An investigation of the uptake of voluntary counselling and testing by adolescents aged 15 to 19 at two integrated youth friendly service sites in Gauteng, South Africa

Voluntary Counselling and Testing (VCT) is at the core of HIV and AIDS prevention, treatment and care programmes. Timely access to treatment and related services requires knowledge of one‟s HIV status. Adolescents are at the epicentre of the HIV and AIDS pandemic. This study investigated the factors that influence adolescents‟ decisions to actively seek VCT. Semi-structured, in-depth individual interviews were conducted with twelve adolescents aged 15 to 19 who were conveniently selected to participate in the study. The study found that adolescents have extensive knowledge regarding the benefits of VCT and acknowledge the need to test for HIV. However, fear of the consequences of testing positive for HIV such as stigmatisation by family and the community, inability to influence their partners to take an HIV test and the inability to make their own decisions and follow through on them still hinder their maximum uptake of VCT. / Sociology / M.A. (Social Behaviour Studies in HIV/AIDS

Strategies for holistic health support of men in polygynous relationships

The study has two separate but related aims, which are to explore the social and health experiences of men who are in polygynous relationships and to develop holistic health and social strategies to support men who are in polygynous relationships. Objectives of the study were to identify the different permutations of polygynous relationships that related to health and social issues; to clarify the social status of men who are in polygynous relationship; to explore and describe the health experiences of men in polygynous relationship; to explore and describe the social experiences of men in polygynous relationship; to explore the spiritual experiences of men who are in a polygynous relationship; to describe and to generate holistic strategies to support men who are in a polygynous relationships. Method: In this study, the researcher used descriptive and interpretive phenomenological processes to develop a range of holistic strategies to support men who were in polygynous relationships. The researcher described the experiences and developed interpretations based on the lived experiences that the men reported. Findings: Polygyny remains the reality within the Bapedi tribe in Sekhukhune area. The practice of polygyny is not only for the affluent as indicated in most literature but is also practiced as a corrective strategy for families who are experiencing marital problems. Polygyny is not viewed as abusive to the women and children but rather beneficial to the women. It helps to reduce the risks of the development of cervical cancer that is predisposed by frequency of sexual intercourse. Polygyny promotes the morals within the communities, as children grow up within the two parents’ environment as opposed to the rising numbers of single parent families. Polygyny practice in the Sekhukhune areas is a voluntary choice and not a forced marital arrangement. Conclusions and recommendations: The researcher recommended policy guidance to support health practitioners with strategies to assist members of polygynous families in need of help. The experiences of HIV sero-discordant couples at the Perinatal HIV Research Unit in Soweto, Gauteng Province

This qualitative exploratory and descriptive study explored and described the experiences of HIV sero-discordant couples post diagnosis at the Perinatal HIV Research Unit in Soweto, Gauteng Province. Data was collected using in-depth interviews with each individual from seven HIV sero-discordant couples. Following thematic analysis, four themes emerged: immediate response to HIV sero-discordant results; challenges in relation to disclosure of HIV sero-status; limited information on HIV sero-discordant; and the impact of HIV sero-discordant on the couples. All these were mainly linked to poor counselling and inadequate support to HIV sero-discordant couples. The findings of this study have both clinical and policy development implications. Recommendations have been put forward for development of contextual relevant HIV Sero-discordant Couple Counselling and support guidelines focusing of enhancing knowledge and skills of health care professionals responsible for counselling and supporting HIV sero-discordant couples. / Health Studies / MA (Public Health)

Factors influencing the adolescent pregnancy rate in the Greater Giyani Municipality, Limpopo Province

This quantitative, explorative and descriptive survey attempted to determine factors that influence the adolescent pregnancy rate in the Greater Giyani Municipality. Data were gathered from adolescent girls attending four selected high schools. Non-probability convenient sample of 147 respondents was used with 100% return rate. Data was collected using a questionnaire which had a reliability of 0.65. Data were analysed using the SAS/Basic computer program, version 9.2. Findings indicated that 56.34% of respondents reported key psychosocial variables such as peer pressure and 58.90% of them changed values as contributory to high pregnancy rates. .Health services were reported as not freely available and relationships with nurses significantly cited as poor by 72.41% respondents with regard to maintenance of confidentiality. Needs of poverty-stricken families : perspectives from adult members

Many families have been deprived access to basic human needs like food, clothing, shelter, protection, education and health services because of poverty. In South Africa poverty remains a challenge, resulting from economic and social inequalities, where 40% of its population still live in poverty (Landman, Bhorat, Van der Berg & Van Aardt 2003). The study aimed at ascertaining and describing the needs identified by adult members living in poverty, in the Lukhanji Municipality. The researcher used a probability, systematic, random sampling method to obtain respondents for the study. A non-experimental, descriptive, quantitative research approach was adopted. A questionnaire was used to collect data from 150 respondents, after which a numerical data analysis was done with the assistance of a statistician. Unemployment could be ascribed to lack of education and skills, which exposed families to poor living conditions, ill-health, insecurity and other social ills. An investigation into young women workers' experiences of the HIV and AIDS response of small and medium sized enterprises in a semi urban area in KwaZulu-Natal

Young women working in SMEs are increasingly being affected by HIV and AIDS, and SMEs are expected to respond to HIV and AIDS through workplace policies and programmes. This requires commitment from SMEs to help young women who face various gendered vulnerabilities to HIV and AIDs. The study, whose purpose was to investigate young women workers’ experiences of the HIV and AIDS response of small and medium enterprises in a semi urban area in KwaZulu-Natal, used a qualitative approach and collected data from three SMEs in Pietermaritzburg during the period of the study. Findings of the study indicate that the majority of respondents from all three SMEs agree on experiencing limited or no HIV and AIDS policies and programmes in SMEs. With reference to incomplete or limited responses, two SMEs provide financial assistance for funerals. However, some aspects such as education and awareness, monitoring, management commitment, provision of medical aid, facilitation of peer education, appointment of an HIV officer and more were not evident. Factors encouraging or discouraging men in the informal sector to attend HIV counselling and testing (HCT) in South Africa : a case study of Pretoria

In English / The purpose of this study was to establish factors affecting the participation of men working in the informal sector economy of South Africa in HIV counselling and testing (HCT) or voluntary counselling and testing (VCT). This includes the exploration of reasons for acceptance and non-acceptance of HIV testing, knowledge and perceptions about HIV testing and behaviours and attitudes towards HIV testing. Data was collected through a structured questionnaire and a sample of fifty (50) men was purposively selected. It was found that the reasons for acceptance of HIV testing are satisfactory and reasons for non-acceptance are still a concern, and that several issues such as education about HIV testing and HIV in general still need to be addressed. Comparison of packed school lunches of boys and girls in primary schools in East London

Submitted in fulfilment of the requirements for the degree Master of Applied Science in Food and Nutrition, Durban University of Technology, 2015. / Objective: To determine the contribution of packed school lunches to the daily food intake of girls and boys in two Primary Schools in East London. Methods: The study was conducted among 199 girls and boys aged 9-13 years. Three 24-Hour recalls and a Food Frequency Questionnaire were completed during an interview with the participants to gather data on dietary patterns over a period of three consecutive days. Additionally, the contents of one lunch box per participant were recorded and weighed. Anthropometrics and socio-demographics were also completed during the interview. Results: The three 24-Hour Recall nutrient measurements revealed a low energy intake in 91 percent for the girls and 77 percent for the boys who were consuming below the recommended Estimated Energy Requirement for energy. The lunchboxes contributed one-third of the daily nutrient intake of the children. The 24-Hour recall revealed an energy-dense, carbohydrate-based diet. The contribution of total fat (30-32%) to the total energy is higher than the World Health Organization (WHO) recommendation of 15-30 percent. The daily fruit and vegetable intake (215.1g and 216.9g), according to the 24-Hour recall and lunchbox analysis respectively, was insufficient compared to the WHO-recommendation of >400g /day. Although the mean intake of most of the nutrients was sufficient, a large number of the participants did not meet requirements for the age group. The risk of overweight was high (24% for girls and 29.2% for boys) with 1.5 percent falling into the obese category. Conclusion: The results of the study indicated a high-fat and carbohydrate intake and a very low fruit and vegetable intake. The girl participants had better food choices for the lunchboxes but the majority of the participant’s daily intake did not meet the basic requirements of a balanced diet. The risk of overweight in the age category is an increasing problem among low- and high-income countries. Nutritional education should concentrate on healthy food choices in school lunchboxes as a large part of the day is spent at school.

Challenges of implementing HIV voluntary counselling and testing (VCT) campaigns for higher education distance learning students : case study of UNISA-Sunnyside Regional Office

This study investigated the challenges faced by implementers of VCT campaigns for higher education distance learning students. Qualitative and explorative approaches, using a case study, were employed as the research methodology. It has been concluded that the administrative planning procedures of VCT campaigns were not properly followed, which resulted in the following difficulties: defining the roles and responsibilities of stakeholders, using limited resources, and the inability to reach all the students, in order to get them to actively participate in the campaigns. Lack of coordination of HIV and AIDS activities and the absence of monitoring and evaluation also impacted negatively on the success of VCT campaigns. This study recommends that VCT campaigns have a proper task team constituting of experts in VCT campaign operations, in order to strategically plan and coordinate all the campaigns' activities. HIV knowledge and sexual risk behaviour of grade 12 learners in the Cape Metropole, Cape Town

Thesis (MCurr)--Stellenbosch University, 2013. / ENGLISH ABSTRACT: The HIV pandemic threatens the social, emotional, and physical development of all persons, especially the youth. Adolescents are more at risk of contracting HIV as their lifestyle often involves sexual exploration and experimentation. Effective educational interventions are central to HIV prevention in South Africa. Being a clinical nurse practitioner in a primary health care (PHC) facility, the principal investigator observed that school learners failed to practice safe sex and demonstrated little knowledge about HIV/AIDS prevention. The aim of the study was to investigate the reported level of HIV knowledge and sexual risk behaviour of grade 12 school learners in the Eastern Sub-District of the Cape Metropole, Cape Town. A descriptive, non-experimental, research design was employed with a primarily quantitative approach. The study population comprised grade 12 learners from high schools in the Eastern Sub-District of the Cape Metropole in Cape Town (N=7940). A total of 92 participants from four schools (2 public and 2 private) were included in the sample by using a cluster sampling method. A self-completion semi-structured questionnaire was used to collect the data. Data was collected by the principal investigator and a trained field worker. Ethical approval was obtained from the Health Research Ethics Committee of the Faculty of Health Sciences, Stellenbosch University (N11/07/225). Permission to conduct the research was obtained from the Department of Education. Reliability and validity were assured by means of a pilot study and the use of experts in the field of nursing research and statistics. Descriptive statistics were used to analyse data. Statistical associations were determined using ANOVA and the Mann-Whitney U tests. The qualitative data was analysed thematically and then quantified. The results show that the average HIV/AIDS knowledge score of participants was 60.73%. However, many gaps in HIV/AIDS knowledge were identified. Only 77.2% (n=71) of participants knew the meaning of HIV, 80.4% (n=74) did not know all the ways in which HIV can be transmitted and only 8.7% (n=8) knew how to safely use a condom. The majority of participants (67.4%; n=62) believed in the myth that HIV can be cured and 18.5% (n=17) reported that a traditional healer can cure HIV. With regard to risky behaviour, half of the participants at the time of the study (51%; n=47) reported sexual engagement and 20% (n=9) of these respondents did not use condoms. Furthermore, 25% (n=23) had used alcohol before having sex. There were no association found between the knowledge about HIV/AIDS of participants and their sexual risk behaviour. In view of these study findings, participants are exposing themselves to high risk sexual behaviour that may increase their chances of acquiring sexually transmitted infections including HIV. Several recommendations were identified, including the strengthening of HIV and STI education linked to sexual risk reduction, open communication and additional information sources, availability of condoms at schools and improved access to HIV testing at schools. / AFRIKAANSE OPSOMMING: Die jeug se sosiale, emosionele en fisiese ontwikkeling word deur die MIV-pandemie gekortwiek. Die studie het dit ten doel om te bepaal wat die MIV-kennis vlakke, en die seksuele gedrag risiko van Graad 12 leerders in die Oostelike sub-distrik van die Kaapstadse Metropool is. ʼn Beskrywende, nie-eksperimentele navorsingsontwerp is gebruik, met ʼn hoofsaaklike kwantitatiewe benadering. Uit die studie populasie van Graad 12 leerders in die Oostelike sub-distrik van die Kaapstadse Metropool hoërskole (N=7940), is ʼn steekproef van 92 deelnemers uit vier hoërskole (twee staatskole en twee privaatskole) gekies – die trossteekproefnemingsmetode is gebruik. Data is versamel deur middel van ʼn semi-gestruktureerde vraelys wat deur die deelnemers self voltooi is. Toestemming vir die uitvoer van die studie is verkry van die Etiese Komitee van die Mediese Fakulteit van die Universiteit van Stellenbosch (N11/07/225), asook die Wes-Kaapse Departement van Onderwys. Die betroubaarheid en geldigheid van die studie is verseker deur die uitvoer van ʼn voorstudie, en is verder versterk deur gebruik te maak van kundiges in die veld van statistiek en verpleegnavorsing. Data is ontleed deur middel van beskrywende statistiese metodes en assosiasies is bepaal deur gebruik te maak van variansie-analise (“ANOVA”) en Mann-Whitney U toetse. Die bevindinge is in frekwensie tabelle en histogramme vervat. Die kwalitatiewe data is gekodeer en gekategoriseer, waarna temas geïdentifiseer is. Alhoewel die studie-bevindinge aangedui het dat die deelnemers ʼn gemiddelde MIV/VIGS-kennis telling van 60.73% behaal het, is verskeie leemtes in hulle bestaande kennis geïdentifiseer. Slegs 77.2% (n=71) van die deelnemers het geweet wat MIV beteken, terwyl 80.4% (n=74) nie geweet het hoe MIV oorgedra word nie. Slegs 8.7% (n=8) van die deelnemers het kennis gehad rakende veilige kondoom gebruik. Die meerderheid van die deelnemers (67.4%; n=62) glo dat MIV genees kan word en 18.5% (n=17) het aangedui dat MIV deur ʼn tradisionele geneesheer genees kan word. Hoë-risiko gedrag, spesifiek seksuele aktiwiteit (51%; n=47%) sonder kondome (20%; n=9) is rapporteer. ʼn Verdere 25% (n=23) van die deelnemers het rapporteer dat hulle alkohol gebruik voor seks, maar daar was geen assosiasie tussen die vlak van MIV/VIGS-kennis en hoë-risiko gedrag nie. Die bevindinge dui daarop dat die deelnemers hulself blootstel aan hoë-risiko seksuele gedrag met die gevolg dat hul kans om MIV te kry verhoog. Die aanbevelings, gegrond op die bevindinge, sluit in: ʼn groter fokus op onderrig wat verband hou met MIV en seksueel oordraagbare infeksies wat gekoppel is aan ʼn verlaging in hoe-risiko seksuele gedrag, openhartige kommunikasie en bykomende inligtingshulpbronne, beskikbaarheid van kondome by skole, asook verbeterde toegang tot MIV toetsing by skole.
