Les déterminants de l’utilisation des services en santé mentale dans le sud-ouest de Montréal

Objectif : L’objectif principal de cette thèse est d’examiner les déterminants de l’utilisation des services de soins pour des raisons de santé mentale dans le sud-ouest de Montréal. Données et méthodes : L’étude utilise les données de la première phase du projet portant sur « le développement d’une zone circonscrite d’études épidémiologiques en psychiatrie dans le sud-ouest de Montréal ». Les données ont été collectées entre mai 2007 et août 2008 auprès d’un échantillon de 2434 personnes sélectionnées au hasard dans tout le territoire de l’étude. De cet échantillon, nous avons sélectionné un sous-échantillon de personnes ayant eu au moins un diagnostic de santé mentale au cours de la dernière année. 423 personnes ont rencontrées ce critère et constituent l’échantillon pour les analyses de la présente thèse. Le modèle comportemental d’Andersen a servi de cadre pour le choix des variables à analyser. Parce que l’approche socio-spatiale a été privilégiée pour modéliser les déterminants de l’utilisation des services, les analyses ont été effectuées à l’aide de quatre logiciels distincts à savoir : SPSS, AMOS, ArcGIS et MlWin. Résultats : Les résultats montrent que 53,66% de notre échantillon ont utilisés au moins un service de santé pour des raisons de santé mentale. On constate néanmoins que les déterminants de l’utilisation des services en santé mentale sont à la fois complexes et spatialement inégalement réparties. En ce qui concerne les caractéristiques sociodémographiques et cliniques, les femmes et ceux qui perçoivent la stigmatisation envers les personnes ayant un problème de santé mentale utilisent plus les services. Le nombre de diagnostics de santé mentale est aussi associé à l’utilisation des services. L’augmentation du nombre de diagnostics entraîne une augmentation de l’utilisation des services (=0,38; p<0,001). D’autres variables comme l’âge, le statut matrimonial, la taille du ménage, le soutien social et la qualité de vie influencent indirectement l’utilisation des services. À titre illustratif toute augmentation de l’âge entraîne une augmentation du soutien social de (=0,69; p<0,001) qui à son tour fait diminuer la détresse psychiatrique (= -0,09 (p<0,05). Or, toute augmentation d’une unité de détresse psychiatrique entraîne une augmentation de l’utilisation des services (=0,58 (p<0,001). Sur le plan spatiale, il existe une corrélation positive entre l’utilisation des services et la défavorisation matérielle, la défavorisation sociale et le nombre d’immigrants récents sur un territoire. Par contre, la corrélation entre la prévalence de la santé mentale et l’utilisation des services est négative. Les analyses plus poussées indiquent que le contexte de résidence explique 12,26 % (p<0,05) de la variation totale de l’utilisation des services. De plus, lorsqu’on contrôle pour les caractéristiques individuelles, vivre dans un environnement stable augmente l’utilisation des services (O.R=1,24; p<0,05) tandis que les contextes défavorisés du point de vue socioéconomique ont un effet néfaste sur l’utilisation (O.R=0,71; p<0,05). Conclusion : Les résultats de l’étude suggèrent que si on veut optimiser l’utilisation des services en santé mentale, il est important d’agir prioritairement au niveau de la collectivité. Plus spécifiquement, il faudrait mener des campagnes de sensibilisation auprès de la population pour combattre la stigmatisation des personnes ayant un problème de santé mentale. Sur le plan de la planification des soins de santé, on devrait augmenter l’offre des services dans les territoires défavorisés pour en faciliter l’accès aux habitants. / Objective: The objective of this thesis is to examine the determinants of health services use for mental health reasons in the southwest of Montreal. Data and methods: The study uses data from the first phase of the project « le développement d’une zone circonscrite d’études épidémiologiques en psychiatrie dans le sud-ouest de Montréal ». Data were collected from May 2007 to August 2008 among a sample of 2.434 persons randomly selected throughout the study area. From this sample, we selected a sub sample of people with at least one mental health diagnosis during the past year. 423 persons met the criteria and constitute the sample for analysis of this thesis. The behavioral model of Andersen provided the framework for the selection of variables to analyze. Because the socio-spatial approach was preferred to model the determinants of services use, the tests were performed using four different software including: SPSS, AMOS, ArcGIS and MlWin. Results: The results show that 53.66% of our sample had used at least one health service for mental health reasons. It should be note, however, that the determinants of mental health services use are both complex and spatially unevenly distributed. Regarding the socio-demographic and clinical characteristics, women and those who perceive stigma against people with mental health problems use more services. The number of mental health diagnoses is also associated with health services use. Health services utilization increase with the number of diagnosis (=0.38; p<0.001). Other variables such as age, marital status, household size, social support and quality of life indirectly influence use of services. For instance, any increase of one year of age leads to increased social support of 0.69 (p<0.001) which in turn reduced the psychiatric distress of 0.09 (p<0.05). Any increase of one unit of psychiatric distress leads to increased use of services of 0.58 (p<0.001). At the spatial scale, there is a positive correlation between health service use and material deprivation, social deprivation and the number of recent immigrants in a neighbourhood. However, there is a negative correlation between the prevalence of mental health and health services use. Further analyses indicate that the neighbourhood explains 12.26% (p<0.05) of the total variation of health services use. After controlling for individual characteristics, living in a stable neighbourhood increases service use (O.R=1.24; p<0.05) while deprive socio-economic neighbourhood hinders use of services (O.R=0.71; p<0.05). Conclusion: Results of this study suggest that if we want to optimize the use of mental health services, it is important to act primarily at the community level. More specifically, awareness campaigns should be conducted to combat stigmatization of people with mental health problems. In term of health services planning, it is urgent to increase the number of health services in disadvantage neighbourhoods to facilitate accessibility to residents.

Durant le Néolithique en France (5700-2100 ans av. J.-C.), plusieurs groupes culturels principalement définis à partir du mobilier se succèdent. Les pratiques funéraires témoignent de changements majeurs au cours de cette période. Les sépultures d’abord individuelles, ont tendance à être regroupées, avant que la monumentalisation et la collectivisation des tombes se mettent en placeau Néolithique moyen pour devenir combiné et quasi exclusives à la fin du Néolithique. L’objectif de ce travail est l’étude des individus immatures dans divers contextes (pratiques funéraires, recrutement, analyse spatiale) et une discussion de leur considération sociale à partir des résultats obtenus.Une analyse multi-scalaire, fondée sur un inventaire de 8124 sites, associe les données archéologiques et biologiques et elle a été couplée à un SIG. La base de données ainsi constituée a vocation à être disponible sous la forme d’une plateforme collaborative en ligne.Huit sites du Néolithique moyen et de la fin du Néolithique ont fait l’objet d’une étude plus approfondie avant d’être intégrés à l’ensemble de l’échantillon. Les résultats documentent une organisation spécifique aux individus immatures mise en place à l’échelle du site. Une diversité de traitements est également visible à une échelle plus large, autorisant certains regroupements de sites d’un point de vue géographique et chronologique. Les différences observées semblent refléter des âges de rupture dans la considération sociale des individus immatures et conduisent à une discussion sur la présence de tranches d’âge au sein de la cohorte des défunts non-adultes, comme autant d’étapes sociales depuis la naissance jusqu’à l’âge adulte. / In France, during the Neolithic time period (5700-2100 BC), several cultural groups have been identified. They are mostly defined according to their set of artefacts. Funerary practices show major changes during this period. In the begining of Neolithic burials were simple and gathered, following with monumentalisation and collectivization at the middle Neolithic, before becoming exclusive at the end of the Neolithic. The aim of this research is to focus on immature individuals within these various contexts (funerary practices, age distribution and spatial analysis) and to discuss the social statut of young individuals within the society through the collected data.A multi-scalar study, based on an inventory of 8124 settlements in France combines biological and archaeological data and includes a GIS analysis. This elaborated data base is designed to become an online collaborative platform.Special attention is given to eight funerary sites dated respectively to middle Neolithic and to the end of Neolithic before their integration in the whole sample. Results document a specific spatial distribution of the immature individuals inside the funerary sites. Various funerary treatments are also identified on a larger scale, allowing the grouping of some sites both geographically and spatially. These differences seem to reflect breaking age at death linked to the social consideration of the immature deceased, from birth to adulthood.

Diversidade funcional em comunidades de peixes lagunares no Sul do Brasil

Os paradigmas da teoria de metacomunidades atribuem diferentes graus de importância à dispersão, filtros ambientais, interações bióticas e processos estocásticos na organização de comunidades. Incluir atributos funcionais em conjunto com aspectos espaciais da estrutura da paisagem pode resultar em uma poderosa ferramenta para a investigação dos diferentes processos que atuam na organização de comunidades biológicas. Neste estudo, utilizamos o potencial de análise e de levantamento de hipóteses que os atributos funcionais proporcionam em uma metacomunidade de peixes, formada por 37 lagoas em uma bacia hidrográfica na região costeira do sul do Brasil (29º37’ a 30º30’ de latitude Sul e 49º74’ a 50º24’ de longitude Oeste). Os objetivos deste trabalho foram identificar qual é a relação entre a diversidade taxonômica com índices de redundância e diversidade funcional. Também verificar se variáveis espaciais são determinantes na variação de índices funcionais, e das composições taxonômica e funcional, e se ocorrem padrões de convergência e de divergência de atributos. Através de sistemas de informação geográfica (imagens Spot e Landsat-TM5), estas lagoas foram mapeadas e delas foram obtidas variáveis estruturais (área, forma, distância do mar, coeficiente de variação da área, conectividade primária e conectividade estuarina). Os dados da ictiofauna foram obtidos através de amostragem padronizada, utilizando-se redes de espera, e uma série de atributos relacionados às habilidades de dispersão e de uso de recursos alimentares foram tomados. A diversidade taxonômica demonstrou ser fortemente correlacionada com a redundância e a diversidade funcional. Os modelos que melhor explicam a redundância funcional são aqueles que incluíram a forma e o coeficiente de variação da área das lagoas, mas a diversidade funcional não foi predita significativamente por nenhuma variável espacial. Não foram encontrados padrões de convergência e de divergência de atributos, e lagoas semelhantes em suas características espaciais não possuem composição funcional similar. Nossos resultados sugerem que o paradigma neutro de metacomunidades é a abordagem que melhor explica a estruturação deste sistema, o qual prediz equivalência funcional entre espécies. / The metacommunity theory paradigms attribute different degrees of importance to dispersal, environmental filtering, biotic interactions and stochastic processes in community assembly. To include the jointly use of functional traits with the spatial aspects of landscape structure could result in a powerful tool for the investigation of the different processes involved in the organization of biological communities. In this study, we used the potential of analysis and survey of hypotheses that functional traits provide in a fish metacommunity, composed by 37 lagoons in a river basin in the coastal region of southern Brazil (29º37' to 30°30' south latitude and 49º74' to 50°24' west longitude). The aims of this work were to identify the relation between taxonomic diversity indices with redundancy and functional diversity. Also, it was verified if spatial variables are determinants for the variation in functional indices, taxonomic and functional composition of fishes. We also look for trait convergence and trait divergence assembly patterns. Through geographic information systems (Spot e Landsat-TM5 images), these lagoons were mapped and were quantified structural variables (area, shape, distance to the ocean, coefficient of variation of area, primary connectivity and estuarine connectivity). Ichthyofauna data were obtained through standardized sampling, using gillnets, and a set of traits related to dispersal abilities and use of food resource were obtained. The taxonomic diversity showed to be strongly correlated with functional diversity and redundancy. The models that best explain the functional redundancy are those involving the shape and area variation coefficient, however, the functional diversity was not significantly predicted by any spatial variable. We did not find trait convergence and trait divergence assembly patterns, and lagoons that share similar spatial features do not have similar functional composition. Our results suggest that the neutral metacommunity paradigm is the approach which has the best explanation for the structure of this system, which predicts functional equivalence among species.

ENABLING LARGE-SCALE HYDROLOGIC AND HYDRAULIC MODELING THROUGH IMPROVED TOPOGRAPHIC REPRESENTATION

<p>Topography is one of the primary drivers of physical processes in the rivers and floodplains. Advances in remote-sensing and survey techniques have provided high-resolution representation of the floodplains but information regarding the 3D representation of river channels (commonly known as river bathymetry) is sparsely available. Field surveys along an entire river network in a watershed remains infeasible and algorithms for estimating simple but effective characterization of river channel geometry are hindered by an incomplete understanding of the role of river bathymetry in surface and subsurface processes. </p> <p> The first objective of this dissertation develops an automated framework – System for Producing RIver Network Geometry (SPRING) for improving the geospatial descriptors of a river network. The tool takes as input the DEM and erroneous river centerline to produce spatially consistent river centerlines, banks, and an improved representation of river channel geometry. SPRING can process entire river networks and is not limited single reach applications. The proposed framework is flexible in terms of data requirements, resolution of output datasets and user preferences. It has a user-friendly graphic user interface (GUI) and is appropriate for large-scale applications since it requires minimal user input.</p> <p> A better understanding of the role of bathymetric characteristics in surface-subsurface hydrology and hydrodynamics can facilitate an efficient incorporation of river bathymetry in large river networks. The second objective explores the level of bathymetric detail required for accurately simulating surface and subsurface processes by developing four bathymetric representations using SPRING with reducing level of detail. These bathymetric configurations are simulated using a physically based tightly coupled hydrologic and hydrodynamic model to estimate surface and subsurface fluxes in the floodplains. Comparison of fluxes for the four bathymetric configurations show that the impact of river bathymetry extends beyond surface routing to surface water – groundwater interactions. Channel conveyance capacity and thalweg elevation are the most important characteristics controlling these interactions followed by channel side slope and channel asymmetry. </p> <p> The final objective aims to develop benchmarks for bathymetric characteristics for accurately simulating flooding related physical processes. The sensitivity of surface and subsurface fluxes to error in channel conveyance capacity is investigated across reaches with varying geomorphological characteristics. SPRING is used to create six bathymetric configurations with varying range of error in channel conveyance capacity (ranging from 25% to 300%). They are simulated using a tightly coupled physically distributed model for a flood event and the estimates of water surface elevation, infiltration and lateral seepage are compared. Results show that incorporating channel conveyance capacity with an error of within 25% significantly improves the estimates of surface and subsurface fluxes as compared to those not having any bathymetric correction. For certain reaches, such as those with high drainage area (>1000km<sup>2</sup>) or low sinuosity (< 1.25), errors of up to 100% in channel conveyance capacity can still improve H&H modeling.</p>

GIS presentation includes geodetic point field in specified location and tasks assignment for students. Designed system allows editing and managing of data already existed.

Paysages emblématiques des campagnes de l’Ouest, les bocages ont connu une série de transformations rapides et importantes ces quarante dernières années : érosion du linéaire de haies, changements des usages du sol, rationalisation et agrandissement du parcellaire, banalisation et massification des bâtiments agricoles, enfrichement des secteurs les plus difficiles à exploiter. L’ampleur des ajustements observés souligne le décalage entre ces formes paysagères héritées d’une longue histoire agraire, et l’évolution des systèmes agricoles qui contribuent à les produire.Si la transformation des paysages de bocage est généralement attribuée au tournant « productiviste » du modèle agricole français, les processus socio-techniques à l’origine de ces évolutions sont plus rarement explicités. L’objet de ce travail est d’évaluer plus spécifiquement la contribution de l’agrandissement des exploitations agricoles à la dynamique d’évolution des paysages bocagers de l’Ouest de la France.Ce travail de géographie s’appuie dans un premier temps sur une analyse statistique à l’échelle du grand Ouest de la France, visant à étudier l’effet des transformations foncières sur les structures spatiales agricoles (assolements, parcellaire, linéaires boisés…). Dans un second temps, les dynamiques paysagères et foncières de quatre espaces d’étude (Bessin, Bocage Virois, Sud Manche, Pays d’Auge), situés en domaine laitier et bocager mais présentant des configurations agricoles variées, ont été étudiées entre 2003 et 2016. Ce travail s’appuie notamment sur un dispositif méthodologique original, articulant au sein d’un système d’information géographique à échelle parcellaire (1) la construction d’un suivi à échelle spatio-temporelle fine des dynamiques paysagères (évolution du maillage bocager, de la trame parcellaire et de l’occupation du sol) et (2) la reconstitution de l’évolution de la mosaïque des parcellaires d’exploitations par appariement de plusieurs millésimes du registre parcellaire graphique. L’exploitation de cette base de données spatio-temporelle a permis de mieux comprendre le rôle de la transformation foncière des exploitations agricoles dans la dynamique des paysages bocagers. Enfin, une enquête de terrain a été réalisée auprès de 150 agriculteurs équitablement répartis dans les quatre espaces d’étude, afin d’appréhender les conséquences sociales, techniques et productives de l’agrandissement à l’échelle des exploitations agricoles, qui diffèrent singulièrement selon le type de trajectoire foncière suivie. / Symbolic landscapes of the countryside of the West of France, the bocage landscapes have undergone a series of transformations these last forty years : decrease in hedgerow density, land uses changes, plots extension, normalization and enlargement of farm buildings, spatial extension of wilderness… The intensity of landscape transformations highlights the contradiction between these landscape forms produced by a long agrarian history and the contemporary evolutions of farming systems. The transformation of hedgerow landscapes is generally attributed to the "productivist" turn of the French agricultural model. However, the socio-technical processes behind these changes are more rarely explained.The first purpose of this geography work is to study the effects of changing agricultural systems on agricultural spatial structures, based on a statistical analysis at the scale of the West of France. In a second step, the landscape and land dynamics of four study areas (Bessin, Bocage Virois, Sud Manche, Pays d'Auge), located in the dairy and bocage domain, have been studied between 2003 and 2016. This work is based on an original methodological device, set up in a geographical information system. This structuration of geographic information makes possible to (1) monitor the landscape dynamics (evolution of the hedgerow density, land cover and plot morphology changes) at a fine spatial and temporal scale and (2) to reconstruct the evolution of the mosaic of farm plots, by matching land-parcell identification systems across the time (2007, 2011, 2013). From this spatio-temporal database, we characterized the coevolution of landscape structures and farm territories across the time, in order to better understand the landscape consequences of farm enlargment.Finally, a field survey was carried out among 150 farmers equitably distributed in the four study areas, in order to apprehend the social, technical and productive consequences of the farms enlargment, which differ singularly according to the type of land trajectory followed.

Designing multimodal public transport networks using metaheuristics

The public transport system in South Africa is in a precarious state, capturing no more than 50% of the passenger market. The three public transport modes that are currently utilized—train, bus, and minibus-taxi—are competing for market share instead of complementing one another. Furthermore, most public transport networks have not been properly redesigned over the past three decades. Improvements were initiated reactively in the past: transit stops and routes were added or removed from the network when demand fluctuated. This reactive process has diminished the confidence of commuters in the public transport networks, forcing commuters to use private transport. A proactive redesign method is needed—one that includes all the modes of public transport, and anticipates an increase in demand and rapid development in geographic areas, while ensuring good accessibility to the network. Current network design models do not include multiple modes of public transport, and are based on the geographical layout of developed cities and their particularities, which makes them unsuitable for the South African environment with its unique land use disparities. This dissertation proposes a multimodal network design model that is capable of designing real world and large scale networks for the South African metropolitan areas. The City of Tshwane Metropolitan Municipality (CTMM) transport network area was used to develop and test the model, which consists of four components. The Geographic Information System (GIS) component has a central role in storing, manipulating, and exchanging the geographic data within the model. For the GIS the appropriate input data is identified, and a design for the geo-database is proposed. The Population Generation Algorithm (PGA) component translates the demographic data into point data representing the transit demand in the study area. The Bus Stop Placement Algorithm (BSPA) component is a metaheuristic that searches for near-optimal solutions for the placement of bus stops in the study area. A novel solution approach proposed in this dissertation uses geographic data of commuters to evaluate the bus stop placement in the study area. The Multimodal Network Design Algorithm (MNDA) component also employs a metaheuristic, enabling the design of near-optimal multimodal networks. The addition of multiple modes to the Transit Network Design Problem (TNDP) is also a novel and significant contribution. The two metaheuristic components are first tested on a test network, and subjected to a comprehensive sensitivity analysis. The purpose of this study was to evaluate the spatial distribution of geochemistry in Swedish till and whether interpolations could predict unknown values of geochemistry between samples at a national spatial level, Sweden, and a regional, the county of Västerbotten. The information of the spatial distribution of the elements and the interpolations accurateness has several applications. For example, the establishing of infrastructure.  Which, in Sweden, is regulated by Naturvårdsverkets guidelines for sensitive land management (KM) and less sensitive land management (MKM) since elements can be harmful for the environment and health above certain levels. The guidelines for waste disposal of soil with levels of less than slight risk (MRR) does also acquire knowledge about the background levels of an area which are found in the C-horizon. The aim of the study was therefore to answer the following questions 1) Which interpolation method provides the most accurate prediction for the various elements at the national and regional level? 2) How does the calculated levels of geochemistry differ from the sampled ones for the two spatial levels? The study was conducted by studying the levels of ten elements in the C-horizon at the two spatial levels were the regional had greater sample density than the national. The interpolations that were used for these elements were the local interpolation methods kriging, inverse distance weighting (IDW), natural neighbour, thiessen polygons and triangular irregular network (TIN). Samples were gathered from Sweden’s geological Surveys (SGU) for the elements that are regulated by Naturvårdsverkets guidelines which was why these ten elements were studied: arsenic, barium, cobalt, chromium, copper, nickel, lead, antimony, vanadium, and zinc. The interpolations were done with two thirds of  the data. A validation was done with the remaining third by calculating root mean square error (RMSE). For the interpolation method with the lowest RMSE, the mean absolute square error (MAPE) was calculated for all the validation points to see how the calculated levels differed from the samples. The result showed that kriging and IDW were the most accurate interpolation methods for the data but that some of the studied elements need even greater sample density to become more correct. This can be solved by doing a cross-validation of the existing data. Furthermore, the interpolations were more accurate at the regional level for elements except antimony. The higher accuracy can be explained by the higher sample density at the regional level. At the national level the interpolations worked better in the north of Sweden than in the south which needs further studying. Overall, the interpolations were the least accurate when the levels of the elements were low, which may be the reason why antimony showed higher RMSE at the regional level. In conclusion the study showed that it is possible to use interpolations to predict values at unknown places with different accurateness.
