Identification de facteurs de risque d'insuffisance rénale en trauma

Contexte: la survenue d’IRA chez les patients ayant subi un traumatisme est une problématique qui a été peu étudiée jusqu’à ce jour. La présence de cette atteinte rénale a été démontrée comme étant associée à un risque accru de morbidités et de mortalité chez les sujets atteints. Objectifs: identifier les facteurs prédictifs d’insuffisance rénale ou plus récemment appelée atteinte rénale dans cette population particulière et tenter de trouver des facteurs qui peuvent être mesurés dans les premières heures de la prise en charge du patient. Aussi, nous avons cherché à savoir si l’injection de produit de contraste est associée à un risque accru d’insuffisance rénale aiguë dans cette population. Méthodes et résultats: la recherche a eu lieu à l’Hôpital du Sacré-Coeur de Montréal, un centre de traumatologie tertiaire en milieu urbain. Nous avons utilisé le registre des patients hospitalisés en traumatologie dans notre centre hospitalier entre 2002 et mars 2007 de même que les banques de données de laboratoire et de radiologie pour obtenir les données sur la créatinine et les examens avec produits de contraste. Finalement, une revue de dossiers structurée fut conduite pour recueillir le reste de l’information requise. L’incidence d’IRA dans la population étudiée est estimée à environ 5 %. Une analyse cas témoins fut conduite pour identifier les facteurs prédictifs d’IRA. Quarante-neuf cas d’IRA diagnostiqués par le médecin traitant et 101 témoins sélectionnés au hasard ont été analysés. Les facteurs prédictifs suivants ont été identifiés à l’analyse univariée : la première valeur de créatinine obtenue (p<0,001), l’instabilité hémodynamique (p<0,001), les antécédents d’insuffisance rénale chronique tels que notés dans le dossier par le médecin traitant (p=0,009), une maladie cardiaque (p=0,007), une chirurgie dans les 48 premières heures suivant le traumatisme (p=0,053), le niveau de gravité du traumatisme (Injury Severity Score) (p=0,046) et l’injection de produit de contraste au cours des 48 heures suivant le trauma (p=0,077). Parmi ces facteurs, deux ont été identifiés comme prédicteurs indépendants d’IRA à l’analyse multivariée. Une des valeurs était la première valeur de créatinine obtenue RC = 6,17 (p<0,001, IC95 % 2,81 – 13,53) pour chaque augmentation de 0.5mg/dL de créatinine. L’autre facteur était la présence d’instabilité hémodynamique RC 11,61 (p<0,001, IC95 % 3,71 – 36,29). Conclusion: des informations obtenues tôt dans la prise en charge du patient permettent de prédire le risque d’IRA chez ces patients. L’administration de contraste (intraveineuse ou intra-artérielle) ne s’est pas avérée un facteur indépendant de prédiction d’insuffisance rénale aiguë dans cette population dans le modèle multivarié. / Background: acute kidney injury (AKI) has important mortality and morbidity complications. Few studies have looked at predictors of acute renal failure in a trauma patient population. Objectives: we sought to identify factors associated with AKI that can be assessed in the early hospital stay of trauma patients. We also specifically assessed if the administration of radiological contrast was a predictor of AKI. Methods: we conducted a nested case-control study from the trauma registry of an urban Level I trauma center which includes data on more than 6 000 subjects. The cases consisted of 49 patients with a diagnosis of AKI by their treating physician in the first 7 days following their trauma between 2002 and 2007 (March 2007). The controls were randomly selected for a 1:2 case to control ratio. Data were retrieved from the prospective trauma registry database. Additional data were also obtained via the hospital laboratory and radiology databases. Finally, a structured chart review was conducted to obtain the remaining information. Univariate analyses were conducted. Elements with a significance level of <0.1 were included in a multivariate logistic regression model. Results: predictors identified in the univariate analysis were: the first creatinine value obtained (p<0,001), hemodynamic instability (p<0,001), history of coronary artery disease (p=0,007), history of chronic renal insufficiency as per physician’s diagnosis in the chart (p=0,009), surgery in the 48 hours following the trauma (p=0,053), and, injection of contrast in the 48 hours following the trauma (p=0,077). In the final multivariate model, two factors were statistically significant. One factor was the first creatinine value p<0,001, OR 6,17 CI95 % (2,81 – 13,53) for each increase of creatinine by 0,5mg/dL. The other factor was the presence of hemodynamic instability p<0,001 OR 11,61 CI95 % (3,71 – 36,29). Conclusion: easily obtained information in the emergency department can aid in predicting the risk of AKI in a trauma population. Early administration of radiological contrast was not an independant predictor of AKI in this population.

L'étude des mécanismes physiologiques au cours d'une plongée subaquatique estessentielle à la compréhension des accidents qui lui sont associées. Le plongeur subit denombreuses contraintes issues du milieu dans lequel il évolue. Parmi ces contraintes, qui leplus souvent se combinent, nous pouvons citer l'immersion, l'exposition au froid,l'hyperbarie, les variations des conditions d'oxygénation (hypoxie, hyperoxie) et l'exercicephysique. L'objectif de notre travail a été d'apprécier les modifications de la fonction cardiovasculaireet de son contrôle neuro-végétatif induites par ces contraintes rencontrées enplongée chez des sujets sains, novices en plongée, et également chez des chasseurs sousmarins. L'étude de la circulation artérielle et de la fonction endothéliale était basée surl'échographie (bi-dimensionnelle et Doppler). Des mesures de tonométrie d'applanation etautres mesures ultrasonores ont complété nos données hémodynamiques. L'étude du systèmeneurovégétatif était basée sur l'analyse de la variabilité de l'intervalle RR (contrôle cardiaque)et de la variabilité de la pression artérielle (contrôle vasculaire). Après un exercice aigu de pédalage de 45 minutes à haute intensité, l'augmentationpost-exercice du flux sanguin et de la vasodilatation endothéliale a été réduite aux musclesparticipant activement à l'exercice comparé aux muscles non actifs. Pour expliquer cesdifférences, la contribution de l'inflammation locale et du stress oxydatif élevé présents dansles muscles actifs de l'exercice pourrait avoir un rôle. / During a dive, subjects undergo environmental stressors such as immersion, coldexposure, hypoxia, hyperoxia and physical exercise. All these stressors may be responsible forchanges in cardiovascular system and consequently modified autonomic nervous control. Theaim of this work was to assess physiological changes induced by diving to better understandinjuries reported during this activity. Investigations were performed in healthy men and inspearfishermen. Hemodynamic changes and endothelial function were assessed by 2-Dimensional and Doppler echocardiography. Arterial wall compliance was estimated by pulsewave analysis. Autonomic nervous activity was assessed by power spectral density of heartrate variability (cardiac control) and blood pressure variability (vasomotor control). After an acute cycling exercise of 45 minutes in high intensity, the increase postexerciseof the blood flow and the endothéliale vasodilatation was reduced to musclesparticipating actively in the exercise compared with no active muscles. To explain thesedifferences, the contribution of the local inflammation and the important oxydative stress inthe active muscles of the exercise could have a role. Thermoneutral head-out water immersion induce hemodynamic and arterial changes. At rest, we observed a brachial arterial vasodilatation. This might be attributed to endothelialrequest inferred by increase of the peripheral arterial debit flow. Endothelial reactivity did notseem to be modified. With cycling exercise in low intensity,hemodynamic differencesobserved disappear. At rest, normobaric hyperoxia didn't affect blood pressure but induced an increase insystemic vascular resistances.

Terapia hemodinâmica guiada por saturação venosa central contínua em pacientes pediátricos submetidos à cirurgia cardíaca ensaio clínico randomizado / Hemodynamic therapy guided by continuous central venous oxygen saturation in pediatric patients undergoing cardiac surgery: a clinical randomized study

Introdução: A terapia hemodinâmica perioperatória baseada em metas modificou a evolução dos pacientes pediátricos cirúrgicos de alto risco, demonstrando ser eficaz na redução de complicações. O objetivo deste estudo foi avaliar o efeito de um protocolo perioperatório guiado por metas baseado na adequação da saturação venosa central mensurada de forma contínua na perfusão tecidual e nas complicações pós-operatórias de pacientes pediátricos submetidos a cirurgia cardíaca. Métodos: Estudo prospectivo, randomizado, e controlado, realizado no Instituto do Coração do Hospital das Clínicas da Faculdade de Medicina de Universidade de São Paulo(Incor-HCFMUSP). Pacientes pediátricos submetidos a cirurgia cardíaca eletiva foram randomizados para grupo terapia hemodinâmica baseado na monitorização da saturação venosa contínua pelo cateter PediaSat (grupo intervenção) ou para grupo padrão. O grupo intervenção foi tratado no intraoperatório e nas primeiras 24 horas de pós-operatório de acordo com um protocolo guiado por metas, baseado na adequação da saturação venosa central (SvcO2 >= 65% nas cardiopatias acianóticas e >= 55% nas cardiopatias cianóticas). O desfecho primário foi a depuração do lactato nas primeiras 24 horas de pós-operatório. Os desfechos secundários foram a ocorrência de complicações pós-operatórias durante a internação hospitalar (insuficiência renal, vasoplegia, baixo débito cardíaco, arritmia, infecção, sangramento e reoperação), tempo de ventilação mecânica, tempo de uso de inotrópicos e de vasopressores e tempo de internação na unidade de terapia intensiva e no hospital e mortalidade operatória. Também foram analisados dados hemodinâmicos e variáveis de perfusão tecidual dos pacientes no período perioperatório. Resultados: No período de julho de 2014 a março de 2016, 65 pacientes foram incluídos no estudo. O número de pacientes que apresentou depuração dos níveis de lactato maior que 30% foi semelhante nos dois grupos (60% no grupo padrão e 59,4% no grupo intervenção, P=0,919). Não houve diferenças em relação a quantidade de dobutamina ou de transfusão de hemácias entre os grupos durante o protocolo. Em relação aos desfechos secundários, o grupo intervenção apresentou menor tempo de ventilação mecânica em comparação como grupo padrão (24,59 h ± 19,69 vs. 85,63 h ± 86,54, P < 0.001), menor tempo de uso de vasopressor [0 h (0-0) vs. 0 h (0-54), P = 0,021], e menor tempo de internação na unidade de terapia intensiva [6 dias (4-10) vs. 8 dias (6-21), p = 0,030 ]. Não houve diferenças entre os grupos em relação aos outros desfechos. Conclusões: Em crianças submetidas a cirurgia cardíaca, uma estratégia de terapia hemodinâmica guiada tendo como alvo a adequação da saturação venosa central de oxigênio não teve impacto na depuração dos níveis de lactato em 24 horas. Entretanto, a estratégia de terapia hemodinâmica guiada foi associada a redução do tempo de ventilação mecânica, da necessidade de vasopressores e do tempo de internação na unidade de terapia intensiva / Introduction: Perioperative hemodynamic therapy through defined goals improves outcomes in high surgical risk patients. The aim of this study was to evaluate whether the use of a perioperative hemodynamic protocol based on the optimisation of the continuous oxygen central venous saturation reduces complications in children undergoing cardiac surgery. Methods: This was a randomized clinical study, performed at the Instituto do Coração do Hospital das Clínicas da Faculdade de Medicina de Universidade de São Paulo. Pediatric patients undergoing cardiac surgery were randomized to hemodynamic therapy based on continous monitorization of central venous saturation of oxgen through the PediaSat catheter (intervention group) or to usual care group. The intervention group during the intraoperative and in the first 24 hours of postoperative were submitted to a hemodynamic protocol of care based on the optimization of central venous saturation (SvcO2 >= 65% in acyanotic disorders and >= 55% in cyanotic disorders). The primary outcome was lactate clearance in 24 hours of postoperative. Secondary outcomes were postoperative complications during hospital stay (acute kidney injury, vasoplegic shock, low cardiac output syndrome, arrhythmia, infection, bleeding and reoperation), duration of mechanical ventilation, duration of inotropes and vasopressors, length of intensive care unit and hospital stay) and death. We also evaluated hemodynamic data and perfusion tissue markers in patients during perioperative care. Results: In the period of July 2014 to March 2016, 65 patients were included in the study. The number of patients presenting lactate clearance higher than 30% in 24 hours was not different between groups (60.6% in the usual care group and 59.4% in the intervention group, P=0.919). There was no difference between groups in the use of dobutamine and in the number of transfused red blood cells units during the protocol. The intervention group had a lower duration of mechanical ventilation when compared to usual care group (24.59 h ± 19.69 s. 85.63 h ± 86.54, P < 0.001). The intervention group had also lower duration of vasopressor therapy [0 h (0-0) vs. 0 h (0-54), P = 0,021], and reduced length of intensive care unit stay [6 days (4-10) vs. 8 days (6-21), p = 0,030].There was no differences between groups in other outcomes. Conclusions: In pediatric patients undergoing cardiac surgery, a strategy of hemodynamic optimization aiming to adequate central venous saturation of oxygen had no influence in lactate clearance. However, hemodynamic therapy was associated with a lower duration of mechanical ventilation, less needing of vasopressors and shorter length of intensive care unit stay

Hämodynamische Effekte einerImmunadsorption mit nachfolgender Immunglobulin-G-Substitution bei Patienten mit dilatativer Kardiomyopathie

Für die dilatative Kardiomyopathie gibt es bisher keine kausalen Therapiemöglichkeiten. Verschiedene kardiale Autoantikörper sind an der Pathogenese der Erkrankung beteiligt. Diese können durch eine Immunadsorptionsbehandlung entfernt werden. Das Ziel dieser Studie war es, die hämodynamischen Effekte einer Immunadsorption mit nachfolgender Immunglobulin-G-Substitution zu untersuchen. Dazu wurden 18 Patienten (NYHA III-IV, linksventrikuläre Ejektionsfraktion < 30 %) eingeschlossen und zufällig der Behandlungs- oder Kontrollgruppe zugeteilt. An drei aufeinanderfolgenden Tagen erfolgte eine Immunadsorptionsbehandlung. Abschließend wurde polyklonales Immunglobulin-G substituiert. Die Behandlung wurde insgesamt viermal in monatlichen Abständen durchgeführt. Die hämodynamischen Messungen erfolgten mit Hilfe eines Swan-Ganz Thermodilutionskatheters. Nach drei Monaten waren der Herzindex und der Schlagvolumenindex in der Behandlungsgruppe signifikant angestiegen. Der systemvaskuläre Widerstand wurde signifikant gesenkt. Außerdem wurde ein Anstieg der echokardiographischen linksventrikulären Ejektionsfraktion sowie eine Reduktion des linksventrikulären endsystolischen und enddiastolischen Durchmesser Index beobachtet. Die echokardiographischen Veränderungen waren auch im Verlauf nach einem halben Jahr stabil nachweisbar. Die günstige Beeinflussung der hämodynamischen Parameter wurde zudem von einer Verbesserung der klinischen Beschwerdesymptomatik - repräsentiert durch die Einteilung nach der NYHA-Klassifikation - begleitet. Hingegen wurden in der Kontrollgruppe für alle genannten Parameter keine signifikanten Veränderungen beobachtet. Die Effekte der Immunadsorption werden hauptsächlich auf die Elimination der Autoantikörpern zurückgeführt, wenngleich andere Einflüsse - wie ein Einfluss der Immunglobulinsubstitution oder anderer immunmodulatorischer Komponenten - nicht ganz ausgeschlossen werden können. Die Ergebnisse zeigen deutlich, dass Immunadsorptionsbehandlungen bei dilatativer Kardiomyopathie als Ergänzung zur medikamentösen Basistherapie zur Verbesserung der Herzfunktion und zur Linderung der Beschwerdesymptomatik beitragen. / There is no causal therapy for the treatment of dilated cardiomyopathy. Various cardiac antibodies have been detected. These antibodies are extractable by immunoadsorption. The aim of the study was to assess the hemodynamic effects of immunoadsorption and subsequent immunoglobulin G substitution. 18 patients with dilated cardiomyopathy (New York Heart Association III-IV, left ventricular ejection fraction < 30%) were randomly assigned either to the treatment group or to the control group. The patients were treated by immunoadsorption on three consecutive days. After the final session polyclonal immunoglobulin G was substituted. The procedure was repeated four times at one-month intervals. Hemodynamic mesurements were made by Swan-Ganz-catheterization. After three months cardiac index and stroke volume index increased significantly. Systemic vascular resistence decreased. Although, the echocardiographic ejection fraction increased, while there was a reduction of the left ventricular endsystolic and enddiastolic diameter index. The echocardiographic changes persisted after six months. The hemodynamic changes were accompanied by an improvement regarding the New York Heart Association classes in the treatment group. In contrast, all parameters did not change throughout the six months in the control group. Presumably the hemodynamic effects result from elimimation of cardiac antibodies. Although there may be an influence of different immunmodulatory components or of the immunoglobulin G substitution, too. In conclusion, Immunoadsorption and subsequent immunoglobulin G substitution improves cardiovascular function in addition to conventional drug therapy.

Functional ultrasound imaging (fUSi) to assess brain function in physiological and pathological conditions : application to stroke / Imagerie fonctionnelle par ultrason pour évaluer les fonction cérébrales en conditions physiologique et pathologique : application à l'AVC

Depuis le milieu du XXème siècle, les techniques d’imagerie fonctionnelles ont un rôle grandissant dans notre compréhension sur les fonctions du cerveau en conditions physiologique et pathologique. Bien que l’IRMf fasse partie des techniques les plus communément utilisées pour l’imagerie du cerveau complet lors d’études préclinique et clinique, cette modalité souffre de sa résolution spatiotemporelle et sa sensibilité pour enregistrer finement les fonctions et activités cérébrales. Récemment l’imagerie fonctionnelle par ultrason (ifUS) a subi des développements permettant d’être complémentaires à l’IRMf ainsi qu’aux autres techniques d’imagerie cérébrales classiquement employées. Contrairement aux ultrasons focalisés conventionnels, l’imagerie hémodynamique proposé par l’ifUS repose sur une illumination ultrasonore plane permettant la détection des globules rouges en mouvement et la mesure de leur vitesse dans les micro-vaisseaux cérébraux. De ce fait, l’ifUS est indirectement lié à l’activité cérébrale d’où l’importance d’une meilleure compréhension des mécanismes du couplage neuro-vasculaire liant l’activité neuronale et les variations cérébrales d’apport en sang. De plus, cette technique a le potentiel pour fournir des informations précises sur les processus de certaines pathologies à la fois sur des modèles précliniques et chez l’homme. Dans un premier temps, j’exposerais mes travaux sur les récents développements techniques permettant l’ifUS in vivo (i) en condition chronique, (ii) sur l’animal éveillé, libre de mouvement et effectuant une tache comportementale et (iii) des vaisseaux capillaires chez le rongeur et l’homme. Dans un second temps, je démontrerais que l’ifUS in vivo peut fournir des informations nouvelles sur des pathologies telles que l’accident vasculaire cérébrale. / Since the middle of the 20th century, functional imaging technologies are making an increasing impact on our understanding on brain functions in both physiological and pathological conditions. Even if fMRI is nowadays one of the most used tool for whole brain imaging in pre-clinical and clinical studies, it lacks sufficient spatiotemporal resolution and sensitivity to assess fine brain function and activity. Functional ultrasound imaging (fUSi) has been recently developed and presents a potential to complement fMRI and other existing brain imaging modalities. Contrary to conventional ultrasound using focus beams, fUSi relies on hemodynamic imaging based on ultrasound plane-wave illumination to detect red blood cells movement and velocity in brain micro-vessels. Consequently, the fUSi signal is indirectly related to brain activity and it is therefore important to better understand the mechanisms of the neurovascular coupling linking neural activity and cerebral blood changes. Here again, fUSi may provide relevant information about disease processes in preclinical models but also in humans. First, I will present recent technical developments allowing in vivo fUSi (i) in chronic condition, (ii) in freely moving and behaving rats and (iii) in rodents and human brain capillaries. Second, I will demonstrate how fUSi could provide new insights in brain pathologies such as stroke.

Efeitos hemodinâmicos de duas estratégias de ventilação mecânica protetora em um modelo suíno de Síndrome do Desconforto Respiratório Agudo / Hemodynamic effects of two strategies of protective ventilation in porcine model of acute respiratory distress syndrome

A Síndrome do Desconforto Respiratório Agudo (SDRA) cursa com insuficiência respiratória e necessidade de ventilação mecânica invasiva (VMI). A mortalidade desta síndrome é elevada, mas pode ser reduzida com o uso de estratégias protetoras de ventilação mecânica que usam baixo de volume corrente (VC), geralmente 6ml/Kg ou menos, para minimizar a lesão causada pelo ventilador. Algumas destas estratégias de ventilação protetora utilizam valores elevados de PEEP que podem produzir instabilidade hemodinâmica. O objetivo deste estudo foi comparar, em um modelo experimental suíno de SDRA, os efeitos hemodinâmicos causados por duas estratégias de ventilação mecânica protetora: uma que utiliza PEEP ajustado pela Tomografia de Impedância Elétrica (OLA) e a outra, a mais comumente utilizada na prática clínica (ARDSNET), em que a PEEP é ajustada através de uma tabela. Foram utilizados 17 animais após randomização (8 no grupo OLA; 9 no grupo ARDSNET) que foram ventilados por 42 horas. As variáveis foram analisadas utilizando ANOVA de medidas repetidas. Antes da radomização, os grupos não apresentavam diferenças nos parâmetros hemodinâmicos e respiratórios. Considerando o período de 42 horas de estudo: 1) o grupo OLA utilizou valores maiores de PEEP, obteve uma melhor oxigenação, mas com valores de PaCO2 mais elevado; não houve diferença entre os grupos para o platô de pressão, mas o delta de pressão foi menor no grupo OLA o que pode minimizar o risco de lesão pulmonar induzida pelo ventilador; 2) o grupo OLA apresentou valores de índice cardíaco (IC) menores durante o estudo, mas não houve um comprometimento aos órgãos alvos conforme pode ser constatado pelos valores normais do lactato arterial e pelos valores de saturação do sangue venoso misto maiores do que no grupo ARDSNET; 3) pela análise de regressão múltipla pode se observar que o aumento da PEEP esteve associado com aumento da pressão média de artéria pulmonar (PMAP), mas redução do IC e da pressão arterial média, enquanto que o aumento da PaCO2 esteve associado com um aumento da PMAP e do IC. Concluímos que uma estratégia ventilatória protetora para SDRA cujo ajuste de PEEP é guiado por TIE, quando comparada com outra estratégia protetora comumente utilizada na prática clínica por um período de 42 horas, determina uso de valores maiores de PEEP, o que melhora a oxigenação e minimiza o delta de pressão, mas causa redução do IC que não resulta em isquemia tecidual. / The Acute Respiratory Distress Syndrome (ARDS) is associated with respiratory failure and need for invasive mechanical ventilation (IMV). The mortality of this syndrome is high, but can be reduced with the use of protective strategies of mechanical ventilation using low tidal volume (VT), usually 6ml/Kg or less, to minimize the damage caused by the ventilator. Some of these strategies of protective ventilation use high levels of PEEP that can produce hemodynamic instability. The aim of this study was to compare, in an experimental pig model of ARDS, the hemodynamic effects caused by two strategies of protective ventilation: one strategy adjusts PEEP values using Electrical Impedance Tomography (EIT-OLA) and the other, the most commonly used in clinical practice (ARDSNET), in which PEEP is adjusted through a table. Seventeen animals were randomized (8 in OLA group; 9 in ARDSNET group) that were ventilated for 42 hours. The variables were analyzed using repeated measures ANOVA. Before randomization, the groups had no difference in hemodynamic and respiratory parameters. Considering the period of 42 hours of study: 1) OLA group used higher values of PEEP, had a better oxygenation but with higher values of PaCO2; there was no difference between groups for the plateau pressure, but the delta pressure was lower in OLA group which can minimize the risk for ventilator-induced lung injury; 2) the OLA group showed lower values of cardiac index (CI) during the study, but there was no damage to target organs as showed by normal blood lactate values and values of mixed venous blood saturation greater than in ARDSNET; 3) multiple regression analysis revealed that, increasing PEEP was associated with increasing in mean pulmonary artery pressure (MPAP), but reduction CI and mean arterial pressure, while increasing in PaCO2 was associated with an increase in MPAP and CI. We conclude that a protective ventilatory strategy for ARDS whose setting of PEEP is guided by EIT, when compared with other protective strategy commonly used in clinical practice for a period of 42 hours requires the use of higher values of PEEP, which improves oxygenation and minimizes delta pressure, but causes reduction of CI that does not result in tissue ischemia.

Devido às dificuldades encontradas na Medicina Veterinária com relação à circulação extracorpórea, despertou-se o interesse pela técnica de parada circulatória total (\"Inflow Occlusion\") para correção cirúrgica de algumas patologias cardíacas, principalmente estenose de valvas semi-lunares. O objetivo deste trabalho consistiu em comparar as possíveis alterações hemodinâmicas que podem ocorrer em cães submetidos à parada circulatória total através da técnica do \"Inflow Occlusion\" por diferentes períodos de tempo. Utilizou-se neste estudo, doze cães, subdivididos em dois grupos, sendo submetidos à parada circulatória total, por sete ou oito minutos. Realizou-se análise hemodinâmica durante quatro momentos do período transoperatório. Os parâmetros de oxigenação foram obtidos por meio da conjunção dos dados fornecidos pelo cateter de artéria pulmonar e pela análise hemogasométrica. Diferença estatisticamente significativa foi encontrada na comparação entre os grupos avaliados, cinco minutos após a restituição da circulação (momento M3), para a pressão arterial média (PAM) e para o índice de trabalho sistólico do ventrículo direito (ITSVD) e do ventrículo esquerdo (ITSVE). Ocorreram dois óbitos transoperatórios no grupo de animais submetidos à oito minutos de parada circulatória total, devido a fibrilação ventricular. Frente aos óbitos ocorridos, conclui-se que o \"Inflow Occlusion\" foi exequível em cães sadios por sete minutos, sendo contra-indicado para períodos de oito minutos. Entretanto, estudos clínicos envolvendo a técnica em questão, precisam ser realizados para que a referida técnica possa ser aplicada, seguramente, em animais portadores de disfunções cardiovasculares. / Because of difficulties related to extracorporeal circulation in Veterinary Medicine, more interest occurred about circulatory arrest trough \"Inflow Occlusion\" technique to correct surgically some cardiac pathologies, such as semilunar valves stenosis. The aim of this study was to compare possible hemodynamic alterations that can occur in dogs submitted to circulatory arrest using \"Inflow Occlusion\" technique for different periods of time. Twelve mongrel dogs were used in this study divided in two groups, each one submitted to seven or eight minutes of total circulatory arrest. Hemodynamic evaluation was realized during four moments on transoperatory period. Oxygenation parameters were obtained trough pulmonary artery catheter data and hemogasometric analysis. Statistical difference between groups were found on M3 moment, five minutes after circulatory arrest, for mean arterial pressure (MAP), right ventricular systolic stroke index (RVSSI) and left ventricular systolic stroke index (LVSSI). There were two transoperatory deaths on group B, because ventricular fibrillation. Because these deaths, we can say that \"Inflow Occlusion\" can be realized up to seven minutes on healthy dogs and is contra-indicated for periods of eight minutes. However, clinical studies should be performed in order to use this technique routinely with safe in dogs presenting cardiovascular abnormalities.

Introdução: O uso de pressão positiva contínua em vias aéreas (CPAP) causa modificações hemodinâmicas favoráveis em pacientes com insuficiência cardíaca. Na presença de disfunção ventricular esquerda, mecanismos hemodinâmicos propostos associam seu uso ao aumento do débito cardíaco, através da redução do enchimento e da pressão transmural do ventrículo esquerdo. Estes efeitos talvez colaborem para o sucesso dessa terapia em pacientes com congestão pulmonar. Entretanto, até onde extensa revisão bibliográfica nos permite afirmar, não foram estudados os efeitos hemodinâmicos do CPAP em pacientes com estenose mitral, etiologia comum de congestão pulmonar em alguns países em desenvolvimento. Objetivos: Avaliar a resposta hemodinâmica dos pacientes portadores de estenose mitral sintomática durante o uso de CPAP. Métodos: Foram incluídos quarenta pacientes portadores de estenose mitral. Os critérios de exclusão foram: presença de disfunção ventricular esquerda, de outra valvopatia de grau moderado ou importante, de trombo intracavitário ao ecodopplercardiograma e de doença pulmonar obstrutiva crônica. Os pacientes foram avaliados em 3 condições diferentes: 1. condição basal, realizada ar ambiente; 2. uso de CPAP 7 cm H2O e 3. uso de CPAP 14 cm H2O. Foi realizada a medida de pressão arterial invasiva (aorta) e utilizado cateter de artéria pulmonar para a medida da pressão arterial pulmonar e débito cardíaco. Foram coletas amostras de gasometria arterial e venosa central. A variação percentual do volume sistólico (deltaVS) foi calculada tendo o valor na condição basal como 100% e foi correlacionada com as características clínicas, ecodopplercardiográficas, hemodinâmicas e gasométricas da condição basal. Os pacientes foram classificados em 3 grupos: grupo I - deltaVS <= -10%, grupo II - deltaVS entre -9,9% e +9,9% e grupo III - deltaVS >= +10%. Resultados: Durante a aplicação do CPAP, freqüência cardíaca, pressão arterial sistêmica e pressão arterial pulmonar não variaram significativamente. Na segunda condição, a deltaVS se correlacionou com a pressão arterial pulmonar diastólica e média, pressão de oclusão de capilar pulmonar e gradiente transvalvar mitral ao cateterismo. A análise dos grupos não identificou qualquer variável capaz de distingui-los. No uso de CPAP 14cmH2O, a deltaVS se correlacionou positivamente com a classe funcional, pressão sistólica do ventrículo direito (ao ecodopplercardiograma), freqüência cardíaca, pressão arterial sistêmica sistólica, pressão arterial pulmonar sistólica, diastólica e média, pressão de oclusão da artéria pulmonar e gradiente transvalvar mitral ao cateterismo. Além disso, nesta condição, a deltaVS se correlacionou inversamente com o volume sistólico indexado e saturação venosa de oxigênio. A análise comparativa dos grupos evidenciou, no grupo III, valores significativamente superiores de pressão sistólica do ventrículo direito (ao ecodopplercardiograma), pressão arterial pulmonar sistólica, diastólica e média, pressão de oclusão da artéria pulmonar e gradiente transvalvar mitral ao cateterismo. Não houve variável capaz de distinguir o grupo I do II. Conclusão: O uso de CPAP aumentou o volume sistólico nos pacientes com estenose mitral de maior impacto clínico e hemodinâmico, caracterizados por maior limitação funcional, hipertensão pulmonar e gradiente transvalvar mitral e menor saturação venosa de oxigênio e volume sistólico indexado. / Background: Continuous positive airway pressure (CPAP) impacts hemodynamic changes in heart failure patients by impending cardiac filling and reduces cardiac afterload by reducing left ventricular transmural pressure. These effects may collaborate for the success of this therapy in patients with pulmonary congestion. However, to your knowledge, no study has evaluated the hemodymanic effects of CPAP in patients with mitral stenosis, which is a common etiology of pulmonary congestion in some developing countries. Objectives: analyze the effects of CPAP on hemodynamic parameters in patients with symptomatic mitral stenosis. Methods: Forty patients with symptomatic mitral stenosis were included. They were monitored by aorta pressure and the insertion of a pulmonary artery catheter. Hemodynamic and gasometrical data were collected in each study period. Patients were evaluated in three study periods. Baseline condition was during spontaneous breathing. We applied CPAP 7 cm H2O in the second period and 14 cm H2O in the last period of the study. Were excluded patients with left ventricular systolic dysfunction, other valvopathy more than mild, and left atrial thrombi assessed by echocardiography and patients with history or symptom of chronic obstructive pulmonary disease. Percentage changes of stroke volume (deltaSV) were calculated with the values at baseline set as 100% and were correlated to clinical, ecocardiographic, hemodynamic and gasometrical data (at baseline). Patients were classified in group I if deltaSV was <= -10%, group II if deltaSV was between -9,9% and +9,9% or group III if deltaSV >= +10%. Results: During CPAP use, heart rate, systemic blood pressure and pulmonary artery pressure did not significantly change. At CPAP 7 cmH2O, deltaSV correlated positively with baseline values of diastolic pulmonary artery pressure, pulmonary artery occlusion pressure and mean transmitral gradient. There was no variable capable to distinguish the groups. At CPAP 14 cmH2O, deltaSV correlated positively with functional class, pulmonary artery systolic pressure (measured by echocardiography), heart rate, systolic blood pressure, systolic pulmonary artery pressure, diastolic pulmonary artery pressure, pulmonary artery occlusion pressure and mean transmitral gradient. Not only, at CPAP 14 cmH2O, deltaSV correlated inversely to indexed stroke volume and central venous saturation. Responders (group III) had superior pulmonary artery pressure (measured by echocardiography), systolic pulmonary artery pressure, diastolic pulmonary artery pressure, pulmonary artery occlusion pressure and mean transmitral gradient. The comparison between groups I and II did not evidence any variable capable to distinguish them. Conclusion: CPAP improved stroke volume in those with mitral stenosis with greater clinical and hemodynamic impact, characterized by higher functional limitation, pulmonary hypertension and mean transmitral gradient and lower central venous saturation and indexed stroke volume.

However, some techniques are still poorly studied in veterinary medicine due to difficulty in accessing specific nerves. The present study aimed at reviewing the thoracic anatomy, where TPB was conducted, determining values between reference points in order to implement this technique, assessing blockade’s quality and hemodynamic effects resulting from 2.5 mg/kg of bupivacaine 0.5% administration in dogs’ paravertebral space. Initially, structures involved when performing TPB were identified during an anatomical study in cadavers. In the second stage, eight healthy male or female dogs, mixed-breed, weighing 16.33 ± 4.04 kg, were submitted to TPB under general anesthesia with isoflurane. The blockage of thoracic T5, T6 and T7 nerves was performed with 2.5 mg/kg of bupivacaine 0.5%, guided by a neuro stimulator. Subsequently, the animals were anesthetized following the same protocol used in the previous stage for pulmonary-artery catheterization via the femoral vein, in order to assess hemodynamic effects of TPB. During TPB performance, the distances between the skin and the transverse process of thoracic vertebrae (STD) as well as the skin and paravertebral space (SPD) were obtained. Isoflurane supply was discontinued and analgesia evaluation was performed by pinprick test and hemostat pressure in conscious animals, after anesthesia recover. During hemodynamic evaluations the animals were kept anesthetized under spontaneous ventilation, while central venous pressure (CVP), cardiac output (CO), pulmonary artery pressure (PAP), pulmonary artery occlusion pressure (PAOP) and other cardiovascular variables were measured. Arterial and mixed venous blood were collected for blood gas analysis such as pH, oxygen partial pressure (PO2), carbon dioxide partial pressure (PCO2), bicarbonate (HCO3-), base excess (BE), anion gap (AG) and electrolytes such as sodium (Na+), potassium (K+), calcium (Ca2+) and chloride (Cl-), tissue oxygenation variables such as oxygen delivery (DO2), consumption (VO2) and oxygen extraction (OEF) were also calculated. Evaluations were made just before TPB (T0) and every 20 minutes in the next 80 minutes (T20, T40, T60, T80). Anatomical study revealed that some thoracic muscles’ innervation come from the brachial plexus. The average obtained for STD was 3.81 ± 1.07 cm and for SPD was 6.25 ± 0.93 cm. The anesthetic block was observed in 3.63±2.77 dermatomes during 250.25 ± 44.02 minutes. The variables CVP, CO, PAP, PAOP, DO2, OEF and PO2 in mixed-venous blood, increased significantly after bupivacaine administration. Similarly, PO2 from mixed venous blood as well as DO2 and OEF increased significantly during hemodynamic evaluations. TPB provided anesthesia in a limited region of the chest wall with clinically irrelevant hemodynamic effects, however this technique did not satisfactorily anesthetized all thoracic muscles’ layers so it should be used in combination with general anesthesia. / O bloqueio paravertebral torácico (BPT) é uma técnica de anestesia locorregional que fornece anestesia/analgesia no local da cirurgia com estabilidade hemodinâmica, baixa incidência de complicações e poucas contraindicações. No entanto, esta ferramenta ainda é pouco estudada na medicina veterinária devido à dificuldade de acesso aos nervos a serem bloqueados. O objetivo deste estudo foi rever a anatomia da região envolvida na realização do BPT, determinar as medidas entre os pontos de referência para execução desta técnica anestésica e avaliar a qualidade do bloqueio e as alterações hemodinâmicas decorrentes da administração de 2,5 mg/kg de bupivacaína a 0,5% no espaço paravertebral torácico de cães. Inicialmente, foi feito um estudo anatômico em cadáveres para identificar as estruturas envolvidas na execução do BPT. Na segunda etapa do estudo, oito cães saudáveis machos ou fêmeas, SRD, pesando 16,33 ± 4,04 kg, foram submetidos a anestesia geral com isofluorano para realização do BPT. O bloqueio dos nervos torácicos T5. T6 e T7 foi feito com 2,5 mg/kg de bupivacaína a 0,5%, com o auxílio de um neuroestimulador. Na terceira etapa, os mesmos animais foram anestesiados com isofluorano e, após introdução de um cateter de artéria pulmonar pela veia femoral, os animais foram submetidos ao BPT com 2,5 mg/kg de bupivacaína a 0,5% para avaliação das possíveis alterações hemodinâmicas. Foram obtidas as distâncias entre a pele e o processo transverso (DPt) e entre a pele e o espaço paravertebral torácico (DEp). Em seguida o fornecimento de isofluorano foi interrompido e a avaliação da analgesia foi realizada pelo pinçamento da pele com pinça hemostática. Para as avaliações hemodinâmicas os animais foram mantidos anestesiados com isofluorano sob ventilação espontânea e foram mensurados a pressão venosa central, (PVC), o débito cardíaco (DC), a pressão da artéria pulmonar (PAP) e a pressão da artéria pulmonar ocluída (PAPo), além de outras variáveis cardiovasculares indiretas calculadas. O sangue arterial e venoso misto foi coletado para avaliação do pH, pressão parcial de oxigênio (PaO2) e de dióxido de carbono (PCO2), bicarbonato (HCO3-), excesso de base (BE), ânion GAP (AG), eletrólitos sódio (Na+), potássio (K+), cálcio (Ca2+) e cloro (Cl-) e das variáveis de oxigenação tecidual como oferta (DO2), o consumo (VO2) e a extração de oxigênio (ERO2). As avaliações foram feitas imediatamente antes do BPT (T0) e, posteriormente, a cada 20 minutos (T20, T40, T60, T80). O estudo anatômico revelou que a inervação de alguns músculos da parede torácica é proveniente do plexo braquial. A DPt média foi de 3,81 ± 1,07 cm e a DEp média de 6,25 ± 0,93 cm. O bloqueio anestésico foi observado em 3,63 ± 2,77 dermátomos durante 250,25 ± 44,02 minutos. A PVC, o DC, a PAP e PAPo, a DO2, a ERO2 e a PaO2 no sangue venoso misto aumentaram significativamente após a administração da bupivacaína. O BPT promove anestesia em uma região limitada da parede torácica com alterações hemodinâmicas sem significância clínica, mas pode não anestesiar de forma satisfatória todas as camadas musculares da parede torácica, devendo ser utilizado em associação com a anestesia geral.
