Magnetnorezonantna dijagnostika akutnog pankreatitisa / Magnetoresonant diagnosis of pancreatitis acuta

<p>Akutni pankreatitis predstavlja zbirni pojam dinamičkih, lokalnih i sistemskih patofiziolo&scaron;kih procesa nastalih iznenadnim prodorom aktivnih litičkih pankreasnih enzima u žlezdani parenhim. Cilj istraživanja je da se Utvrditi senzitivnost difuzione sekvence magnetne rezonance (DWI) radi utvrđivanja morfolo&scaron;kih promena parenhima kod akutnog pankreatitisa. Poređenje difuzione mape i difuzionog koeficijenta kod pacijenata sa akutnim pankreatitisom i kod pacijenata sa morfolo&scaron;ki urednim parenhimom pankreasa na magnetnoj rezonanci. Utvrditi da li postoje statistički značajne razlike difuzionog koeficijenta kod pacijenata sa akutnim pankreatitisom u odnosu na pol. Utvrditi da li postoje statistički značajne razlike difuzionog koeficijenta kod pacijenata sa akutnim pankreatitisom u odnosu na godine. Odrediti prelomnu tačku difuzionog koeficijenta kod pacijenata sa akutnim pankreatitisom. Studija je bila prospektivnog karaktera i obuhvatilo je 30 ispitanika sa morfolo&scaron;ki urednim parenhimom pankreasa i 30 sa dijagnozom akutnog pankreatitisa unutar 72 sata od početka simptoma. Svi pacijenti su pregledani magnetnom rezonancom u Centru za radiologiju, Kliničkog Centra Vojvodine. Rezultati ukazuju da postoje razlike difuzionog koeficijenta kod pacijenata sa akutnim pankreatitisom i kontrolne grupe. Takođe smo dokazali da difuzioni koeficijent zavisi od pola i starosti i utvrdili smo prelomnu tačku difuzije za rano dijagnostikovanje akutnog pankreatitisa.</p> / <p>Acute pancreatitis is defined as cumulative term of dynamic local and general pathophysiological processes caused by sudden penetration of active lithic pancreatic enzymes in the glandular parenchyma. Goal of this research is to note the changes (sensitivity) in values of diffusion weighted images (DWI) in acute pancreatitis and to determine morphological changes in glandular parenchyma of pancreas. Comparation of DWI between patients with acute pancreatitis and patients with normal pancreatic parenchyma based on magnetic resonance (MRI). We also want to determine whether there were statistically significant differences of DWI in patients with acute pancreatitis in relation to sex and age. One of our goals also was to determine breakpoint of DWI as a sure sign of acute pancreatitis. This was prospective study and included 30 patients with morphologically healthy parenchyma of the pancreas (control group) and 30 with the diagnosis of acute pancreatitis &ndash; in first 72 hours of the onset of symptoms. All patients were examined on MRI in department of Radiology of Clinical Center of Vojvodina. Our results indicate that was a big difference of DWI between patients with acute pancreatitis and control group. We prove that DWI depends on the sex and age. 1,77x10-6mm/s2 was breakpoint which indicates acute pancreatitis.</p>

Estudo dos processos linfoproliferativos cutâneos de células B objetivando demonstrar a difícil distinção diagnóstica entre os linfomas cutâneos e pseudolinfomas. Foram avaliados 38 casos de processos linfoproliferativos cutâneos de células B. Foi realizada revisão de prontuários médicos e exames anátomo-patológicos. Foram estudados 25 casos de linfomas cutâneos, 7 de pseudolinfomas e 6 casos onde não foi possível a distinção diagnóstica entre as entidades em estudo. As características clínicas, histológicas e imunoistoquímicas foram descritas para cada grupo. A análise estatística foi realizada demonstrando a similaridade entre os linfomas cutâneos e os pseudolinfomas / Study of cutaneous B-cell lymphoproliferative process with the purpose of demonstrating the difficulty of distinguishing between cutaneous lymphomas and pseudolymphomas. 38 cases of cutaneous B-cell lymphoproliferative processes were evaluated. A review of medical records and histophatologic material was performed. The study comprised 25 cases of cutaneous lymphomas, 7 cases of pseudolymphomas and 6 cases where a diagnosis distinguishing between the entities in study was not possible. It described clinical, histophatologic and immunohistochemical characteristics of each group. A statistical analysis showing the similarity between lymphomas and pseudolymphomas was performed

Análise morfométrica das fibras colágenas e reticulínicas na extrofia vesical / Morphometric analysis of collagen and reticulin fibers in classical bladder exstrophy

Trabalho prospectivo estudando a matriz extracelular da parede vesical em pacientes com extrofia vesical comparados ao grupo controle, pela microscopia óptica comum e luz polarizada com morfometria, quantificando-se as fibras colágenas e reticulínicas. Estudou-se 17 pacientes de ambos os sexos, biopsiando-se toda a parede vesical e empregando colorações HE, PS e Reticulina. Diferenças estatisticamente significantes foram notadas na análise quantitativa de fibras colágenas e reticulínicas o número de fibras colágenas foi significativamente maior e o número de fibras reticulínicas foi menor no grupo de pacientes com extrofia vesical quando comparado ao grupo controle. / A prospective study was done to evaluate the detrusor muscle\'s extracellular matrix in classical bladder exstrophy in comparison to a control group, by use of light microscopy and polarization method with morphometry, to quantify collagen and reticular fibers. Seventeen patients from both sexes were analysed and samples were obtained from the bladder and stained with Haematoxylin-eosin, Picrosirius red and the silver impregnation method. There were significant differences when comparing the quantity of collagen and reticular fibers. The collagen fibers were more abundant in the exstrophy bladders compared to controls, while the reticular fibers were present in smaller amounts.

"Estudo comparativo da função erétil em pacientes portadores da forma digestiva da Doença de Chagas" / Study comparative of the erectile function in patient bearers in the digestive way of the Chagas' disease

O objetivo deste estudo foi correlacionar as alterações causadas pela doença de Chagas no sistema nervoso autônomo e a possibilidade destas alterações provocarem disfunção erétil. Foram incluídos 60 pacientes, do sexo masculino, entre 40 e 70 anos, sendo 30 com a forma digestiva da doença de Chagas e 30 como grupo controle negativos para tripanossomíase; foram utilizados o questionário auto-aplicável do Índice Internacional de Função Erétil (IIFE) e o algoritmo de pontuação para análise e interpretação do questionário.Os resultados obtidos neste estudo comprovaram que a doença de Chagas não afetou a função erétil dos pacientes, que não há diferença significativa se a lesão é do esôfago ou de esôfago e cólon e que a doença de Chagas não deve ser considerada um fator de risco para a disfunção erétil / The objective of this study was to correlate the alterations caused by the Chagas'disease in the autonomous nervous system and the possibility of these alterations they provoke erectile dysfunction. Sixty men - between 40 and 70 years-old - were selected for the stydy: 30 men with the digestive form of the Chagas'disease and 30 of them had negative serologic findings for T. Cruzi. It was used the questionnaire of the International Index of Erectile Function (IIEF) and the punctuation algorithm for interpretation of the questionnaire. The results this study they proved that the Chagas'disease didn't affect the patients' erectile function, that there is not significant difference if the lesion is of esophagus or esophagus and colon and Chagas'disease should not be considered a risk factor for the erectile dysfunction

Composite Bioinks With Mesoporous Bioactive Glasses - A Critical Evaluation of Results Obtained by In Vitro Experiments

Besides osteoconductivity and a high degradation rate, mesoporous bioactive glasses (MBGs) are specific for their highly ordered channel structure and high specific surface area, making them suitable as drug and/or growth factor delivery systems. On the other hand, the mesoporous channel structure and MBG composition can have an effect on common cell evaluation assays, leading to inconclusive results. This effect is especially important when MBG is mixed in composite bioinks, together with cells. Additionally, the hydrogel component of the ink can influence the degradation of MBG, leading to different ion releases, which can additionally affect the analyses. Hence, our aim here was to show how the MBG structure and composition influence common cell viability and differentiation assays when calcium (Ca)- or magnesium (Mg)-containing glass is part of an alginate-based composite bioink. We suggested pre-labeling of cells with DiI prior to bioprinting and staining with calcein-AM to allow identification of metabolically active cells expressing signals in both green and red channels, allowing the use of fluorescence imaging for cell viability evaluations in the presence of high amounts (7 wt %) of MBGs. The release and uptake of ions during degradation of CaMBG and MgMBG were significantly changed by alginate in the composite bioinks, as confirmed by higher release and uptake from bulk glasses. Additionally, we detected a burst release of Mg²⁺ from composites only after 24 h of incubation. Furthermore, we demonstrated that released ions and the mesoporous channel structure affect the measurement of lactate dehydrogenase (LDH) and alkaline phosphatase activity (ALP) in bioprinted composite scaffolds. Measured LDH activity was significantly decreased in the presence of CaMBG. On the other hand, the presence of MgMBG induced increased signal measured for the ALP. Taken together, our findings show how composite bioinks containing MBGs can interfere with common analyses, obtaining misleading results.

Victims and survivors: stable isotopes used to identify migrants from the Great Irish Famine to 19th century London

No / Historical evidence documents mass migration from Ireland to London during the period of the Great Irish Famine of 1845-52. The rural Irish were reliant on a restricted diet based on potatoes but maize, a C(4) plant, was imported from the United States of America in 1846-47 to mitigate against Famine. In London, Irish migrants joined a population with a more varied diet. To investigate and characterize their diet, carbon and nitrogen isotope ratios were obtained from bone collagen of 119 and hair keratin of six individuals from Lukin Street cemetery, Tower Hamlets (1843-54), and bone collagen of 20 individuals from the cemetery at Kilkenny Union Workhouse in Ireland (1847-51). A comparison of the results with other contemporaneous English populations suggests that Londoners may have elevated delta(15) N compared with their contemporaries in other cities. In comparison, the Irish group have lower delta(15) N. Hair analysis combined with bone collagen allows the reconstruction of perimortem dietary changes. Three children aged 5-15 years from Kilkenny have bone collagen delta(13) C values that indicate consumption of maize (C(4)). As maize was only imported into Ireland in quantity from late 1846 and 1847, these results demonstrate relatively rapid bone collagen turnover in children and highlight the importance of age-related bone turnover rates, and the impact the age of the individual can have on studies of short-term dietary change or recent migration. Stable light isotope data in this study are consistent with the epigraphic and documentary evidence for the presence of migrants within the London cemetery.

Interprétation automatique de données hétérogènes pour la modélisation de situations collaboratives : application à la gestion de crise / Automatic interpretation of heterogeneous data to model collaborative situations : application to crisis management

Les travaux présentés dans ce manuscrit s’appliquent au domaine de la gestion de crise française, et notamment à la phase de réponse qui suit un évènement majeur, comme une crue ou un accident industriel. Suite à l’évènement, des cellules de crise sont activées pour prévenir et traiter les conséquences de la crise. Elles font face, dans l’urgence, à de nombreuses difficultés. Les parties-prenantes sont nombreuses, autonomes et hétérogènes, la coexistence de plans d’urgence engendre des contradictions et des effets en cascade se nourrissent des interconnexions entre réseaux. Ces constats arrivent alors que les données disponibles sur les réseaux informatiques ne cessent de se multiplier. Elles sont, par exemple, émises par des capteurs de mesures, sur des réseaux sociaux, ou par des bénévoles. Ces données sont l’occasion de concevoir un système d’information capable de les collecter pour les interpréter en un ensemble d’information formalisé, utilisable en cellule de crise. Pour réussir, les défis liés aux 4Vs du Big data doivent être relevés en limitant le Volume, unifiant (la Variété) et améliorant la Véracité des données et des informations manipulées, tout en suivant la dynamique (Vélocité) de la crise en cours. Nos états de l’art sur les différentes parties de l’architecture recherchée nous ont permis de définir un tel système d’information. Ce dernier est aujourd’hui capable de (i) recevoir plusieurs types d’évènements émis de sources de données connues ou inconnues, (ii) d’utiliser des règles d’interprétations directement déduites de règles métiers réelles et (iii) de formaliser l’ensemble des informations utiles aux parties-prenantes. Son architecture fait partie des architectures orientées évènements, et coexiste avec l’architecture orientée services du logiciel développé par le laboratoire Centre de Génie Industriel (CGI). Le système d’information ainsi implémenté a pu être éprouvé sur un scénario de crue majeure en Loire Moyenne, élaboré par deux Services de Prévision des Crues (SPC) français. Le modèle décrivant la situation de crise courante, obtenu par le système d’information proposé, peut être utilisé pour (i) déduire un processus de réponse à la crise, (ii) détecter des imprévus ou (iii) mettre à jour une représentation de la situation en cellule de crise. / The present work is applied to the field of French crisis management, and specifically to the crisis response phase which follows a major event, like a flood or an industrial accident. In the aftermath of the event, crisis cells are activated to prevent and deal with the consequences of the crisis. They face, in a hurry, many difficulties. The stakeholders are numerous, autonomous and heterogeneous, the coexistence of contingency plans favours contradictions and the interconnections of networks promotes cascading effects. These observations arise as the volume of data available continues to grow. They come, for example, from sensors, social media or volunteers on the crisis theatre. It is an occasion to design an information system able to collect the available data to interpret them and obtain information suited to the crisis cells. To succeed, it will have to manage the 4Vs of Big Data: the Volume, the Variety and Veracity of data and information, while following the dynamic (velocity) of the current crisis. Our literature review on the different parts of this architecture enables us to define such an information system able to (i) receive different types of events emitted from data sources both known and unknown, (ii) to use interpretation rules directly deduced from official business rules and (iii) to structure the information that will be used by the stake-holders. Its architecture is event-driven and coexists with the service oriented architecture of the software developed by the CGI laboratory. The implemented system has been tested on the scenario of a 1/100 per year flood elaborated by two French forecasting centres. The model describing the current crisis situation, deduced by the proposed information system, can be used to (i) deduce a crisis response process, (ii) to detect unexpected situations, and (iii) to update a COP suited to the decision-makers.
