Using a Human Factors Approach to Assess Program Evaluation and Usability of the Ventilator Associated Pneumonia ProtocolBritton, Dana M., Britton, Dana M. January 2017 (has links)
Ventilator-associated pneumonia (VAP) is a healthcare-associated infection (HAI), or more specifically, a healthcare-associated pneumonia (HAP) that can lead to significant morbidity and mortality in hospitalized patients that are being mechanically ventilated. There are established evidence-based guidelines in existence designed to reduce or eliminate VAP from occurring and when properly maintained have been shown to reduce the incidence of VAP. Nurses are at the frontline adhering to the VAP protocol through its integration into their workflow. It is yet unknown what elements of the protocol and workflow contribute to a successful VAP reduction in occurrence and increased patient safety. This program evaluation project, guided by an adapted Systems Engineering Initiative for Patient Safety (SEIPS) model, takes a human-factors approach towards answering these questions. It specifically examines the VAP protocol in a large urban southwestern teaching hospital to evaluate program effectiveness using a human factors approach. Building on the work of Carayon, et al. (2006) and Jansson et al. (2013), I present the findings from this program evaluation project using an adapted SEIPS model that sought to evaluate the VAP prevention program from a human factors perspective addressing the following aims: Aim 1. Determine the effectiveness of using the adapted SEIPS model to evaluate a VAP quality improvement (QI) project; Aim 2. Evaluate a VAP QI program taking a human factors approach; and Aim 3. Using the adapted SEIPS model, identify elements of the VAP bundle that nurses perceive as strength and weaknesses. The project was completed with the following findings: Based on this work the adapted SEIPS model demonstrates usefulness for evaluating QI projects. It would be interesting to continue this work with QI projects to see how well it performs.

Profesinio mokymo įstaigose taikomi pasiekimų vertinimo metodai ir jų įtaka mokinių mokymosi motyvacijai / The methods of assessment of students’ achievements in vocational schools and influencing the motivation for studyingAkelaitienė, Eglė 29 September 2008 (has links)
Self - assessment including clear and comprehensible aims of training also can be treated as means of studying motivation. The focus in this research is going to be put on the methods of assessment of students’ achievements influencing the motivation for studying and stand point to the assessment applied. The process of research is connected with special studies called “Professional education” organized in Vytautas Magnum University. 27 participants of the mentioned studies above and vocational teachers from different teaching institutions took part in a three day theoretical – practical seminar which was held in June in 2007. There was a wide range of lectures based on assessment of students’ achievements. Besides, the teachers were to perfume individual practical tasks – to prepare assessment tools. The research is based on two stages. The first stage characterizes the assessment tools prepared by the teachers mentioned. The second stage includes the analysis of students’ questionnaire data. It’s like a playback talking about the methods of assessment accepted by the teachers of profession. According to the research, for assessment of students’ achievements, simple tests or practical tasks are applied. From the point of view of the questioned students, the mentioned methods of assessment are acceptable for them, quite objectively... [to full text]

This thesis focuses on the ultrasonic non-destructive testing and evaluation (NDT&E) of composite materials contributing in part to the concept of structural health monitoring (SHM). Specifically, adhesion phenomena and aging of composite sandwich core honeycomb structures are of crucial interest for aeronautics. The properties related to these two phenomena were studied using theoretical models, numerical simulations and experimental results. Based on previous works, using Lamb waves allow to characterize adhesion and aging. Additional simulations were carried out with the finite element software Comsol Multiphysics©. Adhesion defects such as delamination or debonding at the interfaces have been detected and located by means of the Short Term Fourier Transform (STFT) by sliding window, both numerically and experimentally. In view of NDT industrial rates, phased array transducers allow inspection and characterization of local bonding. By comparison, the Debye series method (DSM) was conducted locally to solve the inverse problem of a cut plane of a honeycomb composite. First, sandwich structure aging was investigated using Lamb waves in the air. Second, the aging was evaluated by measuring the electromechanical impedance of a transducer in contact with the samples. These experimental results show that the resonance frequencies and widths variations depend on the degree of porosity, indicating the sensitivity of these methods to the aging.

A Bayesian cost-benefit approach to sample size determination and evaluation in clinical trialsKikuchi, Takashi January 2011 (has links)
Current practice for sample size computations in clinical trials is largely based on frequentist or classical methods. These methods have the drawback of requiring a point estimate of the variance of treatment effect and are based on arbitrary settings of type I and II errors. They also do not directly address the question of achieving the best balance between the costs of the trial and the possible benefits by using a new medical treatment, and fail to consider the important fact that the number of users depends on evidence for improvement compared with the current treatment. A novel Bayesian approach, Behavioral Bayes (or BeBay for short) (Gittins and Pezeshk, 2000a,b, 2002a,b; Pezeshk, 2003), assumes that the number of patients switching to the new treatment depends on the strength of the evidence which is provided by clinical trials, and takes a value between zero and the number of potential patients in the country. The better a new treatment, the more patients switch to it and the more the resulting benefit. The model defines the optimal sample size to be the sample size that maximises the expected net benefit resulting from a clinical trial. Gittins and Pezeshk use a simple form of benefit function for paired comparisons between two medical treatments and assume that the variance of the efficacy is known. The research in this thesis generalises these original conditions by introducing a logistic benefit function to take account of differences in efficacy and safety between two drugs. The model is also extended to the more general cases of unpaired comparisons and unknown variance. The expected net benefit defined by Gittins and Pezeshk is based on the efficacy of the new drug only. It does not consider the incidence of adverse reactions and their effect on patients’ preferences. Here we include the costs of treating adverse reactions and calculate the total benefit in terms of how much the new drug can reduce societal expenditure. We describe how our model may be used for the design of phase III clinical trials, cluster randomised clinical trials and bridging studies. This is done in some detail and using illustrative examples based on published studies. For phase III trials we allow the possibility of unequal treatment group sizes, which often occur in practice. Bridging studies are those carried out to extend the range of applicability of an established drug, for example to new ethnic groups. Throughout the objective of our procedures is to optimise the costbenefit in terms of national health-care. BeBay is the leading methodology for determining sample sizes on this basis. It explicitly takes account of the roles of three decision makers, namely patients and doctors, pharmaceutical companies and the health authority.
