Difference-makers in human affective distress: perspectives on causation and recovery gained from qualitative inquiry into lived experience

The biomedical conceptualisation of “depression” as a disease entity stems from biological science rooted in a 17th century paradigm, and is an inappropriately positivist idea which discounts individual agency, disregards social context underscored in the descriptive epidemiology, fails to accommodate inter-subjectivity and process, medicalizes suffering, and serves a managed care model of health administration. A qualitative study using in-depth interviews was undertaken to elicit perspectives from individuals who had lived experience of biomedical (pharmacological) treatment for affective distress and who self-reported having attained satisfactory recovery. Thematic analysis of interview data clustered around three main categories: (1) multifactorial conceptual understandings; (2) context pertinent to the experience of distress and recovery – including a variety of stressors – and (3) trajectories of recovery from acute distress to negotiation of ambivalence toward treatment, enlisting of supports, and reclaiming of agency. Relationships with service providers marked by trust, empathy, and hope were valued as primary difference-makers. / May 2017

Through Satisfaction Assessment Scales Brazilian was aimed in this study to evaluate the degree of satisfaction of residents and staff of these homes, to reflect whether such dwellings or not brought benefits proposed by the Law. It was found that only 1.9% of the professionals they said very dissatisfied with the service, the majority, 62.7%, are satisfied with the work they do. With regard to residents, some data draw attention, such as 44.4% feel respected by professionals, 66.7% feel that professionals understand well your problems. After analyzing the verbal material collected in interviews of professionals and residents, it was possible to understand the dissatisfaction of professionals as to salary issues and incentives for training, and realize how much the occupant is received by the service. / Com o advento da Reforma Psiquiátrica e a promulgação da Lei 10.216-12 que instituiu uma nova concepção para o tratamento de pessoas com doenças mentais, a implantação dos Centros de Atenção Psicossocial e Residências Terapêuticas se impuseram como respaldo para o novo modelo de atendimento a estes pacientes. Por meio de Escalas Brasileiras de Avaliação da Satisfação objetivou-se neste estudo avaliar o grau de satisfação de moradores e funcionários dessas residências, para refletir se tais moradias trouxeram ou não benefícios propostos pela Lei. Foi constatado que apenas 1,9% dos profissionais se disseram muito insatisfeitos com o serviço, a maioria, 62,7%, estão satisfeitos com o trabalho que realizam. Com relação aos residentes, alguns dados chamam a atenção, tais como 44,4% se sentem respeitados pelos profissionais, 66,7% sentem que os profissionais compreendem muito bem seus problemas. Após analisar o material verbal coletado nas entrevistas dos profissionais e dos moradores, foi possível compreender as insatisfações dos profissionais quanto a questões salariais e incentivos à qualificação, além de perceber o quanto o morador é acolhido pelo serviço.

Bakgrund: Unga vuxna som sökte vård sökte oftast inte för den psykiska ohälsan utan istället för något somatiskt. Det är viktigt att få de yngre vuxna att söka sig till vården när behov uppstår och underlätta till en god förbindelse mellan sjukvården och de unga vuxna. Syfte: Syftet med denna studie var att beskriva unga vuxnas upplevelser av sjuksköterskans bemötande av psykisk ohälsa vid kontakt med hälso- och sjukvården. Metod: Studien genomfördes som en litteraturöversikt. Resultat: För de unga vuxna var det inte lätt att komma in i vården på rätt vårdnivå om personen inte kom i kontakt med en sjuksköterska som hade ett holistiskt synsätt. Vården för de unga med psykisk ohälsa bör vara tillgänglig och informationen om tillgängligheten behöver ständigt upprepas. Slutsats: Det är viktigt att sjuksköterskor har kunskap och erfarenhet av unga psykiska människor för att kunna hjälpa dem med den psykiska ohälsan när de söker vård. Relationen mellan patienten och sjuksköterskan är av stor vikt för den fortsatta kontakten med vården för den unge vuxna. Det är viktigt att de känner tillit och förtroende. / Background: Young adults who seek care usually seek not for mental illness, but rather something somatic. It is important to get the younger adults to seek out health care when needed and facilitate a good relationship between healthcare and young adults. Purpose: The purpose of this study was to gain an insight into how the young adults turning to the care and nurses' awareness of how little confidence built up in these patients. Method: The study was conducted as a literature survey. Results: For the young adults, it is not easy to come into care at the right level of care if the person does not come into contact with a nurse who has a holistic approach. The care for the young with mental illness should be available and the information about the accessibility needs are constantly repeated. Conclusion: It is important that nurses have knowledge and experience of young psychic people in order to help those with mental illness when seeking care. The relationship between the patient and the nurse is of great importance for the continued contact with the care for the young adults. It is important that they feel trust and confidence.

Philosophical perspectives on the stigma of mental illness

This thesis is concerned with philosophical perspectives on the stigma of mental illness, with each chapter exploring different philosophical issues. Chapter one delineates the central concept around which the rest of the work revolves: the stigma of mental illness. It provides an outline of the stigma mechanism, how it applies to mental illness, why it is such a large public health concern and what has been done so far to combat it. Chapter two is concerned with the application of recent literature in the philosophy of implicit bias to the topic of mental illness. It suggests that we have hitherto been preoccupied with explicit formulations of the stigma mechanism, but argues that there are distinctive issues involved in combatting forms of discrimination in which the participants are not cognisant of their attitudes or actions, and that anti-stigma initiatives for mental illness should take note. Chapter three applies the philosophical literature concerning the ethics of our epistemic practices to the stigma of mental illness. It contains an analysis of how epistemic injustice- primarily in the forms of testimonial injustice and stereotype threat- affects those with mental illnesses. The fourth chapter brings in issues in the philosophy of science (particularly the philosophy of psychiatry) to explore the possibility of intervening on the stigma process to halt the stigma of mental illness. The first candidate (preventing labelling) is discounted, and the second (combatting stereotype) is tentatively endorsed. The fifth chapter is concerned with how language facilitates the stigma of mental illness. It suggests that using generics to talk about mental illness (whether the knowledge structure conveyed is inaccurate or accurate) is deeply problematic. In the former, it conveys insidious forms of social stereotyping. In the latter, it propagates misinformation by presenting the category as a quintessential one.

Mental Illness in the Workplace: Understanding How Managers Respond to their Employees Living with a Mental Illness

This present research explores the following research question: How do managers respond to employees who are living with a mental illness? In answering this research question, I explore the challenges mental illnesses pose to managers, how managers successfully and unsuccessfully respond to those challenges, and the potential positive outcomes associated with encountering an employee who is living with a mental health condition. I answer my guiding research question through five in-depth semi-structured interviews and by relying on six structured and targeted narrative interviews that had been conducted for a previous research project. All interviews were conducted with managers across a variety of industries who have experience working with at least one employee who has lived with a mental health condition. Based on the themes that emerge in these interviews, I document the challenges managers face. I also identify several ‘facilitators of success’, defined as behaviours or actions that managers believed had some degree of positive impact on helping an employee living with a mental illness. The insights developed through this research will better inform the research community regarding managers’ experiences working with employees with a mental illness, as well help inform organizations regarding how they can better equip their managers in dealing with employee mental illness.

Lone Star Insanity: Efforts to Treat the Mentally Ill in Texas, 1861-1929

During the mid-nineteenth century, the citizens of Texas were forced to keep their mentally disturbed family members at home which caused stress on the caregivers and the further debilitation of the afflicted. To remedy this situation, mental health experts and Texas politicians began to create a system of healing known as state asylums. The purpose of this study is to determine how Texas mental health care came into being, the research and theories behind the prevention and treatment programs that asylum physicians employed to overcome mental illness, in addition to the victories and shortcomings of the system. Through this work, it will be shown that during the 1850s until the 1920s institutions faced difficulty in achieving success from many adverse conditions including, but not limited to, overcrowding, large geographical conditions, poor health practices, faulty construction, insufficient funding, ineffective prevention and treatment methods, disorganization, cases of patient abuse, incompetent employees, prejudice, and legal improprieties. As a result, by 1930, these asylums were merely places to detain the mentally ill in order to rid them from society. This thesis will also confirm that while both Texas politicians and mental health experts desired to address and overcome mental illness in Texas, they were unable to do so due to arguments, selfishness, corruption, failures, and inaction on the part of both sides. However, this thesis will ultimately reveal it was lack of full support from Texas legislators, deriving from the idea that this system was not one of their top priorities among the state’s concerns, that led to the inability of the Texas mental health care system to properly assist their patients.

A phenomenological study of the perceptions on childhood mental illness by Northern Sotho-speaking elderly people in Mankweng Community, Limpopo Province

Thesis (M.A.(Clinical Psychology)) -- University of Limpopo, 2012 / The aim of the study was to explore the notions of childhood mental illness by Sepedi-speaking elderly people in the Mankweng community of Limpopo Province (South Africa). Using the phenomenological method, the researcher sought to understand these elderly people’s notions with a view to: i) describing their conceptualizations of childhood mental illness; ii) determining their notions of the types and causes of mental illness; and iii) describing their views regarding the management of childhood mental illnesses. A total of 8 participants who were selected through snowball sampling were interviewed using unstructured interviews. The results of this study suggest that; i) childhood mental illness is better understood if described than defined, ii) childhood mental illness is manifested through behavioural and physical features, iii) there are many causal factors attributed to childhood mental illness, and iv) both Western and African traditional methods are recognized in the treatment of childhood mental illness in this rural community. The results are discussed in the context of the psychological literature on multicultural counseling and psychotherapy. / Department of Health and Social Development

Detta kan skapa en ohälsa och ett onödigt lidande som finns kvar hos individen även i vuxen ålder enligt stress- sårbarhetsmodellen. Det är sjuksköterskans ansvar att även se till patientens familjemedlemmars hälsa. Syfte: Att få kunskap om vuxna barns upplevelser av att ha haft en förälder med psykisk sjukdom under sin uppväxt samt hur de upplevt och upplever hälsa. Metod: Litteraturöversikt enligt Fribergs metod. Denna litteraturöversikt innehåller tio studier som är hämtade från Cinahl Complete, Medline with full text och Pubmed. Abstrakt lästes med helikopterperspektiv och både kvalitativa och kvantitativa studier valdes av relevans för studiens syfte. Författarna diskuterade likheter och skillnader i studiernas resultat. Teman och subteman definierades. Resultat: Tre teman definierades; upplevelser av ombytta roller, känslomässiga upplevelser samt upplevd hälsa som vuxen. Diskussion: Studiernas tre mest utmärkande resultat diskuteras i relation till Travelbees omvårdnadsteori om mellanmänskliga relationer, vad information till det minderåriga barnet har för betydelse och hur man kan arbeta preventivt för att främja upplevelsen av hälsa hos barn med en psykiskt sjuk förälder. / Background: Mental illness is a common health problem in Sweden, and may not only cause patient suffering, but also his or the patient`s family, particularly under aged children. Parents with mental illness may neglect or be less attentive to the children`s basic needs, and according to the stress-vulnerable model this may cause poor health under unnecessary life-long suffering for the child. It is the nurse`s responsibility to also ensure the patient`s family members´ health. Aim: To gain knowledge of adult children’s experiences of having a parent with mental illness during his or her childhood, as well as how they experience their own health. Method: Literature review by Friberg´s method. This literature review includes ten studies, which are taken from Cinahl complete, Medline with full text and Pubmed. Abstract was read by helicopter perspective and both qualitative and quantitative studies were selected. The authors discussed the similarities and differences in the results of the studies. Themes and subthemes were defined. Results: Three themes were defined; experiences of role change, the emotional experiences and experiences of health of an adult. Discussion: The three most characteristic themes of the study are discussed in relation to Travelbee's nursing theory of interpersonal relationships, the importance of information given to the underaged child, and how to work preventive in order to promote the experience of health in children with mentally ill parents.
