The diploma is about the topic of multidisciplinarity in community care of people with mentally illness. I chose this topic because I meet clients who have lived for various periods of time knowing that they suffer of mental illness. Over time, I began deeply perceive how mental illnesses affect all areas of client's lives. In the modern sense of community-based psychiatric services, multidisciplinary cooperation should become a provider of more comprehensive, better quality and more effective services for people with mental illness who require health care, but also social care that facilitates inclusion in the society of people who suffer of mental illness. Multidisciplinary collaboration is an interdisciplinary collaboration in social and health services, illustrates different theories that use various terminologies. The collaboration of experts from different disciplines, in addition to the term multidisciplinary, can be referred also as interdisciplinary or intersectoral. Another reason for choosing the topic of the thesis is the increas of discussions and even practical steps within the Strategy of Psychiatric Reform in the Czech Republic. The theoretical part of this diploma thesis aims to summarize the basic information about rehabilitation of a family as a relatively new phenomena in Czech social work with families. Firstly I describe according to a disposable literature functions of a family, its importance to a child as well as the most common problems a family can face. Than I place rehabilitation of a family as an approach into the Czech system of care of vulnerable children and describe disadvantages of the current system. Lastly I decribe the characteristics of rehabilitation of a family as a multidisciplinary approach to family social work. The empirical part of this thesis is based on semi-structured interwievs with profesionals in the field of rehabilitation of a family as well as clients of family rehabilitation services. It aims to find out how can we use educational activities when working with families in danger of social failure. Die Behandlung tracheotomierter Patienten mit schwerer Dysphagie : eine explorative Studie zur Evaluation eines interdisziplinären Interventionsansatzes / Treating tracheotomized patients with severe dysphagia : an explorative evaluation of a multidisciplinary treatment protocol

In der neurologischen Rehabilitation werden in zunehmendem Maße tracheotomierte Patienten mit schweren Dysphagien behandelt. Daher sollte den hierzu bisher entwickelten Interventionsverfahren eine evidenzbasierte Grundlage gegeben werden. In der vorliegenden Arbeit wird ein multidisziplinärer Behandlungsansatz zur Trachealkanülenentwöhung und Dekanülierung vorgestellt, der auf der Grundlage der relevanten Forschungsliteratur und klinischen Beobachtungen entwickelt wurde. Des Weiteren wird erstmals eine systematische Evaluation eines multidisziplinären Trachealkanülenmangements vorgenommen und es werden explorative Daten zum Rehabilitationsverlauf dargestellt. In einem retrospektiven Vergleich wurden hierzu die Dekanülierungs- und Komplikationsraten sowie die Dauer der Trachealkanülenentwöhnung zweier Patientengruppen gegenübergestellt, die vor bzw. nach Einführung des beschriebenen multidisziplinären Trachealkanülenmanagements im REHAB Basel, Schweiz, behandelt wurden. Der rehabilitative Verlauf der multidisziplinär behandelten Gruppe wurde mittels der Messinstrumente FIM (Functional Independence Measure) und EFA (Early Functional Abilities) untersucht. Der Vergleich der Dekanülierungs- und Komplikationsraten ergab eine vergleichbare Effektivität der beiden Behandlungsansätze. Darüber hinaus zeigte sich eine signifikante Verkürzung der Kanülenentwöhnungsphase bei Anwendung des multidisziplinären Vorgehens, so dass dieses als effizenter zu beurteilen ist. Die Verlaufsanalyse der multidisziplinär behandelten Patienten ergab erst nach der Dekanülierung einen signifikanten Zuwachs der funktionellen Selbständigkeit in Alltagsaktivitäten. Bei der Mehrzahl der Patienten konnte ein vollständiger oraler Kostaufbau nach der Dekanülierung erreicht werden. / In neurological rehabilitation there is a growing need for information about treatment of tracheotomized dysphagic individuals and treatment methods have to be evaluated objectively. This dissertation presents a multidisciplinary approach for the treatment of tracheotomized dysphagic patients that was developed based on research findings and clinical experiences. Furthermore it presents a first approach to a systematic evaluation of a multidisciplinary treatment protocol and explorative data about the rehabilitative progress in this patient group. In a retrospective analysis mean cannulation times and the success rate of decannulation from patients were compared before and after introduction of the multidisciplinary procedure in a rehabilitation centre, REHAB Basel, Switzerland. Furthermore, the rehabilitation progress was analyzed by means of the assessment tools ‘Functional Independence Measure (FIM)’ and ‘Early Functional Abilities (EFA)’. Decannulation rates and success of decannulation were comparable in both groups of patients. With regard to mean cannulation times, however, a significant reduction was found in the group who underwent multidisciplinary treatment. This indicates a higher efficiency of the multidisciplinary approach, whereas, with regard to effectiveness, the two approaches seem to be comparable. After decannulation the patients of the multidisciplinary group showed clear functional improvements in performing activities of daily living. Most of these patients were able to return to full oral nutrition after decannulation. The multidisciplinary approach was found to be more efficient than the former intradisciplinary protocol as it led to a safe but faster decannulation of tracheotomized dysphagic patients. The explorative data concerning rehabilitation progress in these patients supports the importance of the development of evidence-based treatment protocols that lead to a fast and safe decannulation. This can be considered the basis for further significant improvement of the functional independence of the tracheotomized dysphagic patient.

New conceptual model for design development of smart clothing

Smart Clothing, the convergent future of the electronics and clothing industries, struggles to reach its true potential and enter the mass market because of 1) imbalanced contributions from the electronics and clothing sectors, 2) the lack of an integrated approach to optimise the input from the different areas, and 3) the unclear direction of the products. There is a need for an NPD process that balances all contributions and addresses new values based on user requirements. Moreover, a strategic approach, that challenges the development teams to go beyond their existing creative boundary and reconciles their differences, is required. According to the research, Smart Clothes should take the design approach of functional clothing and focus on the area of sportswear, personal healthcare and physical monitoring, as they fit the users’ lifestyle and requirements. Since social acceptance is an important factor, Smart Clothes must also have a good design and whilst, at the same time, perform all the basic functions that ordinary garments do. They should allow the user to personalise the styles and functions according to the benefits, with respect to product lifecycle and disassembly. A conceptual model of the NPD process was developed and tested with experts in this field. The proposed model provides the basis for a computer software to plan and manage product development teams and activities at the front-end of the NPD process. It offers several advantages: 1. Combining the NPD models and those of collaborative development 2. Providing a holistic view of Smart Clothing development 3. Clarifying of the roles of all participants within the collaborative development teams 4. Describing the responsibilities and expected contributions of all participants 5. This diploma thesis addresses the ongoing reform of psychiatric care in the Czech Republic. The primary pillars of reform are presented here and all projects through which individual steps of reform are implemented are elaborated in detail. There are four projects from the Czech Republic Ministry of Health: Deinstitutionalization of inpatient psychiatric facilities; Introduction of a Multidisciplinary Approach to the Mentally Ill; New Services; and CDZ I, II, III. And there are two projects from the National Institute of Mental Health (NÚDZ): Destigmatization of psychiatric issues and VIZDOM, and the ÚZIS Project. Examples of best practices from Italy, the United Kingdom, and France are also presented. This diploma thesis captures the current state of psychiatric care reform in the Pilsen region. Longtemps considérée comme une période perturbée, l’Antiquité tardive apparaît aujourd’hui comme une époque riche de changements et d’influences mêlées, induisant des problématiques d’étude multiples. Ces particularités se traduisent notamment par des spécificités dans les traitements funéraires. Afin de les restituer au mieux, une étude à la fois biologique, topographique, archéologique et historique s’impose. Nous avons choisi cette approche pluridisciplinaire pour aborder les ensembles funéraires urbains et péri-urbains de Vienne et Arles, du IIIème siècle au VIème siècle de notre ère.Le protocole d’étude s’attache à observer les caractéristiques biologiques des échantillons issus de l’ensemble des nécropoles connues, simultanément aux données taphonomiques, à l’organisation spatiale intra- et inter-sites, mais aussi à la topographie à l’échelle de la ville ou aux archives historiques. Cette approche novatrice, mettant en œuvre des outils méthodologiques originaux, s’avère pertinente et a permis de mettre en évidence les mutations et évolutions qui s’opèrent dans la gestion et la représentation des morts à la fin de l’Antiquité. Les ensembles funéraires montrent des faciès très différents en fonction de la période chronologique et il existe bien une spécificité des nécropoles du IVème siècle, entités « intermédiaires » où subsistent de nombreuses pratiques du Haut-Empire, avant la mise en place des premiers ensembles clairement chrétiens, à une époque plus tardive où l’Eglise est devenue dominante. / Long thought of as a time of social upheaval, Late Antiquity seems rather to have been a period of wide-ranging social change encompassing a mixture of influences that pose many problems for study of the period. These multiple influences affect funeral treatments of the period and studying them requires consideration of multiple lines of evidence: historical, archaeological, anthropological, and biological. We chose this multidisciplinary approach in the study of urban and peri-urban burial sites of the antic cities of Vienne and Arles from the 3rd century to the 6th century A.D.Our study protocol allows us to observe the biological characteristics of samples simultaneously to taphonomic data, spatial organization within and between sites, but also topography data or historical archives. This innovative approach, implementing original methodological tools, is relevant. It highlighted the changes in the management and the representation of the dead and Death in Late Antiquity. The burial sites show very different profiles depending on the time period and we can see specificities in the necropolis of the 4th century,” intermediate” structures where many practices of the High Empire are still used, before the establishment of new Christian structures in different places of the city, which have moved again the necropolises locations.

Towards the incorporation of environmental education in the Namibian Secondary School Curriculum

Environmental education (EE) as an approach to all education is needed to be incorporated in all the subjects of the school curriculum. In Namibia, there are Ministerial documents that support the incorporation of education About, In/ Through and For the environment within the curriculum. Even though there are documents that support this, EE continues to suffer barriers that hinder its effective incorporation into the curriculum. The findings reveal that EE in Namibia is incorporated into the traditional environmental subject homes only. The barriers that prevent the incorporation of EE into the Namibian broad curriculum for secondary schools are, amongst others, the lack of adequate teacher education programmes and the lack of interdepartmental collaboration at school level. These barriers and many others have been identified and discussed in detail in this investigation. Os atuais e persistentes cenários de enchentes da Região Metropolitana de São Paulo, e suas consequências em seu espaço e cotidiano, evidenciam a complexidade dessa problemática. A recorrência das enchentes sugere que, mesmo diante da adoção de medidas de controle, combate e mitigação, tais ações demonstraram-se insuficientes, estando associadas a limitações das metodologias e dos modelos a serem abordados. Este estudo relaciona-se a esta última proposição, testando e aplicando formas de integração das abordagens geomorfológicas, às abordagens oriundas da hidrologia e da hidráulica. O objetivo foi analisar as variabilidades temporal e espacial dos resultados de simulações hidrológicas e hidráulicas e compará-los aos mapeamentos obtidos pela Geomorfologia Antropogênica a partir da cartografia geomorfológica evolutiva na bacia hidrográfica do córrego Três Pontes (SP), de modo a identificar potenciais de complementariedade e integração dessas metodologias. Primeiramente, realizou-se a fotointerpretação e reconstituição das morfologias originais (1962) e das formas antropogênicas (2011), gerando cartografias de perturbação que assumiram diferentes graus de susceptibilidades às inundações e ao escoamento superficial concentrado. Em seguida, modelos hidrológicos (Ven Te Chow SCS e Hidrograma Unitário SCS, pelo ABC6) e hidráulico (pelo HECRAS) foram aplicados nos cenários reconstituídos anteriormente, determinando o aumento da vazão de pico e das áreas inundadas entre os anos estudados para tormentas de diferentes tempos de retornos. Os resultados das modelagens hidrológicas e hidráulicas demonstraram aumento significativo da vazão de pico e uma ampliação expressiva da área inundada na BHTP entre 1962 e 2011. Comparados os limites de áreas inundadas obtidos pela modelagem hidráulica à cartografia de vulnerabilidade à inundação, constatou-se uma compatibilidade de resultados. Por um lado, foram identificadas e discutidas limitações relacionadas aos pressupostos, parâmetros e coeficientes de ambas as metodologias; por outro lado, foram assinalados potenciais de integração entre elas. De modo geral, pode-se afirmar que os resultados obtidos contribuem ao conhecimento hidrológico e geomorfológico urbanos, em especial à área de estudo, e constitui um esforço interdisciplinar de aproximar duas especialidades fundamentais ao planejamento territorial urbano. / The current and persistent flooding scenarios in the Metropolitan Region of São Paulo, and their consequences on its space and life, point out to the complexity of this issue. The recurrence of flooding suggests that, even though policies of control, ordering and mitigation were adopted, its measures demonstrated not to be enough, associated with limitations of the methodologies and hydrological and hydraulic forecast models. This research is related to this last proposition, testing and applying ways in which the approaches of geomorphology, hydrology and hydraulics, can be integrated. Therefore, the study aims to analyze the temporal and spatial variability of hydrologic and hydraulic simulations results and to compare them with the mappings obtained by Anthropogenic Geomorphology from evolutionary geomorphological mapping at the stream Três Pontes basin, in order to identify the dialogues and potentials of integration between these methodologies. Firstly, airphotos and satellite images were interpreted and the original morphologies (1962) and anthropogenic forms (2011) reconstructed and mapped, from which disturbance maps were produced considering different susceptibilities to flooding and concentration of discharges. Afterwards, hydrological models (Ven Te Chow-SCS and Unit Hydrograph-SCS using ABC6 ) and hydraulic models (using HEC-RAS ) were applied considering the same scenarios previously reconstituted and, as a result, the increase on peak discharge and of flooding areas, between 1962 e 2011, were determined for different recurrence intervals. When compared, the boundaries of flooded areas obtained through the hydraulic modeling and the flood susceptibility cartography, demonstrated the compatibilities between the results. Limitations of some parameters, coefficients and assumptions related to both methodologies were identified and discussed, as well as, the potential of integration between the two. As a whole, the obtained results contribute to the urban hydrological and geomorphological knowledge, especially in the studied area, and constitute an interdisciplinary effort to approximate two fundamental territorial planning fields.
