Avaliação da saúde bucal, dores orofaciais, disfunções mandibulares e saliva em pacientes com diagnóstico de transtornos alimentares / Evaluation of oral health, orofacial pain, mandibular dysfunction and saliva in patients with eating disorders

INTRODUÇÃO: A Anorexia Nervosa (AN) e a Bulimia Nervosa (BN) destacam-se como os principais Transtornos Alimentares (TA). A presença de dor orofacial e de disfunção temporomandibular poderia desenvolver quadros de dores crônicas da face, podendo interferir no estado emocional, além de dificultar a ingesta alimentar - comprometendo ainda mais o estado nutricional desses pacientes. MÉTODOS: Foram avaliados 64 pacientes distribuídos nos grupos: Grupo A (AN Restritiva): 07; Grupo B (AN Purgativa): 19; Grupo C (BN): 16; Grupo D (Grupo-controle): 22 pacientes. A avaliação foi realizada por meio da avaliação pela ficha clínica odontológica, questionário de xerostomia, índice de higiene oral, aplicação do Protocolo de Pesquisa para Dor Orofacial (Ficha EDOF-HC e questionário RDC/TMD - Versão em Português) e avaliação de diversos parâmetros salivares (fluxo salivar, pH, capacidade tampão, amilase, peroxidase, ácido siálico, proteína total, eletrólitos), da saliva estimulada e não estimulada, no Laboratório de Biologia Oral da Faculdade de Odontologia da USP. RESULTADOS: Pacientes com TA apresentaram diferenças significativas quanto ao CPOD (p=0,000) em relação ao grupo-controle, principalmente comparando-se os grupos purgativos com o controle (p < 0,05). Queixas como \"sangramento gengival\" (p=0,043) e sinais como erosão e perimólise (p < 0,001) e sensação de boca seca (p=0,000) foram mais prevalentes em pacientes com TA. A queixa de \"ardor bucal\" (p=0,005) foi relatada somente entre os pacientes que praticavam a autoindução de vômitos. A condição periodontal (p=0,02), e o índice de higiene oral (p < 0,001) mostraram-se mais satisfatórios nos indivíduos do grupo-controle. As queixas de dor na face foram mais prevalentes em pacientes com TA (p=0,004) em relação ao controle, porém, sem diferenças entre os grupos restritivos e purgativos (p=0,442). Os transtornos musculares (p=0,010) e alterações articulares patológicas (p=0,051) estão entre as principais alterações encontradas com a aplicação do questionário RDC/TMD. A higiene oral, os parâmetros salivares e o CPOD não têm relação com a presença de dor miofascial. As LCNC são mais prevalentes nos grupos A, B e C em relação ao grupo D (p=0,03). O fluxo salivar mostrou-se reduzido nos pacientes dos grupos A, B e C em relação ao grupo-controle, tanto na saliva não estimulada (p=0,000) quanto na estimulada (p=0,001). Em relação ao pH, não houve diferenças significativas na saliva não estimulada (p=0,374) e na estimulada (p=0,121). Os valores da capacidade tampão também foram diferentes na saliva não estimulada (p=0,002) e estimulada (p=0,000), sendo os valores superiores no grupo-controle. Na saliva não estimulada, a amilase estava aumentada no grupo C (p=0,000), o ácido siálico estava aumentado no grupo D (p=0,000) e o Na estava reduzido no grupo D (p=0,05). Na saliva estimulada, a amilase e o ácido siálico também estavam aumentados no grupo C (p=0,008 e p=0,006, respectivamente), porém, o Na estava aumentado no grupo D (p=0,010). CONCLUSÕES: Pacientes com TA apresentam condição bucal (dentária e periodontal) pior em relação a indivíduos saudáveis, maior prevalência de sinais de erosão, de perimólise e de LCNC, bem como, maior número de queixas de sensação de boca seca, ardor bucal e de sangramento gengival, e pior índice de higiene oral e maior prevalência de queixas de dor orofacial em relação aos indivíduos saudáveis. O fluxo salivar é reduzido e há alterações na capacidade tampão, amilase, ácido siálico e sódio presentes na saliva. A queixa de \"ausência dentária\" é mais prevalente em pacientes com TA e com dor orofacial. METHODS: We evaluated 64 patients divided into two groups: Group A (AN Restrictive): 07; Group B (AN Purgative): 19; Group C (BN): 16; Group D (control group): 22 patients. The evaluation was conducted by evaluating the dental clinic record, xerostomia questionnaire, oral hygiene index, applying the research protocol for Orofacial Pain (EDOF-HC form and questionnaire RDC / TMD - Portuguese version) and evaluation of several salivary parameters (salivary flow, pH, buffering capacity, amylase, peroxidase, sialic acid, total protein, electrolytes), of the stimulated saliva and unstimulated in the Oral Biology Laboratory (School of Dentistry - USP) . RESULTS: Patients with ED showed significant differences in DMFT (p = 0.000) compared to the control group, especially comparing the purgative groups with the control (p < 0.05). Complaints such as \"gingival bleeding\" (p = 0.043) and signs such as erosion and perimólise (p < 0.001) and dry mouth (p = 0.000) were more prevalent in patients with ED. The complaint of \"burning mouth\" (p = 0.005) was reported only in patients who practiced self-inflicted vomiting. Periodontal status (p = 0.02) and the oral hygiene index (p < 0.001) were more satisfactory in the control group of individuals. Complaints of pain in the face were more prevalent in patients with ED (p = 0.004) compared to control, however, no differences between the restrictive and purgative groups (p = 0.442). Muscle disorders (p = 0.010) and pathological articular changes (p = 0.051) are among the major changes found with the application of the RDC / TMD questionnaire. The oral hygiene, salivary parameters and the DMF is not related to the presence of myofascial pain. The LCNC are more prevalent in groups A, B and C compared to group D, (p = 0.03). Salivary flow was shown to be reduced in patients in groups A, B and C in the control group, both in unstimulated saliva (p = 0.000) and the stimulated (p = 0.001). Regarding pH, no significant differences in the unstimulated saliva (p = 0.374) and stimulated (p = 0.121). The values of buffer capacity were also different in unstimulated saliva (p = 0.002) and stimulated (p = 0.000), with higher values in the control group. In unstimulated saliva amylase was increased in group C (p = 0.000), sialic acid was increased in D group (p = 0.000) and was reduced in the D group (p = 0.05). The stimulated saliva amylase and sialic acid were also increased in group C (p = 0.008 and p = 0.006, respectively), however, it was increased in group D (p = 0.010). CONCLUSIONS: Patients with ED present oral health (dental and periodontal) worse compared to healthy subjects, higher prevalence of signs of erosion of perimólise and LCNC as well as higher number of dry mouth complaints, oral burning and bleeding gingival, and worse index of oral hygiene and greater prevalence of orofacial pain complaints compared to healthy individuals. Salivary flow is reduced and there are changes in buffering capacity, amylase, sialic acid and sodium present in saliva. The complaint of \"dental absence\" is more prevalent in patients with ED and orofacial pain. Comportamentos de risco para transtornos alimentares em adolescentes e adultos portadores de diabetes tipo 1 / Risk behavior for eating disorder in teenagers and adults suffers of diabetes type 1

Introdução - Pacientes portadores de diabetes tipo 1 que manifestam comportamentos de risco para transtornos alimentares (TA) possuem grandes chances de complicações microvasculares e macrovasculares e maiores taxas de mortalidade. Objetivo Avaliar a freqüência de comportamento de risco para TA em adolescentes e adultos portadores de diabetes tipo 1 bem como sua relação com variáveis socioeconômicas, nutricionais, relacionadas ao diabetes e satisfação corporal. Metodologia: Foram aplicados: Teste de Investigação Bulímica de Edinburgh (BITE), Teste de Atitudes Alimentares (EAT), Escala de Compulsão Alimentar Periódica (ECAP) e Escala de Silhuetas de Stunkard, além da mensuração de peso, estatura e cálculo do IMC. Foram verificadas associações ou diferenças estatísticas por meio dos testes de Qui-Quadrado e T-Student e Regressão Logística para identificar fatores que influenciaram o risco da doença. Resultados: Um total de 58,7% dos pacientes apresentaram escore positivo em algum dos testes utilizados para risco de TA. Houve diferença significante entre os grupos com e sem risco para TA com relação ao IMC (p=0,009), gênero (p=0,001), omissão de insulina (p=0,003), uso de insulina NPH (p=0,006), ausência do uso da insulina Glargina (p=0,021), ausência do uso da técnica de contagem de carboidratos (p=0,019), controle da glicemia (p=0,049) e insatisfação corporal (p=0,001). Conclusão: Portadores de diabetes tipo 1 apresentaram grande freqüência de comportamentos de risco para o TA, o que requer atenção e cautela dos profissionais de saúde. / Introduction - Patients suffers of type 1 diabetes that reveal risk behavior for eating disorders (ED) have more chances of micro and macro vascular complications and a high mortality level. Objective Evaluate the frequency of risk behavior for ED in teenagers and adults suffers of type 1 diabetes as well as its relation to the socially economical variable, nutritional variable, related to diabetes and body satisfaction. Methodology: There was applied: The Bulimic Inventory Test of Edinburgh (BITE), Eating Attitudes Test (EAT), Binge Eating Scale (BES) and The Stunkard´s Figure Rating Scale, besides weight measure, height and BMI calculation of the patients. There were verified associations or statistical differences through tests of Chi-Square and T-student and Logistical Regression to identify the factors that influenced the risk of the disease. Results: A total of 58,7% patients presented positive score in tests used for ED risk. There was a significant difference among the groups with and without risk for ED in relation to the BMI (p=0,009), gender (p=0,001), omission of insulin (p=0,003), usage of NPH insulin (p=0,006), lack of usage of Glargine insulin (p=0,021), lack of usage of the technique of carbohydrate counting (p=0,019), blood glucose control (p=0,049) and body dissatisfaction (p=0,001). Conclusion: The suffers of type 1 diabetes presented high frequency of risk behavior for ED, which require more attention when caring for patients for the prevention of the disease.

Exploring the interaction of emotional intelligence and coping in the development of eating disorders

Eating disorders remain a phenomenon that escapes full comprehension, resulting in frustration for those who suffer from the disorders, their families, and their therapists. It is becoming increasingly necessary to describe the mechanism by which eating disorders develop, in order to effectively treat and prevent these disorders. The aim of this study was to illuminate factors that contributed to the development of eating disorders within the individual contexts of the lives of the participants, as well as how those factors interacted in context to culminate in the development of an eating disorder. This qualitative study was conducted from an interpretive perspective. The findings show how individual contextual factors interact to produce a marked fear of gaining weight, which is driven by fear of negative evaluation, and that the eating disorder behaviour serves specific functions that are related to coping with stress within the lives of the participants. / Psychology / M. The effects on staff of working in an eating disorders unit

1 online resource (vii, 138 leaves : ill.) / Following an awareness of an increased turn over of staff in the eating disorder unit as compared with other specialised units, in a psychiatric hospital in Johannesburg, South Africa, Tara Hospital, the researcher was motivated to investigate the issue. An exploratory, descriptive based research was chosen to explore and gain information about the topic and its implications. A qualitative research approach was used to gain insight into the perceptions and needs of the team working on the unit. The case study method was used. A pilot study was undertaken to test the validity of the interview schedules. The sampling category was a non- probability one. Individuals were selected from the population of staff working on the unit. Participants were selected from two groups, those who were presently working on the unit and those who had previously worked on the unit and now working in other units. Interview procedure involved personal semi-structured interviews conducted by the researcher and analysed qualitatively and a structured interview questionnaire analysed quantitatively. The researchers assumption that many staff members move from working in an eating disorders unit was confirmed and is due to the following: Staff turnover is due to constant exposure to occupational stress and burnout. Feelings of helplessness, a sense of being unappreciated and excessive exposure to conflict from the patients. In addition, staff experience minimum rewards leading to lowered job satisfaction due to the patients slow recovery rates and a poor prognosis of the illness. Staff also experience a change in their eating patterns and an increased awareness around food and food issues. Recommendations to the staff include: * Psycho-education on eating disorders. * Implementation of strategies to provide supportive care for all staff members. * Education on stress management and strategies to prevent staff burnout and lowered job satisfaction. * A multidisciplinary teamwork approach by the staff, when working in the unit. / Social Work / M. Title: Kinesiotherapy in the treatment of patients with eating disorders Summary: The aim of our experiment was evaluate the effect of kineziotherapy (psychomotor therapy) on body image in the treatemnet of the patiens with eating disorders in The Department of Psychiatry on General University Hospital in Prague. The study included 10 women divided into two groups according to the psychiatrist`s diagnosis of anorexia nervosa and bulimia nervosa. The effectiveness of kinesiotherapy was also evaluated about the type of treatment - inpatient of the department for eating disorders and outpatient of the day care institution for eating disorders. We used a validated questionnaire Body Attitude Test (BAT) which is used for negative evaluation of body size, lack of familiarity with one`s own body and general body dissatisfaction. Then we used Test of silhouette according to Stunkard and Test for the evaluation of sensory stimuli according to Petrie. Due to the small sample size, we supplemented the quantitative part with the qualitative interview with open questions. It allowed us to understand the interaction of individuals with their own body. Title: Acipimox during Short-Term Exercise Exerts A Negative Feedback of Growth Hormone on Ghrelin Secretion in Patients with Bulimia Nervosa and in Healthy Women: The Role of Lipolysis Objective: Eating disorders, such as bulimia nervosa (BN) and anorexia nervosa (AN), are characterized by abnormal eating behavior. The main features of BN are binge-eating and inappropriate compensatory methods to prevent weight gain. The appetite-modulating peptide ghrelin is secreted by the stomach and shows a strong release of growth hormone (GH). A potential GH-ghrelin feedback loop between stomach and the pituitary has been recently reported. Acipimox (Aci), an analogue of nicotinic acid, inhibits lipolysis in adipose tissue (AT) and reduces plasma glycerol and free fatty acids (FFA) levels. Exercise and Aci are stimulators of GH secretion. We suppose that a negative feedback from increased GH levels during exercise may play a role in reducing plasma ghrelin levels. We surmised that altered baseline activity and exercise-induced activation of the sympathetic nervous system (SNS) results in excessive stimulation of lipolysis associated with negative energy balance and may lead to abnormal AT metabolism in patients with BN. Eating disorders have been studied from different points of view lately. They have occured in the centre of various psychiatric and psychologic diagnostics and therapy surveys and they have also been analysed as a sociocultural element of our lifestyle norms, our values and image standards. The number of men and women suffered from these disorders cannot be neither compared nor omitted. It is statistically clear that about ten per cent of anorexics and bulimics are male patients. Going West, the number of male patients is increasing. These disorders in men are usually connected with jobs, mostly those such as jockeys, gymnasts and dancers. Bulimia and anorexia are reported as typically women disorders therefore men hardly any time concede they can suffer from this problem and need a specialist´s advice.. The aim of the work has been to map eating behaviour, a lifestyle, physical activities and risk factors responsible for eating disorder incidence such as a distorted view of the body and being on a diet among men between the ages of 15 to 30. A quantitative technique approach using questionnaires was applied. There were stated four hypotheses concerning the relation between body acceptance and physical activities, methods used for body weight control and differences in lifestyles of secondary school students.

Salivary flow was shown to be reduced in patients in groups A, B and C in the control group, both in unstimulated saliva (p = 0.000) and the stimulated (p = 0.001). Regarding pH, no significant differences in the unstimulated saliva (p = 0.374) and stimulated (p = 0.121). The values of buffer capacity were also different in unstimulated saliva (p = 0.002) and stimulated (p = 0.000), with higher values in the control group. In unstimulated saliva amylase was increased in group C (p = 0.000), sialic acid was increased in D group (p = 0.000) and was reduced in the D group (p = 0.05). The stimulated saliva amylase and sialic acid were also increased in group C (p = 0.008 and p = 0.006, respectively), however, it was increased in group D (p = 0.010). CONCLUSIONS: Patients with ED present oral health (dental and periodontal) worse compared to healthy subjects, higher prevalence of signs of erosion of perimólise and LCNC as well as higher number of dry mouth complaints, oral burning and bleeding gingival, and worse index of oral hygiene and greater prevalence of orofacial pain complaints compared to healthy individuals. The complaint of \"dental absence\" is more prevalent in patients with ED and orofacial pain. Les troubles du comportement alimentaire (TCA) touchent une part importante de la population et constitue un réel problème de santé publique, il est indispensable d’approfondir nos connaissances sur la physiopathologie des TCA afin d’identifier des perspectives thérapeutiques. L’écriture d’une revue systématique a permis d’évaluer la prévalence des TCA selon le sexe, l’origine géographique ou l’âge de la population étudiée et confirme son augmentation au cours des 15 dernières années. L’analyse des données épidémiologiques des patients avec un TCA de la cohorte Eating Disorder and Longitudinal Survey (EDILS) a souligné le stress et le régime alimentaire comme facteurs de survenue et des similarités entre les comorbidités des 3 catégories larges de TCA (Restrictive, boulimique et compulsive). Ces comorbidités étaient fortement associées entre elles et, selon un cercle vicieux pourrait contribuer au maintien de la physiopathologie des TCA. La littérature suggère l’implication d’une dérégulation peptidergique de la prise alimentaire au cours des TCA qui pourrait inclure un mécanisme immunitaire. Les analyses des échantillons plasmatiques de la cohorte ont permis de mieux définir les profils biologiques de peptides et immunoglobulines associés aux 3 catégories larges de TCA et d’évaluer leur utilité pour le phénotypage des patients. La collecte de selles dans la cohorte offre également la possibilité de préciser les profils de microbiote au cours des différents types de TCA. Cette thèse a donc permis de mieux définir les caractéristiques cliniques des TCA et les perturbations de l’axe microbiote-intestin-cerveau associées, en particulier les dérèglements biologiques. / Eating disorders (ED) affect a large population and are a serious public health issue. It is thus essential to deepen our knowledge of ED pathophysiology in order to open new therapeutic perspectives. The prevalence of ED according to sex, geographical origin or age of the study population and its increase over the last 15 years has been highlighted in a systematic review. The analysis of patients with ED from the Eating Disorder and Longitudinal Survey (EDILS) cohort underlined stress and / or diet as triggering factors and noted similarities between the comorbidities of the 3 broad categories of ED (Restrictive, bulimic and compulsive). These comorbidities were strongly associated with each other, and contributed in a vicious circle to the perpetuation of ED. The literature suggests during ED a dysregulation of neuropeptide signaling of food intake that may include some immunological mechanisms. Analyzes of plasma samples from the EDILS cohort allowed a better definition of the peptides and immunoglobulins profiles associated with the 3 broad categories of ED and to evaluate their utility for phenotyping patients. The collection of feces in the cohort offers the possibility of refining the specificity of the microbiota according to different type of ED. This thesis allows to better define the clinical characteristics of 3 broad categories of ED and disturbances of the microbiota-intestine-brain axis, particularly the associated biological dysregulations of neuropeptide signaling.
