An Assessment of the Feasibility of Environmental Exposure Data for Syndromic Surveillance

INTRODUCTION: Syndromic surveillance is a method of rapid disease detection based on categories of syndromes, or signs, experienced before the full onset of disease. It is increasingly being used by government agencies and health departments to identify disease outbreaks in a timely manner. Environmental exposures are known to induce respiratory and gastrointestinal symptoms, tend to have a seasonality component, and adversely affect the health of millions of people. OBJECTIVE: In this study, we assess the availability of environmental exposure data for air pollution (PM2.5, ozone, and NO2), pollen, and water contaminant exposure for use in a syndromic surveillance project. We also evaluate: 1) the general proximity of HMO populations to monitors, and 2) distribution of SES characteristics of the area populations with respect to monitor locations. METHODS: We collected exposure data, patient population data, and Census tract SES data for two metropolitan areas where Kaiser Permanente (KP) provides medical services: Atlanta, Georgia and the northern Virginia, District of Columbia (DC), and Baltimore area. Exposure data for air pollution and pollen were collected for 2013-2014. Straight-line distance from a monitor to the nearest KP clinic, and from each Census tract centroid, to the nearest air pollution or pollen monitor was computed using the Euclidean distance formula. RESULTS: Air pollution is routinely monitored by a Federal mandate, is universally available, and easily obtained. Pollen data is collected by private entities, which in some cases hinders access. Water quality data is generally publically available, but it is collected at the source and not easily traceable to water delivery endpoints. In both Atlanta and DC, Maryland, and Virginia most of the clinics (78% and 94%, respectively) are located within 10 miles of an air pollution monitor; approximately 83% and 94% of the KP populations were located within 10 miles of an air pollution monitor. SES populations differ substantially by race, age, income, and education with respect to the nearest monitor. However, the median and interquartile range of various air pollutants does not differ much across the monitors – indicating that, on average, there is little SES gradient in type of level of air pollution exposure. CONCLUSIONS: Overall, this study adds knowledge regarding future considerations about the coverage of environmental monitors and to what extent exposure measure estimates can be assigned to certain populations located near monitors.

Methamphetamin emissions from contaminated building materials

Over 110,000 known methamphetamine (meth) clandestine labs were reported in the U.S. from 1999 to 2008. The production of meth is regarded as a national epidemic, and can lead to substantial contamination of indoor materials. Due to its chemical and physical properties, meth residual can persist on indoor surfaces and in indoor air for prolonged periods. Unfortunately, most remediation techniques lack strong scientific support and cleanup standards are inconsistent across different states. A better understanding of the mechanistic interactions between meth and indoor environment can help improve remediation strategies and the development of regulations. In this study, equilibrium partitioning coefficients (Ke) between a meth surrogate and wall materials were estimated based on laboratory experiments. The resulting Ke values were then used in a screening model to predict meth decay rates from wall materials and indoor air. The effects of ventilation and indoor air mixing on meth off gassing were explored. / text

Characterizing the Natural Attenuation Potential of Chlorinated Ethenes Contaminated Sites

Site characterization methods for measuring the occurrence, magnitude, and rate of microbial mediated transformation processes were evaluated to assess the implementation of monitored natural attenuation (MNA) at chlorinated ethenes contaminated sites. A model site in Arizona, the Park-Euclid WQARF site in Tucson, was selected for the study. Field, geochemical, and compound specific carbon isotope fractionation (CSI) data confirm intrinsic biodegradation is occurring in the perched aquifer. Use of the BIOCHLOR model and a screening protocol support the potential for reductive dehalogenation found in the perched aquifer. Biotransformation of tetrachloroethene to cis-1,2-dichloroethene (cis-DCE) was achieved in microcosm studies. Transformation of cis-DCE to vinyl chloride and to ethene is, at the moment, the laboratory rate limiting step. PCR analysis established that the aquifer contains Dehalococcoides sp. and other dechlorinating microorganisms, though genes that encode for enzymes capable of achieving complete dehalogenation of the chlorinated contaminants were confirmed only in one monitoring well. The regional aquifer shows little evidence of intrinsic biodegradation. This study corroborates that CSI analysis can be used as an additional line of evidence to evaluate and verify MNA. Microbial analysis provides relevant information about the capabilities of native microbial communities to carry out reductive dehalogenation and thus, to naturally attenuate chlorinated compounds at a contaminated site. The combination of microcosm studies, CSI analysis, and bacterial DNA identification is becoming a convincing line of evidence for the assessment of MNA application to chloroethenes contaminated sites.

Determination of exposure of humans to selected mycotoxins with particular reference to aflatoxins.

Mycotoxins are poisonous secondary metabolites commonly produced by fungi and are involved in human disease conditions known as mycotoxicoses. There is evidence to show that food eaten by the rural Black population of Southern Africa is contaminated with mycotoxins. A tenuous relationship exists between the occurrence of mycotoxins in foods and certain disease conditions in humans. In order to verify this relationship, efforts have, in the past, been made to detect mycotoxins and their metabolites in physiological fluids and tissues. The difficulty with this approach is that mycotoxins in the body have short half lives, being rapidly excreted or metabolised to other forms. More recently it has been shown that aflatoxin B1, as its activated epoxide, can conjugate with macromolecules such as nucleic acids and proteins. These survive for much longer than the free toxins and by suitable methods can be isolated and measured. This allows for a much better estimate of exposure of the individual to aflatoxin. This study reviews and evaluates screening methods for the detection and analysis of mycotoxin contamination in rural foodstuffs such as maize and groundnuts. Methods for the production of aflatoxin-lysine and protein adducts are motivated and developed then used in the identification of naturally occurring adducts in humans. Life-History Trade-offs in Northern Leopard Frog (Lithobates [Rana] Pipiens) Tadpoles: Interactions of Trace Metals, Temperature, and Ranavirus

Emerging infectious diseases, pollution and climate change are associated with amphibian extinction events worldwide; however, direct causation is often obscured by the interactions of these stressors. Elucidating the possible synergies between metal contamination and disease is, therefore, critical in advancing our knowledge of the co-evolutionary mechanisms in host-pathogen systems and helping with the ability to better forecast the spread of diseases in metal-stressed environments. Additionally, increasing ecotoxicological research has improved our understanding of the complex influence trace metals may have on the physico-chemical nature of aquatic systems; however, the discrepancy in concentration-response within the toxicological literature makes it difficult to accurately define the range of toxicity, often due to the variability in media used in experimentation. The first chapter of this thesis reports an evaluation of copper, nickel and copper/nickel concentrations on several Northern Leopard Frog (Lithobates [Rana] pipiens) larvae life history traits within field collected smelting effluent water. Overall, results indicated that copper had a stronger negative impact on survival than nickel. However, tadpoles exposed to copper displayed increased growth and developmental patterns while those exposed to nickel demonstrated opposing life history traits. These results indicate that tadpoles are displaying different fitness strategies, in terms of survival and life history, in the presence of increased copper and/or nickel stress.

When seafood feeds the spirit yet poisons the body : developing health indicators for risk assessment in a Native American fishing community

Current US government risk assessment and management regulations and policies are based on a position that views risk as an objective measure of a predictable physiological morbidity or mortality outcome that is not otherwise connected to social or cultural beliefs and values. Whereas human health risk assessments are meant to determine the probability of adverse impacts from particular hazards, the conventional risk assessment framework fails to consider Native American definitions of health and so risk. This study was conducted with the Coast Salish Swinomish Indian Tribal Community of Washington State, where contamination of their aquatic natural resources has been found. By conducting two series of interviews with traditional high-use seafood consumers, experts and elders from the Swinomish Indian Tribal Community, and by averting use of what I describe herein as ‘conventional’ fish consumption survey, the study allowed interviewees to provide a more complex narrative set of details and information that bestowed a much more accurate picture of the reasoning behind seafood consumption habits within the community. Among the more salient points that emerged from the interviews was that seafood represents a symbolic, deeply meaningful food source that is linked to a multi-dimensional ‘Swinomish’ concept of health. Yet drastic changes in access, harvest and consumption have occurred over time, and continue to this day. A health evaluation tool was also devised using simple descriptive scaled rankings to elucidate non-physiological health risks and impacts in relation to contaminated seafood. Findings demonstrate that community cohesion, food security, ceremonial use and knowledge transmission all play primary roles as concerns the Swinomish notions of health, and that these indicators are regarded as equally important when juxtaposed to physical indicators of health. Thus, to eat less seafood—as prescribed by current policy and decision-making procedures when contamination is present—is actually detrimental to the multi-dimensional concept of health as defined by the Swinomish. The evaluation tool may be used in conjunction with the conventional risk assessment framework to more accurately and comprehensively deduce risks and impacts.

Novel methods for the separation and intervention of Salmonella typhimurium for food safety applications

This work begins with chemotaxis studies involving Salmonella typhimurium. Known chemical attractants (ribose, aspartic acid, etc.) and repellents (nickel chloride, sodium acetate, etc.) were tested to direct bacteria swimming patterns. It was found that high concentrations of both attractant and repellent, approximately 10% chemical in deionized (DI) water, yielded better separation results than lower concentrations, such as 1% and .1% chemical in DI water. Utilizing these attractants or repellents appropriately can allow live bacteria to be directed in a desired manner in a microfluidic device, while dead bacteria, which yield no response, can be separated into a waste reservoir. Another important aspect of bacteria separation is preconcentration, or the process of concentrating bacteria in a usable amount of liquid for further analysis in a microfluidic device. This study introduces a method of capturing Salmonella typhimurium through the use of magnetic nanoparticles (MNPs) without functionalizing them with antibody or amine coatings. Based on the work by Deng et al., MNPs were prepared in various ways to alter their diameter and surface characteristics to achieve optimal bacteria capture efficiency. A capture efficiency of approximately 94% has been achieved by altering chemical quantities in the MNP fabrication process. A macro-scale flow cell prototype was designed and characterized in order to ‘clean’ large volumes of buffer and separate the bacteria-MNP aggregates through the use of a magnetic field. Finally, intervention of bacteria is a significant topic in food safety applications. This study utilizes Fe (III) to inhibit bacteria growth. This chemical was used in the presence of Salmonella, E. coli, Staphylococcus, and Pseudomonas. Further experiments were conducted with raw chicken and lettuce contaminated with Salmonella typhimurium. Using as little as .005M Fe III in DI water, up to 5 orders of magnitude reduction in bacteria growth was seen on test plates as compared to control plates.

Tirtas pienas, mėsa, bulvės, grūdai ir geriamasis vanduo iš Šakių rajono. Buvo nustatomas 137Cs savitasis aktyvumas ir bendrieji alfa ir beta aktyvumai. Radiometriniai tyrimai atlikti radiacinės saugos centre. / We have summarized the data concerning accumulation of 137Cs in soil and the transition of the radionuclides to plants and animals. The presented survey gives some information about the distribution and accumuliation of 137Cs in different organs and tissues of animals about the possible methods of reducing the amount of radionuclides in the organism of animals and in products of animal origin. There was tested milk, meat, potatoes, grain and drinking water from Šakiai region. There was instituted the specific activity of 137Cs and general alpha and beta activities. The radiometric tests were performed in the Radiation Protection Centre.

Šiuo metu negalima atsakyti į klausimą, ar šalies gyventojų apšvita dėl 90Sr ir 137Cs maiste jonizuojančiosios spinduliuotės, įvertinta pagal atliktų maisto radiologinių tyrimų rezultatus, buvo ir yra tikrai tokia, kokia buvo nustatyta ar nustatoma, t. y. trūksta duomenų apie tyrimų rezultatų reprezentatyvumą. Aplinkosauginiu ir ekonominiu požiūriu įvertinti maisto radiologinio monitoringo optimalumą bei parengti jo keitimui rekomendacijas dėl taršos aplinkoje lygių ir maisto produktų importo kaitos tendencijų yra aktualu, vengiant nepagrįstai didelių radiologinių tyrimų apimčių. Optimalus mėginių kiekis ir tinkamos mėginių rūšys įgalintų išvengti nepagrįstai didelės apšvitos gyventojams, tinkamai įvertinus vidutines dozes dėl maiste esančių 90Sr ir 137Cs jonizuojančiosios spinduliuotės. Maisto radiologinio monitoringo optimizavimas svarbus ir dėl pastarųjų metų tendencijų atskirose valstybėse, taip pat ir Lietuvoje, planuojant plėtoti branduolinę energetiką. Įvertinus naujų technologijų su jonizuojančiosios spinduliuotės šaltiniais plėtrą atskirose socialinėse ekonominėse srityse yra būtina optimali radiologinio monitoringo programa, garantuojanti gyventojų apsaugą nuo jonizuojančiosios spinduliuotės. / Optimization methods of radiological monitoring of food for the measurements of 90Sr and 137Cs activity concentrations are not estimated. No determination is made in which extent the results of measurements of activity concentration in the samples reflect the amount of radionuclides in all food consumed. The estimation of the ways for the optimization of radiological monitoring of food and preparation of recommendations for the changes in the monitoring program according the levels of radioactivity in the environment and amount of food imported is relevant for the optimization of measurement program not to extend the program and to save allocations. Optimization of food radiological monitoring program should help for the protection of the public to avoid exposure due to 90Sr and 137Cs in food when the average annual dose estimation is made correctly. Optimization of food radiological monitoring is important in nowadays when the planning of development of nuclear energy is going on in different countries including Lithuania. To take into account the development of new technologies using sources of ionizing radiation the optimization of food radiological monitoring program should assure the radiation protection of public.

Assesing biological recovery from acidification and metal contamination in urban lakes from Sudbury, Canada : a paleolimological approach

The acidification and metal contamination of freshwater resources are major environmental concerns in many areas, with Sudbury (Ontario, Canada) having been amongst the most severely impacted. Many scientific investigations have examined the effects of these environmental stressors on aquatic systems, but relatively little is known about the biological recovery process following smelter emission reductions. Therefore, paleolimnological techniques were utilized to determine if diatom and scaled chrysophyte assemblages have recovered toward their pre-disturbance conditions as a result of reduced anthropogenic inputs. Pre-industrial algal assemblages were primarily dominated by circumneutral to alkaline and pH-indifferent taxa. However, there was a shift toward acid-tolerant species in all study lakes with the onset of open pit roasting and smelting operations. Coinciding with emission reductions, scaled chrysophyte assemblages in two of the three study sites have shown evidence of biological recovery. Given the population growth of the city of Sudbury over the last century, and the lack of scientific information regarding cultural eutrophication trends in the region, paleolimnological techniques were also used to track long-term biological changes within diatom assemblages related to cultural disturbances. Historically, oligotrophic diatom taxa primarily dominated the algal assemblages in each of the four study lakes. With the onset of urban environmental stressors there was a shift toward taxa which thrive in more productive systems. In addition, diatom assemblages appear to track increased lakewater pH through time. Finally, geochemical analysis tracked the increase in copper and nickel concentrations in lake sediment with the onset of open pit roasting and smelting activities and the subsequent decline in concentration with emission controls. Metal concentrations in recently deposited lake sediments remain elevated compared to pre-industrial concentrations. Paleolimnological studies comparing pre- and post-disturbance algal assemblages are of interest to lake managers as these data will aid in setting realistic mitigation targets for freshwater systems impacted by acidification, cultural eutrophication, and metal contamination, and will help gauge biological recovery mechanisms.
