Esta tese se orientou pelo objetivo de compreender a configuração do campo da atenção à saúde a partir da produção social da definição dos conceitos que o organizam - direito e descentralização. Para realizá-la defini uma atitude filosófica - a hermenêutica, e uma metodologia - o construcionismo, que permitissem um olhar diferente daquele usualmente aplicado a esse campo e que se orienta pelo positivismo sistêmico. Busquei, também, um referencial teórico que propiciasse a compreensão, de modo articulado e não reducionista, de fenômenos sociais e relações de poder, tendo optado por uma leitura, influenciada por Gadamer, de Pierre Bourdieu e Carlos Matus. Para acompanhar o jogo social e a interação entre atores, escolhi o foco na definição de conceitos, entendendo o discurso como uma prática social, política e ideológica, capaz de gerenciar significados, definir posições dos sujeitos e construir objetos. Parti, então, em busca dos diversos significados potenciais de direito à saúde e descentralização, utilizando-os, ao longo da narrativa, como referência para acompanhar a produção social desse campo. A história que conto começa em 1988 com um campo dominado por atores sociais que definem direito com foco na cidadania e descentralização como devolução parcial, para os governos municipais, com ênfase nas dimensões política, social, administrativa e operacional, até chegar, em 2001, a um campo hegemonizado pelo foco na pobreza e em direitos restritos de consumidores; onde descentralização é definida como um misto de privatização, delegação e devolução parcial, sempre sob a forma, predominantemente, operacional e administrativa. Essa redefinição de conceitos produz uma série de acumulações, descritas ao longo do texto, que provocam a reconfiguração do campo e novas posições dos sujeitos, sem a necessidade de alterar as regras básicas que o regem. Como todo texto, esse também é uma representação, a criação de uma autora, um desempenho de um ponto de vista particular. Nesse sentido, a autoria é, simultaneamente, a representação da experiência dos outros e de um experimento, que é o próprio ato de escrever. O texto desta Tese é expressão desse processo, tendo sido criado de modo a ser coerente com a atitude filosófica e com o referencial teórico escolhido. / The objective of this thesis is to understand the configuration of the health care field as a result of the social production of its core concepts' definition - right and decentralization. In order to achieve this objective, I began by defining a philosophical attitude - hermeneutics, and a methodology - constructionism, that allowed a look, different from the dominant one - based on systemic positivism. I also looked for a theoretical framework to understand social phenomena and power relations, choosing a reading, influenced by Gadamer, of Pierre Bourdieu and Carlos Matus. To follow the social game and the interaction among actors, I choose the focus on concept definitions, understanding discourse as a social, political and ideological practice, that can manage meanings, define positions and construct objects. The next step was the search for the various potential meanings of health as a right and decentralization, that were used, during the narrative, as a reference to follow the field social production. The history begins, in 1988, in a field dominated by social actors that define right with a focus on citizenship and decentralization as partial devolution to municipal governments, emphasizing political, social, administrative and operational dimensions; and arrives in 2001 , when the field is dominated by the focus on poverty and restrict consumer rights, where decentralization is defined as a mix of privatization, delegation and partial devolution, always under a form, predominantly, operational and administrative. This redefinition of concepts produces facts, described during the narrative, that change the field configuration and establish new subjects positions, without the need of changing the basic rules. As all texts, this is also a representation, a piece of authorship, a performance from a particular point of view. In this sense, the authorship is, simultaneously, the representation of others experience and of an experiment - the act of writing. The text produced is the expression of this process, and was created to be coherent with the chosen philosophical attitude and theoretical framework.
