Prerequisites and Responsibility for Appropriate Prescribing - the Prescribers' View

The overall aim of this thesis was to explore aspects of the subjective views and experiences of doctors as prescribers, focusing on responsibility for and factors of importance in achieving appropriate prescribing. To provide insights into the prescriber’s perspective the study designs were qualitative. In the first studies secondary care doctors’ perceptions of appropriate prescribing and influences in prescribing were investigated in interviews. The doctors perceived that appropriate prescribing needed continuous revision. From the perspective of the prescribers the definition of prescribing could be rephrased as: “the outcome of the recurring processes of decision making that maximises net individual health gains within society’s available resources”. Among the influences in prescribing were guidelines, colleagues and therapeutic traditions. In the subsequent studies the experiences of exchanging information regarding a patient’s drugs in an electronic patient medical record (e-PMR) shared between primary and secondary care and views of responsibility was explored, using focus groups with both primary and secondary care doctors. Considering the gap between health care levels, doctors’ views of responsibility in prescribing and exchange of information are of concern. The doctors expressed how they assume information to be in the e-PMR and active information transfer has decreased. On the other hand, they experienced an information overload in the e-PMR system. There is a need for improved and structured communication between health-care givers. Taking responsibility to review all the patient’s medications was perceived as important, but described as still not done. Lack of responsibility taken was often due to acts of omission, i.e. that doctors did not make needed changes to the list of medications due to different barriers. The barriers rested both with individual doctors and the system, but to ensure solutions that are realisable in practise, perspectives of the doctors need to be taken into consideration when overcoming those barriers.

A 3D Virtual Environment Development Platform for ASD Therapy Tools

Thesis (MSc (Computer Science))--University of Stellenbosch, 2009. / ENGLISH ABSTRACT: The aim of this thesis is to develop a generic 3D virtual environment development platform for autism spectrum disorder (ASD) therapy tools. The potential of using computerised therapy tools for ASD therapy is well known. However, the development of such tools is expensive and time-consuming, and is language and culture speci c. This work intends to alleviate these problems. The design of the platform is based on known game engine designs, but adapted for the requirements of ASD therapy tools. It supports standard features such as 3D rendering, animation and audio output. Speci c features, aimed at ASD therapy tools and educational games, included in our engine are: replays, data capturing, remote monitoring over a network and language localisation. We also implemented an input hardware abstraction layer to allow support for non-standard input peripherals in the future, without modifying existing game implementations. Furthermore, to separate the development of games and tools from the engine, we include wrapper libraries in our engine for Lua and Java. We successfully developed our engine and implemented a number of prototype therapy tools and educational games. These implementations confirmed that the engine works as expected. Some of these programs are currently in use at a local primary school. / AFRIKAANSE OPSOMMING: Die doel van hierdie tesis is om 'n 3D virtuele omgewing en ontwikkelingsplatform vir outistiese spektrum versteuring (OSV) terapiemiddels te ontwikkel. Die gebruik van rekenaargebaseerde terapiemiddels vir OSV terapie is bekend. Om sulke terapiemiddels te ontwikkel is egter duur, tydrowend en is dikwels gerig op spesi eke taal- en kultuurgroepe. Hierdie werk het dit ten doel om hierdie probleme te bowe te kom. Die ontwerp van die platform is gebaseer op die ontwerp van bekende videospeletjie-enjins, maar is aangepas vir die benodigdhede van OSV terapiemiddels. Dit ondersteun standaard funksionaliteit soos 3D uitbeelding, animasie en klank. Ons platform sluit in spesi eke funksionaliteit, wat gerig is op OSV terapiemiddels en opvoedkundige speletjies, naamlik: kykweer, datavaslegging, afstandswaarneming oor 'n netwerk en taal-lokalisering. Verder is 'n abstrakte koppelvlak vir toevoerapparatuur ontwikkel, wat dit moontlik maak om in die toekoms nie-standaard toevoerapparatuur te ondersteun, sonder om bestaande speletjies se implementasies aan te pas. Verder, om die ontwikkeling van speletjies en terapiemiddels te skei van die enjin, is koppelvlakke ontwikkel wat dit moontlik maak om die enjin in Lua en Java te gebruik.

An analysis of urban transport demand in Windhoek : a case study of the Katatura and Khomsdal residential areas

Existing travel patterns in Windhoek are influenced by the past land use policy of restricted urban development in the former townships ofKatutura and Khomasdal. Thus residents in these townships are faced with longer distances to reach quality urban servtces. This thesis examined the impact on travel demand of changes in land use policies aimed at stimulating growth in the two areas using the HLFM computer model. The results showed that, policy induced land use changes may stimulate population and employment growth in the neglected areas, but with no effects on the present travel patterns. The model needs to be refined to fit the peculiarity of the study area. Nevertheless, information provided in the thesis could be used in assessing areas where development trends will go if any ofthe land use policy tested in the study is adopted. / Geography / M.A. (Geography)

Title: The effect of airway control on stance dynamic stability Objectives: The aim of this thesis is to find out whether and how will the influence of vocal cords modulation be manifested on the dynamic stabilization of the standing body during translational shifts of the supporting surface of different intensities and A-P directions. Thus, building on the findings of Massery et al (2013). Methods: The thesis has the character of qualitative research. The experiment was attended by 23 healthy probands, of which 7 men and 16 women aged 20-40 years. Spirometry was used to test the objectivity of airway airflow during breathing / phoning maneuvers with different vocal cords positioning and dynamic computer posturography using the Neurocom Smart Equi Test System and its Motor Control Test, which evaluated the effectiveness of automatic postural responses. We connected the posturograph with the spirometer using the Kistler accelerometer (type 8766A100BB). The course of the experiment was simultaneously recorded by a camera (GoPro Hero 7). The Smart EquiTest System generated three postural perturbations of different intensity (S - sub treshold, M - threshold, L - saturating) in two directions (anterior translation / posterior translation). Developing soft tissue thickness values for South African black females and testing its accuracy

In forensic science one frequently has to deal with unidentified skeletonised remains. When conventional methods of identification have proven unsuccessful, forensic facial reconstruction (FFR) may be used, often as a last resort, to assist the process. FFR relies on the relationships between the facial features, subcutaneous soft tissues and underlying bony structure of the skull. The aim of this study was to develop soft tissue thickness (STT) values for South African black females for application to FFR, to compare these values to existing literature or databases, and to test the accuracy and recognisability of reconstructions using these values. It also established whether population-specific STT values are necessary for FRR. Computerised tomography scanning was used to determine average populationspecific STT values at 28 facial landmarks of 154 black females. The Manchester method of facial reconstruction was employed to build faces, for which antemortem photographs were available, on two skulls that were provided by the South African Police Service’s (SAPS) Forensic Science Laboratory. Different data sets of STT values, namely values from this study, two sets of data from American blacks and a South African mixed ancestry group, were used to build four faces for each of the skulls. Two identification sessions were then held. In the first session, 30 observers were asked to select matches from a random group of 20 photographs of black females which included the two actual images. The identification rates calculated for each photograph revealed that the highest rates of a positive match were for the reconstructions based on South African values. In the second session another group of 30 volunteers were asked to match to each photograph the most similar of the four reconstructions made of that particular individual. The reconstructions with STT values from the current (South African) study were selected more often than the other data sets. Although shortcomings do exist, the identification sessions indicated that FFR can be of value. Furthermore, population-specific STT values are important, since skulls reconstructed using these values were selected or identified statistically significantly more often than the others. Wanneer die konvensionele metodes van identifikasie onsuksesvol is, mag forensiese gesigsrekonstruksie (FGR) gebruik word, dikwels as `n laaste uitweg, om die proses te help. FGR is afhanklik van die verhouding tussen die gelaatstrekke, subkutane sagte weefsels en onderliggende benige struktuur van die skedel. Die doel van hierdie studie was om sagte weefsel dikte (SWD) waardes vir Suid-Afrikaanse swart vroue te ontwikkel vir gebruik met FGR, om hierdie waardes te vergelyk met bestaande literatuur of databasisse, en die akkuraatheid en herkenbaarheid van rekonstruksies waar hierdie waardes gebruik was te toets. Dit is gedoen ten einde vas te stel of bevolking-spesifieke SWD waardes nodig is vir FGR. Gerekenariseerde tomografie skandering is gebruik om die gemiddelde bevolkingspesifieke SWD waardes op 28 gesigslandmerke van 154 swart vroue te bepaal. In die tweede sessie was `n ander groep van 30 vrywillgers gevra om die mees soortgelyke van die vier rekonstruksies by die foto van die betrokke individu te pas. Die rekonstruksies met SWD waardes van die huidige (Suid Afrikaanse) studie was meer dikwels gekies as die van ander data stelle. Hoewel verskeie tekortkominge bestaan, het die identifikasie sessies getoon dat FGR van waarde kan wees. The predictive validity of learning potential and English language proficiency for work performance of candidate engineers

The aim of this research was (1) to provide empirical data of learning potential and English language proficiency for work performance; (2) to establish whether race and gender influence work performance; (3) to evaluate practical utility and to propose recommendations for selection purposes. The Learning Potential Computerised Adaptive Test and the English Literacy Skills Assessment were used as measuring instruments to measure learning potential and English language proficiency respectively. Work performance data were obtained from the normal performance data system of the company where the research was conducted. ANOVA results showed differences between race and gender groupings. A regression analysis confirmed the predictive validity of learning potential and English language proficiency on work performance. The Spearman rho correlation coefficient (p < 0.05) showed a significant positive correlation between the investigated variables / Industrial and Organisational Psychology / M. A. Deciphering maintenance challenges through computerized maintenance management system in Ethiopian manufacturing industries

Abstracts in English and Xhosa / Given the need for prompt responses to today’s dynamic market, maintenance and maintenance management functions are becoming increasingly fundamental for industrial companies. Reduction of waste and bottlenecks, particularly the reduction of overproduction and work in process, accentuates the impact of malfunctions or failures of equipment on production. As such, it has become ever more urgent for proactive world-class maintenance to turn to computer-based support for efficacious management. To successfully manage activities such as the scheduling of activities, the planning of preventive maintenance actions, the analysis of data (to reduce the occurrence of malfunction and failures), and augment the absolute performance of the maintenance function, industrial companies implement information systems enabled by computerised maintenance management systems (CMMS) to deliver timely and accurate information. However, while various CMMS are available on the market, not all meet the exactitudes of each industrial company. The overall objective of this thesis is threefold: to explore major barriers and obstacles that have a negative impact on implementing CMMS; to identify the most common critical success factors (CSFs) that have a positive impact on implementing CMMS; and to develop an optimised CMMS model suitable to the context of Ethiopian manufacturing industries. To achieve this, an exploratory descriptive research design was employed, utilising both quantitative and qualitative data-gathering techniques, including structured interviews and questionnaires. Both qualitative and quantitative findings Suggest that the most important CSFs for CMMS implementation were work planning and scheduling and work identification and responsibilities, while the Ethiopian manufacturing firms lagged behind in the case of information technology and appraisal. Overall, key isues in maintenance management range from several maintenance techniques, information systems, scheduling, and optimisation models. This thesis is projected to be a useful source of information for both maintenance managers and stakeholders in CMMS decision making. Las nuevas tecnologías en la administración de justicia. La validez y eficacia del documento electrónico en sede procesal

La tesis se encarga de analizar, por un lado, la integración y el desarrollo de las nuevas tecnologías en la Administración de Justicia; y, por otro, los parámetros que constituyen la validez y eficacia del documento electrónico. La primera cuestión se centra en la configuración de los Sistemas de Información de la Oficina Judicial y del Ministerio Fiscal, así como de la informatización de los Registros Civiles, donde el art. 230 LOPJ es la pieza clave. Se estudian sus programas, aplicaciones, la videoconferencia, los ficheros judiciales y las redes de telecomunicaciones que poseen la cobertura de la firma electrónica reconocida, donde cobran gran relevancia los convenios de colaboración tecnológica. La digitalización de las vistas quizá sea una de las cuestiones con más trascendencia, teniendo en cuenta que el juicio es el acto que culmina el proceso. Aunque no todos los proyectos adoptados en el ámbito de la e.justicia se han desarrollado de forma integral, ni han llegado a la totalidad de los órganos judiciales. El objetivo final es lograr una Justicia más ágil y de calidad, a lo cual aspira el Plan Estratégico de Modernización de la Justicia 2009-2012 aprobado recientemente.En referencia a la segunda perspectiva, no cabe duda que el Ordenamiento jurídico y los tribunales, en el ámbito de la justicia material, otorgan plena validez y eficacia al documento electrónico. Nuestra línea de investigación se justifica porque cada vez son más los procesos que incorporan soportes electrónicos de todo tipo, ya sea al plantearse la acción o posteriormente como medio de prueba (art. 299.2 LEC). Entre otros temas examinamos el documento informático, la problemática que rodea al fax, los sistemas de videograbación y el contrato electrónico. / La tesi s'encarrega d'analitzar, per una part, la integració i el desenvolupament de les noves tecnologies dins l´Administració de Justícia; i, per l'altra, els paràmetres que constitueixen la validesa i l'eficàcia del document electrònic. La primera qüestió es centra en la configuració dels Sistemes d´Informació de l´Oficina Judicial i del Ministeri Fiscal, així com de la informatització dels Registres Civils, on l'art. 230 LOPJ es la peça clau. S'estudien els seus programes, aplicacions, la videoconferència, el fitxers judicials i les xarxes de telecomunicacions que tenen la cobertura de la firma electrònica reconeguda, on cobren gran rellevància els convenis de col·laboració tecnològica. La digitalització de les vistes tal vegada sigui una de les qüestions amb més transcendència, tenint amb compte que el judici es l'acte que culmina el procés. Però no tots el projectes adoptats en l'àmbit de la e.justicia s'han desenvolupat d'una manera integral ni han arribat a la totalitat dels òrgans judicials. L'objectiu final es assolir una Justícia més àgil i de qualitat, al que aspira el Pla Estratègic de Modernització de la Justícia 2009-2012 aprovat recentment. En referència a la segona perspectiva, no hi ha dubte que l´Ordenament jurídic i els tribunals, en l'àmbit de la justícia material, donen plena validesa i eficàcia al document electrònic. La nostra línia d'investigació es justifica perquè cada vegada son més el processos que incorporen suports electrònics de tot tipus, ja sigui quant es planteja l'acció o posteriorment como a medi de prova (art. 299.2 LEC). Entre altres temes examinem el document informàtic, la problemàtica que envolta al fax, els sistemes de videogravació i el contracte electrònic. / The thesis seeks to analyse, on the one hand, the integration and development of the new technologies in the Administration of Justice; and, on the other, the parameters which constitute the validity and efficiency of the electronic document.The first question centres on the configuration of the Information Systems of the Judicial Office and the Public Prosecutor, as well as the computerisation of the Civil Registers, where the art. 230 LOPJ it's the part key. Their programmes, applications, the Video Conferencing, the judicial registers and the telecommunication networks which are covered by the recognised electronic signatures, are studied, where the agreements on technological collaboration gain great relevance. The digitalisation of evidence might perhaps be one of the questions with most consequence, bearing in mind that the judgment is the act by which the process is culminated. Although not all the projects adopted within the compass of e.justice have developed completely nor have reached all the judicial organs. The final objective is to achieve an agile, quality Justice, to which the recently approved Strategic Plan for the Modernisation of Justice aspires.With reference to the second perspective, there is no doubt that the juridical Ordinance and the tribunals within the compass of material justice grant full validity and efficacy to the electronic document. Our line of investigation is justified because there are more and more processes which are sustained by electronic supports of all kinds, whether it be at the establishment of the action or later, as a proof of it (art. 299.2 LEC). Amongst other things, we examine the computerised document, the problems which surround the fax, the systems for video recording and the electronic contract.
