Relationships Between D-Dimer Levels and Stroke Risk as Well as Adverse Clinical Outcomes After Acute Ischemic Stroke or Transient Ischemic Attack: A Systematic Review and Meta-AnalysisYuan, Bing, Yang, Tong, Yan, Tao, Cheng, Wenke, Bu, Xiancong 27 March 2023 (has links)
Objective: Abnormal elevation of D-dimer levels is an important indicator of
disseminated intravascular clotting. Therefore, we hypothesized that high D-dimer levels
were associated with the risk of stroke and adverse clinical outcomes of patients with
acute ischemic stroke (AIS) or transient ischemic attack (TIA).
Methods: The present meta-analysis aimed to systematically analyze the associations
between D-dimer and the risk of stroke as well as the clinical outcomes of patients with
post-stroke or TIA. Meanwhile, dose–response analyses were conducted when there
were sufficient data available. Three electronic databases including Pubmed, the Embase
database, and the Cochrane Library were searched by two investigators independently.
All the pooled results were expressed as risk ratios (RRs).
Results: Finally, 22 prospective cohort studies were included into this meta-analysis.
The results suggested that high D-dimer levels were associated with increased risks of
total stroke (RR 1.4, 95%CI 1.20–1.63), hemorrhagic stroke (RR 1.25, 95%CI 0.69–2.25),
and ischemic Stroke (RR 1.55, 95%CI 1.22–1.98), and the dose-dependent relationship
was not found upon dose–response analyses. Besides, the high D-dimer levels on
admission were correlated with increased risks of all-cause mortality [RR 1.77, 95%
confidence interval (CI) 1.26–2.49], 5-day recurrence (RR 2.28, 95%CI 1.32–3.95), and
poor functional outcomes (RR 2.01, 95%CI 1.71–2.36) in patients with AIS or TIA.
Conclusions: On the whole, high D-dimer levels may be associated with the risks of total
stroke and ischemic stroke, but not with hemorrhagic stroke. However, dose–response
analyses do not reveal distinct evidence for a dose-dependent association of D-dimer
levels with the risk of stroke. Besides, high D-dimer levels on admission may predict
adverse clinical outcomes, including all-cause mortality, 5-day recurrence, and 90-day
poor functional outcomes, of patients with AIS or TIA. More studies are warranted to
quantify the effect of D-dimer levels on the risk of stroke or TIA, so as to verify and
substantiate this conclusion in the future.

Head and neck cancer patients often experience swallowing disorders placing them at risk for aspiration and malnutrition. This study examined the effects of electrical stimulation to the pharyngeal wall on swallowing function in post-surgical head and neck cancer patients. Swallowing of liquid, pudding, and cookie consistencies was examined using videofluoroscopy before, and 30 minutes after, a ten minute application of electrical stimulation the pharyngeal wall in 5 male patients experiencing moderate-severe dysphagia. A total of ten measures of swallowing function were obtained from pre- and post-videofluoroscopy studies. Changes were observed post-stimulation in duration of posterior pharyngeal wall to base of tongue contact, total number of swallows, cricopharyngeal opening durations, and pharyngeal transit time. Preliminary findings indicate that electrical stimulation of the pharynx may impact certain features of swallowing in head and neck cancer patients who experience dysphagia. However, further studies are required to confirm the present findings, explore the mechanisms responsible for these changes, and investigate the effect on swallowing function as a result of manipulating stimulus frequencies, intensities and durations. / Speech-Language Pathology

Sibling relationships and the interaction within these relationships play a central role in children’s general development and will therefore impact on functional outcomes of children with severe speech and language disabilities. Siblings of children with disabilities have often felt excluded from family interactions, possibly causing negative attitudes towards participation with their siblings, in turn impacting negatively on the functional outcomes of their siblings with disabilities. The main aim of this research is to determine the attitudes of typically-developing children towards participation with their younger siblings with severe speech and language disabilities in four everyday life situations, namely: play (major life areas), communication, activities of daily living as part of domestic life and interpersonal relationships (time and responsibility issues). Twenty-seven older, typically-developing siblings of children with severe speech and language disabilities were selected to complete the measuring instrument (SAS). The results indicated that the participants held the most positive attitudes towards participation in play (major life areas) but that they held less positive attitudes towards communication participation and that they were least positive about participation in interpersonal relationships (time and responsibility issues). This study succeeded in documenting attitudes of siblings towards participating in four everyday life situations with their younger sibling. It is envisaged that these results will provide tentative guidelines for how activity-based interventions can best be structured to support both the child with a disability and their sibling. Although the female menopause has been extensively characterized as a well-defined symptomatic state of oestrogen deficiency, which responds relatively well to oestrogen replacement therapy, the symptomatic state of androgen deficiency in men is poorly defined and uncertainty exists whether it responds to testosterone replacement. It has been proposed that hypothalamic-pituitary-testicular (HPT)-axis function (responsible for the production of androgens) and regulation could be viewed as a âbarometerâ of health status in older men and that potential alterations in HPT-axis function and regulation reflect subclinical and clinical deficits in function and health, which may result in an aged phenotype of human health and disease in older men. The HPT-axis constitutes a well-defined, tractable, clinically-relevant, biological system, which may permit insight into the mechanisms underlying the expression of ageing-related phenotypes of human health and disease. By using a different lens â such as the genetic background; the compensatory responses within the HPT-axis; the syndromes of androgen deficiency; the ethnic background of an individual or the life course trajectory of function and health from conception into older age â to magnify potential dysregulation in the HPT-axis will it be possible to visualize and understand the phenotypic expression of human male ageing as a gradient of functional and health outcomes. This will allow for a better understanding of the physiological mechanics underlying symptomatic expression of dysregulation in the HPT-axis.
