Philosophiae Doctor - PhD / The thesis explores the role of semiotic structuring of health information in relation to language,
multimodality and health literacy and the affordances for agentive participation among consumers of
two leading South African medical schemes - Discovery Health Medical Scheme (Discovery Health)
and the Government Employees Medical Scheme (GEMS).
The focus is on who has access to health information, how this information is constructed and what
the semiotic health habitat looks like for citizen-consumers. Through a virtual ethnographic approach
the thesis explores the design of genres of health information artefacts: application forms, application
guides, a comic book, and a variety of website images.
The choice to study the commercial package of a private health industry is aimed at finding and
defining codes of practice in health communication that could be replicable in the public health sector.
A new perspective emerging out of the thesis is how semiotic structuring of style, stance-taking, and
choice of registers affects reading positions, and how these determine with what voice citizenconsumers
can engage with this information.

Previous issue date: 2006-07-19 / This paper seeks to analyze the process of implementation of the Participative Budget in Alagoinhas- BA (2001-2005). The historic molding of the practice of citizenship in Brazil imposed an anti-participation of the popular classes and of the different social subjects including the political class and the civil society that has been facing a new form of administration that have adopted social participation and direct democracy as the basic primacy; as well as how to understand up to which point the Participative Budget has contributed to social participation, local authority and a health system with a greater resolubility. The study is of qualitative nature in a historical-social perspective. The working up process of the Participative Budget includes various social subjects (govemment, social movements and legislative power) with educational matrixes of political, social, cultural and ideological meaning that made possible the dialogue between the subjects and the object of the study. The semi-structured interview was used so to permit a greater freedom for the interviewer as well as the person interviewed in order to contribute to the development of the proposed theme through its form of leaming the reality it lives in. The participative observation of the elaboration, execution and evaluation of the Participative Budget and the consulting of documents that portray its implementation had a complementary character. The treatment of the empiric material had as basis the contents analysis, taking into account the multiple dimensions of the investigative process, in the description, explanation and comprehension of the study in focus. The study reveals that the Participative Budget is build as a pedagogical instrument of citizenship of social control and local power upon the use of public resources. The Participative Budget conforms itself as a democratic and participative project with popular support, being implemented as of 2001 with advances and set backs in its process of formation with regard to social participation. There existed convergence among the people interviewed in the acknowledgment of the Participative Budget as a tool of social control of the municipal executive and legislative actions by the civil society and the formation of different subjects to perform citizenship. One of the consensual questions, for the advocates as well as the opponents of the Participative Budget, was the definition and election of an excessive number of priorities in 2001 and 2002. Their putting into practice would result in an expenditure of funds fairly above the investment capacity of the municipality. Hence it was not possible to define new priorities as long as the ones that had been defined previously were carried out, causing political wear to the Participative Budget members. Nevertheless the govemment nucleus showed understanding as to the claims of the people of Alagoinhas making possible a democratic discussion of the problems that are present in the local govemment. The survey identified that from 2001 on, the local govemment got the full administration of the health care system receiving direct funds from the Ministry of Health without the state intermediacy. This process represented not only a greater entrance of resources from the national fund into the municipal health fund, but also an increase in the counterpart of the local govemment in health investments. It was concluded that the Participative Budget is a process of participative construction and of citizenship permeated by conf1icts and contradictions in the service of the social necessities of a local reality. / Esta disserta??o busca analisar o processo de implementa??o do or?amento participativo no munic?pio de Alagoinhas, entre 2001-2005. Considerando o hist?rico cerceamento do exerc?cio da cidadania no Brasil, que imp?s uma antiparticipa??o das camadas populares, aqui se prop?e investigar, como os diferentes sujeitos sociais, incluindo a Classe Pol?tica e a Sociedade Civil, se confrontaram ante uma nova forma de gest?o que tinha a Participa??o Social e a democracia direta como primado b?sico. Assim como compreender at? ponto o or?amento participativo (OP) contribuiu para ampliar a Participa??o Social, o Poder Local, um sistema de sa?de com maior resolubilidade para a popula??o e o exerc?cio da cidadania plena conquistando melhor qualidade de vida para os mun?cipes alagoinhenses. Toma como pressupostos te?ricos b?sicos a conforma??o social do poder pol?tico, econ?mico, cultural e ideol?gico no espa?o local e nacional que buscou historicamente dificultar de todas as formas o exerc?cio da cidadania pelas camadas populares e a implementa??o do OP, como um instrumento de mobiliza??o da sociedade civil, o que contribuiu para ampliar a Participa??o Social e o Poder Local no processo de constru??o da cidadania. Esse foi permeado por conflitos e contradi??es, pela complexidade das demandas e necessidades sociais apresentadas pelos diferentes sujeitos sociais (gestores, trabalhadores, usu?rios, lideran?as comunit?rias, e opositores) no cen?rio local. Tratava-se de um estudo de natureza qualitativa, em uma perspectiva hist?rico-social, cr?tica e reflexiva. Ao analisar o processo de elabora??o do OP , incluindo diferentes sujeitos sociais (Governo, Movimentos Sociais e Poder Legislativo) com diferentes matrizes formativas de cunho pol?tico, econ?mico, social, cultural e ideol?gico que possibilitou interlocu??es entre os sujeitos e objeto de estudo, exigindo assim uma abordagem cr?tica que buscasse em determinado lugar e tempo hist?rico o fundamento do processo. Optou-se pela entrevista semi-estruturada, por permitir maior liberdade tanto ao pesquisador, quanto ao entrevistado, de contribuir para o desenvolvimento do tema proposto, atrav?s da sua forma de apreender a realidade em que vive. A observa??o participante da elabora??o, execu??o e avalia??o do OP e a consulta aos documentos que retrataram a sua implementa??o tiveram car?ter complementar. Foi feitas a an?lise vertical e horizontal dos dados coletados pelas entrevistas, e triangula??o com a observa??o participante e os documentos, contemplando as m?ltiplas dimens?es do processo investigativo, na descri??o, explica??o e compreens?o do foco em estudo. Teve-se como objetivo analisar o processo de implementa??o do OP em Alagoinhas-BA, no per?odo 2001/2005, discutir a participa??o dos diferentes sujeitos sociais (Classe Pol?tica e Sociedade Civil), na constru??o do OP como instrumento pedag?gico de cidadania, e apontar as articula??es entre as demandas da sa?de e o OP. Se no in?cio prevaleceu interesse corporativo na defini??o de prioridades de cada comunidade, posteriormente os aspectos coletivos foram mais valorizados. O governo foi "blindado" pelo trabalho do OP, ao se conseguir aprovar 84 dos 86 projetos enviados ? C?mara Municipal que fora contr?ria ao processo durante 2001/2004. O OP representou um caminho para as camadas populares conquistarem importante espa?o no Pode Local. O OP demonstrou ser um instrumento pedag?gico de cidadania. Contudo, urge maior investimento na qualifica??o dos conselheiros do OP .

Previous issue date: 2008-10-31 / This study addresses the interdisciplinary training in perspective for the Brazilian Health System (SUS) in view of graduation students in the areas of health of Federal University of Rio Grande do Norte s (UFRN) former students of the course Health and Citizenship (SACI). Emphasizes also the importance of commitment to social policies, particularly with those focused on the area of health. This is a case study with a qualitative approach. There was the lifting of data through documental research, from 14 portfolios of learning, which are weekly records of students from the respective discipline, corresponding to 2005.2 to 2007.2 semesters. From the analysis undertaken, inferred that the methodology problematizing, used by the discipline, enables students to understand that learning is not restricted to the confined university walls. It shows the health from a complex and concrete social reality, allowing students to an interdisciplinary dialogue in search of the transformation of this reality. It means an opportunity to interact with the dynamics of society in their area of activities, developing a relationship of solidarity in the formation of the citizen. Moreover, it was clear the direction of experienced interdisciplinary and recognized by students of various professions that make up the discipline. Thus, the SACI in preparation for construction of SUS, helps to form a new professional, more committed to the promotion and with a collective work in health / O presente estudo aborda a interdisciplinaridade na perspectiva da forma??o para o Sistema ?nico de Sa?de (SUS) na ?tica de estudantes da ?rea da sa?de da Universidade Federal do Rio Grande do Norte (UFRN) egressos da disciplina Sa?de e Cidadania (SACI). Ressalta, igualmente, a import?ncia do compromisso com as pol?ticas sociais e, em particular, com aquelas voltadas para a ?rea da sa?de. Trata-se de um estudo de caso com abordagem qualitativa. Realizou-se o levantamento dos dados atrav?s de pesquisa documental, a partir de 14 portf?lios de aprendizagem, que s?o registros semanais dos estudantes da respectiva disciplina, correspondendo aos semestres 2005.2 a 2007.2. Da an?lise empreendida, inferiu-se que a metodologia problematizadora, utilizada pela referida disciplina, possibilita aos estudantes compreender que a aprendizagem n?o se limita aos restritos muros universit?rios. Ela situa a sa?de partindo de uma realidade social concreta e complexa possibilitando aos estudantes um di?logo interdisciplinar na busca da transforma??o desta realidade. Significa uma oportunidade de interagir com a din?mica da sociedade no seu espa?o de a??es, desenvolvendo uma rela??o de solidariedade na forma??o do cidad?o.
