AssociaÃÃo de polimorfismos dos genes de citocinas com susceptibilidade para infecÃÃo por micobactÃrias / Association of polymorphisms of cytokine genes with susceptibility to mycobacterial infection

Os estudos de polimorfismos dos genes de citocinas podem ser utilizados como uma ferramenta de anÃlise antropolÃgica para determinar perfis genÃticos distintos, para traÃar relaÃÃes entre populaÃÃes e para promover o entendimento das disparidades raciais na susceptibilidade genÃtica a doenÃas. O objetivo deste estudo foi correlacionar os polimorfismos das citocinas, TNF-&#945; -308, TGF-&#946;, cÃdon 10 e cÃdon 25, IL-10 -1082, -819, -592, IL-6 -174, e IFN-&#947; +874 com a susceptibilidade e a proteÃÃo Ãs infecÃÃes por M. tuberculosis e M. leprae. A casuÃstica incluiu 194 pacientes, sendo 65 com hansenÃase e 129 com tuberculose. Como controles foram selecionados 168 voluntÃrios saudÃveis. Todos os indivÃduos assinaram termo de consentimento informado e o estudo foi aprovado pelo Comità de Ãtica local. Para a obtenÃÃo do DNA genÃmico e amplificaÃÃo foram coletados 3-5ml de sangue perifÃrico, sendo utilizado na extraÃÃo do DNA o kit Easy-DNA (Invitrogen). Os SNPs dos genes das citocinas TNF-&#945; (-308G>A), TGF-&#946; cÃdon 10 e 25, IL-10 (-1082G>A, -819C>T, -592C>A), IL-6 (-174G>C) e IFN-&#947; (+874T>A) foram tipificados por reaÃÃo em cadeia da polimerase, empregando-se iniciadores de seqÃÃncia especÃfica (PCR-SSP) (One-Lambda). Utilizou-se para a anÃlise estatÃstica o programa GraphPad prism 5, sendo o valor de &#961; determinado atravÃs do Teste de Fisher bicaudal com significÃncia em &#961;<0,05. Durante a comparaÃÃo dos resultados entre os grupos estudados observou-se que nÃo houve diferenÃas entre as freqÃÃncias dos genÃtipos de TNF&#945;. Entretanto, na anÃlise dos genÃtipos de IL10, houve uma reduÃÃo da freqÃÃncia do GCC/GCC nos casos (7,1% vs 17,9%, p=0,006). Esta reduÃÃo tambÃm foi observada quando analisados os genÃtipos IL-6 GC (22,1% vs 32,8%, p=0,04) e IFN&#947; TA (36,6% vs 51,6%, p=0,01). No entanto, as freqÃÃncias dos genÃtipos GG de IL-6 (72,1% vs 57,0%, p=0,008) e AA de IFN&#947; (54,0% vs 41,4%, p=0,03) estavam aumentadas nos casos. Ademais, houve uma tendÃncia para aumento da freqÃÃncia do CC/GC de TGF&#946; (7,5% vs 0,8%, p=0,06). Dessa forma pode-se concluir que os genÃtipos: IL-10 (GCC/GCC), IL-6 (GC) e IFN-&#947; (TA) conferiram efeito protetor, enquanto os genÃtipos TGF-&#946; (CC/GC), IL-6 (GG) e IFN-&#947; (AA) conferiram efeito de susceptibilidade para infecÃÃo por micobactÃrias. Por outro lado, os alelos T no cÃdon 10 e G no cÃdon 25 de TGF-&#946; determinaram efeito protetor contra tuberculose. Em contraste, o alelo -174G de IL-6 determinou susceptibilidade Ãs infecÃÃes por micobactÃrias. / Cytokine genetic polymorphisms studies are helpful to determine diverse genetic profiles, to correlate populations, and to promote the knowledge of ethnic influence on the genetic susceptibility to diseases. The aim of the present study was to correlate TNF-&#945; -308, TGF-&#946;, codon 10 and codon 25, IL-10 -1082, -819, -592, IL-6 -174, and IFN-&#947; +874 polymorphisms with the susceptibility and the protection to M. tuberculosis and M. leprae infection. The study included 194 patients, 65 of them with leprosy and 129 with tuberculosis, and 168 healthy volunteers. The local Ethical Committee approved the study and all of the participants consented. To DNA purification and amplification 3-5 mL of peripheral blood was collected. An Easy-DNA (Invitrogen) kit was used to purification. Cytokines SNPs TNF-&#945; (-308G>A), TGF-&#946; codon 10 and 25, IL-10 (-1082G>A, -819C>T, -592C>A), IL-6 (-174G>C) and IFN-&#947; (+874T>A) were amplified by the polymerase chain reaction using sequence-specific primers (SSP-PCR) (One-Lambda). Statistical analysis was proceeded using GraphPad prism 5, and p values were determined by the Fisher test, with significance when p<0.05. Frequencies of TNF&#945; genotypes were not different among patients and controls. However, IL-10 genotypes analyses showed a reduction on the frequency of the GCC/GCC in patients (7.1% vs 17.9%, p=0.006). This reduction was also observed with the IL-6 GC (22.1% vs 32.8%, p=0.04) and IFN&#947; TA (36.6% vs 51.6%, p=0.01) genotypes. In contrast, the frequencies of the IL-6 GG (72.1% vs 57.0%, p=0.008) and the IFN&#947; AA (54.0% vs 41.4%, p=0.03) genotypes were increased in patients. Furthermore, there was a trend to an increase in the frequency of the TGF&#946; CC/GC genotype (7.5% vs 0.8%, p=0.06). In conclusion, IL-10 (GCC/GCC), IL-6 (GC) and IFN-&#947; (TA) genotypes conferred protection against, while TGF-&#946; (CC/GC), IL-6 (GG) and IFN-&#947; (AA) genotypes determined susceptibility to mycobacteria infection. On the other hand, T allele in codon 10 and G allele in codon 25 of the TGF-&#946; gene, determined protection against tuberculosis. In contrast, the IL-6 -174G conferred susceptibility to mycobacteria infection.

Conselho Nacional de Desenvolvimento CientÃfico e TecnolÃgico / Com o objetivo de enfatizar a importÃncia do diagnÃstico laboratorial de micobactÃrias em sÃtios extrapulmonares, exceto renal, utilizou-se neste trabalho a reaÃÃo em cadeia da polimerase (PCR). Na PCR foram empregados iniciadores especÃficos para a detecÃÃo de micobactÃrias do complexo Mycobacterium tuberculosis (MTB) e nÃo pertencentes ao complexo Mycobacterium tuberculosis (MNTB). Foram comparados os resultados obtidos na PCR com os mÃtodos microbiolÃgicos convencionais. Foram analisadas 114 amostras clÃnicas (lÃquidos corporais) provenientes de pacientes internados ou em atendimento ambulatorial, com suspeita clÃnica de tuberculose extrapulmonar, exceto renal. As amostras foram coletadas no perÃodo de 2001 a 2003 e cedidas pelo LaboratÃrio de Microbiologia do LaboratÃrio Central do Hospital UniversitÃrio Walter CantÃdio (HUWC-UFC) e pelo LaboratÃrio Central de SaÃde PÃblica da Secretaria de SaÃde do Estado do Cearà (LACEN-CE). A baciloscopia e a cultura foram negativas em todas as amostras. A pesquisa molecular para o gÃnero Mycobacterium foi positiva em dez amostras e, para o complexo Mycobacterium tuberculosis, foi negativa em todas as amostras. / In order to focus on the importance of laboratorial diagnosis of extrapulmonary mycobacteria, except renal, we employed the polymerase chain reaction (PCR). Clinical samples (body fluids) were tested by PCR for the presence of Mycobacterium tuberculosis complex (MTB) and nontuberculous mycobacteria (MNTB). We compared the results obtained through the PCR with the ones obtained through conventional microbiological methods. The 114 clinical samples were obtained from internal patients or from or in ambulatorial attendance with clinical suspicion of extrapulmonary tuberculosis, except renal. The samples were collected from 2001 to 2003, and ceded to this research by the LaboratÃrio de Microbiologia do LaboratÃrio Central do Hospital UniversitÃrio Walter CantÃdio (HUWC-UFC), and by the LaboratÃrio Central de SaÃde PÃblica da Secretaria de SaÃde do Estado do Cearà (LACEN-CE). The direct microscopy and culture were negative in all the samples. The molecular research, for the Mycobacterium genus was positive in 10 samples, and for the Mycobacterium tuberculosis complex was negative in all the samples.
