Implante venosos homólogo conservado em glutaraldeído da veia jugular em eqüinos: avaliação clínica, ultra-sonográfica e histopatológica

Objetivando testar a viabilidade do enxerto homólogo de jugular, conservado em glutaraldehído, nos eqüinos, foram utilizados 16 cavalos, submetidos ao implante deste enxerto. Os 16 animais foram observados durante 30 dias, período em que 6 animais tiveram os enxertos retirados cirurgicamente. Os 10 animais restantes prosseguiram as avaliações até os 60 dias quando também tiveram os enxertos retirados. Considerou-se os aspectos clínicos, ultra-sonográficos e histopatológicos. Os animais operados não mostraram alterações clínicas sistêmicas, apenas nos primeiros dias no local da cirurgia. O fluxo sanguíneo da veia jugular operada mostrou-se presente na maioria dos animais no período pósoperatório imediato e tardio. No exame ultra-sonográfico, observou-se a presença de fluxo sanguíneo, além de mensuradas a área vascular ingurgitada nas regiões cranial, caudal e do enxerto, a medida da espessura da parede vascular destas regiões e aspecto dos trombos, quando presentes. A formação de deposição de trombos murais na região do enxerto, foi observada em 14 animais, sem entretanto, representar total obstrução do fluxo sanguíneo. A histopatologia mostrou a progressiva substituição do enxerto por células mononucleares, fibroblastos, colágeno, neovascularização e a recanalização dos trombos. Objetivando testar a viabilidade do enxerto homólogo de jugular, conservado em glutaraldehído, nos eqüinos, foram utilizados 16 cavalos, submetidos ao implante deste enxerto. Os 16 animais foram observados durante 30 dias, período em que 6 animais tiveram os enxertos retirados cirurgicamente. Restabelecimento funcional do tendão extensor digital longo submetido à tenectomia em eqüinos: observação macroscópica, ultra-sonográfica e histopatológica

A ocorrência do restabelecimento da função do tendão foi considerada quando o membro operado apresentou capacidade de elevar e avançar o membro operado, em locomoção a passo. As observações ultra-ultra-sonográficas foram realizadas nos momentos préoperatório, a cada 15 dias e no dia do restabelecimento da capacidade funcional. A avaliação macroscópica quanto ao aspecto e características da ferida, foi realizada a cada 48 horas até o momento do restabelecimento funcional do tendão. A mensuração das lesões feita por planimetria, iniciou-se no dia da cirurgia e repetiu-se a cada 10 dias, até o restabelecimento funcional do membro. Biópsias das lesões e tecidos neoformados foram realizadas no dia do restabelecimento funcional do membro. O restabelecimento funcional do membro submetido à ressecção parcial do tendão deu-se em média no 45º dia, com as feridas apresentando aspecto granulomatoso tendo evoluindo com evidente retração, sem epitelização da lesão. O acompanhamento ultra-sonográfico revelou evolução cicatricial das lesões, com tecido de preenchimento de densidade variável e intensa neovascularização, sem mostrar reorganização estrutural do tendão. À histopatologia observou-se tecido neoformado, vascularizado, com infiltrado leucocitário e deposição colágena, sem completa epitelização. O tecido cicatricial formado na região da ressecção do tendão, imaturo e sem a organização estrutural do tendão normal, mostrou-se suficiente para o restabelecimento funcional do membro operado / The objective of this work was to study the long digital extensor tendon in its functional restoration. Ten equines have been undergone long digital extensor tendon partial resection. The tendon function has been considered restorated when the operated limb showed ability to lift and go forward under walk locomotion. The ultrasonographic examination has been performed prior to surgery, every 15 days and at the functional ability restoration day. The macroscopic evaluation relating to the wound characteristic and aspect has been carried out every 48 hours up to the tendon functional restoration. The lesion measurement by planimetry has been initiated at the surgery day and repeated every 10 days up to the limb functional restoration. Lesion and newly formed tissue biopsies have been performed at the limb functional restoration day. The functional restoration of the limb under tendon partial resection has occurred around the 45th day, with the wounds showing granulomatous aspect, evolved with clear retraction, without lesion epithelization. The ultrasonographic follow-up revealed lesion healing with newly formed tissues with variable density and high neovascularization without tendon structural reorganization. The histopathology has shown newly formed vascularized tissue, with leukocytic infiltrate and collagenous deposition without full epithelization. The cicatrization tissue formed in the resection region of the tendon, immature and without the structural organization of the normal tendon, has shown to be able to functional restoration of the operated limb

Durante vinte e oito dias (D0-27), os animais foram avaliados em momentos específicos, quanto aos parâmetros físicos, termográficos, laboratoriais, ultrassonográficos e venográficos, compreendendo quatro períodos de avaliação: o momento pré-indução à tromboflebite (MPIT); período de indução da tromboflebite (MIT), que teve 72 horas de duração; o período de tromboflebite (MT), onde os animais foram avaliados por dez dias; e avaliados em grupos divididos (GF x GVF) após a aplicação das técnicas de correção de trombose, o período pós-operatório (MPT). Dados quantitativos foram comparados seguindo media e desvio padrão. As análises laboratoriais foram avaliadas pela comparação dos dados de momento e tratamento, pela análise de variância. Através da análise... / The jugular thrombosis in horses commonly occurs in iatrogenic situations secondary to endotoxemia, and disseminated intravascular coagulation, which may lead to death. Prophylactic treatment with antithrombotic agents has reduced the severity of the cases. However, in situations where there chronic cases, with intense adhesion of thrombus to the vessel, drug therapy may be unsatisfactory and surgical intervention is required. The objective of this study is to evaluate the applicability of thrombectomy with Vollmar ring and watch the action of the passage of the Fogarty catheter through adhered thrombi. 10 healthy horses were used and divided into two groups of five animals each, treated with Vollmar ring and Fogarty catheter (GVF) and only with the Fogarty catheter (GF), after 13 days from unilateral right jugular vein thrombosis induction. During twenty-eight days (D0-28), the animals were evaluated at specific times, as the physical parameters, thermography, laboratory, ultrasound and phlebography, comprising four evaluation periods: the pre-induction of thrombophlebitis time (MPIT); Induction period of thrombophlebitis (MIT), which is 72 hours duration; thrombophlebitis the period (MT), where the animals was evaluated for ten days; and evaluated in divided groups (GF x GVF) after application of thrombosis correction techniques, the postoperative period (MPT). Trombectomia com cateter de Fogarty no tratamento da trombose jugular experimental em eqüinos

O diagnóstico é relativamente simples, baseado nos achados físicos, nas imagens angiográficas e ultra-sonográficas. A terapêutica conservativa empregada em grande parte dos casos é insatisfatória. O objetivo deste trabalho foi avaliar em eqüinos a aplicabilidade da trombectomia com cateter de fogarty, técnica esta rotineiramente empregada pela medicina humana, no restabelecimento da perviedade vascular. Foram utilizados 10 eqüinos divididos em 2 grupos, com cinco animais cada, em que se induziu a trombose de jugular unilateral direita, através do acesso cirúrgico à veia e aplicação de sutura estenosante e injeção de glicose a 50%. No grupo controle avaliou-se a evolução da tromboflebite sem qualquer tipo de intervenção terapêutica. Os animais do grupo tratado foram submetidos a trombectomia com cateter de Fogarty. Foram avaliados os parâmetros físicos gerais, regionais, ultra-sonográficos e angiográficos contrastados, nos momentos pré-indução (MPRÉ), indução da trombose (MTI) e dez dias de evolução da trombose (M13). A técnica empregada induziu a tromboflebite, que obstruiu completamente um segmento da veia jugular de todos os animais. Os animais do grupo controle mantiveram os trombos obstruindo totalmente o lume vascular até o final do período de avaliação, sendo que avaliações regionais mostraram principalmente o edema parotídeo e o ingurgitamento vascular, cranial a tromboflebite de jugular. Nos exames ultra-sonográficos e angiográficos, o grupo tratado apresentou todas as veias pérvias ao final do experimento, com remissão total dos sinais clínicos. Concluiu-se que a técnica da trombectomia com cateter de Fogarty foi eficiente na desobstrução da veia jugular submetida à trombose experimental. / Thrombosis of jugular vein is a frequent problem in the equine medicine, implying many times in fatal consequences. The diagnosis is relatively simple, based in the clinical findings, angiographics images and ultrasonographycs. The therapeutic employed to a large extent of the cases is unsatisfactory. The objective of this work was to evaluate in horses the applicability of the thrombectomy with Fogarty catheter, technique routinely used in medicine, in the reestablishment of the perviousness vascular. 10 horse divided in 2 groups had been used, with five animals each, and induced thrombosis of right unilateral jugular vein, through the surgical access to the vein and application of stenotic suture and glucose 50% injection. In the controlled group it was evaluated evolution of the thrombophlebitis without any type of therapeutical intervention. The animals of the treatead group had been submitted to the thrombectomy with Fogarty catheter. General clinical parameters were analysed at the moment of the preinduction (MPRÉ), induction of thrombosis (MTI), and ten days of evolution of thrombosis (M10). The employed technique induced the thrombophlebitis that completely obstructed a segment of the jugular vein in all the animals. The animals of the control group had kept the thrombus totally obstructing the vascular lumen until the end of the period of evaluation, being that regional evaluations had mainly shown parotid edema and the vascular dilated, cranial the thrombophlebitis of jugular vein. The treated group presented all the pervious veins to the end of the experiment, confirmed by angiographics and ultrasonographics examinations, with total remission of the clinical sign. So far, it was concluded that the technique of the thrombectomy with catheter of Fogarty was efficient in eliminating the obstruction of the jugular vein submitted to experimental thrombosis.

Dessensibilização dos nervos palmares em eqüinos: estudo comparativo experimental do álcool benzílico 0,75% e álcool etílico absoluto (95,5% PA)

After gait and lameness score analysis, the lateral and medial branchs of the left palmar nerve of 5 horses was injected with 5 ml of 0,75% benzyl alcohol -Sarapin -(Group A) and 5 horses was injected with 5 ml of pure ethyl alcohol or absolute alcohol (Group B). The animals were submitted to lameness evaluation and solar sensibility tests during the next months. In the Group A, which was injected with benzyl alcohol -Sarapin , the solar pain returned 5 months latter while in the Group B, injected with ethyl alcohol (ethanol or absolute), the solar surface was still desensitized 6 months latter. The histopathological examination of the palmar nerve showed that the perineural injection of benzyl alcohol 0,75% - Sarapin , (Group A), resulted in axonotmesis, characterized by axonal nerve degeneration, without disruption of connective tissue elements (endo, peri and epineurium), with good prognosis for recovery the nerve conduction. In the Group B, the perineural injection of ethyl alcohol absolute (ethanol), resulted in neurotmesis, characterized by disruption of all the axon and supporting connective tissue structures, with poor prognosis for nerve regeneration. It was concluded that chemical neurolysis with alcohol is an option for temporary or permanent nerve blocks in horses but the minimum concentration and number of injections required for neurolysis must be established.

Hipotetizou-se que a mudança de decúbito durante a anestesia reduziria significativamente a oxigenação sanguínea (PaO2) e que a instituição da ventilação controlada seria capaz de prevenir a diminuição da PaO2 após a mudança de decúbito. Foram utilizados 16 eqüinos adultos hígidos, sem raça definida, com peso corpóreo de 444,3 ± 42,5 Kg, provenientes do Regimento de Cavalaria “Dragões da Independência”. Os animais foram submetidos a procedimentos cirúrgicos onde fosse necessária a mudança de decúbito. Mediante jejum alimentar de 12 horas e hídrico de 4 horas, os animais foram premedicados com acepromazina (0,05 mg/kg, IM) e após 30 minutos, com xilazina (0,5 mg/kg, IV). A anestesia foi induzida com diazepam (0,1 mg/kg, IV) e cetamina (2,2 mg/kg, IV) e mantida com halotano diluído em O2. A dobutamina foi empregada para manter a pressão arterial média acima de 70 mm Hg durante todo o procedimento. Os animais foram equitativamente divididos em 2 grupos, sendo que no grupo VE a anestesia foi mantida sob ventilação espontânea, enquanto no grupo VC a anestesia foi mantida sob ventilação controlada (frequência respiratória: 6 mov/min, relação inspiração/expiração: 1/3, volume corrente: 15 mL/kg e pressão de pico inspiratório entre 25 a 30 cm H2O). Os procedimentos cirúrgicos foram iniciados em decúbito lateral esquerdo (DLE) e, após 75 minutos, os animais foram reposicionados em decúbito lateral direito (DLD) até o término da cirurgia. Durante a VE, observou-se... / In horses, changes in body position during anesthesia may contribute to a reduction in arterial oxygenation because the non-dependent lung (functional) is suddenly compressed. It was hypothesized that the change in recumbency during anesthesia would significantly reduce the arterial oxygenation (PaO2) and the institution of controlled ventilation would prevent the descrease in PaO2 after the change in body position. A total of 16 healthy adult mixed breed horses horses of the Cavalry Regiment “Dragões da Independência”, weighting 444,3 ± 42,5 Kg, were used. All animals underwent procedures in which a change in body position would be necessary to perform surgery. Food and water were withheld for 12 and 4 hours respectively. All animals received acepromazine (0.05 mg/kg, IM), followed 30 minutes later by xylazine (0.5 mg/kg, IV). Anesthesia was induced with diazepam (0.1 mg/kg, IV) and ketamine (2.2 mg/kg, IV) and maintained with halothane in oxygen. Orientador : José Luiz de Mello Nicoletti / Resumo:Nesta pesquisa foi estudada a dessensibilização química dos nervos Palmares em eqüinos normais. Como modelo experimental, usou-se uma ferradura especial adaptada com parafusos para induzir claudicação, cujos graus foram registrados antes e após a injeção perineural de dois neurolíticos. No Grupo A (5 animais), usou-se o álcool benzílico 0,75% (Sarapin ) e no Grupo B, o álcool etílico absoluto (100%). Foram realizados periodicamente testes de sensibilidade da sola em vários momentos à partir de 12 horas pós-infiltração (M12) até 180 dias (M180). Observou-se que nos animais do Grupo A, infiltrados com Sarapin , à partir do 150º dia após a injeção, houve um retorno progressivo da sensibilidade na região solar, enquanto que nos animais do Grupo B, aos 180 dias, os testes de sensibilidade da sola ainda mostravam resultado negativo. Os exames histopatológicos realizados nos segmentos dos nervos palmares infiltrados, mostraram que no Grupo A, infiltrado com álcool benzílico 0,75% (Sarapin ) prevaleceram alterações características de axonotemese, o que sugere tendência de regeneração do nervo e restabelecimento da condução do estímulo nervoso enquanto que no Grupo B, infiltrado com álcool etílico absoluto, prevaleceram alterações histopatológicas próprias de neurotemese, características de degeneração nervosa permanente.
