Noise in early childhood education centres: the effects on the children and their teachers : a thesis presented in partial fulfilment of the requirements for the degree of Doctor of Philosophy at Massey University, Wellington, New Zealand

Although the effects of noise on children’s learning in school classrooms is well documented, there is very little on the effects of noise on preschool children. There are strict legal requirements for the daily noise exposure an adult worker can received in the workplace but nothing to control the noise children can receive in school and early education. There is also little or no data on how sound affects a child, compared to an adult. The early years of life are critical for the development of speech, hearing and auditory processes, as well as being the most vulnerable time for middle ear infections. This work sets out to determine the typical noise levels in early childhood centres and the effects on a range of children and their teachers. Reverberation times in most centres were found to well exceed the 0.6 seconds prescribed by the Australasian standard for schools and learning spaces. Very high levels of noise were recorded in a number of centres with a significant number of children and staff members, exceeding the maximum daily sound exposure of 100% permitted for workers in industry. A range of special needs children were identified as being particularly at-risk to noise, with the most adverse outcomes reported for those experiencing sensory integration disorder. Yet, even though high levels of noise were recorded, the majority of respondents in a survey of teachers rated the lack of sufficient space for the number of children present as the main issue, and inclement weather as the greatest environmental condition contributing to noise (by confining children indoors, especially over long periods of time). Hearing tests on the children were not permitted under the strict human ethics criteria to which this study had to conform, but simple hearing tests on a small group of teachers, revealed that hearing loss could be a serious occupational health issue. The legal issues of noise control and management in early childhood education have been addressed in this thesis, current legal frameworks reviewed, and recommendations presented for future consideration.

Schoolbag carriage : design, adjustment, carriage duration and weight : a thesis presented in fulfilment of the requirements for the degree of Doctor of Philosophy in Ergonomics at Massey University, Palmerston North, New Zealand

There is anecdotal and scientific evidence to suggest that schoolbag carriage is associated with musculoskeletal discomfort (MSD) and possibly long-term back pain. Thus schoolbag carriage is an area of concern for students, parents and both education and health professionals. A schoolbag weight limit of 10% of body weight (BW) is currently recommended. However, it is based on subjective observations rather than objective findings and does not consider other aspects of schoolbag carriage such as schoolbag design and adjustment or carriage patterns. Five studies were conducted in order to determine the effects on students' responses to schoolbag carriage of schoolbag design, adjustment, carriage duration and weight. Backpack design had a significant effect on reported musculoskeletal discomfort and choice of backpack. Schoolbag hip-belt and shoulder strap adjustment and weight significantly affected shoulder strap tension forces and shoulder interface pressure in simulated schoolbag carriage. Using activity monitoring, school students were found to spend approximately two hours carrying their schoolbags each day. This usually comprised 11-15 times per day of 8-9 minutes of carriage. Using this temporal pattern information, 16 boys (13-14 years) were exposed to a simulated school day using schoolbags weighing 0, 5, 10, 12.5 and 15% BW and an additional condition of 10% BW with tighter shoulder straps. Posture, rating of perceived exertion (RPE), muscular strain and reported ability to walk and balance were significantly affected when schoolbag load reached 10% BW. However, despite these findings, the magnitude of self reported muscular strain and MSD suggested that 15% BW may be too heavy for school students. Thus, 10% BW may be an appropriate upper schoolbag weight limit for a typical school day. Using a psychophysical approach the mean (standard deviation) maximum acceptable schoolbag weight (MASW) selected by 16 school boys (13-14 years) was 10.4(3.8) %BW. This finding agrees with the findings of the previous study and supports the current schoolbag weight recommendation of 10% BW. The results of the five studies can be used in developing schoolbag carrying guidelines to help reduce the prevalence of MSD amongst school students.

Objetivo: Desarrollar una metodología para evaluar el nivel de protección respiratoria de respiradores, mascarillas quirúrgicas y mascarillas comunitarias que usa la población peruana, usando partículas de un tamaño similar a las que contienen al virus activo del SARS-CoV-2. Materiales y métodos: Se ha determinado una relación lineal directa entre el logaritmo de la concentración de partículas suspendidas en aire y el tiempo transcurrido; por lo cual es posible comparar la cantidad de partículas internas y externas a la mascarilla o respirador en un mismo periodo y conocer el porcentaje de protección respiratoria de cada muestra evaluada. Resultados: Se ha logrado implementar una metodología para evaluar el nivel de protección respiratoria ante aerosoles menores a 5,0 μm. Asimismo, el empleo de accesorios como ligas o ajustadores detrás de cabeza y nuca, y el uso de clips nasales robustos, incrementan significativamente el nivel de protección respiratoria ante partículas con alta probabilidad de contener al SARS-CoV-2. Conclusiones: Se observa una concordancia entre los valores de protección respiratoria obtenidos y los esperados, considerando el nivel de filtración del material empleado de cada mascarilla quirúrgica o respirador, y su nivel de ajuste. Se observó un incremento significativo en los niveles de protección respiratoria. / Objective: To develop a methodology for evaluating the level of respiratory protection provided by res- pirators, surgical masks and community face masks used by the Peruvian population; protection was evaluated against particles of a size similar to those containing active SARS-CoV-2 virus. Materials and methods: A direct linear relationship has been determined between the logarithm of the concentration of airborne particles and the elapsed time; thus, it is possible to compare the quantity of particles inside and outside of the mask or respirator in the same time period, as well as to obtain the percentage of re- spiratory protection for each evaluated sample. Results: A methodology was established to evaluate the level of respiratory protection against aerosols smaller than 5.0 μm. Also, the use of accessories such as garters or adjusters behind the head and neck, and the use of strong nasal clips, significantly increased the level of respiratory protection against particles with a high probability of containing SARS-CoV-2. Conclusions: We found concordance between the obtained respiratory protection values and those ex- pected, considering the filtration level of the material used for each surgical mask or resp s well as the tightness. A significant increase in the levels of respiratory protection was observed.

COVID-19 y adultos mayores: comparación espaciotemporal entre mortalidad y vacunación en la provincia de Buenos Aires, Argentina

El objetivo de este estudio fue analizar las variaciones espaciotemporales de la mortalidad por COVID-19 en adultos mayores y de la vacunación contra la COVID-19 en esta población. Se utilizaron datos de defunciones por COVID-19 y de personas de 70 a más años inmunizadas con la primera dosis de vacunas contra esta enfermedad, en partidos de la provincia de Buenos Aires, desde el 29 de diciembre del 2020 al 30 de junio de 2021. Se emplearon técnicas de escaneo espaciotemporal para detectar conglomerados. Los partidos del Gran Buenos Aires que tuvieron mayor vacunación con la primera dosis de la vacuna Sputnik V en población de 70 a más años, entre mediados de marzo y principios de abril de 2021, coincidieron mayormente en registrar una disminución de la mortalidad en esta población, entre finales de abril y finales de junio de 2021. Este estudio mostró algunos indicios del impacto positivo de la aplicación de la primera dosis de la vacuna Sputnik V en el Gran Buenos Aires.

Seguridad de las vacunas contra la COVID-19

El desarrollo y producción de vacunas seguras y eficaces contra la enfermedad por coronavirus 2019 (COVID- 19) ofrece la esperanza para el control de la pandemia actual. Los eventos adversos posteriores a la inmunización son respuestas indeseadas o acontecimientos involuntarios que siguen a la vacunación, y que deben ser cuidadosamente vigilados, ya que todas las vacunas, incluyendo las desarrolladas contra el SARSCoV- 2, requieren cumplir con los criterios de seguridad para su administración en humanos. Se recopiló la información de la base de datos de PubMed/Medline durante los meses de agosto de 2020 a noviembre de 2021. La mayoría de los eventos adversos identificados en los ensayos clínicos fueron leves o moderados; sin embargo, se identificaron eventos trombóticos asociados a algunas vacunas basadas en vectores virales contra la COVID-19 en estudios de seguimiento, aunque se requiere la conclusión de los distintos estudios en curso y vigilancia poscomercialización para determinar todos los posibles eventos adversos y de especial interés.

Experiencias de telesalud en un hospital especializado en salud mental durante la pandemia de COVID-19 en Perú

El objetivo de este artículo es dar a conocer las experiencias de telesalud en un hospital especializado en salud mental en Lima, Perú durante la pandemia de COVID-19. En concordancia con las disposiciones, como el cierre temporal de la atención presencial y el confinamiento obligatorio entre los meses de marzo a diciembre del 2020, se brindó progresivamente atención a través de llamadas telefónicas, videollamadas o plataformas digitales; se realizaron 57398 atenciones de teleconsultas y telemonitoreos; 4411 orientaciones en salud mental; 295 teleinterconsultas en psiquiatría; 42 actividades de telecapacitación y 29 sesiones educativas en salud mental dirigidas a la población general. Concluimos que la implementación de la telesalud para la atención de los usuarios del Hospital Hermilio Valdizán contribuyó al cuidado de la salud mental y permite reducir las brechas de acceso a la atención especializada en psiquiatría por las consecuencias de la COVID-19.

Det elektriska isolationssystemet för stora roterande maskiner består av ett kompositmaterial av glimmertejp och ett värmehärdande harts. Hartset hjälper till att mekaniskt stabilisera lindningsstrukturen i statorn samt ersätter luftinneslutningar i isolationssystemet för att undertrycka bildandet av elektriska urladdningar. Vakuumtryckimpregneringsprocessen (VPI) är den föredragna tekniken för att impregnera glimmerisolationen med det värmehärdande hartset. Hartset som används i VPI-processen är sammansatt av flera nyckelkomponenter, inklusive den härdbara polymeren, härdare som deltar i tvärbindning, reaktiva utspädningsmedel för förbättrade processegenskaper och ytterligare tillsatser som katalysatorer och stabilisatorer. Olika hartskemier har använts under åren för VPI-processen. Det finns dock betydande farhågor när det gäller påverkan på miljö- och arbetshälsa för några av dessa komponenter. Den ökade medvetenheten om de skadliga effekterna av olika kemikalier har drivit på arbetet med att utveckla hartser med reducerade flyktiga organiska föreningar som kan vara skadliga för såväl miljön som de som hanterar hartset i stora mängder. Dessutom har stränga EU-regler klassificerat vissa härdare och reaktiva utspädningsmedel som ’substances of very high concern’, vilket har lett till ett stort behov av att hitta alternativ för dessa föreningar. Detta examensarbete består av en litteraturstudie som har genomförts med hänsyn till de önskade egenskaperna för VPI-hartser för att utvärdera potentiella kandidater som alternativ till härdare och reaktiva utspädningsmedel. Fyra olika impregneringshartser har tagits i beaktande som alternativ. Olika härdningskinetikparametrar har uppmätts med olika karakteriseringstekniker såsom infraröd spektroskopi av Fouriertransform i realtid, reologi och differentiell scanningkalorimetri. Eftersom de termiska, elektriska och mekaniska egenskaperna är nödvändiga för att säkerställa långvarig livslängd för industriellt nyttjade roterande maskiner, har olika egenskaper studerats genom att utföra dynamisk mekanisk analys, drag- och böjningstestning samt dielektrisk spektroskopi, genom att härda hartserna under lämpliga tids- och temperaturförhållanden. Alla de fyra hartserna visade varierande härdningskinetikparametrar och egenskaper som har korrelerats till hartskemin samt att hartsernas egenskaper har utvärderats i jämförelse med varandra. / The electrical insulation system for large rotating machines consists of a composite material of mica tape and a thermosetting resin. The resin helps in mechanically stabilizing the winding structure in the stator as well as replaces air inclusions in the insulation system to suppress the formation of discharges. Vacuum pressure impregnation (VPI) is the preferred technique to impregnate this mica tape with the thermosetting resin. The resin used in VPI process is composed of several key components, including the thermoset polymer, a hardener that participates in crosslinking, reactive diluents for improved processability and and additional additives like catalysts and stabilizers. Different chemistries have been used over the years for the VPI process. However, there are significant concerns regarding the environmental and occupational health and safety of some of these components.  The increasing awareness of the harmful effects of various chemicals has driven efforts to develop resins with reduced volatile organic compounds which can be detrimental to both the environment as well as those who are handling the resin in large quantities. Furthermore, stringent EU regulations have classified some hardeners and reactive diluents as substances of very high concern which has resulted in pressing need to find alternatives for these compounds.  This thesis, thus, consists of a literature study which has been performed taking the desired properties for VPI resins into consideration to evaluate potential candidates as alternatives for hardeners, and reactive diluents. Four different chemistries of impregnation have been taken into consideration as alternatives. Different curing kinetics parameters have been measured by different characterization techniques such as real time Fourier transform infrared spectroscopy, rheology, and differential scanning calorimetry. As the thermal, electric, and mechanical factors are necessary to ensure long term lifespan of industrial machines, different properties have been studied by performing dynamic mechanical analysis, tensile and flexural testing as well as dielectric spectroscopy by curing the resins under appropriate time and temperature conditions. All the four resins showed varied curing kinetics parameters and properties which have been correlated to the chemistry involved in the resin as well as evaluated in comparison to each other.

Annoyance due to relatively high levels of sound and noise, above 50 dB, has been well documented in noise assessment literature. The potential for annoyance or disturbance from low amplitude sound, below 50 dB to the threshold of an individual’s hearing, is not as well documented. The thesis presents a new approach to the measurement and assessment of intrusive noise and low amplitude sound. Acoustical and sound quality measures are integrated with measures of loudness, pitch, dissonance and tonality to provide physical measures of sound. Individual amenity is assessed with respect to personal noise sensitivity and personal attitudes to sound in the environment, the environment itself and the perceived qualities of the audible sound. A decision-support methodology to integrate perceived noise with noise performance indicators, annoyance criteria, personal noise sensitivity and amenity is presented. A method for rating intrusive noise is derived. Designs for sound measurement and calibration instrumentation are described. Methods to measure and assess low amplitude sound are presented. Recreational Exposure To Freshwater Cyanobacteria: Epidemiology, Dermal Toxicity And Biological Activity Of Cyanobacterial Lipopolysaccharides

Cyanobacteria are common inhabitants of freshwater lakes and reservoirs throughout the world. Under favourable conditions, certain cyanobacteria can dominate the phytoplankton within a waterbody and form nuisance blooms. Case reports and anecdotal references dating from 1949 describe a range of illnesses associated with recreational exposure to cyanobacteria: hay fever-like symptoms, pruritic skin rashes and gastro-intestinal symptoms (the latter probably related to ingestion of water) are most frequently reported. Some papers give convincing descriptions of allergic responses to cyanobacteria; others describe more serious acute illnesses, with symptoms such as severe headache, pneumonia, fever, myalgia, vertigo and blistering in the mouth. A U.S. coroner recently found that a teenage boy died as a result of accidentally ingesting a neurotoxic cyanotoxin from a golf course pond; this is the first recorded human fatality attributed to recreational exposure to cyanobacteria. One of the main public health concerns with exposure to freshwater cyanobacteria relates to the understanding that some blooms produce toxins that specifically affect the liver or the central nervous system. The route of exposure for these toxins is oral, from accidental or deliberate ingestion of recreational water, and possibly by inhalation. Cyanobacterial lipopolysaccharides (LPS) are also reported to be putative cutaneous, gastrointestinal, respiratory and pyrogenic toxins. The aims of this project were to enhance the understanding of public health issues relating to recreational exposure to cyanobacteria by conducting epidemiological and laboratory-based toxicology studies. A prospective cohort study of 1,331 recreational water users was conducted at various sites in southern Queensland, the Myall Lakes area of New South Wales, and central Florida. The study design sought to make improvements over previously published epidemiological studies, in that an unexposed group was recruited from cyanobacteria-free waters, cyanobacterial toxins were measured in site water samples, and respondents were asked to rate the severity of reported symptoms. This study has shown an increased likelihood of symptom reporting amongst bathers exposed to high cyanobacterial cell density (measured by total cell surface area) compared to those exposed to low cyanobacteria-affected waters. Mild respiratory symptoms appear to be the predominant symptom category. A clinical dermatology study to examine delayed-contact hypersensitivity reactions to cyanobacterial extracts was conducted. The study groups were 20 patients presenting for diagnostic skin patch testing at the Royal Brisbane Hospital's dermatology outpatient clinic; a convenience sample of 20 individuals was recruited from outside the hospital as a control group. One patient developed unequivocal reactions to several cyanobacteria extracts, with no dose-response pattern seen, indicating that the reactions were allergic in nature. A mouse model of delayed-contact hypersensitivity, the mouse ear swelling test, has demonstrated that the purified toxin cylindrospermopsin, a highly water-soluble compound, is capable of producing cutaneous injury. Encrusting lesions were seen on abdominal skin during the induction phase of these experiments. Delayed-contact hypersensitivity reactions were also demonstrated with this toxin. LPS from two non-axenic cyanobacterial samples - Cylindrospermopsis raciborskii and Microcystis aeruginosa - were extracted and purified. Thermoregulation studies were performed using a mouse model of rectal temperature measurement. Separate groups of mice were injected with these LPS extracts at two dose levels. Thermoregulation studies were also conducted with purified cyanobacterial toxins: microcystin-LR and cylindrospermopsin and anatoxin-a; cyanobacterial LPS samples purified by collaborators in Adelaide were also investigated for thermoregulation potential. These experiments have shown that the LPS extracts are weakly active when compared with responses to much lower doses of Escherichia coli LPS. Cylindrospermopsin also produces hypothermic responses in mice - comparable to pyrexia in larger mammals - although at a later stage than was seen with LPS extracts. Supplementing the experimental observations of the toxicology and immunotoxicology of cyanobacterial LPS are insights gained from beyond the cyanobacteria literature on the mechanisms of toxicity of different LPS structures. Cyanobacterial LPS was initially suggested to be toxic in the 1970s, at a time when lipid A, the active moiety of LPS, was thought to be identical across all Gram-negative bacteria. More recent work raises questions about the attribution of cutaneous and gastrointestinal illness to cyanobacterial LPS, with the understanding that some bacterial lipid A structures are LPS antagonists, LPS is not toxic by the oral route, and LPS is not reported as a toxin in the clinical dermatology literature. Gut-derived lipopolysaccharides, however, exert potent synergistic effects with a variety of xenobiotic hepatotoxins, and the well-known shock-like syndromes associated with severe cylindrospermopsin and microcystin poisoning deserve further scrutiny from the perspective of immunotoxicology.
