If You Make it, Will They Come?: The Impact of the Affordable Care Act and Organizational Characteristics on Hispanic Mental Health Care Organizations

Thesis advisor: Rocío Calvo / Hispanics are less likely than non-Hispanic whites to use mental health service, even after controlling for various social, environmental, and health factors. Mental health services disparities between Hispanics and non-Hispanic whites have been well-documented and consistent over time. However, very little is known about the impact mental health care organizations have on Hispanics’ access to mental health care, especially since the implementation of the Patient Protection and Affordable Care Act (ACA). The three papers in this dissertation utilize the 2010, 2014, and 2016 waves of the National Mental Services Survey (N-MHSS) to assess the impact of the ACA on Hispanics’ access to mental health care and mental health care organizations’ provision of integrated services. The N-MHSS is a national repository of data on the mental health organizations in the United States. This dataset was created to report the characteristics and client enrollment at mental health care organizations. Paper 1 uses the 2014 N-MHSS to describe the structural characteristics of mental health care organizations according to the proportion of Hispanics they serve and the organizations’ structural characteristics in Medicaid expansion and non-expansion states. Paper 2 uses the 2010, 2014, and 2016 N-MHSS waves to examines the impact of the ACA and the health safety net on Hispanic admissions at mental health care organizations. These three waves were merged together using a repeated cross-sectional design to assess whether Hispanic admissions increased after the implementation of the ACA. The final paper uses the 2014 and 2016 N-MHSS waves to assesses whether integrated care has increased at Hispanic-serving organizations compared with mainstream organizations two years after the implementation of the ACA. This paper also assessed whether the increased funding for integrated services under the ACA has disproportionately affected mainstream organizations compared with Hispanic-serving organizations. / Thesis (PhD) — Boston College, 2018. / Submitted to: Boston College. Relationship Between Mental Health Facilities' Pre-Intake Practices and Children's First Appointment Attendance Rates

Between 20% and 57% of patients at community mental health centers miss their first mental health appointment, resulting in wasted resources and longer wait times for other patients. To date, only one peer-reviewed quantitative study examined the relationship between appointment reminder practices and children's and adolescents' first appointment attendance rates for community mental health centers. The purpose of this quantitative study was to examine the relationship between first appointment wait times, electronic appointment reminders, phone appointment reminders, and first appointment attendance for U.S. community mental health centers, along with the potential moderating effects of age, ethnicity, and gender. Blumer's symbolic interaction theory served as the theoretical framework. Secondary data on was collected from 12 Texas-based community mental health centers covering 5,260 patients. Binary logistic regression uncovered the following key study findings: (a) wait time and message type significantly predicted appointment attendance; (b) e-mail appointment and text reminders were significantly more effective than were phone reminders; and (c) age, gender, and ethnicity moderated the relationship between wait time and message type, and first appointment attendance. The implications for positive social change include improving our understanding of the optimal appointment reminder tactics that increase children's and adolescents' mental health appointment attendance rates, improve operating efficiency of community health centers, and increase the likelihood that children and adolescents will receive needed mental health services.

Esse estudo teve como objetivo analisar o atendimento das demandas de saúde mental na rede de atenção à saúde no município de Cordeirópolis, interior de São Paulo. Tal análise compreendeu a descrição da estrutura desta rede e a percepção dos profissionais sobre a identificação e cuidado das demandas de saúde mental. Para tanto, realizou-se uma pesquisa qualitativa, transversal do tipo descritivo-exploratória. Foram utilizados dados primários e secundários. Os dados foram coletados entre junho de 2013 e maio de 2014 através de análise documental e entrevistas semiestruturadas. Os dados secundários foram obtidos a partir de registros do Departamento de Informática do Sistema Único de Saúde (DATASUS), dos documentos que embasam o funcionamento dos serviços estudados (planilhas com dados dos acolhimentos no ano de 2012, o projeto de implantação do Centro de Atenção Psicossocial - CAPS e o Plano de Funcionamento do serviço) além do Plano Municipal de Saúde \"2010- 2013\". Já os dados primários foram obtidos através de entrevistas com 20 profissionais de saúde de nível médio e superior que atuavam nos serviços estudados a saber, CAPS, Ambulatório de Saúde Mental, Pronto-Socorro e Estratégias de Saúde da Família. O referencial teórico adotado para análise foram os pressupostos de Bárbara Starfield em relação à organização dos sistemas de saúde sob a lógica da Atenção Primária a Saúde. Assim os dados foram analisados utilizando a análise documental e análise de conteúdo. Os resultados apontaram que a rede de atenção psicossocial do município estava estruturada com importantes pontos de atenção (CAPS, o Ambulatório de Saúde Mental, as Estratégias de Saúde da Família e Pronto-Socorro) e equipes com número e diversificação considerável de profissionais. Em relação à percepção dos participantes sobre a saúde mental, identificou-se uma justaposição de visões que ora corroboram o paradigma biomédico, ora o conceito ampliado de saúde. Esta diversidade de concepções ilustra o fato que saúde e saúde mental se entrecruzam e confundem-se dentro da história da humanidade, tendo sido tratadas como distintas apenas com o nascimento da psiquiatria, quando essa especialidade toma para si o objeto \"psíquico\", salientando a divisão mente-corpo. Essa diferença de concepções, pode também explicar as ações ora focadas na remissão de sintomas, tratamento medicamentoso e encaminhamento para internação, ora com vistas para aspectos psicossociais, como por exemplo, escuta, acompanhamento psicólogico, criação de vínculo, oficinas de geração de renda, passeios, dentre outras. Dessa forma, o presente estudo permitiu demarcar uma série de potencialidades da rede de saúde do município em questão no atendimento a demanda de saúde mental, no entanto foram também destacadas algumas recomendações importantes para sua melhoria / This study aimed to analyze the treatment for mental health demands in the health care network in Cordeirópolis, São Paulo. Secondary data were obtained from the Unified Health System database (Departamento de Informática do Sistema Único de Saúde - DATASUS), data of the respective services and the Municipal Health Plan \"2010-2013\". For document analysis, we selected the documents that support the service operation: spreadsheets with data screening of 2012, the implementation project of the Community Health Center (Centro de Atenção Psicossocial - CAPS) and this Service Operation Plan. For the primary data, we selected 20 health professionals at middle and senior levels who worked in the these services: CAPS, Outpatient Mental Health Clinic, First Aid Station and Family Health Program. Thus the data were analyzed using document analysis and content analysis. The results showed that the psychosocial care\'s network of the council was structured with important services (CAPS, the Outpacient Mental Health Clinic, Family Health Program and First Aid Station) and teams with considerable diversity and number of professionals. Regarding the participants\' perceptions about mental health, we identified a juxtaposition of views that either corroborates the biomedical paradigm and the wider concept of health at other times. The diversity of views presented by participants illustrates the fact that health and mental health are intertwined and confused in the history of mankind and have been treated as distinct, only with the birth of psychiatry, when this specialty takes on the \'psychic\' object, emphasizing the mind-body division. This difference of views, can also explain the actions focused on remission of symptoms, drug treatment and referral for hospitalization, sometimes overlooking psychosocial aspects, such as listening, psychological counseling, bond\'s creation, CAPS\'s patients in workshops for income, tours, among others. Thus, the present study allows demarcate a series of network capabilities of the studied municipal health service in the demand for mental health, however were also highlighted some important recommendations for improvement

Objetivamos contribuir com o planejamento, gestão e a sustentação clínica de uma rede de serviços de saúde mental a partir da compreensão da experiência subjetiva da crise em pessoas diagnosticadas com transtorno do espectro esquizofrénico para que possa dar resposta a essas situações. A metodologia é qualitativa, baseada na Hermenêutica Filosófica de Hans-Georg Gadamer e a Hermenêutica Crítica de Paul Ricoeur. Para isto nos valemos da observação participante em Centros de Atenção Psicossocial, e de grupos focais com usuários desses serviços como principais ferramentas de construção do material de campo. Nossos achados apontam que são diferentes as intervenções na crise experimentadas pelos usuários em hospitais psiquiátricos e em Centros de Atenção Psicossocial. Também diversas são as estratégias implementadas por eles para antecipar e/ou lidar com esse difícil momento. Finalmente são destacadas contribuições e desafios para o planejamento, a gestão e a sustentação clínica dos serviços públicos de saúde mental encarregados de dar resposta à crise / Abstract: This is a multicenter study, developed in the cities of Rio de Janeiro / RJ, Salvador / BA and Campinas / SP. We aim to contribute to the planning, management and clinical support of a network of mental health services from the understanding of the subjective experience of the crisis on people diagnosed with schizophrenic spectrum disorder, in order to respond to these situations. The methodology is qualitative, based on the Philosophical Hermeneutics of Hans-Georg Gadamer and Paul Ricoeur's Critical Hermeneutics. Participant observation in Psychosocial Care Centers and focus groups with users of such services was the main tools for building material field. Our findings suggest that crisis interventions are different in the experience of patients at psychiatric hospitals and Psychosocial Care Centers. Also different are the strategies implemented by them to anticipate and / or deal with this difficult moment. Deinstitutionalization and Its Discontents: American Mental Health Policy Reform

In 1963, President John F. Kennedy signed the Mental Retardation and Community Mental Health Centers Construction Act, establishing the beginnings of deinstitutionalization in the United States. By some counts, this Act was a stupendous policy success—by others, a dismal failure. 50 years later, no cohesive national mental health care policy has emerged to deal with increased rates of mental illness among the homeless and the incarcerated. However, California has made enormous strides to create a state policy which provides adequate services to the mildly, moderately, and severely mentally ill as well as adequate funding for those services through Proposition 63, the Mental Health Services Act, passed in 2004. This paper reviews mental health policy history from Colonial America to the present, paying special attention to JFK's deinstitutionalization in 1963 and the discontents that followed. It takes a special look at California's mental health care policy history and the strides the state has made to better serve the mentally ill.
