Examining Collaboration Within Child Welfare Multidisciplinary Teams: How Home-Based Therapists Respond to Conflict

Indiana University-Purdue University Indianapolis (IUPUI) / When the child welfare system becomes involved with a family in need of services it does so with the goal of concluding its involvement by finding a safe and permanent placement for the children, ideally with their parents. This challenging and complicated work often has many issues that need to be addressed before a successful closure can occur. To achieve this goal, multiple service providers with various backgrounds, degrees, and professions are tasked with working with each other and the family through a collaborative team called a multidisciplinary team (MDT). However, collaboration is not always guaranteed, and conflict can emerge as the team attempts to best serve the family. This conflict may emerge among professionals and between professionals and the family. Although the underlying factors of collaboration and conflict have been documented and studied, research on the process of resolving conflict when it occurs in MDTs is severely lacking in the literature. Furthermore, MDTs specific to the child welfare system also lack the focus they deserve within the child welfare literature. This grounded theory study addresses the gap by focusing on child welfare MDTs and specifically on home-based therapists (N=20) to determine not only their perceptions of facilitators and barriers to collaboration but also the process that they and their fellow service providers engage in when addressing and resolving conflict. In conducting this qualitative study, this researcher used grounded theory to construct a theory outlining the processes that home-based therapists utilize to resolve conflict within MDTs, starting with the emergence of the conflict and detailing the decision making process through the team’s reaction and the ultimate decision or final result. In the future, these findings could be used to aid and train other MDT members as they face their own conflicts with the hope that a more efficient conflict resolution process will lead to a more effective MDT that keeps its focus on the family and provides the needed treatment and services in a timely manner.

O trabalho voltado à saúde tem recebido uma atenção especial em dois enfoques: a humanização do atendimento e a formação de equipes multidisciplinares. A primeira propõe a valorização da dimensão psicossocial do atendimento, processo amplo, complexo, que passa pela quebra de protocolos, como a relação de autoridade entre profissional e paciente; a formação de equipes multidisciplinares encontra dificuldades inerentes ao caráter racional das ciências especializadas, que fragmenta o saber, perdendo a visão de totalidade. Promover a humanização e o trabalho multidisciplinar, foi a proposta de uma equipe atuando em uma academia de ginástica, criada para atender portadores de HIV/Aids. Formada por alunos e profissionais da Educação Física, Fisioterapia, Nutrição e Psicologia, a equipe reunia-se para discutir aspectos técnicos e psicossociais referentes ao atendimento. O presente estudo teve como objetivo identificar elementos relacionados à apropriação do trabalho, produção de conhecimento derivado do atendimento prestado, e seus desdobramentos para a formação do profissional envolvido. Entrevistas individuais, realizadas junto aos participantes da equipe, permitiram a identificação de conteúdos referentes à avaliação do trabalho realizado, cujos significados foram, posteriormente, agrupados em categorias temáticas: a) Integração, cujos conteúdos se referiam à interação na equipe e desta com as pessoas atendidas; b) Produção do Conhecimento, envolvendo elementos para a construção do saber dentro da equipe, enquanto produto da apropriação da experiência informal ou acadêmica; c) Avaliação, enquanto crítica e autocrítica sobre o trabalho realizado, em função de metas não atingidas ou relacionadas ao trabalho multidisciplinar; d) Perspectivas, ou propensões para a continuidade das atividades, sejam no programa ou na carreira profissional. Tais categorias temáticas, enquanto elementos subsidiários á criação de um background para o atendimento humanizado em saúde, apontam para necessidades de mudanças importantes nos processos de formação em saúde, envolvendo formas de apropriação do trabalho no interior das equipes multidisciplinares. / The work directed to health has received special attention in two approaches: the humanization of health service and the formation of multidisciplinary teams. The first one is considered to ad value to the psychosocial service, a wide and complex process which goes through protocol breaks, such as, the relation between professionals and patients; the formation of multidisciplinary teams finds difficulties inherent to the rational aspect of specialized sciences, that breaks up knowledge, losing the global vision. To promote the humanization and multidisciplinary approach was the proposal of a team working in a health club, established to take care of HIV/Aids patients. Created by students and professionals from Physical Education, Physiotherapy, Nutrition and Psychology, the team congregated itself to discuss technical and psychosocial aspects referring to the service. The present study had as a purpose, to identify elements related to the appropriation of work, knowledge production derived from the service, and its developments to the formation of involved professionals. Individual interviews, carried through the team´s participants, allowed the identification of contents referred to the evaluation of work covered, whose meanings were, later, grouped in thematic categories: a) Integration, whose contents related to the teams interaction and its interaction with the people taken care of; b) Knowledge production, involving elements to the construction of knowledge in the team, while a result of the appropriation of informal academic experience; c) Evaluation, while critical and self-critical on the carried work, either as a matter of unreached goals or not related to multidisciplinary work; d) Perspectives, or propensities for the continuity of the activities, either in the program or in the professional career. Such thematic categories, while subsidiary elements to the creation of a humanized service background in health, point to necessary and important changes in the process of health formation, involving forms of appropriation work inside the multidisciplinary teams.

As exigências do cenário mercadológico requerem adaptações nos modelos de trabalho das empresas que visam sobrevivência no longo prazo por meio da satisfação das necessidades de clientes. Nesse contexto, o processo de desenvolvimento de produtos (PDP) é um processo crítico e de crescente importância. Sua alta relevância estratégica para as organizações, devido especialmente às rápidas mudanças nos ambientes de negócios, tem conduzido à necessidade da utilização de equipes multidisciplinares no processo, objetivando maximizar o valor agregado por meio dos conhecimentos específicos dos profissionais envolvidos. Nesse sentido, o conhecimento de mercado, preponderantemente possuído pelas equipes de vendas, é subsídio essencial para o desenvolvimento de novos produtos aderentes ao potencial de mercado. Portanto, a transferência de conhecimento entre os times de maneira colaborativa é fator crítico de sucesso. Assim, o presente trabalho propõe um método que auxilia a diagnosticar a efetividade de transferência de conhecimentos para o trabalho colaborativo entre os setores de desenvolvimento de produto e vendas. O método proposto apoia a identificação dos fluxos de conhecimento entre estas áreas, os tipos de conhecimento compartilhados e ferramentas utilizadas. In this context, new product development (NPD) is a critical process with increasing importance. The high strategic relevance of the NPD process in organizations, mainly due to the rapid changing environments, led to the need of adopting multidisciplinary teams to maximize the added value through the specific knowledge of the professionals involved. In this sense, market knowledge, which is mostly possessed by sales teams, is an essential subsidy for the development of new products that are adherent to the market potential. Therefore, knowledge transfer between teams working in a collaborative approach is a critical factor of success. Thus, this work proposes a method that assists to diagnose the effectiveness of knowledge transfer for the collaborative work between NPD and sales departments. The proposed method helps to identify knowledge flows between these areas, the types of knowledge to be shared and tools used. This is developed by means of an action research in an organization of the communication and media sector.

Nigerian Cancer Survivors' Perceptions of Care Received From Health Care Professionals

Cancer patients worldwide receive care from multidisciplinary teams, and patients sometimes have little or no knowledge about the different treatment options available, making communication with their care providers an important influence in how they perceive their care. Patient satisfaction with care is an important factor in determining survivorship. The increasing prevalence of cancer worldwide has become a huge public health issue. The World Health Organization has warned that the potential gains from combatting infectious diseases in Nigeria might be lost because of the increasing prevalence of cancer. The purpose of this grounded theory study was to obtain the perceptions of 30 cancer survivors in Nigeria about their care in federally funded hospitals to improve cancer care in the country. Through a constructivist grounded theory (CGT) approach, data from 30 in-depth interviews with the cancer survivors were analyzed. Data analysis included open coding, selective coding, theoretical coding, and memo writing. Three themes emerged from the data analysis: positive perception of care (P), service improvement (S), and good care experience (G), all of which were cyclically related to form the PSG substantive theory, which indicates that better health care to cancer patients will improve preventative and curative cancer care services in Nigeria. This theory was validated by relating it to 3 theories in the literature as recommended in CGT research. The implication for positive social change is that the results might provide information that can help health care professionals to improve how they communicate with, relate to, and care for cancer patients and their families in Nigeria and other countries.

General Education Teachers and Classroom-Based Interventions: Knowledge, Training, and Building-Level Influences

Intervention assistance (IA) programs have been developed as a mechanism for avoiding costly special education referrals and for supporting teachers in their instruction of students with varying needs within the general education classroom (Safran and Safran, 1996). Although IA programs are designed to be consultative, multidisciplinary approaches to assisting teachers, some studies report that teachers conduct the majority of classroom-based interventions for a given student on their own prior to referring students to an IA team (Wilson, Hagen, Gutkin, and Oats, 1998). It is important to determine what interventions or strategies teachers commonly consider and what factors are associated with breadth and depth of intervention knowledge. The purpose of the present study was to replicate a portion of the research of Wilson et al. (1998), which assessed general education teachers knowledge of classroom-based interventions. The present study also extended the work of Wilson et al. by using an exploratory descriptive/nonexperimental design to examine the degree to which teachers individual professional characteristics, as well as the IA practices of the schools in which they work, were related to their knowledge of interventions. Twenty-nine general education teachers in Hillsborough County, FL responded to a vignette describing a typical classroom-based problem in a structured-interview format. Participants responses were then counted and coded for (a) how specifically interventions were described, and (b) what types of interventions the teachers used (e.g., instructional, behavioral, etc.). Teachers also completed a brief demographic questionnaire, which included items about the IA programs at their schools, as well as their individual referral history over the last two years, and the degree to which they had been trained in classroom-based interventions. Results were similar to Wilson et al. with regard to number of intervention ideas, but teachers were more specific than in previous investigations. Descriptive data regarding teachers' characteristics as problem-solvers and their perceptions of IA at their school are offered, but few noteworthy relationships were identified between these variables and structured interview outcomes. Nevertheless, the present study offers a glimpse into the intervention practices of general education teachers. Implications for both school psychology practice and research are offered.
