Bezprostřední a odložené rekonstrukce prsu / Delayed and Immediate Breast Reconstruction

OBJECTIVES : This work studies the indications, methods, results , satisfaction and partner relationships in immediate (IBR ) and deferred breast reconstruction ( DBR ) to objectively consider the benefits and indications IBR . METHOD : IBR between 1/2002-12/2012 underwent 51 ( 33.33 %) women ( 29-58 years, mean 41.5 , median 40.5 ) ; DBR 102 ( 66.67 %) ( 31-64 , mean 47.5 , median 47 ), data were obtained from medical records , questionnaires interviews and questionnaires , processed by statistical analysis RESULTS : Indications IBR : ≤ pT2N0M0 , low grade tumor ; DBR : ≥ 1 year of remission. Age at IBR was lower than the DBR ( p- 0.0004 ) Statistical differences in the modes of life after reconstruction the IBR a DBR were observed ( p- 0.1935-0.9659 ) predominates full and prevailing contentment. IBR does not burden patients ( 55 to 160 min, average 91.1 and 139.3 min, median 75 and 135 min ) between unilateral and bilateral operations are not statistically significant differences ( p -value 0.1065 ) . Complications prolonging healing rare - IBR 5 ( 8.33 %) , DBR 6 ( 5.8 % ) and mortality generalization low - IBR and 1 ( 1.96 % s ) DBR 1 and 2 ( 0.98 % and 1 , 96%) . Satisfaction with IBR was reported by 84.09 % , with 86.11 % DBR . The DBR was found 4 times greater risk of life or relationship. SUMMARY:...

Novel Cues Reinstate Cocaine-Seeking Behavior and Induce Fos Protein as Effectively as Conditioned Cues

abstract: The capability of cocaine-associated stimuli in eliciting craving in human addicts, even after extended periods of abstinence, is modeled in animals using cue reinstatement of extinguished cocaine-seeking behavior. This study aimed to examine brain activation in response to cocaine cues in this model apart from activation produced by test novelty using a novel cue control. Rats trained to self-administer cocaine paired with either an oscillating light or tone cue underwent daily extinction training and were then tested for reinstatement of extinguished cocaine-seeking behavior elicited by response-contingent presentations of either their assigned cocaine-paired cue or the alternate, novel cue. Additional controls received saline infusions and cue presentations yoked to a cocaine-trained rat. Brains were harvested for Fos immunohistochemistry immediately after the 90-min reinstatement test. Surprisingly, conditioned and novel cues both reinstated responding to a similar degree; however magnitude of reinstatement did vary by cue modality with the greatest reinstatement to the light cues. In most brain regions, Fos expression was enhanced in rats with a history of cocaine training regardless of cue type with the exception of the Cg1 region of the anterior cingulate cortex, which was sensitive to test cue modality. Also Fos expression within the dorsomedial caudate-putamen was correlated with responding in the novel, but not conditioned, cue groups. In subsequent experiments, we observed a similar pattern of reinstatement in rats trained and tested for sucrose-seeking behavior, whereas rats trained and tested with the cues only reinstated to a novel light and tone, but not a familiar cue. The results suggest that novel cues reinstate responding to a similar extent as conditioned cues regardless of whether animals have a history of operant-delivered drug or a natural reinforcer. Furthermore, similar brain circuits as those involved in cocaine-seeking behavior are activated by novel cues, suggesting converging processes exist to drive conditioned and novel reinforcement seeking. / Dissertation/Thesis / M.A. Psychology 2012

The Interaction of Active or Passive Roof Bolts, Stress Conditions, and the Immediate Roof Strata in a Longwall Mine in the United States

Safety is the most important aspect to the mining industry. Research having a direct and positive impact on mine safety is always needed and should be supported. There are several different types of primary roof supports that are used to try to maintain stable mine workings in order to extract the coal safely. The majority of roof bolts installed annually in underground coal mines in the USA use resin cartridges (Tadolini, 2006). The standard roof bolt used is a fully grouted resin rebar bolt. This is considered a passive (un-tensioned) support. Technological advances in roof support anchored with resin grout systems have resulted in the development of supports that are active (tensioned during installation). Generally it is thought that active anchors are superior in performance as roof support except in highly laminated weak roof. The assumption is that since an active system applies some pre-tension to the roof, it will provide a more stable roof beam. There is, however, a lack of real data to back up this perception. A systematic attempt is through this project that addresses this knowledge gap. Without a clear idea on how different roof bolt systems work in-situ, the consequences could have a significant impact on the safety of the miners working underground as well as production costs. Supports are now often only chosen based on perceived ideas instead of real data. Such assumed superior performance of the support system could lead to false economy. Essentially, it needs to be demonstrated to what degree or under what conditions, if any, these technological advances in roof support improve ground conditions and opening stability and ultimately improve the safety of the miners. There is an argument regarding whether passive bolts could truly be better than active bolts. The focus is centralized towards a cost and performance issue. The overall goal of the proposed project was to develop an understanding of the interaction between different roof bolt types and the immediate strata in a longwall headgate as the longwall face progresses. In order to monitor the interactions between the strata and roof bolts, specially instrumented bolts were installed with six strain gauges that were electronically accessed. A look into the initial bolt loads was used to compare three different bolt types (both tensioned and untensioned) upon installation. These loads continued to be monitored and analyzed as the longwall face approached and passed the location of the instrumented bolts. Along with analyzing instrumented bolt data compared to a passing longwall face, a limited finite element model was set up within Flac3D to represent real conditions, and compare the in situ data collected to the computer outputs in order to establish beginning phases of validating the results.

O desempenho do equilíbrio vem sendo associado a melhora do desempenho atlético (ou seja, saltos verticais, sprints, tarefas de mudança de direção) e até mesmo está sendo considerado uma condição importante para o atleta tornar-se de alto nível. Atualmente equipes e atletas estão utilizando o treino de equilíbrio como uma modalidade de exercício para melhorar o equilíbrio, prevenir lesões nas extremidades inferiores, reabilitar a propriocepção e função neuromuscular. Este treinamento, tem como objetivo gerar instabilidade ao atleta e com isso gerar melhora do controle postural, propriocepção e alguns casos força. A melhora controle postural se dá através dos mecanismos neuronais, envolvendo redes espinhais e supraespinhais. No nível da coluna vertebral, o treino de equilíbrio inibe a excitabilidade reflexa espinhal devido a um aumento na inibição pré-sináptica. Melhorias no desempenho do equilíbrio estão associadas a redução da excitabilidade córticoespinal e cortical (nível supraespinhal), gerando regulações no reflexo. Além disso, adaptações favoráveis após treinamento de equilíbrio podem estar associadas as mudanças estruturais na massa cinzenta e substância branca no lobo pré-frontal em indivíduos jovens saudáveis. Portanto, mudanças geradas no Sistema Nervoso Central (SNC) podem contribuir na melhora do desempenho do equilíbrio e nos efeitos que tendem a prevenir as lesões. Apesar do treinamento equilíbrio ser uma ferramenta efetiva para a melhoria do controle postural e variáveis neuromusculares, ainda há contradição na literatura em relação aos seus efeitos no desempenho de atletas e pouco se sabe sobre o efeito imediato deste treinamento. Preparadores físicos e fisioterapeutas devem considerar que os indivíduos submetidos a treinos de equilíbrio possam apresentar déficit imediato. O objetivo desta pesquisa é determinar quais parâmetros biomecânicos são influenciados após o treinamento equilíbrio em atletas de forma imediata e crônica. As coletas foram realizadas em 45 participantes do sexo feminino e 15 do masculino com idade entre 18 e 35 anos, sendo que para o experimento 1 e 2 foram avaliadas trinta jogadoras de basquete, no experimento 3 atletas de diversas modalidades, quinze mulheres e quinze homens. No experimento 1 e 3 foram realizadas avaliações imediatamente após o treinamento de equilíbrio, 15 minutos e 30 minutos. No experimento 1 foram avaliadas as variáveis de equilíbrio (área, comprimento e velocidade do centro de pressão - COP) e do salto contramovimento (altura e força de reação do solo na aterrissagem), já no experimento 3 foram realizados testes isométricos e isocinético para verificação do torque e com eletromiografia nos músculos reto femoral, vasto lateral e vasto medial para cálculos da eficiência neuromuscular. No experimento 2 foi realizado treinamento equilíbrio com jogadoras de basquete por oito semanas. Foram divididas em dois grupos (controle e treinamento), realizaram avaliações pré e pós treinamento. As avaliações realizadas foram senso de posição ativa, cinestesia, teste isométrico e teste isocinético de joelho dominante, equilíbrio unipodal e salto contramovimento. Após a verificação da normalidade (ShapiroWilk) e homogeneidade (Levene), foi utilizado o teste T independente para comparar as variáveis dependentes entre grupos. Para verificar o efeito do treinamento de equilíbrio, foi aplicada a análise de variância (ANOVA) two-way. Na sequência utilizou-se a ANOVA one way, com correção de Bonferroni para análises par a par quando encontradas interações ou quando encontrados somente efeitos principais. Para todas as variáveis foram adotados o nível de significância de α < 0.05. Após 8 semanas de treino, o grupo treinamento melhorou o desempenho do salto, do equilíbrio olhos fechado e olhos aberto membro não dominante, do senso de posição, torque isométrico e isocinético, mas não apresentou melhora no equilíbrio olhos abertos membro dominante e cinestesia. Já no efeito imediato apenas a altura do salto contramovimento não apresentou déficit imediatamente pós o treino de equilíbrio. As demais variáveis, força de reação do solo, equilíbrio unipodal, torque isométrico, torque isocinético e eficiência neuromuscular apresentaram déficit imediatos ao treino e algumas delas levando não retornaram ao valor basal após 30 minutos de treino. / Balance is an important prerequisite for learning complex, life-related motor skills. Balance performance has been associated with improved athletic performance (ie, vertical jumps, sprints, change-of-direction tasks) and is even considered to be an important condition for the athlete to become a high-level athlete. Currently teams and athletes are using balance training as an exercise modality to improve balance, prevent injury to the lower extremities, rehabilitate proprioception and neuromuscular function. This training aims to generate instability to the athlete and with that improves athletes’ postural control through neuronal mechanisms, involving spinal and supraspinal networks. At the spinal level, balance training apears to inhibit spinal reflex excitability due to an increase in presynaptic inhibition. Improvements in balance performance can be associated with substantially reduce corticospinal and cortical excitability so that the traininginduced reflex down-regulation is associated with improvements in balance performance (supraspinal level). In addition, favorable adaptations after balance training may be associated with structural changes in gray matter and white matter in the prefrontal lobe in healthy young subjects. Therefore, changes generated in the Central Nervous System (CNS) may contribute to improved balance performance and to effects that tend to prevent injury. Although balance training is an effective tool for the improvement of postural control and neuromuscular variables, there is still contradiction in the literature regarding its effects on the performance of athletes and little is known about the immediate effect of this training. Physical trainers and physiotherapists should consider that individuals submitted to balance training may present immediate deficit. The aim of this research is to determine which biomechanical parameters are influenced after the balance training in athletes immediately and chronically. The samples were collected in 45 female participants and 15 male participants aged 18 to 35 years. For the experiment 1 and 2, thirty basketball players were evaluated, in the experiment 3 athletes of various modalities, fifteen women and fifteen men. In experiments 1 and 3, evaluations were performed immediately after the balance training, 15 minutes and 30 minutes. In the experiment 1, were evaluated the variables of balance and countermovement jump, and in the experiment 3, was performed isometric and isokinetic tests with electromyography in the rectus femoris, vastus lateralis and vastus medialis muscles. In experiment 2, balance training was performed with basketball players for eight weeks. They were divided into two groups (control and training), performed pre and post training assessments. The evaluations performed were a sense of active position, kinesthesia, isometric test and isokinetic test of dominant knee, unipodal balance and countermovement jump. After verification of normality (Shapiro-Wilk) and homogeneity (Levene), the independent T-test was used to compare the dependent variables between groups. To verify the effect of the balance training, the ANOVA two-way was applied. An ANOVA one way, with Bonferroni correction, was used for parity analyzes when interactions were found or when only main effects were found. For all variables, the level of significance of α <0.05 was adopted. After 8 weeks of training, the training group improved the performance of the jump, the closed eyes balance and open eyes non-dominant limb, the sense of position, isometric and isokinetic torque, but did not show improvement in the open eyes limb dominant balance and kinesthesia. Already in the immediate effect only the height of the countermovement jump did not present deficit immediately after the balance training. The other variables, ground reaction force, unipodal balance, isometric torque, isokinetic torque and neuromuscular efficiency presented immediate training deficit, and some of them did not return to baseline after 30 minutes of training. / CAPES: 001

This thesis mapped the view of the DNR issue through the eyes of nurses working at the resuscitation department and ICU. A DNR (Do Not Resuscitate) order indicates situations when starting immediate resuscitation is not indicated in respiratory or cardiac arrest. DNR is ordered by a doctor after evaluation of the overall clinic condition and further prognoses of a patient. A patient him/herself has the right to decide on not starting resuscitation in the form of a request expressed in advance. The main task of the nurse continues to care for the patient and her/his family. Qualitative research showed thad nursing care remains unchanged even in a patient with DNR order. Nurses see DNR first of all as termination of patient suffering and consider DNR definitely acceptable in old and terminally ill people. Non resuscitation of a young person or a child is unacceptable for them. They consider the decision making itself an ethical problem. There is vain and inefficient treatment burdening a person on one side and person?s autonomy and participation in deciding on his/her own fate.

O estudo inclui 10 pacientes que receberam 10 implantes e próteses provisórias unitárias e 5 pacientes que receberam 25 implantes, 5 implantes cada paciente entre os foramens mentoniano, estes foram unidos e receberam uma prótese tipo protocolo. Todas as próteses foram instaladas imediatamente após a cirurgia. Em um intervalo de 6 e 12 meses todos os pacientes foram radiografados e avaliados clinicamente. Não foram registradas perdas no grupo de implantes unitários e o grupo de implantes que recebeu prótese protocolo perdeu 2 implantes. Não houve alterações nos parâmetros periodontais (clínicos e radiográficos) entre os períodos de 6 e 12 meses, e as diferenças encontradas entre os implantes unitários e unidos, parecem estar mais relacionados ao local de inserção (maxila - mandíbula). Podemos concluir que a carga imediata é um procedimento previsível tanto para implantes unitários como em implantes unidos. / The study includes 10 patient that received 10 implants and unitary temporary prostheses and 5 patient that received 25 implants, 5 implants each patient one in the area previous of jawbone, these were united and they received a prosthesis type protocol. All of the prostheses were installed immediately after the surgery. In an interval of 6 and 12 months all the patients were x-rayed and evaluated clinically. Losses were not registered in the group of unitary implants and the group of implants that it received prosthesis protocol lost 2 implants. There were not alterations in the periodontal parameters (clinical and radiographic) among the periods of 6 and 12 months, and the differences found among the unitary and united implants, they seem to be more related to the insert place (jawbone - jaw). We can conclude that the immediate load is a previsible procedure so much for unitary implants as in united implants.

Immediate Early Genes Anchor a Biological Pathway of Proteins Required for Memory Formation, Long-Term Depression and Risk for Schizophrenia

While the causes of myriad medical and infectious illnesses have been identified, the etiologies of neuropsychiatric illnesses remain elusive. This is due to two major obstacles. First, the risk for neuropsychiatric disorders, such as schizophrenia, is determined by both genetic and environmental factors. Second, numerous genes influence susceptibility for these illnesses. Genome-wide association studies have identified at least 108 genomic loci for schizophrenia, and more are expected to be published shortly. In addition, numerous biological processes contribute to the neuropathology underlying schizophrenia. These include immune dysfunction, synaptic and myelination deficits, vascular abnormalities, growth factor disruption, and N-methyl-D-aspartate receptor (NMDAR) hypofunction. However, the field of psychiatric genetics lacks a unifying model to explain how environment may interact with numerous genes to influence these various biological processes and cause schizophrenia. Here we describe a biological cascade of proteins that are activated in response to environmental stimuli such as stress, a schizophrenia risk factor. The central proteins in this pathway are critical mediators of memory formation and a particular form of hippocampal synaptic plasticity, long-term depression (LTD). Each of these proteins is also implicated in schizophrenia risk. In fact, the pathway includes four genes that map to the 108 loci associated with schizophrenia: GRIN2A, nuclear factor of activated T-cells (NFATc3), early growth response 1 (EGR1) and NGFI-A Binding Protein 2 (NAB2); each of which contains the "Index single nucleotide polymorphism (SNP)" (most SNP) at its respective locus. Environmental stimuli activate this biological pathway in neurons, resulting in induction of EGR immediate early genes: EGR1, EGR3 and NAB2. We hypothesize that dysfunction in any of the genes in this pathway disrupts the normal activation of Egrs in response to stress. This may result in insufficient electrophysiologic, immunologic, and neuroprotective, processes that these genes normally mediate. Continued adverse environmental experiences, over time, may thereby result in neuropathology that gives rise to the symptoms of schizophrenia. By combining multiple genes associated with schizophrenia susceptibility, in a functional cascade triggered by neuronal activity, the proposed biological pathway provides an explanation for both the polygenic and environmental influences that determine the complex etiology of this mental illness.

Influência do selamento imediato da dentina na resistência de união de diferentes sistemas adesivos junto a cimentações de restaurações indiretas / Influence of immediate dentin sealing on bond strength of different adhesive systems on the cementation of indirect restorations

Hipótese do estudo: A presente pesquisa hipotetiza que a técnica de cimentação está diretamente ligada a resistência adesiva de restaurações indiretas. Objetivos: O presente estudo experimental verificou se o selamento imediato da dentina, com diferentes sistemas adesivos, apresentaram melhores resultados de resistência de união junto a cimentação resinosa de restaurações indiretas. Métodos: Foram utilizados 48 terceiros molares humanos, divididos em seis grupos de acordo com as técnicas de cimentação testadas: grupo 1) Sistema Variolink II (Ivoclar Vivadent); grupo 2) Clearfill SE (Kuraray) + Variolink II; grupo 3) Selamento com Clearfill SE + Sistema Variolink II após 24h; grupo 4) Selamento com Exite DSC (Ivoclar Vivadent) + Sistema Variolink II; grupo 5) Selamento com Clearfill + Tetric Evo Flow (Ivoclar Vivadent) + Sistema Variolink II após 24h; Grupo 6) selamento com Exite DSC + Tetric Evo Flow + Sistema Variolink II após 24h. Oito amostras de cada grupo foram para máquina de cortes seriados (Isomet 1000) para obtenção de palitos com 1mm2 de área. Os palitos de cada grupo foram submetidos ao teste de microtração. O teste de microtração foi utilizado após 24h da cimentação. Os dados foram estatisticamente analisados pelo teste de Kruskal-Wallis e o teste de Dunn (5%). Diferenças significativas foram detectadas (p=0,0074). Resultados: Os grupos 1 e 2 apresentaram mediana de resistência de união intermediária, sendo estatisticamente semelhantes aos demais grupos. O grupo 6 apresentou mediana significativamente mais alta que os grupos 3, 4 e 5, os quais foram semelhantes entre si. Adicionalmente os tipos de fratura foram analisados com auxílio de lupa estereoscópica, em todos os grupos a porcentagem de fraturascoresivas em dentina ou mistas foi menor em comparação as fraturas adesivas e coesivas em cerômeros. Conclusão: O selamento imediato da dentina com adesivo de condicionamento total junto a resina flow demonstrou maior resistência adesiva. / Hypothesis of the study: This study hypothesizes that the cementation technique is directly related to the bond strength of indirect restorations. Objectives: The present experimental study examined whether the immediate dentin sealing with different bonding systems, the best-performing bond strength in the resin cementation of indirect restorations. Methods: A total of 48 human third molars was divided into six groups according to the techniques of tested: group 1) Variolink System II (Ivoclar Vivadent), group 2) Clearfill SE (Kuraray) + Variolink II, Group 3) with Seal Clearfill SE + System Variolink II after 24h, group 4) Sealing successfully DSC (Ivoclar Vivadent) + Variolink System II, Group 5) Sealing with Clearfill + Tetric Evo Flow (Ivoclar Vivadent) + System Variolink II after 24h, Group 6) sealing successfully DSC + + Tetric Evo Flow System Variolink II after 24h. Eight samples of each group were machine serial sections (Isomet 1000) to obtain sticks with 1 mm2 in size. The sticks of each group were subjected to microtensile testing. The microtensile test was performed used after 24h of cementation. The data were statistically analyzed by Kruskal- Wallis test and Dunn (5%). Significant differences were detected (p = 0.0074). Results: Groups 1 and 2 had a median of intermediate bond strength, being statistically similar to other groups. The sixth group median significantly higher than groups 3, 4 and 5, which were similar. Besides the types of fracture were analyzed with the aid of a stereomicroscope. Conclusions: The immediate detin sealing with total etch adhesive and the flowable resin showed higher bond strength.

Foram pesquisados os seguintes aspectos: índices de placa e de sangramento, estabilidade dos implantes por frequência de ressonância (FR) e perda óssea marginal. Foram avaliados 76 implantes, em 19 pacientes, divididos em 2 grupos: A (n=12, All-on-four® - Nobel Biocare®) e B (n=7, Neopronto® - Neodent®). Os pacientes foram acompanhados nos momento da instalação dos implantes (inicial), 6 meses, 1, 2 e 7 anos após a instalação. Os resultados mostraram uma taxa de sucesso de 100% para os implantes do grupo A e de 90% para o grupo B. Os índices de placa não apresentaram diferenças estatisticamente significativas e foram, no grupo A, de 71,87%, 63,54% e 47,92%, e, no grupo B de 26,78%, 41,07% e 37,50%, para as avaliações de 1, 2 e 7 anos, respectivamente. O índice de sangramento para o grupo A foi de 43,75%, 42,71% e 43,75% e para o grupo B foi de 53,57%, 51,79% e 48,21%, nos tempos de avaliação de 1, 2 e 7 anos, respectivamente, sem diferenças estatísticas. Não foi observado supuração em nenhum tempo de avaliação. Os resultados de FR não diferiram estatísticamente entre os dois grupos, entretanto, diminuíram entre a avaliação inical (grupo A = 69,27ISQ e grupo B = 68,29ISQ) e de 6 meses (grupo A = 64,65ISQ e grupo B = 66,50ISQ), e aumentaram nos anos seguintes para os 2 grupos, sendo para o grupo A de 66,17ISQ, 66,05ISQ e 69,87ISQ e para o grupo B de 67,78ISQ, 68,13ISQ e 73,61ISQ para os tempos de 1, 2 e 7 anos, respectivamente. Em relação a perda óssea, existiu diferença significativa entre grupos e foi de 0,49mm e 0,67mm após 1 ano e de 1,03mm e 1,27mm, após 7 anos, para os grupos A e B, respectivamente. Em relação à posição dos implantes, o grupo B apresentou diferença estatística entre os implantes centrais e distais nas avaliações de FR e perda óssea. Concluiu-se que 1- a taxa de sucesso foi de 100% para o grupo A e de 90% para o grupo B; 2- os índices de placa e de sangramento diminuíram ao longo dos anos de avaliação; 3- a FR foi semelhante entre os grupos, mas com diferença significativa no grupo A para o critério tempo e, no grupo B para os critérios tempos de avaliação e posição dos implantes; 4- perda da altura da crista óssea foi diferente significativamente entre os grupos, sendo maior no grupo B. / This study evaluated the success of implants used to support mandibular complete full-arch fixed prosthesis employing two rehabilitation systems with immediate loading after 7 years. The following aspects were investigated: plaque and bleeding index, implant stability by resonance frequency analysis and marginal bone loss. The 76 implants were evaluated in 19 patients divided in 2 groups: A (n=12, All-on-four® - Nobel Biocare®) and B (n=7, Neopronto® - Neodent®). The follow-up visits were at the time of implant placement (initial), 6 months, 1, 2 and 7 years after surgery. The results showed a success rate of 100% for implants in group A and 90% for group B. The plaque index did not differ statistically signifcant and were in group A, 71.87%, 63.54% and 47.92%, and in group B 26.78%, 41.07% and 37, 50% for 1, 2 and 7 years of follow-up, respectively. The bleeding index for group A was 43.75%, 42.71% and 43.75% and for group B was 53.57%, 51.79% and 48.21%, for 1, 2 and 7 years of follow-up, respectively, no statistical differences. Suppuration was not observed at any time of evaluation. The results of FR did not differ significantly between groups, however, decreased between initial (group A = 69,27ISQ and group B = 68,29 ISQ) and 6 months (group A = 64,65ISQ and group B = 66,50ISQ), and increased in the following evaluations for both groups, in group A was 66,17ISQ, 66,05ISQ and 69,87ISQ and group B was 67,78ISQ, 68,13ISQ and 73,61ISQ for 1, 2 and 7 years of follow-up, respectively. In relation to bone loss, a significant difference existed between groups and was 0,49mm and 0,67 mm after 1 year and 1,03mm and 1,27mm after 7 years, for groups A and B, respectively. Regarding the position of the implants, group B showed statistical difference between the central and distal implants for FR and bone loss. It was concluded that 1- Success rate was 100% for group A and 90% for group B; 2 - Plaque and bleeding index decreased over the follow-up; 3- FR was similar between groups, but with a significant difference in group A for thw criterion time, and in group B for the criteria evaluation times and position of the implants, 4- bone loss was significantly different between groups, and higher in group B.

Os implantes imediatos que recebem restaurações imediatas vêm sendo apresentados na literatura como uma opção de tratamento confiável para a reposição de dentes condenados. No entanto, recentemente, dúvidas com relação à estabilidade dos tecidos periimplantares utilizando esta técnica têm sido levantadas. Neste estudo prospectivo, doze pacientes tiveram seus incisivos centrais superiores condenados substituídos por implantes imediatos e restaurações imediatas. Durante 18 meses, foram acompanhados o comprimento das coroas implanto-suportadas e a distância entre a ponta das papilas e a borda incisal dos dentes vizinhos. Não foi observada diferença estatisticamente significativa entre as medidas obtidas. Os resultados obtidos podem ser atribuídos ao rigoroso critério de seleção dos casos e a técnica cirúrgica aplicada. Pode-se afirmar que os tecidos moles periimplantares de implantes imediatos com restaurações imediatas, mantiveram-se estáveis ao longo dos 18 meses de observação seguindo-se os critérios de seleção e execução desse estudo. / The immediate implants that receive immediate restorations have been presented in literature as a reliable alternative to the replacement of condemned teeth. Recently, however, doubts regarding the perimplantar tissue stability have been raised. In this prospective study, twelve patients had their central maxillary incisor replaced by immediate implant and immediate restoration. After 18 months the papillae height and the Zenith of implant supported crown were compared to the dimensions before dental extraction. No statistically significant difference was observed between the obtained measurements. The authors justify the obtained results to the rigorous selection criteria of the cases that had been part of this sample and the surgical technique applied. In the selections criteria and executions followed in this work to the immediate implant placement and immediate restorations, we can state that the soft perimplantar tissues have been remained stable for the 18 months? observation.
