Community health promotion programs for seniors : program focus and contributing factors to composition

The purpose of this study is to investigate the program focus and contributing factors to program composition of five health promotion programs for seniors. The programs are selected using opportunistic sampling from five different local areas in metropolitan Vancouver. The five areas together constitute metropolitan Vancouver. A theoretical framework based on health promotion as a process which enables people to take control of their health promotion programming and recognizes that social, political, and organizational interventions are as important as individual actions, is used to support the purpose of this study. An ethnographic approach is used to collect observational, interview and documentary data on program focus, process and organization. The data are analyzed qualitatively to further the understanding of health promotion as a process central to individual and group empowerment in program focus and organization. The findings confirm that these programs focus predominantly on individual behaviour change efforts and only minimally on underlying environmental and community change factors. In the process of examining these health promotion programs for seniors, themes emerged which shed light on which factors most influence program composition. Program organization and process which involves multiple historical, theoretical and organizational factors are seen to most heavily influence program composition. / Graduate and Postdoctoral Studies / Graduate

A profile of the elderly admitted to the emergency unit of Groote Schuur Hospital : with particular reference to their health care needs

This study is the first of its kind undertaken at Groote Schuur Hospital. It is an attempt to provide a holistic profile of their elderly patients with a view to encouraging further, more specific research, and to provide information for use in the planning of efficient health care for the aged. The study was based on three premises: (i) there is an interrelationship between the ageing process and disease; (ii) a non-disease-specific approach which focusses on the functional status of elderly patients can be used as a predictor of health services consumption; and (iii) any study which promotes understanding of the dynamics of health care of the elderly must also take into account the ageing process and its effect on a particular population within a specific social context. The research spanned 52 weeks (1 March 1989 - 27 February 1990). A sample of nine patients per week was selected from the total population of patients aged 65 and over admitted to the Emergency Unit of Groote Schuur Hospital. Two adult female researchers, using structured questionnaires, constructed in English and comprising subtests, utilising indexes and scales, interviewed respondents and/or household members in their own homes. Data was also obtained from the hospital files. Although essentially descriptive by nature, use was made of groups in regard to variables such as "first admission" (admission to the Emergency Unit), and "readmission" (a previous overnight admission in the preceding year). Statistical analysis, where indicated, was by means of non-parametric tests.

O Programa Acompanhante de Idosos (PAI), política Pública do Município de São Paulo, é uma modalidade de atendimento domiciliar a idosos em situação de fragilidade e vulnerabilidade social. O acompanhante de idosos (ACI) é o trabalhador da equipe que com maior periodicidade realiza atividades no domicílio. A proposta desta pesquisa foi apreender diversos aspectos envolvidos no processo de trabalho dos ACIs do PAI, a partir da opinião dos mesmos. Caracteriza-se como estudo qualitativo, exploratório e descritivo que contou com a participação de onze ACIs, membros das primeiras equipes PAI contratadas pela Associação Saúde da Família, instituição parceira executora da Secretaria Municipal da Saúde de São Paulo. Para alcançar os objetivos propostos, a coleta de dados ocorreu com os ACIs por meio da realização de um grupo focal com quatro sessões, cuja discussão se embasou em um roteiro semi-estruturado. As sessões do grupo foram gravadas em áudio e seu conteúdo transcrito. Os dados foram analisados por meio da técnica de análise de conteúdo temática, a partir de categorias previamente definidas pelo referencial teórico da Psicodinâmica do Trabalho de Christophe Dejours, sendo elas: identidade, trabalho prescrito e trabalho real e inteligência prática. Os resultados indicaram que os participantes eram do gênero feminino, com idade entre 32 a 56 anos e a maioria não casados. Nenhum participante referiu ter realizado curso de cuidador antes de ingressar no PAI. Identificou-se que o ACI é uma nova categoria de trabalhadores, e o ineditismo de seu trabalho influencia diretamente na compreensão de sua função por eles mesmos, pelos idosos atendidos e seus familiares, por outros trabalhadores domiciliares contratados pelos idosos e pelos demais profissionais da equipe. Os ACIs consideraram que seu trabalho é dinâmico, o que faz com que se deparem na prática com demandas imprevistas, para as quais a prescrição se mostra insuficiente para respondê-las. Em decorrência do hiato entre seus trabalhos prescrito e real, estes trabalhadores realizam ações não previstas em sua prescrição, o que, apesar de implicar em riscos por eles assumidos, favorece o aperfeiçoamento do próprio trabalho. Os resultados mostraram aspectos que interferem no exercício do trabalho dos ACIs. Estes dados poderão constituir-se como subsídio para as equipes no apoio a estes trabalhadores, bem como para o aprimoramento do PAI / The Programa Acompanhante de Idosos (PAI), public policy of São Paulo city, is a modality of home care for elderly in a fragile and vulnerable social situation. The elderly companion is the team worker who performs activities more frequently at the elderly\'s. The purpose of this research was to understand various aspects involved in the work process of the elderly companion of PAI according to their own opinion. It is characterized as an exploratory descriptive qualitative study with the participation of eleven elderly companions, members of the first PAI teams contracted by Associação Saúde da Família, executing partner institution of Secretaria Municipal da Saúde de São Paulo. In order to achieve the proposed objectives, the data collection with the elderly companions occurred in a four-session focal group where the discussion was supported by a semi-structured guide. The group sessions were recorded and its content transcribed. The data were analyzed through thematic content analysis based on categories previously defined by the theoretical approach of Psychodynamics of Work of Christophe Dejours, those were identity, prescribed work and actual work and practical intelligence. The results indicated that the participants were female, between 32 and 56 years old, and mostly not married. None of them mentioned having taken any caregiver course before joining PAI. The elderly companion was identified as a new worker category and the novelty of this work influences directly the understanding of their functions by themselves, by the attended elderly and their family members, by other elderly´s workers and by other team professionals. The elderly companions described their work as dynamic, what makes them face unexpected demands, for which the prescription was insufficient to provide. Due to the discrepancy between the prescribed work and the actual work, these workers perform actions that were unanticipated by their prescription. Even though this fact may carry risks for themselves, it contributes to the improvement of their own work. The results presented some aspects that interfere in the accomplishment of the elderly companion´s work. These data may help the elderly companions´ team to support these workers. It may also collaborate to the enrichment of PAI

IntroduÃÃo Consequente ao envelhecimento populacional, as mudanÃas no perfil sociodemogrÃfico e epidemiolÃgico constituem problema de saÃde pÃblica, ensejando a necessidade de criaÃÃo de polÃticas e prÃticas de saÃde voltadas para esta populaÃÃo. Fortaleza à uma cidade com dois milhÃes e meio de habitantes, com um percentual de 7,48% de idosos. Avaliar como a linha de cuidado do idoso ocorre nas redes assistenciais de saÃde de Fortaleza produz informaÃÃes que podem subsidiar a tomada de decisÃo por parte da gestÃo municipal. Objetivo Compreender como a linha de cuidado do idoso acontece nas cinco redes assistenciais de saÃde de Fortaleza, na percepÃÃo de gestores atuantes neste cenÃrio. Metodologia Estudo qualitativo, com abordagem descritiva, tÃcnica de grupos focais e, para organizaÃÃo dos achados, foi empregado o Discurso do Sujeito Coletivo (LEFÃVRE, LEFÃVRE, 2006). Resultados Na Rede Assistencial EstratÃgia SaÃde da FamÃlia, destacam-se as aÃÃes de promoÃÃo da saÃde e a introduÃÃo da caderneta do idoso como instrumento de trabalho. Na Rede Assistencial Especializada, foi constatada significativa demanda reprimida em razÃo da carÃncia de serviÃos especializados no cuidado com o idoso. Na Rede Assistencial Hospitalar, nÃo se observa um cuidado especÃfico para o idoso, uma vez que ele à visto como usuÃrio comum. Na Rede Assistencial de UrgÃncia e EmergÃncia, inclui-se como prioridade a remoÃÃo e, no hospital, o atendimento conforme a gravidade. Na Rede Assistencial SaÃde Mental, uma abordagem psicossocial com envolvimento da comunidade e articulaÃÃo intersetorial. ConsideraÃÃes finais Entre os principais achados deste estudo, tem-se que a linha de cuidado do idoso na cidade de Fortaleza ainda à frÃgil, onde alguns fios se desalinham, ou se rompem, comprometendo a continuidade do cuidado. Ante o estado de transiÃÃo demogrÃfica e o perfil demogrÃfico e epidemiolÃgico atual, observa-se a necessidade de melhor integraÃÃo das redes assistenciais, ancoradas na Rede Assistencial da EstratÃgia SaÃde da FamÃlia, bem como a estruturaÃÃo de novos serviÃos especializados, objetivando melhorar a assistÃncia da populaÃÃo idosa de Fortaleza. / Introduction As a result to the population aging, changes in demographic and epidemiological profile is public health problem, entailing the need to develop health policies and practices for the population. Fortaleza is a city of two and a half million inhabitants, of which 7.48% are elderly. The assessment of how the care for the elderly occurs in health assistance networks in Fortaleza produces information that can support the decision taken by the municipal government. Purpose Understanding how the way of caring for the elderly takes place in five health care networks in Fortaleza, in the perception of managers acting in this scenario. Methodology A qualitative study with descriptive approach and focus group technique. The Discourse of the Collective Subject was used to organize the findings (LEFÃVRE, LEFÃVRE, 2006). Results In the Family Health assistance network (so called EstratÃgia SaÃde da FamÃlia), the actions of health promotion and introduction of the recording book of the elderly as a working tool are highlighted. A significant unmet demand was observed due to the lack of specialized services in caring for the elderly in the Specialized assistance network. Specific care for the elderly was not observed in the Hospital assistance network, as he is seen as common user. In the Emergency assistance network, the removal is included as a priority, and in the hospital, the service applied is according to severity. In Mental Health assistance network, there is a psychosocial approach to community involvement and intersectorial coordination. Final considerations Among the main findings of this study is that the way of care for the elderly in the city of Fortaleza is still weak, where some strands are misaligned, or broken, compromising the continuity of service. Given the state of demographic transition and the demographic and epidemiological profile, there is a need for better integration of health care, anchored in the Family Health Strategy as well as the structuring of new specialized services, aiming to improve care for the elderly population of Fortaleza.
