Diet during pregnancy:dietary patterns and weight gain rate among Finnish pregnant women

Abstract Proper nutrition and optimal weight status in pregnancy are important for both the mother and her child. The present study was aimed at assessment of maternal food and nutrient intake, dietary supplement use, dietary patterns, and weight during pregnancy. Additionally, associations between maternal weight, socio-demographic and perinatal factors and advanced beta cell autoimmunity in the offspring were examined. The results from a one-year cohort of mothers entering the ongoing Type 1 Diabetes Prediction and Prevention (DIPP) study in 1998–99 (n =  797) suggested that healthy food choices were positively correlated with maternal age and education. Dietary supplements were used by 85% of the women. However, the intake of vitamin D did not meet the recommendation and folic acid intake was inadequate in 44% of the pregnant women when both food and supplementation intakes were taken into account. Seven dietary patterns were identified in 3730 pregnant women who entered the DIPP study between 1997 and 2002. The ‘healthy’, the ‘low-fat foods’ and the ‘alcohol and butter’ dietary patterns were positively associated with maternal age and education. The ‘fast food’ dietary pattern was positively associated and the ‘alcohol and butter’ pattern was inversely associated with the rate of maternal weight gain during pregnancy. Altogether, 4093 children and their mothers comprised the study population in which the relationships between maternal initial body mass index, weight gain rate, and the development of beta cell autoimmunity in the offspring were examined. Maternal weight status during pregnancy was not related to the risk of advanced beta cell autoimmunity. A higher level of maternal education was significantly associated with a decreased risk of advanced beta cell autoimmunity in children. More attention should be paid to nutritional guidance among Finnish pregnant women, especially as regards young and less well educated women. Dietary patterns may be useful for risk group identification and they may offer a framework for further research concerning diet and health outcomes among mothers and their children.

Consumo de suplementos con multimicronutrientes Chispitas® y anemia en niños de 6 a 35 meses: estudio transversal en el contexto de una intervención poblacional en Apurímac, Perú.

Objetivos. Evaluar la implementación del programa de suplementación universal con multimicronutrientes “Chispitas®” en la región Apurímac a través de la cantidad y calidad de sobres consumidos y su relación con la anemia, en niños de 6 a 35 meses. Materiales y métodos. Se realizó un estudio transversal usando un muestreo multietápico en el 2010. Se consideró como anemia a los valores de hemoglobina ajustados por altitud menores de 110 g/L. El consumo de multimicronutrientes se categorizó en: menor de 30; de 30 a 59, y 60 o más sobres. La calidad del consumo fue adecuada cuando la madre refería que el niño consumía toda la comida con el suplemento. Se calculó las razones de prevalencia (RPa) ajustadas por regresión de Poisson. Resultados. Se incluyó 714 participantes, 25,3% vivía en hogares pobres y 59,2% en extrema pobreza; 52,6% residía a más de 3000 m de altitud. La prevalencia de anemia fue de 51,3% (IC95%: 47,1-55,4%), 5,4% no recibió la intervención; 60,3% consumió 60 o más sobres y 49,0% los consumió en forma adecuada. No se encontró asociación entre la cantidad de sobres recibidos o consumidos y la anemia (p<0,05). Aquellos niños que consumieron el suplemento en forma adecuada tuvieron menor prevalencia de anemia que aquellos que no lo hicieron (RPa: 0,81; IC95%: 0,68-0,96) Conclusiones. No basta con entregar o consumir la cantidad necesaria de los multimicronutrientes, sino asegurar que el proceso de consumo sea adecuado para lograr una reducción de la prevalencia de anemia, aspecto que debe ser trabajado para mejorar esta intervención. / Objectives. To assess the implementation of the Universal “Chispitas®” Multimicronutrient Supplement Program in Apurimac by determining the quantity and quality of sachets consumed and their connection with anemia, in 6 – 35-month-old children. Materials and methods. A crosscut study was conducted by using a multistage sample in 2010. Anemia was considered to be related to altitude-adjusted hemoglobin values below 110 g/L. The consumption of multimicronutrients was classified as follows: less than 30; 30 to 59, and 60 or more sachets. The quality of consumption was considered to be adequate when the mother indicated that the child ate all his food with the supplement. The rationale for prevalence (RPa) adjusted by Poisson regression was calculated. Results. 714 participants were included, 25.3% of which lived in poor houses and 59.2% in extreme poverty; 52.6% lived at over 3000 m of altitude. The prevalence of anemia was 51.3% (CI95%: 47.1-55.4%), 5.4% did not receive intervention; 60.3% consumed 60 or more sachets and 49.0% consumed them adequately. No association between the number of sachets received or consumed and anemia (p<0.05) was found. Children who consumed the supplement adequately showed lower prevalence of anemia than those who did not (RPa: 0.81; CI95%: 0.68-0.96) Conclusions. In order to reduce the prevalence of anemia, attention should not only focus on giving or consuming the necessary quantity of multimicronutrients, but also on ensuring that the consumption process is adequate, and work needs to be done in this area in order to improve this intervention.

The efficacy of TranQuin® Day Formula supplement on psychological stress in university students

(Homoeopathy) / Psychological stress refers to an individual’s interaction with what he perceives as adverse or threatening phenomena of the external environment (stimulus) and the ensuing physiological response that occurs within the body. Although the stimulus itself may not be harmful, the physiological reaction of the individual to the perceived threat may lead to health consequences. University students may experience greater levels of stress than the average population. They may also experience symptoms of anxiety, irritability, sleep disturbances and impaired memory due to psychological stress. These symptoms may be exacerbated by concomitant use of alcohol and stimulants such as nicotine and caffeine, which students may use as coping mechanisms. Conventional treatment for stress might include anti-depressants and anxiolytics that often have adverse effects. TranQuin® Day Formula is a combination vitamin and herbal supplement formulated to assist the body to cope with stress. Although each individual vitamin and herbal constituent of TranQuin® Day Formula has been thoroughly researched, to date, no research has been conducted on the efficacy of TranQuin® Day Formula dietary supplement for the treatment of psychological stress in university students. The aim of this study was to determine the efficacy of TranQuin® Day Formula supplement on psychological stress in university students, with the use of the Cohen’s Perceived Stress Scale-10 (PSS-10) and Goldberg’s General Health Questionnaire-28 (GHQ-28). Thirty participants, both male and female, between the ages of 18 and 49 years, who obtained a minimum score of 10 on the Cohen’s Perceived Stress Scale-10, were selected to participate in this six week, double-blind, placebo-controlled study. Participants were also requested to complete Goldberg’s General Health Questionnaire-28. The scores obtained by the participants on both stress scales were measured at the beginning of the study (week 0) to obtain a baseline score, in the middle of the study (week 3) and at the end of the study (week 6). The participants were randomly divided into the control and experimental groups. Participants were asked to take two capsules of the supplement or placebo, preferably in the morning after breakfast, or the first meal of the day, for the duration of the study period (6 weeks). Each participant received a daily data sheet which recorded capsules taken and any symptoms experienced, as well as any other medication taken. The results of the study were statistically analysed using the Mann-Whitney-U Test, the Shapiro-Wilk Test, the Wilcoxon Signed Ranks Test, Friedman Test and descriptive statistics.

A composição lipídica das lipoproteínas reflete o conteúdo de ácido graxo (AG) da dieta. A influência dos AG na lipemia e aterogênese varia conforme o grau de saturação, de oxidação e suas proporções na dieta. Vários estudos reportam os efeitos dos ácidos graxos saturados (AGS), monoinsaturados (AGM) e poliinsaturados (AGP) sobre a concentração das lipoproteínas plasmáticas. No entanto, são pouco explorados os efeitos dessa suplementação sobre as propriedades funcionais da lipoproteína de alta densidade (HDL). Desse modo, no presente estudo foi avaliada a transferência de fosfolípides (FL), colesterol livre (CL), ésteres de colesterol (EC) e triglicérides (TG) de uma nanoemulsão lipídica artificial para a HDL, bem como sua atividade antioxidante e a sua composição em ácidos graxos. Além disso, foram avaliadas as atividades das proteínas de transferência de ésteres de colesterol (CETP) e de fosfolípides (PLTP). Foram estudados 84 hamsters Golden Syrian, machos adultos, divididos em 3 grupos os quais receberam suplementação dietética de óleos ricos em AGS (n=28), AGM (n=29) ou AGP (n=27), por gavagem, durante 30 dias. Amostras de sangue foram coletadas, após jejum de 12 h, para determinação do perfil lipídico, atividade da CETP, da PLTP, da paroxonase 1 (PON1) e a composição em ácidos graxos do plasma total e da fração HDL por cromatografia gasosa, bem como a transferência de lípides da nanoemulsão lipídica artificial para a HDL. O método de transferência é baseado na troca de lípides ocorrida entre uma nanoemulsão lipídica artificial, semelhante à estrutura lipídica da LDL, marcada radioativamente com 14C-CL e 3H-TG ou 14C-FL e 3H-EC, usada como doadora de lípides. Após precipitação química da nanoemulsão e das demais lipoproteínas, a capacidade da HDL de receber lípides foi quantificada pela medida da radioatividade presente na lipoproteína. As concentrações de colesterol total, EC, TG e ácido graxo oléico foram menores no grupo AGP em relação aos grupos AGS e AGM (p=0,002, p=0,010, p=0,001, p<0,05, respectivamente). A atividade da PON1, a atividade da PLTP assim como a composição em ácidos graxos da HDL, não diferiu entre os grupos estudados. Por outro lado, a atividade da CETP foi maior no grupo AGM quando comparado aos grupos AGS e AGP (p<0,05). O grupo AGP apresentou maior transferência de EC e menor transferência de FL em comparação aos grupos AGS e AGM (p<0,05). A suplementação dietética de ácidos graxos é capaz de influenciar a funcionalidade da partícula HDL. Dessa forma, o presente estudo fornece subsídios para melhor compreensão do efeito da dieta nos mecanismos relacionados ao metabolismo lipídico nos hamsters / The lipid composition of the lipoproteins reflects the fatty acid (FA) content of the diet . The degree of saturation and oxidation as the dietary content of the FA differently affects the lipemia and the atherosclerosis development. Numerous studies have evaluated the effects of the saturated fatty acids (SFA), monounsaturated fatty acids (MUFA) and polyunsaturated fatty acids (PUFA) on the plasma lipoprotein concentrations. However, few studies explored the effects of the dietary supplementation of these FA on the functional properties of the high-density lipoprotein (HDL). Therefore, in this study, the transfer of phospholipids (PL), free cholesterol (FC), cholesteryl esters (CE) and triglycerides (TG) from an artificial lipidic nanoemulsion to HDL, as its antioxidant activity and total fatty acids composition were evaluated. Also, the cholesteryl ester transfer protein (CETP) and the phospholipid transfer protein (PLTP) activities were determined. We studied 84 adult male Golden Syrian hamsters divided into 3 groups that received dietary supplementation of 0.75 ml of SFA (n=28), MUFA (n=29) or PUFA (n=27) rich oils by gavage for thirty days. Blood samples were collected after 12 h fasting for determination of plasma lipids, CEPT, PLTP and paraoxonase-1 (PON-1) activities, the fatty acids composition of the total plasma and HDL fraction by gas-liquid chromatography and transfer of lipids from the artificial lipidic nanoemulsion to HDL. The later method is based on the lipid transfer between an artificial lipidic nanoemulsion labeled with 3H-TG and 14C-FC or 3H-CE and 14C-PL, structurally similar to the low-density lipoprotein (LDL), used as a radioactive lipid donator. After artificial nanoemulsion and other lipoprotein precipitation, the capacity of HDL to receive lipids was quantified by measuring the radioactivity present in the lipoprotein. Serum total cholesterol, CE, TG and oleic fatty acids concentrations were smaller in the PUFA group than in the SFA and MUFA groups (p=0.002, p=0.010, p=0.001 and p<0,05 respectively). The PON-1, the PLTP activities and fatty acids composition of the HDL fraction were similar between the groups. On the other hand, the CETP activity was greater in the MUFA group than in the SFA and PUFA groups (p<0.05). The PUFA group showed greater transfer of CE and smaller transfer of PL from the nanoemulsion to HDL compared with SFA and MUFA groups (p<0.05). The dietary supplementation of different FA can lead to alterations of the functionality of the HDL particles. Therefore, the present study contributes to a better understanding of the effects of different fatty acid -rich diets on the mechanisms involved in the lipid metabolism in hamsters

Prise de complément en vitamine D ou C et risque de cancer du sein dans la cohorte E3N / Vitamin D or vitamin C supplement use and breast cancer risk in the E3N cohort

Les compléments alimentaires sont de plus en plus consommés en France depuis le début des années 2000. Cependant, l’effet de la prise de compléments alimentaires sur la santé, notamment à long terme, n’est pas clairement établi. Les compléments en vitamine D et en vitamine C font partie des compléments les plus consommés en France, et à des doses généralement beaucoup plus élevées que celles apportées par l’alimentation. Plusieurs études expérimentales suggèrent que la vitamine D et la vitamine C pourraient avoir une influence sur le risque de cancer du sein, le cancer actuellement le plus fréquent chez la femme dans le monde. Cependant, les études épidémiologiques disponibles ne permettent pas de conclure sur cette relation. L’objectif de cette thèse était d’étudier la relation entre prise de complément en vitamine D ou vitamine C et risque de cancer du sein dans la cohorte E3N à partir de données répétées sur la prise de complément alimentaire, et d’étudier si cette relation pourrait être influencée par l’ancienneté de la prise de complément alimentaire et par de potentielles interactions avec certains facteurs spécifiques liés au mode de vie. La prise de compléments alimentaires a été estimée à partir de plus de 55 000 femmes ménopausées de la cohorte E3N suivies entre 1995 et 2008. Nous avons observé que le prise de complément en vitamine D pourrait réduire l’augmentation de risque de cancer du sein associée à la prise de traitement hormonal de la ménopause, mais pourrait être délétère chez les femmes n’ayant jamais utilisé de traitement hormonal de la ménopause et n’étant pas en surpoids, et donc étant peu exposées aux hormones. Concernant la prise de complément en vitamine C, nous avons observé une augmentation de risque chez les femmes ayant déjà des apports alimentaires en vitamine C élevés. Nos résultats ont un impact potentiel en termes de santé publique, mais devront d’abord être confirmés par des études ayant des données répétées et quantitatives sur la prise de compléments alimentaires. Cependant, ces résultats appellent à la prudence quant à l’utilisation de compléments alimentaires tels que la vitamine C en l’absence de déficience nutritionnelle avérée. / The use of dietary supplements has been steadily increasing since the early 2000s. However, little is known on the effect, especially the long-term effect, of dietary supplement use on health. Single vitamin D and vitamin C supplements are widely consumed in France, at doses that generally largely exceed dietary intakes. Experimental studies have suggested potential effects of vitamin D and vitamin C on breast cancer risk, the currently most common cancer in women worldwide. However, data from epidemiologic studies are still inconclusive. The objective of this project was to investigate the relationship between vitamin D or vitamin C supplement use and breast cancer risk in the E3N cohort using regularly updated data on dietary supplement use, and to investigate a potential effect modification by recency of dietary supplement use and by other specific lifestyle factors. The use of dietary supplements has been estimated using data collected from 55 000 postmenopausal women of the E3N cohort followed between 1995 and 2008. We observed that vitamin D supplement use could reduce the increased risk of breast cancer associated with menopausal hormone therapy use, but could be associated with an increased breast cancer risk in women with low hormonal exposure, i.e. women who never used menopausal hormone therapy and are not overweight. Concerning vitamin C supplement use, we observed an increased risk of breast cancer in women who already had high dietary vitamin C intakes. Our results have a potential public health impact, but must be first confirmed by further studies with repeated and quantitative data on dietary supplement use. However, caution can already be advocated towards using dietary supplements such as vitamin C in the absence of clear deficiency in these nutrients.

Effect of supplementing diets with antimicrobials and effective microorganisms on productivity and meat quality of Ross 308 Broiler Chickens

(Animal Production)) -- University of Limpopo, 2016 / This study was conducted to determine the effect of supplementing diets with antimicrobials and effective microorganisms on productivity and carcass characteristics of Ross 308 broiler chickens. The study consisted of two parts. The first part determined the effect of antimicrobial and effective microorganism (EM) supplementations on growth performance of unsexed Ross 308 broiler chickens aged one to 21 days. A complete randomized design was used and 150 unsexed day-old chicks with an initial weight of 42 ± 2g were randomly assigned to five different treatments which were replicated 3 times with each replicate having 10 chicks. The five grower diets had the same nutrients (20% CP and 12MJ/kg) but different supplementation levels of 0g oxytetracycline and 0ml EMs/l of water (UAM0EM0), 0.01g oxytetracycline (UAM0.01EM0), 30ml EMs/l of water (UAM0EM30), 50ml EMs/l of water (UAM0EM50) and 100ml EMs/l of water (UAM0EM100). A quadratic regression model was used to determine dietary effective microorganism supplementation levels for optimal feed intake and live weight of Ross 308 broiler chickens. A linear model was used to determine the relationship between dietary effective microorganism supplementation levels and metabolisable energy intakes. Antimicrobial and effective microorganism supplementations did not have any effect (P>0.05) on growth rate, feed conversion ratio and mortality. Antimicrobial supplementation improved (P<0.05) feed intake and live weight of the chickens. Supplementation with 50ml of EMs/l of water improved (P<0.05) feed intake. Supplementation with 50 or 100ml of EMs per litre of water increased (P<0.05) ME intake of the chickens. Effective microorganism supplementation levels of 72.25 and 48.29ml of drinking water optimized feed intake and live weight, respectively. The second part of the experiment determined the effect of antimicrobials and effective microorganisms on productivity, blood, carcass characteristics and meat quality of male Ross 308 broiler chickens aged 22 to 42 days. The chickens were randomly allocated to five treatments with three replications, each having six chickens. A total of 90 male chickens, with the initial live weight of 452 ± 3g were allocated to the treatments in a complete randomized design. The chickens were fed a grower diet supplemented with 0g oxytetracycline and 0ml EMs/l of water (MAM0EM0), 0.01g oxytetracycline (MAM0.01EM0), 30ml EMs/l of water (MAM0EM30), v 50ml EMs/l of water (MAM0EM50) and 100ml EMs/l of water (MAM0EM100). Antimicrobial and effective microorganism supplementation did not have effect (P>0.05) on feed intake, growth rate, live weight, ME intake, blood glucose and mortality. Poorer (P<0.05) feed conversion ratio was observed with the supplementation of antimicrobial. Blood glucose levels were optimized at an effective microorganism supplementation level of 29.00ml of EM/l of drinking water (Figure 4.05). Supplementing diets with antimicrobials and effective microorganisms did not have effect (P>0.05) on crop, gizzard, proventriculus and large intestine pH values of male chickens. However, supplementation with of 100ml of EMs per litre of drinking water reduced (P<.0.05) the pH of ileum. Effective microorganism supplementation level of 85.00ml per litre of drinking water optimized the crop pH value. Antimicrobial and effective microorganism supplementations did not have influence (P>0.05) on gizzard, proventriculus, small intestine, caecum, large intestine, liver and heart weights of male chickens at 42 days. Effective microorganism supplementation level of 50ml per litre of drinking water reduced (P<.0.05) crop weight. Antimicrobial and effective microorganism supplementations did not have effect (P>0.05) on whole gastro-intestinal tract (GIT), small intestine and caecum length of the chickens. Antimicrobial supplementation reduced (P<0.05) the length of large intestine. Effective microorganism supplementation levels of 41.00, 45.50 and 85.00ml per litre of drinking water optimized crop weights and caecum and large intestine lengths, respectively. Antimicrobial and effective microorganism supplementations did not have any influence (P>0.05) on live weight, carcass weight, breast weight, drumstick weight ad thigh weight. Similarly, antimicrobial and effective microorganism supplementations did not have influence on meat tenderness, juiciness and flavour. There were no antibiotic and effective microbe residues in the meat. It is, therefore, concluded that effective microorganism supplementation did not have much effect on production parameters, carcass characteristics and meat quality of Ross 308 broiler chickens.

A dietary supplement is a product that contains nutrients (e.g. vitamins, minerals, herbs, amino acids or fatty acids) intended to supplement the diet. The aim of the diploma thesis was to suggest a prescription for preparation of three types of dietary supplements which are used to reduce of climacteric symptoms, their production and determination of energetic (nutrition) value on account of detected content of proteins, fats and saccharides in dietary supplements. A part of the diploma thesis was to suggest suitable wrapping and price of prepared dietary supplements. The three dietary supplements were used for analyses, namely capsules, herb syrup and herb drops. The content of proteins was determined by Kjeldahl´s method, total fats were determined by Soxhlet extraction and saccharides were determined by titration with potassium permanganate (Bertrand´s method). Dietary supplements in form of capsules were particular analysed on content of heavy metals (lead, cadmium and arsenal). In herbal drops was in addition determined alcohol content (ethanol). There was from the measured data found out, dietary supplement in form of capsules was chosen as the product with the lowest energetic (nutrition) value.

The aim of this thesis was to design a recipe for the dietary supplements with adaptogenic effects, production, and subsequent analysis (proteins, lipids, carbohydrates), which led to the determination of the energetic value of preparation. Adaptogenic effects are the domain of plants and fungi, known as adaptogens. Adaptogens act non-specifically to the human body - improve the overall condition of the body, accelerate healing and regenerative processes, slow degenerative processes in the body and thus the aging process. As biogenic active stimulators, they affect not only in the physical but also in mental area. Dietary supplements have been designed in three different forms - capsules, syrup, drops. The analysis was also performed in tablets of Vietnamese origin supplied by supervizor, of which entry to our market is being considered. Protein content was determined by Kjeldahl method, lipids by Soxhlet extraction, saccharides by gravimetric method. Dietary supplements were analyzed for heavy metals (Pb, Cd, Hg), and in herbal drops ethanol content was determined.

Another aim is to point to potential danger which could endanger the patient while irrational using of the dietary supplement before the medical interventions (interactions of dietary supplement, side effects of dietary supplement). Methodology: The questionnaire research was conducted in order to obtain the data. It was counducted between 9th May 2017 till 30th June 2018 at 9 departments of the University Hospital in Hradec Králové. In total 332 patients in the time before surgery were asked and 257 completed questionnaires were analysed.
