Infant hearing screening at maternal and child health clinics in a developing South African community

Newborn hearing screening has become an increasingly important element of neonatal care in developed countries whilst only a few fragmented screening programmes are evident in developing countries. The numerous socio-economic, cultural and healthcare barriers in developing contexts do not, however, negate or diminish the need to ensure optimal outcomes for infants with hearing loss through early identification and intervention programmes. South Africa has taken a first step toward addressing this need by publishing a Year 2002 Hearing Screening Position Statement that was produced by the Professional Board for Speech, Language and Hearing Professions of the Health Professions Council of South Africa. Interim recommendations are made toward universal newborn hearing screening programmes in three contexts: well-baby nurseries,; neonatal intensive care units (NICU) and Maternal and Child Health (MCH) clinics through their 6-week immunisation programmes. Although these clinics constitute an unfamiliar hearing screening context, they are essential platforms toward widespread screening of the majority of infants in South Africa. An urgent need therefore exists to ascertain the feasibility of hearing screening programmes at MCH 6-week immunisation clinics in order to guide the future implementation of widespread hearing screening services in South Africa. To attend to this need, an exploratory descriptive design that jointly implements quantitative and qualitative methods in a dominant-less-dominant model of triangulation was utilised to critically describe a screening programme conducted at two MCH clinics in Hammanskraal (a developing, peri-urban South African community). The quantitative methods included a structured interview to compile biographical and risk information; high frequency immittance measurements; hearing screening with OAE and AABR according to specified protocols, and diagnostic assessment of referred infants. The qualitative methods included field notes and critical reflections describing clinics as screening contexts and elucidating interactional processes involved in sustaining programmes. A total number of 510 infant-caregiver pairs were enrolled as subjects during the five-month research period. Results indicate that clinics not only provide a suitable context, but also the possibility of effective collaborations toward facilitating effective initial infant hearing screening programmes. The caregivers and infants who attended the clinics demonstrated significant degrees of socio-economic deprivation. They also reported an increased incidence of risk indicators exacerbating the population’s risk for congenital hearing loss, poor participation in the hearing screening/follow-up process, and subsequent poor involvement in a family-centred early intervention process for infants identified with hearing loss. The screening protocol effectively classified infants into risk categories for hearing loss and established useful norms for high frequency immittance in infants. The efficiency of the programme was acceptable considering the short period of implementation, but inefficient coverage with the AABR and poor follow-up return rates were obtained at the clinics. Despite prevailing barriers, the MCH 6-week immunisation clinics showed promise as platforms for widespread hearing screening programmes for infants in South Africa. Extended frequency amplification, speech recognition and functional performance in children with mild to severe sensorineural hearing loss

A substantial body of research points to the benefits of fitting hearing instruments that provides extended high frequency amplification. Most published research were done on adults or in controlled laboratory settings. It is therefore necessary for peadiatric audiologists to critically assess the effects that this extended high frequency amplification has on the individual child fitted with hearing instruments. A quantitative research method was selected to explore the possible correlations between extended high frequency amplification and the influence this extended high frequency amplification has on speech recognition and functional performance in children with mild to severe sensory neural hearing loss. A quasiexperimental design was selected. This design accommodated a one-group (single-system) pre-test versus post-test design. Baseline assessments were done and all participants were subjected to pre- and post-intervention assessments. Six participants were fitted with hearing instruments which provided extended high frequency amplification. A baseline assessment was done with current hearing instruments after which participants were assessed with the hearing instruments with extended high frequency amplification. Aided audiological assessments were done without the extended high frequencies after which participants were evaluated with the added high frequencies. Speech recognition testing and functional performance questionnaires were used to compare the outcomes obtained with and without the extended high frequency amplification. A t-test was used for hypothesis testing to determine if extended range amplification increased speech recognition abilities and functional performance, and if these increases were statistically significant. Results were varied where some participants performed better and some performed worse with the added extended range amplification during speech recognition testing and functional performances observed at home. These varied results were statistically insignificant. However, statistically significant evidence was obtained to indicate that extended high frequency amplification increased the functional performance observed at school. The study concluded that the paediatric audiologist should know the effect fitting hearing instruments capable of extended high frequency amplification have on speech recognition abilities and functional performances. Fitting hearing instruments with extended high frequency amplification should however be done with caution because not all children benefited from extended bandwidth amplification. This underlines the importance of following a strict evidence-based approach that incorporates objective and subjective assessment approaches. Cette thèse aborde différents aspects de la modélisation de la microstructure du marché et des problèmes de Market Making, avec un accent particulier du point de vue du praticien. Le carnet d’ordres, au cœur du marché financier, est un système de files d’attente complexe à haute dimension. Nous souhaitons améliorer la connaissance du LOB pour la communauté de la recherche, proposer de nouvelles idées de modélisation et développer des applications pour les Market Makers. Nous remercions en particuler l’équipe Automated Market Making d’avoir fourni la base de données haute-fréquence de très bonne qualité et une grille de calculs puissante, sans laquelle ces recherches n’auraient pas été possible. Le Chapitre 1 présente la motivation de cette recherche et reprend les principaux résultats des différents travaux. Le Chapitre 2 se concentre entièrement sur le LOB et vise à proposer un nouveau modèle qui reproduit mieux certains faits stylisés. A travers cette recherche, non seulement nous confirmons l’influence des flux d’ordres historiques sur l’arrivée de nouveaux, mais un nouveau modèle est également fourni qui réplique beaucoup mieux la dynamique du LOB, notamment la volatilité réalisée en haute et basse fréquence. Dans le Chapitre 3, l’objectif est d’étudier les stratégies de Market Making dans un contexte plus réaliste. Cette recherche contribueà deux aspects : d’une part le nouveau modèle proposé est plus réaliste mais reste simple à appliquer pour la conception de stratégies, d’autre part la stratégie pratique de Market Making est beaucoup améliorée par rapport à une stratégie naive et est prometteuse pour l’application pratique. La prédiction à haute fréquence avec la méthode d’apprentissage profond est étudiée dans le Chapitre 4. De nombreux résultats de la prédiction en 1- étape et en plusieurs étapes ont retrouvé la non-linéarité, stationarité et universalité de la relation entre les indicateurs microstructure et le changement du prix, ainsi que la limitation de cette approche en pratique. / This thesis addresses different aspects around the market microstructure modelling and market making problems, with a special accent from the practitioner’s viewpoint. The limit order book (LOB), at the heart of financial market, is a complex continuous high-dimensional queueing system. We wish to improve the knowledge of LOB for the research community, propose new modelling ideas and develop concrete applications to the interest of Market Makers. We would like to specifically thank the Automated Market Making team for providing a large high frequency database of very high quality as well as a powerful computational grid, without whom these researches would not have been possible. The first chapter introduces the incentive of this research and resumes the main results of the different works. Chapter 2 fully focuses on the LOB and aims to propose a new model that better reproduces some stylized facts. Through this research, not only do we confirm the influence of historical order flows to the arrival of new ones, but a new model is also provided that captures much better the LOB dynamic, notably the realized volatility in high and low frequency. In chapter 3, the objective is to study Market Making strategies in a more realistic context. This research contributes in two aspects : from one hand the newly proposed model is more realistic but still simple enough to be applied for strategy design, on the other hand the practical Market Making strategy is of large improvement compared to the naive one and is promising for practical use. High-frequency prediction with deep learning method is studied in chapter 4. Many results of the 1-step and multi-step prediction have found the non-linearity, stationarity and universality of the relationship between microstructural indicators and price change, as well as the limitation of this approach in practice.

Das Erkennen und die Lokalisierung kritischer Risse ist ein wesentlicher Schlüssel für eine sichere und nachhaltige Bauwerksnutzung. In diesem Beitrag wird ein neuartiges, kostengünstiges Sensorsystem vorgestellt, das zur Echtzeit-Zustandsüberwachung von sowohl neuen als auch Bestandsbauwerken geeignet ist. Erste Ergebnisse zeigen, dass das System prinzipiell in der Lage ist, die Gesamtdehnung eines Bauteiles zu erfassen sowie auftretende Risse zu erkennen und zu lokalisieren. Die Erkennungsgenauigkeit hängt dabei von technischen Parametern ab, wodurch das System auf verschiedene Einsatzszenarien angepasst werden kann.

Sensorless Control of Synchronous Machines in Python Using Signal Injection : An implementation of a High-Frequency Square-Wave Injection Algorithm on a Linear and Non-Linear Synchronous Machine Model in open-source Software Motulator

The importance of accurately controlling the speed and torque of Synchronous Machines (SMs) in industry, transportation, aerospace, to name a few, can not be overstated. The driving unit to control the machines are called Variable Speed Drives (VSDs) and they can be designed in many different ways. In this project, a speed sensorless drive using high-frequency square-wave voltage injection is implemented in a open-source Python software called Motulator, developed by Prof. Marko Hinkkanen at Aalto University. The drive is first tested on an already existing linear model of a Permament Magnet Synchronous Machine (PMSM). An equivalent model is built in Matlab/Simulink to benchmark the performance of the implementation in Python. The results suggest that the performance Motulator implementation is satisfactory when compared to the Simulink implementation. Next, a non-linear Synchronous Reluctance Machine (SynRM) is implemented, using data from Finite Element (FEM) simulations of the non-linear flux-current relation. By using the injection scheme (with some tweaks), the speed of the motor is accurately controlled, but a steady-state position error is observed at all operating points. The error is produced due to the cross-saturation effect and a compensation strategy is implemented in an attempt to remove this error. however without full success. / Det är av avgörande betydelse att kunna kontrollera varvtal och vridmomentet hos synkrona elektriska maskiner (SM) inom transport, flyg och rymd, för att nämna några tillämpningar. Drivsystem för att styra de elektriska maskinerna kallas för varvtalsreglerade drivsystem och kan konstrueras på många olika sätt. I det här projektet implementeras ett varvtalsreglerat drivsystem, utan sensor för mätning av varvtalet. Varvtalsestimeringen bygger på att en fyrkantsvåg med hög frekvens injiceras varur det är möjligt att estimera hastigheten. Implementering görs i Python i en open-source programvara kallad Motulator, utvecklad av professor Marko Hinkkanen från Aaltouniversitetet. Regleringen testas först på en redan existerande linjär modell av en permanentmagnetiserad motor. Som jämförelse utvecklas även en motsvarande implementering av regleralgoritm och motor i Matlab/Simulink. Resultaten visar att Motulatorimplementeringen fungerar väl och att simuleringarna stämmer väl överens med Matlab/Simulink-modellen. I nästa steg implementeras en icke-linjär modell av en synkron reluktansmaskin. Det icke-linjära förhållande mellan flöde och ström modelleras med hjälp av data från finita elementsimuleringar (FEM). Simuleringar i Motulator visar att varvtalet i denna motormodell kan kontrolleras för alla olika laster och varvtal. Däremot noteras ett stationärt rotorpositionsfel vid vissa driftpunkter. Felet beror på mättningen av statorinduktansen och en algoritm implementerats för att kompensera effekten av mättningen och därmed eliminera felet. Det visar sig dock att kompenseringsalgoritmen endast fungerar vid vissa driftpunkter.

Three Essays in Applied Econometrics

La tesis engloba tres capítulos: el primero sobre fertilidad y "calidad" infantil, el segundo sobre planificación familiar y salud infantil, y el tercero sobre la estimación de un indicador de la actividad económica agregada regional. Concretamente, el primer capítulo examina empíricamente usando un modelo de diferencia-en-diferencias con efectos fijos, cómo un aumento inesperado de riqueza (proveniente de una transferencia intergeneracional) afecta a la fertilidad y la inversión de los padres en la calidad de los hijos. Se encuentra un efecto negativo en la cantidad de hijos junto a un efecto positivo en la calidad de los hijos demandados por los hogares. Estos efectos ocurren en diferentes momentos del tiempo para la muestra completa. En el corto plazo se observa una reducción de la fertilidad mientras que en el largo plazo se observa un aumento de la inversión en calidad infantil. También se encuentra un efecto positivo en la inversión en calidad infantil a corto plazo para las parejas que ya tenían hijos. En el segundo capítulo se evalúa una política, estudiando el impacto de haber sido expuesto a un programa de planificación familiar que promovió la anticoncepción quirúrgica/esterilización por primera vez en Perú (Programa de Salud Reproductiva y Planificación Familiar o PNSRPF, 1996- 2000). Los resultados muestran un mayor uso de métodos anticonceptivos temporales y permanentes entre las mujeres expuestas al programa y un menor riesgo de mortalidad infantil entre sus hijos. Este efecto es, en parte, debido a la prolongación de la lactancia materna. Finalmente, en el tercer capítulo se estima un indicador resumen de la actividad económica agregada, en frecuencia mensual, para las regiones españolas. Se utiliza un Modelo de Factores Dinámico dada la escasez de datos a nivel regional. Los indicadores estimados muestran la heterogeneidad del sistema productivo entre regiones a través de la inclusión de diferentes variables con el fin de reflejar con mayor precisión la evolución económica regional. Switchable and Tunable MEMS Devices in GaN MMIC Technology

<p dir="ltr">Rapid evolution in wireless technology and the increasing demand for high bandwidth communication for 5G/6G and the Internet of Things (IoT) have necessitated a growing number of components in radio front-end modules in an increasingly overcrowded radio frequency (RF) spectrum. Low-cost ad-hoc radios have drawn consumer interest, enabling new devices like microelectromechanical (MEMS) resonators for on-chip clocking, frequency-selective filters, RF signal processing, and spectral sensing for their small footprint and low power consumption. Gallium nitride (GaN) is an attractive electromechanical material due to its high coupling coefficient, acoustic velocity, and low viscoelastic losses. These benefits enable high-Q MEMS resonators in GaN monolithic microwave integrated circuits (MMICs) with scaling capability up to mm-wave frequencies, making this technology platform a contender for high-performance programmable radios in RF/mm-wave, sensors for harsh environments, and information processing in quantum systems.</p><p dir="ltr">The bias-dependent control mechanism of the 2D electron gas (2DEG) in GaN heterostructures can be exploited to design different switchable and tunable devices for reconfigurable MEMS components. This work presents, for the first time, a comprehensive study of the electromechanical performances of different transduction mechanisms in switchable GaN MEMS resonators. A unique OFF-state shunt design, where the 2DEG in an AlN/GaN heterostructure is utilized to control electromechanical transduction in Lamb mode resonators, is also experimentally demonstrated in this work. To make a valid comparison among switchable transducers, equivalent circuit models are developed to extract key parameters from the measurements by fitting them in both ON and OFF states. The switchable transducer with Ohmic interdigitated transducers (IDTs) and Schottky control gate shows superior performance among the designs under consideration with complete suppression of the mechanical mode in the OFF state and a maximum frequency-quality factor product of 5x10<sup>12</sup>s<sup>-1</sup> and a figure-of-merit of 5.18 at 1GHz in the ON state.</p><p dir="ltr">Over the past few years, there have been numerous efforts to scale the frequencies of MEMS devices in the GaN platform towards mm-wave frequencies. However, challenges remain due to the multi-layer thick buffer, typical in the growth of GaN epilayer on a substrate. This work presents the investigation of SweGaN QuanFINE<sup> </sup>buffer-free and ultrathin GaN-on-SiC for the performance of surface acoustic wave (SAW) devices beyond 10GHz. Finite element analysis (FEA) is performed to find the range of frequencies for the Sezawa mode in the structure. Transmission lines and resonators are designed, fabricated, and characterized. Modified Mason circuit models are developed for each class of devices to extract critical performance metrics and benchmark with the state-of-the-art and theoretical limits for GaN. Sezawa modes are observed at frequencies up to 14.3GHz, achieving a record high in GaN MEMS to the best of our knowledge. A maximum piezoelectric coupling of 0.61% and frequency-quality factor product of 6x10<sup>12</sup>s<sup>-1</sup> are achieved for Sezawa resonators at 11GHz, with a minimum propagation loss of 0.26dB/λ for the two-port devices. The devices also exhibit high linearity with input third-order intercept points (IIP3) of 65dBm at 9GHz.</p><p dir="ltr">This work also investigates tunable acoustoelectric (AE) devices in the QuanFINE platform, leveraging its inherent 2DEG in the AlGaN/GaN heterostructure. Using 9.7GHz Sezawa mode acoustic delay lines, we report the highest frequency of AE in GaN to date. Active and passive AE devices are designed for voltage-dependent non-reciprocity and propagation loss without modification to the standard process for the High Electron Mobility Transistors (HEMTs) in MMICs. Drain/source Ohmic contacts control the drift velocity of the 2DEG, and the Schottky gate modulates 2DEG carrier concentration, resulting in a 30dB/cm separation between forward and reverse acoustic waves for a 2.56kV/cm lateral DC electric field and a maximum change in propagation loss of 50dB/cm for -5V DC at the control gate, respectively. The QuanFINE<sup> </sup>technology with AlGaN/GaN heterostructure enables a platform for switchable MEMS resonators and tunable acoustoelectric devices in MMICs for reconfigurable front end approaching mm-wave frequencies.</p>
