Food Swamps, Obesity & Health Zoning Restrictions on Fast Food Restaurants

<p>Protecting public health is the most legitimate use of zoning, and yet there is minimal progress in applying it to the obesity problem. Zoning could potentially be used to address both unhealthy and healthy food retailers, but lack of evidence regarding the impact of zoning and public opinion on zoning changes are barriers to implementing zoning restrictions on fast food on a larger scale. My dissertation addresses these gaps in our understanding of health zoning as a policy option for altering built, food environments. </p><p>Chapter 1 examines the relationship between food swamps and obesity and whether spatial mapping might be useful in identifying priority geographic areas for zoning interventions. I employ an instrumental variables (IV) strategy to correct for the endogeneity problems associated with food environments, namely that individuals may self-select into certain neighborhoods and may consider food availability in their decision process. I utilize highway exits as a source of exogenous variation .Using secondary data from the USDA Food Environment Atlas, ordinary least squares (OLS) and IV regression models were employed to analyze cross-sectional associations between local food environments and the prevalence of obesity. I find even after controlling for food desert effects, food swamps have a positive, statistically significant effect on adult obesity rates.</p><p>Chapter 2 applies theories of message framing and prospect theory to the emerging discussion around health zoning policies targeting food environments and to explore public opinion toward a list of potential zoning restrictions on fast-food restaurants (beyond moratoriums on new establishments). In order to explore causality, I employ an online survey experiment manipulating exposure to vignettes with different message frames about health zoning restrictions with two national samples of adult Americans age 18 and over (N1=2,768 and N2=3,236). The second sample oversamples Black Americans (N=1,000) and individuals with high school as their highest level of education. Respondents were randomly assigned to one of six conditions where they were primed with different message frames about the benefits of zoning restrictions on fast food retailers. Participants were then asked to indicate their support for six zoning policies on a Likert scale. Subjects also answered questions about their food store access, eating behaviors, health status and perceptions of food stores by type. </p><p>I find that a message frame about Nutrition and increasing Equity in the food system was particularly effective at increasing support for health zoning policies targeting fast food outlets across policy categories (Conditional, Youth-related, Performance and Incentive) and across racial groups. This finding is consistent with an influential environmental justice scholar’s description of “injustice frames” as effective in mobilizing supporters around environmental issues (Taylor 2000). I extend this rationale to food environment obesity prevention efforts and identify Nutrition combined with Equity frames as an arguably universal campaign strategy for bolstering public support of zoning restrictions on fast food retailers.</p><p>Bridging my findings from both Chapters 1 and 2, using food swamps as a spatial metaphor may work to identify priority areas for policy intervention, but only if there is an equitable distribution of resources and mobilization efforts to improve consumer food environments. If the structural forces which ration access to land-use planning persist (arguably including the media as gatekeepers to information and producers of message frames) disparities in obesity are likely to widen.</p> / Dissertation

Beliefs and Opinions of Low-Income Residents Living in a Food Desert in a Gulf Coast State

This study was an investigation of the access problem to fresh and affordable fruits and vegetables and how limited access influenced the beliefs and opinions of residents of a food desert as designated by United States Department of Agriculture. Many researchers have documented that price, availability, and transportation are barriers to fruit and vegetable consumption, particularly among low-income individuals. Researchers in other studies have concluded that these barriers are further exacerbated by an individual's distance from a supermarket or grocery store. However, much less information is available regarding the beliefs and opinions about life in a food desert and how these factors affect food-shopping. Accordingly, this study used the socioecological model framework to explore the beliefs and opinions of the low-income residents who lived in a food desert. Qualitative data were collected from semistructured interviews with 14 low-income participants to learn their beliefs and opinions about access to fresh produce. The qualitative data were transcribed and analyzed, using ATLAS.ti software, to generate themes. Results showed that the participants felt fresh fruits and vegetables were available, but they were not always able to purchase them because of cost. Respondents had an awareness of how their budgets influenced food choices and shopping behaviors as well as how the mode of transportation and mileage from a grocery store impacted food shopping behaviors. The implications for positive social change include creating additional options for food access, such as farmers' markets, community gardens, mobile food trucks, and lowered prices (or subsidies).

Residence in a Deprived Urban Food Environment: Food Access, Affordability, and Quality in a Paraguayan Food Desert

abstract: Food deserts are the collection of deprived food environments and limit local residents from accessing healthy and affordable food. This dissertation research in San Lorenzo, Paraguay tests if the assumptions about food deserts in the Global North are also relevant to the Global South. In the Global South, the recent growth of supermarkets is transforming local food environments and may worsen residential food access, such as through emerging more food deserts globally. This dissertation research blends the tools, theories, and frameworks from clinical nutrition, public health, and anthropology to identify the form and impact of food deserts in the market city of San Lorenzo, Paraguay. The downtown food retail district and the neighborhood food environment in San Lorenzo were mapped to assess what stores and markets are used by residents. The food stores include a variety of formal (supermarkets) and informal (local corner stores and market vendors) market sources. Food stores were characterized using an adapted version of the Nutrition Environment Measures Survey for Stores (NEMS-S) to measure store food availability, affordability, and quality. A major goal in this dissertation was to identify how and why residents select a type of food store source over another using various ethnographic interviewing techniques. Residential store selection was linked to the NEMS-S measures to establish a connection between the objective quality of the local food environment, residential behaviors in the local food environment, and nutritional health status. Using a sample of 68 households in one neighborhood, modeling suggested the quality of local food environment does effect weight (measure as body mass index), especially for those who have lived longer in poorer food environments. More generally, I find that San Lorenzo is a city-wide food desert, suggesting that research needs to establish more nuanced categories of poor food environments to address how food environments emerge health concerns in the Global South. / Dissertation/Thesis / Ph.D. Social Science and Health 2012

Access to Fresh Foods: Socio-Economic Characteristics of Illinois Farmers' Markets Participating in Government Funded Food Assistance Programs

The USDA's Supplemental Nutrition Assistance Program's (SNAP) strives to connect SNAP recipients to local and healthful food sold at Farmers' markets. These efforts are an attempt to provide families, struggling to make ends meet, access to healthy foods and food purchasing power via the Federal Food Stamp program. However, of the 288 listed farmers' markets in Illinois only 25 are listed as SNAP/EBT accepting markets (2010 National Farmers Market Directory). The first part of this study uses the 61st Street Farmers Market as a case study, investigating the practical market operations and interpreting their method of EBT implementation to provide an established model of an existing and thriving farmers' market. Next, I will investigate the socio-economic characteristics (population, race, employment, income, poverty, education, age, and SNAP recipients) of Illinois farmers' markets accepting EBT. I will compare this data to other Illinois Farmers' markets that do not accept EBT and assess which non-accepting market locations would be most suitable for implementing EBT usage in the future. Finally, based on my case study and statistical analysis, I will discuss recommendations for farmers' markets to accept EBT into their markets. Findings indicate that there is no significant difference between the mean ranks of the socio-economic characteristics for the EBT and non-EBT accepting farmers' markets. Of the 8 socio-economic characteristics tested, the populations of SNAP recipients were analogous for each group: EBT and Non-EBT accepting farmers' markets.

The Public Market System of New Orleans: Food Deserts, Food Security, and Food Politics

This study evaluates the public market system in New Orleans, Louisiana by focusing on the history of New Orleans public markets, the privatization of food, and the "greening" of the city with the creation of the Crescent City Farmers Market and other grass roots food activist efforts. Using qualitative methods, ethnographic fieldwork, participant observation and interviewing, issues of food access, food security, food production, food locality, quality, and affordability in New Orleans are explored. The history of public markets in New Orleans and the patterns of market proliferation, regulation, and privatization are significant in the landscape of cultural self-identification, community cohesion, neighborhood networks and economic and ecological development and sustainability. The city's various food shopping arenas and their locations become markers of history, status, rebellion, and of the "other," and become centers for issues of health, economy, politics, and food.

Local Governmental Development of Alternative Food Systems in Distressed Urban Areas

Alternative food systems (AFS) projects are designed to address issues of environmental justice, food security and insecurity, community health disparities between the affluent and the poor, and access to healthful foods in distressed urban areas. Past research has questioned the efficacy and long-term viability of such interventions, particularly in distressed primarily Black urban areas. The purpose of this intrinsic case study (ICS) was to understand the ways in which local governmental entities collaborated with each other and with nongovernmental partners to help develop an AFS in South Florida through the creation of a market garden called the PATCH. Critical race theory was the framework for addressing the challenges associated with community health, empowerment, and socioeconomic issues pertaining to AFS. A critical case sampling strategy was employed in order to study the selected site. Transcribed data from interviews with 6 key informants, observational notes, and publicly available document searches were coded using a thematic posteriori strategy and analyzed diagrammatically. Results revealed 4 primary drivers for the effective creation of AFS including collaboration and partnerships, community empowerment, community involvement, and the leadership role of government. The concept of transcommunality played an integral role in how these primary drivers could be applied between local governmental and nongovernmental partners. Knowledge gleaned from these results can inform the development of effective community and culturally specific AFS that can help address the disparities that race and socioeconomic status play in providing access to healthful foods in South Florida, thereby creating the basis for positive social change in distressed urban areas.

Can urban agriculture become a planning strategy to address social-ecological justice?

Last century witnessed an unprecedented growth of cities which has led to the consolidation of an eminently urbanised world population. Meanwhile, agriculture has adopted industrial methods of production in the shape of large-scale, chemical-laden crops in the countryside, which, together with the liberalisation of global trade, have undermined the livelihood of small-scale peasants throughout the world, forcing many of them out of business. The food industry has responded to the high rates of hunger and malnutrition with an extraordinary increase in production that has not solved food security problems, as these have turned out to be more a question of unequal access to food rather than insufficient supply. Furthermore, the activity of large agri-food corporations has resulted in the degradation of natural ecosystems and an increasing pressure over already overburdened critical resources for food production. Consequently, facing the imminent threat of climate change, more and more voices are questioning the sustainability of the current food system and rising against the burgeoning hunger and escalating inequalities resulting from it. Hence, several alternatives to the neoliberal food system are emerging these days with the aim of reducing social inequalities and curbing environmental degradation, being urban agriculture one of them. Precisely, this thesis explores, from a social-ecological justice perspective, whether urban agriculture can address issues of environmental stewardship and disparities in food distribution. Although the many virtues of urban farming might not be enough to subvert the structures of power that are deeply rooted in the foundations of the present food regime, it could still play a significant role in alleviating the gaps in food needs. However, food security comes only after the core reasons of poverty have been addressed and social justice is achieved in the larger society. The pathway towards a greater social and ecological justice seems to require not only to re-examine how to feed the urban population, but also a significant transformation that goes beyond aspects from the whole food supply chain and embraces societal systemic change.
