Bendruomenės slaugytoja yra pirmasis asmuo, į kurį kreipiasi bendruomenės nariai, ištikus nelaimei, todėl esant poreikiui, slaugytojos teikia pagalbą bet kuriuo paros metu. Pacientų ir slaugytojų santykiai yra labai artimi, visapusiškai pasitiki, todėl slaugytojos neretai atlieka daugelį vaidmenų, reikalaujančius tarpdisciplininių žinių. Tai rodo slaugytojų didelį atsidavimą darbui ir atsakomybę už visapusišką bendruomenės narių sveikatą. Interviu analizė leido pamatyti bendruomenės medicinos punktų veiklos problemas: kitoje įstaigoje prisirašiusių asmenų aptarnavimo problema, bendradarbiavimo trūkumas su socialinių paslaugų teikėjais, bendruomenės slaugytojų savarankiškumo sumažinimas, rizikos grupės šeimų ir alkoholizmo problemos, kaimo retėjimo ir senėjimo problema. Pokyčių bendruomenės medicinos punktuose informantai nenori ir neorganizuoja, tenkinasi esama situacija. Anot respondentų, realiausia perspektyva – medicinos punktų uždarymas. Mobilių paslaugų idėjos įgyvendinimui respondentai griežtai priešinasi, nurodant tam įvairias priežastis: kiltų gyventojų nepasitenkinimas, nevisi bendruomenės nariai noriai įsileistų slaugytojas į namus, sumažėtų teikiamų paslaugų spektras ir PSP paslaugų prieinamumas. / The community nurse is the first person approached by the members of the community when disaster strikes, so if needed, nurses provide assistance at any time of the day. The relationships of patients and nurses are very close, so nurses often perform many different roles, which require interdisciplinary knowledge. It shows great dedication of nurses and the responsibility for comprehensive community health. Interview analysis identified community medical centers operating problems: serving customers which are not signed to the concrete community center, lack of cooperation with social service providers, loss of autonomy of community nurses, problems of families at risk and alcohol abuse, problems of village depopulation and aging. The informants do not organize any changes and are satisfied with the current situation of medical centers. Probably community medical centers will be closed in the future, but respondents were categorically against the idea of portable medical station, arguing, that: it would lead to dissatisfaction of community members, not all members of the community would be willing for nurses to come to their home, the range of services would reduce, the availability of primary health care would reduce.

Pride and prejudice, practices and perceptions : a comparative case study in North Atlantic environmental history

Due to escalating carbon-based emissions, anthropogenic climate change is wreaking havoc on the natural and built environment as higher near-surface temperatures cause arctic ice-melt, rising sea levels and unpredictable turbulent weather patterns. The effects are especially devastating to inhabitants living in the water-worlds of developing countries where environmental pressure only exacerbates their vulnerability to oppressive economic policies. As climatic and economic pressures escalate, threats to local resources, living space, safety and security are all reaching a tipping point. Climate refugees may survive, but they will fall victim to displacement, economic insecurity, and socio-cultural destruction. With the current economic system in peril, it is now a matter of urgency that the global community determine ways to modify their behaviour in order to minimize the impact of climate change. This interdisciplinary comparative analysis contributes to the dialogue by turning to environmental history for similar scenarios with contrasting outcomes. It isolates two North Atlantic water-worlds and their inhabitants at an historical juncture when the combination of climatic and economic pressures threatened their survival. During the sixteenth and seventeenth centuries, the Hebrideans in the Scottish Insular Gàidhealtachd and the Wabanaki in Ketakamigwa were both responding to the harsh conditions of the ‘Little Ice Age.’ While modifying their resource management, settlement patterns, and subsistence behaviours to accommodate climate change, they were simultaneously targeted by foreign opportunists whose practices and perceptions inevitably induced oppressive economic pressure. This critical period in their history serves as the centre of a pendulum that swings back to deglaciation and then forward again to the eighteenth century to examine the relationship between climate change and human behaviour in the North Atlantic. It will be demonstrated that both favourable and deteriorating climate conditions determine resource availability, but how humans manage those resources during feast or famine can determine their collective vulnerability to predators when the climate changes. It is argued that, historically, climate has determined levels of human development and survival on either side of the North Atlantic, regardless of sustainable practices. However, when cultural groups were under extreme environmental and economic pressure, there were additional factors that determined their fate. First, the condition of their native environment and prospect for continuing to inhabit it was partially determined by the level of sustainable practices. And, secondly, the way in which they perceived and treated one another partially determined their endurance. If they avoided internal stratification and self-protectionism by prioritising the needs of the group over that of the individual, they minimised fragmentation, avoided displacement, and maintained their social and culture cohesion.

A psycho-educational curriculum for sport career transition practitioners : development and evaluation

The aim of this thesis is to extend sport career transition knowledge by developing and evaluating a psycho-educational curriculum for sport career transition practitioners. Study 1 provides an extensive overview of organisational intervention programmes for sport career transition, as well as of training and development programmes for sport career transition practitioners from 19 countries worldwide. The purpose of Study 1 was to examine the following five features of sport career transition organisational intervention programmes and training and development programmes for sport career transition practitioners: 1) At the government level, which organisation is responsible for the delivery of sport career transition organisational intervention programmes? 2) What is the typical strategy of sport career transition organisational intervention programmes? 3) Which activities and events are included in sport career transition organisational intervention programmes? 4) Who is responsible for the delivery of sport career transition organisational intervention programmes? and 5) Do sporting organisations have training and development programmes for sport career transition practitioners? Findings from Study 1 were used to inform Study 2 and to supplement the literature review. According to the literature, training and development programmes for sport career transition practitioners remain scarce, while the number of sport career transition organisational intervention programmes is gradually increasing. The purpose of Study 2 was to develop and evaluate a novel psycho-educational curriculum for sport career transition practitioners. There were three stages of development and evaluation: (1) content development, (2) curriculum design and (3) curriculum evaluation. To achieve the goal of Study 2, a focus group and a two-round Delphi method were implemented. The final version of the psycho-educational curriculum is presented in Chapter 5. The purpose of Study 3 was to test a key aspect of the psycho-educational curriculum developed in Study 2 concerning 38 competences of sport career transition practitioners. A curriculum package consisting in the key parts from the overall curriculum was developed for Study 3. The findings revealed that the curriculum package positively affected the confidence of the study participants in 38 competences concerning sport career transition. This demonstrates that the curriculum is effective for the development of sport career transition practitioners, as the study participants exhibited greater confidence on all four factorsDual Career Transition Management, Skill Transfer, Social Support, and Career Planningwhen completing the curriculum package. The discussion in Chapter 7 focuses on both the theoretical and practical implications of the research, limitations and future research directions. In conclusion, the findings presented here deepen the knowledge of sport career transition in the areas of organisational intervention programmes for high performance athletes and training and development programmes for sport career transition practitioners. In addition, a novel psycho-educational curriculum for sport career transition practitioners is developed and evaluated.
