An Evaluation of Computational Methods to Support the Clinical Management of Chronic Disease Populations

Innovative primary care models that deliver comprehensive primary care to address medical and social needs are an established means of improving health outcomes and reducing healthcare costs among persons living with chronic disease. Care management is one such approach that requires providers to monitor their respective patient panels and intervene on patients requiring care. Health information technology (IT) has been established as a critical component of care management and similar care models. While there exist a plethora of health IT systems for facilitating primary care, there is limited research on their ability to support care management and its emphasis on monitoring panels of patients with complex needs. In this dissertation, I advance the understanding of how computational methods can better support clinicians delivering care management, and use the management of human immunodeficiency virus (HIV) as an example scenario of use. The research described herein is segmented into 3 aims; the first was to understand the processes and barriers associated with care management and assess whether existing IT can support clinicians in this domain. The second and third aim focused on informing potential solutions to the technological shortcomings identified in the first aim. In the studies of the first aim, I conducted interviews and observations in two HIV primary care programs and analyzed the data generated to create a conceptual framework of population monitoring and identify challenges faced by clinicians in delivering care management. In the studies of the second aim, I used computational methods to advance the science of extracting from the patient record social and behavioral determinants of health (SBDH), which are not easily accessible to clinicians and represent an important barrier to care management. In the third aim, I conducted a controlled experimental evaluation to assess whether data visualization can improve clinician’s ability to maintain awareness of their patient panels.

The overall purpose of this project is to create a tool that will be able to generate reports for measurements and calculations performed by the Perception system. At present, this is a manual procedure that is both time-consuming, inefficient, and increases the risk of error. Since Perception is developed in C#, the reporting tool will be developed in ASP.NET Core with C# based on the MVC pattern and all data will be stored in Microsoft's SQL server. Transfer of data between systems will be handled by export files that Perception can generate as well as a self-developed import function in the reporting tool. With a few simple settings and options, the operators will be able to generate reports for the selected project and then copy them to the final report. Initial work with design and usability, based on the existing graphic profile, will result in an application that significantly facilitates the work of producing test reports. To make the design appealing, Bootstrap and self-written CSS will be used and to make the user experience as good as possible, JavaScript and AJAX will be used together with jQuery and thus be able to provide relevant information without the need of page reload. Focus will be placed on validating imported data to ensure that correct information is available and stored in the database. To further help the users, relevant messages will show status both in case of problems but also in case of successful procedures. / Det övergripande syftet, för detta projekt, är att skapa ett verktyg som ska kunna generera rapporter för mätningar och beräkningar utförda i systemet Perception. I dagsläget är detta ett manuellt förfarande som både är tidskrävande och ineffektivt vilket också ger en ökad risk för felkällor. Eftersom Perception är utvecklat i C# kommer rapportverktyget att utvecklas i ASP.NET Core med C# baserat på MVC-mönstret och all data kommer lagras i Microsofts SQL server. Överföringen av data mellan systemen kommer att ske med hjälp av de exportfiler som Perception kan generera samt en egenutvecklad importfunktion i rapportverktyget. Med några enkla inställningar och val ska operatörerna kunna generera rapporter för valt projekt och sedan kunna kopiera dem till slutrapporten. Ett inledande arbete med design och användbarhet, som grundar sig på den befintlig grafisk profilen, ska mynna ut i en applikation som väsentligt underlätta arbetet med att producera testrapporter. För att göra designen tilltalande kommer Bootstrap och egenskriven CSS att användas och för att användarupplevelsen ska bli så bra som möjligt kommer JavaScript samt AJAX att användas tillsammans med jQuery och på så vis kunna ge relevant information utan att sidan behöver uppdateras. Stort fokus kommer att läggas på kontroller av importerad data för att säkerställa att rätt information finns representerat samt sparas till databasen. För att ytterligare underlätta för användarna ska relevanta meddelanden ge status både vid problem men också vid lyckade procedurer.

The arrival of internet and its services, including current trends such as social media and networks, has changed and enriched information behaviour, channels and marketing strategies between information suppliers and user. Although the information industry subjects as database vendors are traditional in their marketing strategies, their engagement in social media marketing is proactive and developing. The exploration confirmed that 10 of 12 examined subjects actively develop these channels and support them.

The thesis focuses on various information services over digital archives and institutional repositories in the area of science and research. It aims to ascertain these online services and introduce, analyse and assess selected examples in detail. Their quantitative as well as qualitative features are emphasized - The work is logically divided into four chapters. The first one defines the basic terminology and outlines the contemporary status of scholarly communication on the Internet. The second chapter introduces a comprehensive list of services as well as their suggested typology. The third chapter constitutes the core of the thesis and presents the descriptions and analysys of the selected services. The fourth final part includes the comparison of their relevant parameters, final assessment and the perspectives of their future development. Vertrauenswürdigkeit und Wissenschaftlichkeit von Visualisierungen im partizipativen Prozess der Endlagersuche für hoch radioaktive Abfälle in Deutschland

Nach über 60 Jahren Nutzung ziviler Kernenergie in Deutschland und dem Atomausstieg bis 2022 stellt sich die Frage nach der Endlagerung (hoch-) radioaktiver Abfälle. Für die Endlagersuche in Deutschland trat 2017 das Standortauswahlgesetz (StandAG) in Kraft, das ein partizipatives, wissenschaftsbasiertes und transparentes Verfahren fordert (Deutscher Bundestag, 2020). Um die in diesem Verfahren auftretenden Fragen und Ergebnisse gesellschaftlich und wissenschaftlich fundiert diskutieren zu können, werden von den am Prozess beteiligten Stakeholdern in zunehmendem Maße auch digitale Informationsangebote bereitgestellt. Diese nutzen verschiedene Arten des Informationsdesigns (Umwandlung komplexer Daten in einfachere, nützliche und efektive Informationen, um Bedürfnisse und Ziele Nutzender kontextabhängig zu erfüllen (Herrera, 2013)), u.a. Kartendarstellungen, wie sie auch in anderen partizipativen Verfahren eingesetzt werden, um niederschwellig Information bereitzustellen (Moser et al., 2020). Vertrauen ist besonders wichtig, um den Betroffenen bei der Entscheidung zu helfen, ob sie an öffentlichen Beteiligungsprozessen teilnehmen (Santamaría-Philco & Wimmer, 2018). In früheren Umfragen wurde den verschiedenen Stakeholdern generell ein sehr unterschiedlicher Grad an Vertrauenswürdigkeit zugestanden (Drögemüller, 2017; Hocke, Stolle & Gloede, 2003; TNS Opinion & Social, 2008). Bei kritischer Würdigung der Fähigkeit dieser Stakeholder, die öffentliche Meinung zu beeinflussen, stellt sich die Frage, ob eine Beurteilung des Inhalts anhand des Informationsdesigns oder aufgrund des dem Stakeholder zugestandenen Vertrauens erfolgt. Wissenschaftlichkeit hingegen ist als Kriterium vor allem mit Blick auf das StandAG relevant, welches einen wissenschaftlich fundierten Prozess fordert. The management and diffusion of HIV/AIDS information in institutions of higher learning in South Africa

Thesis submitted in fulfillment of the requirements for the award of the Degree of Doctor of Philosophy in Library and Information Science at the University of Zululand, 2005. / The impact of HIV/AIDS (human immunodeficiency virus/ acquired immune deficiency syndrome) is devastating worldwide especially among tertiary institutions whose constituencies are within the age bracket between 15-45 years. Unfortunately there is still no cure for the disease, and one way of controlling the rampant nature of the pandemic is through educational and enlightening interventions backed by appropriate information. The aim of the study was to assess the framework, nature and scope of the institutional response as well as the appropriateness of HIV/AIDS information dissemination interventions developed and employed by institutions of higher learning in South Africa for the prevention of the spread of the pandemic. The study was informed by theoretical framework grounded on the Diffusion of Innovations theory. Both qualitative and quantitative research design and methodologies were employed largely through survey, observation and document analysis. The study targeted HIV/AIDS service providers, health centers and institutional libraries within all public universities and technikons in South Africa. The respondents within institutions were identified largely through non-probability sampling techniques such as snowball and purposive sampling. The study mapped out the HIV/AIDS response of the higher education sector in relation to programmes offered and strategies and methods that are used to manage the pandemic and disseminate information. The findings reveal that the response of the higher education sector to HIV/AIDS is not uniform, but there is a positive move towards strong management of HIV/AIDS and information diffusion. Secondly, it is observed that the disease has some impact on institutional mandates such as teaching, learning, research and community service. Unexpectedly, the study confirmed widely held views that are also reported in related studies, that the response of academic institutions to the disease is still characterized by silence, denial, discrimination and stigma as most institutions do not address the disease openly. Thirdly, it was established that in those institutions where there is an AIDS Centre the response seemed to be more systematic and well guided as compared to those that relied on the services of the health centre. Fourthly, there was no distinction made in terms of the nature and strength of the institutional response between service providers that had higher academic qualifications and those that did not have. Most highly qualified respondents though had other academic responsibilities, dealt with HIV/AIDS as additional job. Fifthly, it was easy to distinguish between historically advantaged and disadvantaged institutions, as the tatter had interventions that were underdeveloped and limited in scope and depth. Similarly, universities as compared to technikons demonstrated more intense interventions and better resource provision. Sixth, in most institutions management supports the institutional HIV/AIDS management and response. This involvement was evident through observation on the nature of the response, capacity buildings and resources on the ground. However, it was sadly observed that this executive commitment to HIV/AIDS seemed to be overridden by other priorities such as the reconfiguration and reconstruction of the sector. Further, noted that all institutions have HIV/AIDS policies, but some of them have not implemented these policies. Seven, though the Higher Education HIV/AIDS Programme is coordinating the HIV/AIDS response within the whole sector, it does not seem to have all the answers for the systemic problems that are cropping up. Eight, HiV/AlDS information is disseminated by the institutional libraries, HIV/AIDS service providers and health centers. Mostly, information is disseminated in print form while other modem media seemed to be underutilized and repackaging is not extensively done due, partly, to shortage of resources and capacities. The study found strong link between the theoretical models earlier mentioned and results of the study. Specifically, these theories confirmed the importance of the content of HIV/AIDS messages and the value of horizontal and vertical communication strategies. The study recommends that the institutional response needs to be revamped and redesigned to improve the traditional information dissemination strategies that are used by most academic institutions. Information dissemination strategies should be designed in line with current trends in socio-cultural and political lifestyles of young people. However, though there are still flaws and inefficiencies, the sector is responding positively to the epidemic and efforts are being made to synchronize and coordinate the systemic response. The study recommends further research on feasibility, applicability and effectiveness of the centralized coordination of the higher education HIV/AIDS response. It also recommends that the higher education sector should be more involved in the initiative of the Higher Education HIV/AIDS Programme to make valuable contributions based on experiential encounters. Similarly, strategies should be rapidly implemented to redress past imbalances in relation to strengthening capacities and resources of previously disadvantaged institutions to enable them to deal effectively with the disease. Other issues have been unearthed and a model for effective HIV/AIDS management and information diffusion in the sector suggested.
