Spousal Crossover Effects of Job Demands on Health

The primary goal of this dissertation was to examine the crossover effects of an individual's job demands on the perceived health of the spouse. Using a sample of grocery store employees and their partners, crossover relationships were examined for 90 dyads using Structural Equation Modeling. Although the models had good fit, the proposed direct crossover relationships of job demands on health were not supported. Job demands of one partner did not significantly predict health outcomes in the other partner; although follow-up analyses found significant crossover of women's job satisfaction on men's mental health scores. An additional crossover relationship was supported with the finding that crossover of health between spouses (i.e. physical and mental health components of general health) was significant. The second goal of the present dissertation study was to develop a typology of crossover research. All crossover studies to date were reviewed and categorized in a discussion of the proposed typology.

Burnout and job engagement in UK cancer care staff : how do they relate to job stress and satisfaction and turnover intentions

Background: Health care professionals are at risk of developing burnout due to the inherently stressful nature of their work. Burnout has been found to compromise the wellbeing of health care professionals and their ability to provide optimal patient care. Job engagement is proposed to be the antipode of burnout and is concerned with occupational well-being. It is hoped that through a better understanding of factors related to job engagement and burnout, occupational well-being of health care professionals and their ability to care for patients can be improved. Systematic review: A systematic review of the literature on burnout and job engagement in physicians and nurses since 2002 identified seven studies. Findings suggest that burnout and job engagement are independent constructs, albeit negatively related. These findings contribute to the current knowledge about the relationships between burnout and engagement dimensions and provide a framework for interventions aimed at increasing occupational well-being among front line medical professionals. Aims: A study was conducted to assess levels of job engagement and burnout and their relationship with turnover intentions and job satisfaction and stress in the entire workforce of a Cancer Centre in the United Kingdom. Participants and procedure: 150 cancer care workers completed a cross-sectional questionnaire entailing the Maslach Burnout Inventory, the Engagement Indicator, measures of job satisfaction, stress, turnover intentions and demographics. Results: Mean scores of emotional exhaustion did not differ from normative data, while lower levels of depersonalisation and lack of accomplishment were found. Furthermore, engagement scores were significantly higher than in the normative sample and the majority reported high levels of job satisfaction and indicated no turnover intentions. Path analysis provided preliminary support for an exploratory model indicating that engagement mediates the relationship between job stress, burnout and job satisfaction and turnover intentions. Implications: It is important that, despite increasing pressure to reduce costs, service planning is mindful of the continuous and long-term process required to maintain and facilitate engagement and job satisfaction, which appear important to the long term retention of staff. Conclusions: Work overload and a perception of being poorly managed and resourced appear to be risk factors for burnout. However, engaged employees with high levels of personal accomplishment may experience job satisfaction and desire to stay in their jobs despite high levels of occupational stress. Further research is required to identify factors predictive of personal accomplishment and job engagement in oncology services.

No Sistema Único de Saúde Brasileiro (SUS), a reabilitação de pessoas com deficiência física segue um modelo de rede de serviços hierarquizados em diferentes níveis de complexidade. Este estudo tem como objetivos levantar características do modelo de reabilitação física num serviço de referência do SUS e analisar a incorporação do princípio doutrinário da integralidade na atenção à saúde. Realizou-se um estudo de caso com dados secundários de pesquisa sobre a rede de assistência a pessoas com deficiência física na região nordeste do Brasil. A análise de conteúdo dos grupos focais, entrevista semi-estruturada e documentos institucionais foram antecedidos por análise de documentos que sustentam as práticas de reabilitação normativa e conceitualmente. Os resultados apontam para um modelo de reabilitação que supera a concepção tradicional de saúde e de deficiência, mas enfrenta desafios quanto à articulação com o conjunto da atenção à saúde e com a efetivação dos resultados esperados da reabilitação. / Salvador

A intenção de prestar assistência integral ao trabalhador informal em condições precárias ou perigosas deu origem a um estudo descritivo prospectivo de base populacional investigando-se o tempo trabalhado e acidentes ou doenças relacionadas ao trabalho durante quatro meses mediante visitas semanais pelos Agentes Comunitários de Saúde da Equipe. Aplicou-se instrumento de coleta de dados elaborado em projeto-mãe para outra cidade após adaptação às condições locais. Observou-se que 62,1% dos trabalhadores eram do sexo feminino; 43,9% tinham entre 35 e 50 anos; 55,6% trabalhavam por conta própria; 39,9% eram empregados e 8,4% dos entrevistados pagavam a Previdência Social. O motivo de ingresso no setor informal foi o desemprego para 44,1% enquanto 24,6% buscavam melhores rendimentos. A jornada diária de trabalho foi de 8 horas/dia para 39,7%; o trabalho durante 7 dias por semana foi referido por 31,6%; 7,0% referiram a ocorrência de pelo menos um acidente no local de trabalho e 69,0% dos entrevistados alegaram conhecer uma pessoa em perigo em seus locais de trabalho. A contribuição do planejamento para a organização do sistema de saúde e, conseqüentemente, a assistência à saúde dos trabalhadores é valiosa, envolvendo desde gestores até os trabalhadores das equipes de saúde. Faz-se necessária a inserção da saúde do trabalhador como domínio da saúde coletiva, considerando-se ambos como campos de conhecimento complexos, que buscam a promoção da saúde para a coletividade / Abstract: Globalization forces the work relationships to become more flexible, originating a great group of contingent workers that are unprotected, ignored by the public and state services, without right to the onsite health services assistance and control of working conditions. The intention of delivering comprehensive health services to the informal workers in precarious or dangerous conditions originated a descriptive and prospective population based study to investigate the worked time and work related accidents or illnesses during four months by means of weekly visits carried out by a Team of Community Health Agents. A data collection . Instrument elaborated for a mother-project in another city was applied, after being adapted to the local conditions. It was observed that 62.1% of the workers were females; 43.9% were aged between 35 and 50 years; 55.6% worked on their own; 39,9% were informally subcontracted employees and 8.4% of the interviewed ones paid the Social Security monthly fees. The reason for entering to the informal sector was the unemployment for 44.1% while 24.6% searched better incomes. The worked daily journey was of 8 hours a day for 39.7%; working during 7 days per week was related by 31.6%; 7.0% had related the occurrence of at least one accident in their worksite during the research period and 69.0% alleged to know a person in danger in their worksite. The contribution of Health Planning for the organization of the health system and, consequently, the assistance to the health of the workers is fundamental, involving from managers to the Health Workers. Family-Supportive Supervisory Behaviors as a Moderator of the Relationship between Job Strain and Workers' Blood Pressure

Cardiovascular disease is one of the leading causes of death in industrialized nations. Research indicates that job strain may be significantly related to cardiovascular disease in employees with little to no social support. Using the JDC-S model developed by Karasek (1979) and elaborated upon by Johnson and Hall (1988), the family-supportive supervisory behaviors (FSSB) measure created by Hammer et al., (2009), and the blood pressure wrist monitor device Omron317T, this study examined FSSB as a moderator of the relationship between job strain, job demands, job control and workers' blood pressure on work and non-work days. Sixty-nine grocery store workers from a Midwest grocery store chain participated in this study, fifty-six of which were included in the analyses. Though none of the interactions were significant at the .05 level, results indicate that FSSB is significantly related to a number of blood pressure readings at the grand centered mean of job strain, job control, and job demands.
