. The compressive mechanical properties of cellularized scaffolds did not differ much from acellular scaffolds. The collective results indicated increased cellular attachment, infiltration, and bone regenerative protein/cytokine secretion by cells on GE-M scaffolds, which support the addition of a bone-like mineral surface on GE scaffolds. Cellularized PHCE and PHCE-M scaffolds report similar advantages as well as Young’s modulus values in the range of native tissues present in the early stages of bone healing. The results of this study propose that the developed PHCE and PHCE-M scaffolds exhibit good cellular responses and mechanical properties for use in early bone healing applications.

A desnutrição é uma condição clínica decorrente da deficiência de um ou mais nutrientes essenciais para homeostase do organismo. Em crianças continua a ser um dos problemas mais graves de saúde pública no mundo, em virtude de sua grandeza e consequências desastrosas para o crescimento, desenvolvimento e sobrevivência, o acarretando graves comprometimentos na idade adulta. O osso é extremamente sensível a mudanças metabólicas, hormonais e nutricionais. Alterações internas e externas ao organismo podem interferir na aquisição de massa óssea máxima, levar a doenças osteometabólicas e afetar o crescimento de ossos longos. Uma alimentação adequada, com fornecimento de todos os nutrientes necessários ao organismo é crucial para o desenvolvimento do sistema esquelético. No entanto, os estudos de qualidade óssea e crescimento no desnutrido ainda são poucos na literatura. Sendo assim, o nosso objetivo foi avaliar os efeitos da restrição alimentar no tecido ósseo (trabecular e cortical) e na cartilagem de crescimento de ratos em crescimento. Neste estudo, a qualidade do tecido ósseo e da cartilagem de crescimento das tíbias foi analisada em ratos normais e desnutridos. Foram utilizados 72 ratos machos, com massa corpórea inicial de 70 a 80 gramas, que permaneceram alojados em gaiolas individuais e foram aleatoriamente divididos em 2 grupos experimentais com dois subgrupos cada, conforme o tempo de exposição à restrição alimentar: CON1: ratos normais, observados por 56 dias; RA1: ratos submetidos à restrição alimentar, observados por 56 dias; CON2: ratos normais, observados por 70 dias; RA1: ratos submetidos à restrição alimentar, observados por 70 dias. A desnutrição foi induzida por meio da restrição alimentar em 50%, ou seja, metade da quantidade ingerida pelos animais controles. Ao término do período experimental, após 56 e 70 dias, os ossos foram submetidos às análises macroscópica, microtomografia computadorizada (avaliação qualitativa e quantitativa da microestrutura óssea), densitometria óssea (DXA), microscopia óptica (caracterização dos tecidos), histomorfometria (quantificação de colágeno e volume trabecular), de expressão gênica e ensaio mecânico (avaliação da resistência mecânica). O nível de significância estatística foi de 5%. Embora a desnutrição não tenha resultado em alterações na expressão gênica, várias alterações fenotípicas foram observadas na cartilagem de crescimento (diminuição do volume; redução da área total, área de ossificação e espessura; alterações celulares nas zonas hipertrófica e proliferativa; tecido mais fraco na resistência ao cisalhamento), osso trabecular e cortical (redução no tamanho e massa óssea; menor densidade óssea; deterioração na microarquitetura trabecular e cortical e; menos trabéculas com menor deposição de colágeno). Concluiu-se que a restrição alimentar causou efeitos deletérios para a cartilagem de crescimento, osso trabecular e cortical, indicando interferência nos mecanismos de ossificação osteocondral e intramembranosa. / Clinical undernutrition is characterized by the depletion of one or more nutrients that are essential for human homeostasis. Undernutrition in children results in a serious health impact due to the impairment in growth and bone quality in adulthood. Several factors may interfere with the peak mass achieved by an individual and interfere in growth and osteometabolic disorders. An adequate nutritional intake is an essential factor for bone development. However, few studies have investigates the relation between bone quality and growth in malnourished subjects. Therefore, we aimed to study the effects of food restriction on the growth plate and bone tissue (trabecular and cortical) in growing rats. 72 male Wistar rats weighing approximately 70-80g were randomly assigned into two groups and, subsequently subdivided into two subgroups according to the experimental follow-up: CON1: control rats followed by 56 days; RA1: food restricted rats followed by 56 days; CON2: control rats followed by 70 days and, RA2: food restricted rats followed by 70 days. After weaning, food restricted rats were fed 50% of the ad libitum food intake. At the end of each experimental period, the rats were euthanized and tibias were analyzed by morphological measurements, micro-computed tomography (qualitative and quantitative analysis of bone microstructure), dual-energy X-ray absorptiometry (DXA), optical microscopy (tissue characterization), histomorphometry (trabeculae quantity and collagen deposition), gene expression and mechanical test (mechanical strength assessment). Statistical significance was set when p<0.05. Although undernutrition did not alter gene expression, several phenotypic changes were seen at the growth plate (i.e. decreased in growth plate volume, reduction in total growth plate area and thickness, decrease in area of proliferative, hyperthrofic and ossified zones and, decrease in hyperthrofic cells and lesser mechanical resistance), trabecular and cortical bone (i.e. reduction in bone size and mass, bone density, deterioration at the trabecular and cortical microstructure, reduction in trabeculae bone and collagen deposition) of growing rats subjected to food restriction. We concluded that food restriction caused detrimental effects on growth plate, trabecular bone and cortical shaft, evidencing deleterious changes in the mechanisms of osteochondral and intramembranous bone formation.

Estudo do efeito de campo elétrico pulsado no reparo ósseo em tíbia de ratos / Study of the effect of pulsed electric field on bone healing in rats tibia

O conceito de que estimulação elétrica promove o reparo de fraturas tem uma longa história, datada de 1812. O mecanismo pelo qual estimulação elétrica promove a osteogênese ainda é assunto de especulação científica, entretanto esse método não farmacológico, utilizado no presente trabalho, já mostrou efeitos positivos na promoção e estímulo da osteogênese. O presente trabalho objetivou analisar a influência da aplicação de campo elétrico pulsado nas características histológicas e propriedades mecânicas do tecido ósseo em osteotomias experimentais realizadas em tíbias de ratos. Foram utilizados 52 ratos com fraturas cirúrgicas unilaterais do terço médio da tíbia, sem lesão dos músculos e do periósteo. Os animais foram divididos em dois grupos: grupo controle, que não recebeu estimulação e grupo tratado com campo elétrico. Esses foram divididos em dois subgrupos: animais sacrificados no 19º pós-operatório e animais sacrificados no 27º pós-operatório. O membro fraturado foi tratado com campo elétrico pulsado com freqüência fundamental de 1,5 MHz e 200 \'mü\'s de largura de pulso, em sessões diárias de 20 minutos. Realizou-se radiografia desse membro no dia da cirurgia e do sacrifício. Após o tratamento sacrificaram-se os ratos e extraíram-se suas tíbias para a obtenção de material para a análise morfométrica e ensaio mecânico de flexão. Os ensaios biomecânicos revelaram que houve diferença significativa entre as médias dos grupos tratado 18 e 12 dias somente para a tensão, sendo esta maior para o grupo tratado 18 dias, para as demais propriedades mecânicas analisadas não houve diferença significativa. A análise morfométrica determinou que a relação entre os grupos controle 18 dias com tratado 18 dias bem como entre os grupos tratado 12 dias e tratado 18 dias das médias das áreas de osso formado apresentaram diferenças estatisticamente significantes. Esses resultados sugerem que existe um incremento na síntese de matriz orgânica no grupo tratado, não acompanhado linearmente pela síntese mineral. A diferença estatisticamente significativa nas tensões de ruptura, por outro lado, sugere que o aumento no grau de mineralização encontra-se em curso. / The concept that electrical stimulation promotes the repair of fractures has a long history, dated of 1812. The mechanism for which electrical stimulation promotes osteogenesis is still subject of scientific speculation, however that method non-pharmacological, used in the present work has already showed positive effects in the promotion and incentive of osteogenesis. The present work objectified to analyze the influence of the application of electric field pulsed in the histological characteristic and mechanical properties of the bone tissue in experimental osteotomy accomplished in tibias of rats. 52 rats were used with unilateral surgical fractures of the medium third of the tibia, without lesion of the muscles and of the periosteum. The animals were divided in two groups: group control, that didn\'t receive stimulation and treated group with electric field. These were divided in two subgroups: animals sacrificed in the 19th after operation and animals sacrificed in the 27th after operation. The fracture member was treated with electric field pulsed with fundamental frequency of 1,5 MHz and 200 \'mü\'s of pulse width, in daily sessions of 20 minutes. The x-ray of that member took place in the day of the surgery and of the sacrifice. After the treatment the rats were sacrificed and their tibias were extracted for the material obtaining for analysis morfometric and mechanical rehearsal of flexion. The mechanical rehearsals revealed that there was only significant difference among the averages of the treated groups 18 and 12 days for the tension, the treated group 18 days had a larger tension, for the other analyzed mechanical properties there was not significant difference. The averages of the moment of inertia and area went significantly larger for the treated group 18 days compared with the respective averages of the control group controls. The treated group 12 days presented averages significantly larger when compared to the properties: moment of inertia, neutral line and area with the control group 12 days. These results suggest that an increment exists in the synthesis of organic matrix in the treated group, not accompanied lineally by the mineral synthesis. The difference significant statistic in the rupture tensions, on the other hand, suggests that the increase in the mineralization degree meets in course.
