A aquisição do HCV continua a ocorrer em unidades de hemodiálise e está relacionado principalmente com a transmissão nosocomial. O tratamento da hepatite C, com interferon em monoterapia, entre pacientes em hemodiálise, apresenta resultados satisfatórios. Entretanto, um número considerável de pacientes apresenta resposta não-sustentada à terapia. Objetivo: Avaliar se a resposta não sustentada em pacientes infectados pelo HCV em hemodiálise, corresponde a uma recidiva da infecção ou a uma reinfecção com outras cepas virais. Materiais E Métodos: Foram avaliados 15 pacientes em hemodiálise, que eram soronegativos para HBV e HIV, sem história de alcoolismo e uso de drogas injetáveis. Foram incluídos os pacientes que foram tratados com IFN-alfa 3 MU, 3 x/sem por 12 meses e que apresentaram resposta não sustentada (HCV-RNA negativo no 12o mês e positivo no 6o mês pós-tratamento). Quatro pacientes foram incluídos com infecção aguda pelo HCV. Amostras de soro foram coletadas antes do tratamento (T0), ao final do tratamento (T12) e 6 meses após o seu término (T18). A análise da genotipagem foi realizada no T0 e T18 usando o Inno-LiPA HCV II e o seqüenciamento da região NS5B. Uma árvore filogenética foi construída usando os programas Mega e Treeview. Resultados: Antes do tratamento, os respondedores não sustentados apresentavam: genótipo 1a em 8 pacientes (53%), 1b em 4 (27%), 3a em 2 (13%) e 4a em 1 (7%). No período T18, 5 pacientes (33%) mudaram de genótipo: de 1b para 3a (1 caso), 1b para 1a (1), 4a para 1a (1), 1b para 2a (1) e 1a para 2a (1). Um resultado discordante entre o Inno-LiPA e o sequenciamento do NS5B foi encontrado em um paciente. A infecção pelo HCV1a foi detectada com Inno-LiPA em T0 e T18, enquanto a análise filogenética da região NS5B demonstrou uma alteração do HCV1b em T0 para HCV1a em T18. Conclusões: Este estudo permitiu demonstrar, por meio da mudança de genótipo em 5 de 15 pacientes hemodialisados com resposta não sustentada ao tratamento da infecção crônica pelo HCV, que existe possibilidade de reinfecção após o tratamento, uma vez que os pacientes permanecem expostos a ambiente de risco de transmissão da infecção. Estudos futuros de clonagem viral devem ser feitos para complementar estes achados, no sentido de avaliar a real magnitude da reinfecção e dimensionar adequadamente as medidas a serem adotadas para evitar este tipo de ocorrência. / Background/Aims: Hepatitis C virus (HCV) infection remains common among hemodialysis (HD) patients and its occurrence is mainly related to nosocomial spread. Although dialysis patients with HCV infection respond well to Interferon-based therapy, relapse is frequent. Our study aimed at a selected group of HCV-HD patients under interferon therapy who achieved end-of-treatment virological response but became HCV-RNA positive again 6 months after end-of-treatment. We evaluated whether de novo HCV-RNA positivity in these non-sustained responders occurred due to lack of clearance of HCV after the initial response to interferon-alpha (relapse) or due to re-infection with a new strain (re-infection). Methods: Genotyping by Inno-LiPA and by phylogenetic tree analysis using partial HCV-NS5B sequences at two evaluation points: pre-treatment (T0) and 6 months after end-of-treatment (T18). Results: Non-sustained responders (n=15) carried subtypes 1a (8 patients), 1b (4 patients), 3a (2 patients) and 4a (1 patient) before treatment. Identical subtypes were detected in 10 patients at T18. Five patients changed genotypes at T18, suggesting nosocomial re-infection. To our knowledge, these findings have never been reported before in HD patients during HCV treatment. Conclusions: This study emphasizes the importance of epidemiologic measures to control the re-exposure of HD patients previously treated for HCV infection. / BV UNIFESP: Teses e dissertações

Although common, the pathofisiology of steatosis and its role in the progression of fibrosis in patients with HCV is uncertain. Our objective was to quantify the macrovacuolar and microvesicular steatosis and to correlate them with clinical and histophatologic data. The study included needle biopsy of the liver of patients with HCV without previous treatment. The fibrosis and necroinflammatory activity of hepatic damage by HCV were evaluated by METAVIR and Ishak’s scores; Kleiner’s and Brunt’s classification were used as a support for diagnosis of the steatohepatitis by the pathologist. Furthermore, the number of hepatocytes with the macrovacuolar and microvesicular steatosis was quantified in a total number of hepatocytes. Fibrosis and necroinflammatory activity were categorized and semi quantified. A significant association of the fibrosis was found and it was evaluated by the Ishak system between the macrogoticular and the microgoticular steatosis (p=0,017 e p= 0,0113, respectively). The global inflammatory activity (Metavir classification) has presented a linear correlation with the worsening of the fibrosis (< 0,001). The fibrosis which was evaluated by the Metavir system has been correlated with the BMI. The presence of HCV associated with the steatohepatitis has presented a significant correlation with the average of AST/ALT and with the Ishak and Metavir fibrosis, when compared to the data of the patients with HCV, without steatohepatitis (p= 0,006; p= 0,012; p= 0,0098 and p= 0,014, respectively) In our research we conclude that the Ishak fibrosis has been associated with the macrogoticular and microgoticular steatosis. Equally, it was demonstrated that the presence of the steatohepatitis was strongly associated to the fibrosis. / O vírus da hepatite C (VHC) e a esteatose são importantes causas de doença hepática crônica no mundo. Apesar de comum, a fisiopatologia da esteatose, e seu papel na progressão da fibrose em pacientes com VHC, permanece desconhecida. O objetivo deste trabalho foi quantificar esteatose macrovesicular e microvesicular e correlacioná-las com dados clínicos e histopatológicos. O estudo analisou biópsias hepáticas de pacientes portadores do VHC sem tratamento prévio. A fibrose e atividade necroinflamatória foram avaliadas segundo os escores de METAVIR e Ishak; as classificações de Kleiner e Brunt foram utilizadas como suporte para o diagnóstico de esteatohepatite realizado pelo patologista. Ademais, o número de hepatócitos com esteatose macrovesicular e esteatose microvesicular foram quantificados a partir do número total de hepatócitos. A fibrose e atividade necroinflamatória foram classificadas e semi-quantificadas. Foi encontrada associação significante da fibrose avaliada pelo sistema de Ishak entre a esteatose macrogoticular e microgoticular (p= 0,017 e p= 0,0113, respectivamente). A atividade inflamatória global (classificação Metavir ) apresentou correlação linear com a piora da fibrose (< 0,001). A fibrose avaliada pelo sistema de Metavir se correlacionou com o IMC. A presença de VHC associado à esteatohepatite apresentou correlação significante com as médias das AST/ALT, e com a fibrose de Ishak e Metavir, quando comparado aos dados dos pacientes com VHC, sem esteatohepatite (p = 0,006; p = 0,012; p = 0,0098 e p = 0,014, respectivamente). Em nosso trabalho, concluímos que a fibrose de Ishak esteve associado a esteatose macrogoticular e microgoticular. Igualmente, se demonstrou que a presença da esteatohepatite esteve fortemente associado a fibrose.

Avaliação nutricional de pacientes portadores de Hepatite C crônica : perfil antropométrico, dietético e metabólico e seu impacto na presença de fibrose hepática

A Infecção pelo Vírus da Hepatite C (HCV) é caracterizada pela alta taxa de fibrose e cronicidade, além disso, muitos pacientes progridem para cirrose ou ainda, carcinoma hepatocelular. O HCV é considerado um grave problema mundial de saúde pública com um elevado impacto social e econômico. Diversos fatores podem influenciar na progressão da fibrose, tais como: idade superior a 40 anos, sexo masculino, esteatose hepática, resistência insulínica, imunossupressão, atividade necroinflamatória na primeira biópsia hepática entre outros. Estudos demonstram que pacientes com HCV que apresentam mudanças de estilo de vida através da manutenção do peso, alimentação equilibrada além da prática regular de atividade física tendem a melhorar o perfil metabólico, promover a redução da esteatose-hepática e incidência de doença hepática gordurosa não-alcoólica. Porém, ainda existem poucas informações na literatura sobre o estado nutricional e consumo alimentar de pacientes HCV crônicos. O maior entendimento sobre o papel da dieta no curso desta patologia e a sua associação com a presença de fibrose poderá nortear de forma mais precisa a conduta dietética para este grupo de pacientes. Desta forma os autores se propõem a descrever o consumo alimentar, perfil metabólico e o estado nutricional em pacientes portadores do vírus da hepatite C crônicos residentes no Rio Grande do Sul. Foram incluídos 58 pacientes portadores de hepatite C crônica, genótipo 1, 2 e 3, atendidos no Ambulatório de Gastroenterologia – Hepatites (GHE) do Hospital de Clínicas de Porto Alegre no período de 2013 a 2014. Foram excluídos pacientes com cirrose, coinfectados com Vírus da Imunodeficiência Humana (HIV) ou Vírus da Hepatite B (HBV), gestantes e pacientes em tratamento farmacológico ativo. Os indivíduos foram selecionados pelo sistema de agendamento de consultas do hospital. De maneira geral, o grupo de pacientes estudados apresentou IMC (índice de massa corporal) elevado 27,29±3,98 Kg/m² e em contrapartida, os valores de força do aperto de mão estavam diminuídos 28,34±10,52 kgf. A partir do laudo da biópsia hepática pela classificação METAVIR, os pacientes foram divididos em dois grupos. Sem fibrose (F=0) e com fibrose (F=1 a 3). Dentre os 58 pacientes estudados, 39 (67,2%) apresentavam fibrose, sendo mais prevalente o grau F1. Quanto ao consumo alimentar, o grupo com fibrose apresentou valores maiores quanto à ingestão calórica 34,65±11,23 vs 28,84±34,65 Kcal/Kg, ingestão de lipídeos 1,40 (1,01-1,86) vs 1,10 (0,83-1,32) g/Kg e carga glicêmica 221,87 (168,91-307,80) vs 181,01 (140,55-223,45) g (p<0,05). Quanto ao perfil metabólico foram observados valores superiores no grupo que apresentava fibrose referente ao HOMA-IR 3,39(2,44-6,39) vs 2,18(1,60-3,34), Ferritina 378,84±200,13 vs 143,83±33,00 μg/mL, Hematócrito 42,80(39,30-45,80) vs 40,60(38,8-41,50) g/dL e Hemoglobina 14,80(13,90-15,80) vs 13,90(13,20-14,70) g/dL (p<0,05). Demais variáveis não houve diferença estatística (colesterol total, colesterol HDL, Colesterol LDL, triglicerídeos, AST, ALT e GGT). Em conclusão, na amostra estudada, observou-se que pacientes portadores de HCV crônicos, apesar de apresentarem sobrepeso, apresentam estado nutricional comprometido, uma vez que foi demonstrada redução da força do aperto de mão. Ainda, levando-se em consideração a presença de fibrose, pacientes com fibrose consomem mais calorias e gordura, além disso, apresentam pior perfil metabólico, com maiores taxas de resistência insulínica, tais fatores tendem a piorar ainda mais o prognóstico da doença hepática. As evidências que confirmam a associação da infecção crônica pelo HCV com o desenvolvimento de cirrose reforçam a necessidade do diagnóstico precoce da doença e do tratamento dos pacientes com elevado risco de complicações a fim de minimizar a morbidade e mortalidade. / The infection from the Hepatitis C virus (HCV) is characterized by the high rate of fibrosis and chronicity, in addition, many patients progress to cirrhosis or even hepatocellular carcinoma. The HCV is considered a serious global public health problem with a high social and economic impact. Several factors may influence the progression of the fibrosis, such as, over 40 years old, male, hepatic steatosis, insulin resistance, immunosuppression, necroinflammatory activity in the first liver biopsy, among others. Studies show that patients with HCV, who demonstrate lifestyle changes through weight control, balanced diet as well as regular physical activity tend to improve the metabolic profile, reduce hepatic steatosis and incidences of non-alcoholic fatty liver disease. However, there exists very little information in the literature about the nutritional state and dietary intake of chronic HCV patients. A greater understanding of the role of diet in the course of this condition and its association with the presence of fibrosis could more accurately guide the dietetics for this group of patients. Thus, the authors propose to describe the dietary intake, metabolic profile and nutritional state of patients carrying the chronic hepatitis C virus in Rio Grande do Sul. Fifty-eight patients carrying chronic hepatitis C were included, genotypes 1, 2 and 3, outpatients at the Gastroenterology – Hepatitis Clinic (GHE) at Hospital de Clínicas in Porto Alegre in 2013 to 2014. Patients with cirrhosis, co-infected with Human Immunodeficiency virus (HIV) or Hepatitis B virus (HBV), pregnant women and patients on active drug treatment were excluded. The individuals were selected by the appointment scheduling system of the hospital. In general, the patients studied showed and elevated Body Mass Index (BMI) 27.29 ±3.98 Kg/m² and in contrast, the values for handgrip strength were reduced to 28.34 ±10.52. From the liver biopsy report using the METAVIR classification, the patients were divided into two groups. Without fibrosis (F=0) and with fibrosis (F=1-3). Within the 58 patients studied, 39 (67.2%) presented fibrosis, most commonly being of F1 degree. All other variables showed no statistical difference (cholesterol HDL, cholesterol LDL, triglycerides, AST, ALT and GGT). In conclusion, patients carrying the chronic HCV, despite being overweight, presented a compromised nutritional state, once a reduction in handgrip strength was demonstrated. Additionally, patients with fibrosis had an increased energy and fat intake, as well, a poor metabolic profile, with higher rates of insulin resistance; these factors tend to further worsen the prognosis of liver disease. The evidence confirming the link between the chronic infection of HCV with the development of cirrhosis reinforced the need for early diagnosis and treatment of patients with a high risk of complications in order to minimize morbidity and mortality.

Ferritins 378.84 ±200.13 vs 143.83 ±33.00 μg/mL, Hematocrit 42.80 (39.30 – 45.80) vs 40.60 (38.8 – 41.50) g/dL and hemoglobin 14.80 (13.90 – 15.80) vs 13.90 (13.20 – 14.70) g/dL (p<0.05). All other variables showed no statistical difference (cholesterol HDL, cholesterol LDL, triglycerides, AST, ALT and GGT). In conclusion, patients carrying the chronic HCV, despite being overweight, presented a compromised nutritional state, once a reduction in handgrip strength was demonstrated. Additionally, patients with fibrosis had an increased energy and fat intake, as well, a poor metabolic profile, with higher rates of insulin resistance; these factors tend to further worsen the prognosis of liver disease. The evidence confirming the link between the chronic infection of HCV with the development of cirrhosis reinforced the need for early diagnosis and treatment of patients with a high risk of complications in order to minimize morbidity and mortality.

Avaliação nutricional de pacientes portadores de Hepatite C crônica : perfil antropométrico, dietético e metabólico e seu impacto na presença de fibrose hepática

A Infecção pelo Vírus da Hepatite C (HCV) é caracterizada pela alta taxa de fibrose e cronicidade, além disso, muitos pacientes progridem para cirrose ou ainda, carcinoma hepatocelular. O HCV é considerado um grave problema mundial de saúde pública com um elevado impacto social e econômico. Diversos fatores podem influenciar na progressão da fibrose, tais como: idade superior a 40 anos, sexo masculino, esteatose hepática, resistência insulínica, imunossupressão, atividade necroinflamatória na primeira biópsia hepática entre outros. Estudos demonstram que pacientes com HCV que apresentam mudanças de estilo de vida através da manutenção do peso, alimentação equilibrada além da prática regular de atividade física tendem a melhorar o perfil metabólico, promover a redução da esteatose-hepática e incidência de doença hepática gordurosa não-alcoólica. Porém, ainda existem poucas informações na literatura sobre o estado nutricional e consumo alimentar de pacientes HCV crônicos. O maior entendimento sobre o papel da dieta no curso desta patologia e a sua associação com a presença de fibrose poderá nortear de forma mais precisa a conduta dietética para este grupo de pacientes. Desta forma os autores se propõem a descrever o consumo alimentar, perfil metabólico e o estado nutricional em pacientes portadores do vírus da hepatite C crônicos residentes no Rio Grande do Sul. Foram incluídos 58 pacientes portadores de hepatite C crônica, genótipo 1, 2 e 3, atendidos no Ambulatório de Gastroenterologia – Hepatites (GHE) do Hospital de Clínicas de Porto Alegre no período de 2013 a 2014. Foram excluídos pacientes com cirrose, coinfectados com Vírus da Imunodeficiência Humana (HIV) ou Vírus da Hepatite B (HBV), gestantes e pacientes em tratamento farmacológico ativo. Os indivíduos foram selecionados pelo sistema de agendamento de consultas do hospital. De maneira geral, o grupo de pacientes estudados apresentou IMC (índice de massa corporal) elevado 27,29±3,98 Kg/m² e em contrapartida, os valores de força do aperto de mão estavam diminuídos 28,34±10,52 kgf. A partir do laudo da biópsia hepática pela classificação METAVIR, os pacientes foram divididos em dois grupos. Sem fibrose (F=0) e com fibrose (F=1 a 3). Dentre os 58 pacientes estudados, 39 (67,2%) apresentavam fibrose, sendo mais prevalente o grau F1. Quanto ao consumo alimentar, o grupo com fibrose apresentou valores maiores quanto à ingestão calórica 34,65±11,23 vs 28,84±34,65 Kcal/Kg, ingestão de lipídeos 1,40 (1,01-1,86) vs 1,10 (0,83-1,32) g/Kg e carga glicêmica 221,87 (168,91-307,80) vs 181,01 (140,55-223,45) g (p<0,05). Quanto ao perfil metabólico foram observados valores superiores no grupo que apresentava fibrose referente ao HOMA-IR 3,39(2,44-6,39) vs 2,18(1,60-3,34), Ferritina 378,84±200,13 vs 143,83±33,00 μg/mL, Hematócrito 42,80(39,30-45,80) vs 40,60(38,8-41,50) g/dL e Hemoglobina 14,80(13,90-15,80) vs 13,90(13,20-14,70) g/dL (p<0,05). Demais variáveis não houve diferença estatística (colesterol total, colesterol HDL, Colesterol LDL, triglicerídeos, AST, ALT e GGT). Em conclusão, na amostra estudada, observou-se que pacientes portadores de HCV crônicos, apesar de apresentarem sobrepeso, apresentam estado nutricional comprometido, uma vez que foi demonstrada redução da força do aperto de mão. Ainda, levando-se em consideração a presença de fibrose, pacientes com fibrose consomem mais calorias e gordura, além disso, apresentam pior perfil metabólico, com maiores taxas de resistência insulínica, tais fatores tendem a piorar ainda mais o prognóstico da doença hepática. As evidências que confirmam a associação da infecção crônica pelo HCV com o desenvolvimento de cirrose reforçam a necessidade do diagnóstico precoce da doença e do tratamento dos pacientes com elevado risco de complicações a fim de minimizar a morbidade e mortalidade. / The infection from the Hepatitis C virus (HCV) is characterized by the high rate of fibrosis and chronicity, in addition, many patients progress to cirrhosis or even hepatocellular carcinoma. The HCV is considered a serious global public health problem with a high social and economic impact. Several factors may influence the progression of the fibrosis, such as, over 40 years old, male, hepatic steatosis, insulin resistance, immunosuppression, necroinflammatory activity in the first liver biopsy, among others. Studies show that patients with HCV, who demonstrate lifestyle changes through weight control, balanced diet as well as regular physical activity tend to improve the metabolic profile, reduce hepatic steatosis and incidences of non-alcoholic fatty liver disease. However, there exists very little information in the literature about the nutritional state and dietary intake of chronic HCV patients. A greater understanding of the role of diet in the course of this condition and its association with the presence of fibrosis could more accurately guide the dietetics for this group of patients. Thus, the authors propose to describe the dietary intake, metabolic profile and nutritional state of patients carrying the chronic hepatitis C virus in Rio Grande do Sul. Fifty-eight patients carrying chronic hepatitis C were included, genotypes 1, 2 and 3, outpatients at the Gastroenterology – Hepatitis Clinic (GHE) at Hospital de Clínicas in Porto Alegre in 2013 to 2014. Patients with cirrhosis, co-infected with Human Immunodeficiency virus (HIV) or Hepatitis B virus (HBV), pregnant women and patients on active drug treatment were excluded. The individuals were selected by the appointment scheduling system of the hospital. In general, the patients studied showed and elevated Body Mass Index (BMI) 27.29 ±3.98 Kg/m² and in contrast, the values for handgrip strength were reduced to 28.34 ±10.52. From the liver biopsy report using the METAVIR classification, the patients were divided into two groups. Without fibrosis (F=0) and with fibrosis (F=1-3). Within the 58 patients studied, 39 (67.2%) presented fibrosis, most commonly being of F1 degree. Regarding the dietary intake, the fibrosis group showed values greater intake of calories 34.65 ±11.23 vs 28.84 ±34.65 Kcal/Kg, lipid intake 1.40 (1.01 – 1.86) vs 1.10 (0.83 – 1.32) g/Kg and glycemic load 221.87 (168.91 – 307.80) vs 181.01 (140.55 – 223.45) g (p<0,05). Patient data were collected with a confirmed diagnosis of hepatitis C, of 155 medical records for medical care in order to characterized them for clinical, epidemiological and survival. Then they were related criteria found by gender and age group, obtaining the following results: In men is greater the negative viral load and that remain under assessment; genotype 3 appear as the predominant, followed by 1b and 2, the site of infection identification prevails the use of vitamin and intravenous drug, the greater is the degree of fibrosis portal fibrosis F1-F3 and liver and histopathologic activity A1- light and A4-severe, and higher mean viral load. In women it is identified greater number of deaths; genotype 1a; blood transfusion and surgery as a form of contagion; F2 fibrosis in the liver portal fibrosis with rare septal fibrosis F0-F4 or absence of fibrosis and histological activity and moderate A2-A0 absent. In the age group of between 29-59 years of age is higher the dropout rate of treatment; the predominant genotype is 3, followed by 1a; in relation to the site of contagion, transfusion and intravenous drug use; the degree of liver fibrosis, and F4 and F3-F0 no fibrosis; in the analysis of inflammatory activity of severe liver A3, and also A0-away. In patients 60 years of age or older identifies the highest number of deaths and staying in treatment assessment; is the predominant genotype 1b followed by 2; the form of contagion by the use of vitamin and performing some surgery; It is greater the degree of fibrosis in liver fibrosis, portal F1 and histopathological features activity A1-A2-light, moderate and has a higher average viral load. The search results reveal difficulties encountered in manual recording information related to treatment, which leads to reflection of the importance of adequately monitor the follow-up of patients because some do not heal, they resort to transplantation or relapse, further increasing public spending on health. / A hepatite C é atualmente a principal causa de hepatopatias crônicas, cirrose e carcinoma hepatocelular. Tratou-se de um estudo descritivo, transversal, quantitativo, retrospectivo (2000 – 2014) de pacientes com infecção crônica pelo HCV, originários da consulta de ambulatório do HEMOPI, PI. Foram recolhidos os dados do paciente com diagnóstico confirmado de hepatite C, de 155 prontuários para o atendimento médico com o objetivo de caracterizá-los quanto aos aspectos clínicos, epidemiológicos e a sobrevivência. Depois foram relacionados os critérios encontrados por gênero e faixa etária, obtendo-se como resultados: Nos homens é maior a negativação da carga viral e dos que permanecem em avaliação; o genótipo 3 aparece como predominante , seguido do 1b e do 2; na identificação do sitio de contagio prevalece o uso de complexo vitamínico e de droga endovenosa; é maior o grau de fibrose no fígado F1-fibrose portal e F3 e atividade histopatológica A1-leve e A4-severa; e maior carga viral média. Nas mulheres é identificado maior numero de óbitos; o genótipo 1a; transfusão sanguínea e cirurgias como forma de contagio; fibrose no fígado F2- fibrose portal com rara fibrose septal, F4 ou F0-ausência de fibrose e atividade histopatológica A2- moderada e A0-ausente. Na faixa etária situada entre 29 – 59 anos de idade é maior o índice de abandono do tratamento; o genótipo predominante é o 3, seguido do 1a; em relação ao sitio de contagio, por transfusão e uso de droga endovenosa; ao grau de fibrose do fígado, F3 e F4 e F0-ausência de fibrose; na análise da atividade inflamatória do fígado A3- severa, e também A0-ausente. Nos pacientes com 60 anos de idade ou mais se identifica o maior numero de óbitos e os que ficam em avaliação após tratamento; o genótipo predominante é o 1b seguido do 2; a forma de contagio pelo uso de complexo vitamínico e realização de alguma cirurgia; é maior o grau de fibrose no fígado F1-fibrose portal e apresenta atividade histopatológica A1-leve e A2-moderada; e apresenta carga viral média maior. A busca dos resultados revela dificuldades encontradas no registro manual das informações relacionadas ao tratamento, que leva a reflexão da importância em monitorar adequadamente o acompanhamento dos pacientes, pois alguns não curam, recorrem ao transplante ou recidivam, aumentando ainda mais os gastos públicos com a saúde.

Entretanto, muitos estudos nao chegaram a estes resultados57, 64-66. Maior ainda e a controversia na relacao entre o HIV, o HCV e GBV-C, quando em tripla infecca06O, 66,137,139-141,144. Objetivo: Estimar a prevalencia de marcadores de infeccao pelo GBV-C entre pacientes com infeccao pelo HIV e com co-infeccao HIV/HCV e comparar os grupos, procurando identificar impacto da infeccao pelo virus G. Metodos: Estudo observacional, transversal, com inclusao prospectiva de voluntarios, que passaram por entrevista, exame fisico e coleta de sangue para determinacao de carga viral do HIV, contagem CD4, AL T, AST, GGT, HCV RNA quantitativo, genotipagem HCV, anti-E2 e GBV-C RNA. A partir dos resultados dos marcadores para GBV-C, foram alocados em grupos e comparados. Resultados: Foram incluidos 60 voluntarios, 30 infectados somente pelo HIV e 30 co-infectados HIV/HCV. A prevalencia de infeccao pelo GBV-C entre pacientes HIV-positivos (20 por cento) tendeu a ser menor que a descrita na literatura, porem a prevalencia de tripla infeccao HIV/HCV/GBV-C (27 por cento) e semelhante a relatada. A presenca de marcadores para o GBV-C se associou a maior exposicao parenteral e ao aumento da AST, isolado em pacientes HIV-positivos e em conjunto com elevacao da AL T em pacientes co-infectados HIV/HCV. Exceto pelos achados ja descritos, nao identificamos impacto do GBV-C sobre a evolucao de pacientes infectados pelo HIV e co-infectados HIV-HCV. Conclusoes: O GBV-C pode estar sendo transmitido preferencialmente pela via parenteral em nosso meio. Os maiores niveis de AST observados em associacao com GBV-C talvez sejam resultantes da infeccao de outros tipos celulares por este virus. Os maiores niveis de ALT e AST relacionados com a infeccao pelo GBV-C em co-infectados HIV/HCV talvez seja traducao da infeccao simultanea dos hepatocitos por estes tres virus. O numero reduzido de voluntarios em cada grupo certamente prejudicou a analise estatistica / Introduction: It has been almost ten years since the first studies suggesting that a new RNA apathogenic virus called GBV-C could delay the progression of HIV infection into aids55, 56, 17, 19, 20, 59-63. However, many studies couldn’t find this relationship57, 64-66. And the controversy around the simultaneous infection by HIV, HCV e GBV-C is even greater60, 66, 137, 139-141, 144. Objectives: To estimate the prevalence of GBV-C markers among patients infected with HIV and co-infected with HIV and HCV and to compare the groups, searching for differences related with GBV-C infection. Methods: Observational and prospective study. Volunteers were interviewed, had a physical examination and blood samples tested for HIV viral load, CD4 count, ALT, AST, GGT, quantitative HCV RNA, HCV genotyping, anti-E2 and qualitative GBV-C RNA. Accordingly with the results, the volunteers were divided into groups and compared. Results: 60 volunteers were included, 30 infected only by HIV and 30 co-infected with HIV/HCV. The prevalence of GBV-C markers among HIV-positive volunteers (20%) occurred in a small frequency compared to results of previous studies, though the prevalence of triple infection HIV/HCV/GBV-C (27%) was similar to what is found in the literature. The presence of GBV-C was related with more frequent parenteral exposure, higher levels of AST in patients infected only with HIV and higher levels of AST associated with higher levels of ALT in coinfected HIV/HCV patients. Conclusions: In our setting, GBV-C preferencial route of transmission may be parenteral. The higher levels of AST observed when GBV-C markers are present, may reflect the infection of other cell types by this virus. The higher levels of ALT and AST related with GBV-C in co-infected patients may reflect the simultaneous infection of hepatocytes by these three viruses. It has a positive-sense RNA and belongs to the flavivirus family. The HCV shows considerable sequence variability and its variants can be divided into six main genotypes (numbered from 1 to 6) and several subtypes. The genotypes distribution varies from the geographic area and among risk groups. This study had the main aim to study HCV genotypes distribution in patients from a reference hospital in Fortaleza, Ceara. It was investigated a hundred and twenty patients with anti-HCV positive or clinical history suggesting virus infection. Viral RNA was extracted from patients serum and the virus molecular detection was made by PCR, using specific primers to the 5’ non-coding region of viral genome. Genotyping was based in a RFLP technique, using the restriction enzymes HaeIII, RsaI, MvaI and HinfI. Ninety six patients (80%) were positive in the qualitative test for HCV. The mean age of these patients was 44 years old. Surgery clinical history was the most frequent risk factor, followed by blood transfusion, sexual transmitted disease, drugs use, tattoos, dialysis, and occupacional exposure. The relation between qualitative test result and drug users showed statistical significance, with 96% of drugs users being positive for HCV. There was no significant difference in qualitative test result when we analysed blood transfusions done after or before the year 1993. Clinical symptoms and ALT levels also were not predictive of HCV positive result. Genotype 1 was the most prevalent in the studied population (46,9%), followed by genotype 3 (34,4%) and 2 (8,3%). There was also a molecular characterization of one patient with genotype 4 whereas nine samples were not HCV genotyped and were called as undetermined. The epidemiological characteristics such as age, gender, risk factors, ALT levels and clinical manifestations were related with viral genotypes. The HCV genotypes had a homogeneous distribution, in the majority of cases, among the different categories. A statistical significance was observed only when genotype 1 was related to HCV occupational exposure. Co-infections with more than one viral genotype were not observed. / O vírus da hepatite C é considerado como o principal agente causador de hepatite não-A, não-B e, desde sua descoberta em 1989, tem sido reconhecido como a principal causa de doença crônica do fígado em todo o mundo. O VHC possui um genoma de RNA de sentido positivo e é classificado como um flavivírus. O vírus apresenta considerável variabilidade em sua seqüência e as variantes do VHC podem ser divididas em seis genótipos principais (numerados de 1 a 6) e diversos subtipos. A distribuição dos genótipos varia tanto geograficamente, quanto entre os grupos de risco. Este estudo teve como objetivo analisar a distribuição dos genótipos do VHC entre pacientes atendidos em um hospital de referência em Fortaleza, Ceará. Cento e vinte pacientes com sorologia positiva para anti-VHC ou história clínica sugestiva de infecção pelo vírus foram estudados. O RNA viral foi extraído a partir do soro dos pacientes e a detecção molecular do vírus foi feita por PCR, utilizando iniciadores específicos para a região 5 (linha) não-codificadora do genoma viral. A genotipagem foi baseada em técnica de RFLP utilizando as enzimas de restrição HaeIII, RsaI, MvaI e HinfI. Noventa e seis pacientes (80%) foram positivos no teste qualitativo para VHC. A média de idade desses pacientes foi de 44 anos. História clínica de cirurgia foi o fator de risco mais presente, seguido por transfusão sangüínea, DST, uso de drogas, tatuagem, diálise e exposição ocupacional. A relação entre o resultado do teste qualitativo e o uso de drogas apresentou significância estatística, com 96% dos usuários de drogas positivos para VHC. Não houve diferença significativa no resultado do teste qualitativo quando transfusões sangüíneas feitas antes ou depois de 1993 foram analisadas. Manifestações clínicas ou índices de ALT alterados também não foram preditivos de positividade para VHC. O genótipo 1 foi o mais prevalente na população estudada (46,9%), seguido pelos genótipos 3 (34,4%) e 2 (8,3%). O genótipo 4 viral foi detectado em um paciente. Em nove amostras não foi possível a genotipagem do VHC. Esses casos foram denominados indeterminados. Características epidemiológicas como idade, sexo, fatores de risco, índices de ALT e manifestações clínicas foram relacionadas com os diversos genótipos virais. A distribuição dos genótipos virais entre as categorias estudadas ocorreu de forma homogênea na maioria dos casos.
